Abstract

This paper reports a design study for a space-based decametric wavelength telescope. While not a new concept, this design study focused on many of the operational aspects that would be required for an actual mission. This design optimized the number of spacecraft to insure good visibility of approx. 80% of the radio galaxies -- the primary science target for the mission. A 5,000 km lunar orbit was selected to guarantee minimal gravitational perturbations from Earth and lower radio interference. Optimal schemes for data downlink, spacecraft ranging, and power consumption were identified. An optimal mission duration of 1 year was chosen based on science goals, payload complexity, and other factors. Finally, preliminary simulations showing image reconstruction were conducted to confirm viability of the mission. 10. Impacto del Índice Neutrófilo/Linfocito en el pronóstico de los pacientes con linfoma difuso de células grandes B tratados con R-CHOP como primera línea

Abstract

Introduction. The Neutrophil/Lymphocyte Index may be a prognostic factor in patients with diffuse large B-cell lymphoma. Material and methods. Ob-servational, retrospective, crosssectional and analytical study. Eighty three patients with diffuse large Bcell lymphoma, diagnosed between January 2014 and December 2018, were included. The descriptive analysis was done with absolute frequencies, percentages and measures of central tendency. The qualitative variables were analyzed with the chisquare test and the quantitative variables with the Student's T test. To evaluate association, the relative risk was measured with a 95% CI, with a p value <0.05% be-ing significant. A survival analysis was performed with Kaplan-Meier curves. Results. Of 83 patients, 51 (61.4%) were men and 32 (38.5%) were women, with a median age of 61 (26-85) years and a median follow-up of 27 (0.9-73.2) months. Only 11 patients (27.5%) with INL ≥3 achieved a complete response. Overall 6-year survival in this group was 36%. Conclusions. The elevated Neutrophil / Lymphocyte Index at diagnosis is associated with a worse response to firstline treatment with R-CHOP and a lower overall survival, Introducción. El Índice Neutrófilo/Linfocito puede ser un factor pronóstico en los pacientes con linfoma difuso de células grandes B. Material y métodos. Estudio observacional, retrospectivo, transversal y analítico. Se incluyeron 83 pacientes con linfoma difuso de células grandes B, diagnosticados entre enero 2014 y diciembre 2018. El análisis descriptivo se hizo con frecuencias absolutas, porcentajes y medidas de tendencia central. Las variables cualitativas se analizaron con la prueba de chi cuadrada y las variables cuantitativas con la prueba T de Student. Para evaluar asociación, se midió el riesgo relativo con intervalo de confianza al 95%, siendo significativo un valor de p <0.05%. Se realizó un análisis de sobrevida con curvas de Kaplan-Meier. Resultados. De 83 pacientes, 51 (61.4%) fueron hombres y 32 (38.5%) mujeres, con una mediana de edad de 61 (26-85) años y una mediana de seguimiento de 27 (0.9-73.2) meses. Sólo 11 pacientes (27.5%) con INL ≥3 alcanzaron respuesta completa. La sobrevida global a 6 años en el grupo de pacientes con INL ≥3 fue de 36%. Conclusiones. El Índice Neutrófilo/Linfocito elevado al diagnóstico se asocia a peor res-puesta al tratamiento de primera línea con R-CHOP y a menor sobrevida global.

Abstract

The capability of photoreactivation to restore infectivity to sunlight-damaged natural viral communities in offshore, coastal and estuarine waters of the Gulf of Mexico was studied. Findings showed that light-dependent repair, probably photoreactivation, compensated for a huge fraction of sunlight-induced DNA damage in natural viral communities and may be important for the maintenance of high concentrations of viruses in surface waters.

24. Evaluation of an individualized dose titration regimen of patiromer to prevent hyperkalaemia in patients with heart failure and chronic kidney disease

Abstract

Aims: Hyperkalaemia risk precludes optimal renin–angiotensin–aldosterone system inhibitor use in patients with heart failure (HF), particularly those with chronic kidney disease (CKD). Patiromer is a sodium-free, non-absorbed potassium (K+)-binding polymer approved for the treatment of hyperkalaemia. In PEARL-HF, patiromer 25.2 g (fixed dose) prevented hyperkalaemia in HF patients with or without CKD initiating spironolactone. The current study evaluated the effectiveness of a lower starting dose of patiromer (16.4 g/day) followed by individualized titration in preventing hyperkalaemia and hypokalaemia when initiating spironolactone. Methods and results: This open-label 8-week study enrolled 63 patients with CKD, serum K+ 4.3–5.1 mEq/L, and chronic HF, who, based on investigator opinion, should receive spironolactone. Eligible patients started spironolactone 25 mg/day and patiromer 16.8 g/day (divided into two doses), with patiromer titrated to maintain serum K+ 4.0–5.1 mEq/L. Mean (standard deviation) serum K+ was 4.78 (0.51) mEq/L at baseline; weekly values were 4.48–4.70 mEq/L during treatment. Serum K+ of 3.5–5.5 mEq/L at the end of study treatment (primary endpoint) was achieved by 57 (90.5%) patients; 53 (84.1%) had serum K+ 4.0–5.1 mEq/L. One patient (1.6%) developed hypokalaemia, and two patients (3.2%) developed hypomagnesaemia. Spironolactone was increased to 50 mg/day in all patients; 43 (68%) patients required one or more patiromer dose titration. Adverse events (AEs) occurred in 36 (57.1%) patients, with a low rate of discontinuations [four (6.3%) patients]. The most common AE was mild to moderate abdominal discomfort [four (6.3%) patients]. Conclusions: In this open-label study, patiromer 16.8 g/day followed by individualized titration maintained serum K+ within the target range in the majority of patients with HF and CKD, all of whom were uptitrated to spironolactone 50 mg/day, patiromer was well tolerated, with a low incidence of hyperkalaemia, hypokalaemia, and hypomagnesaemia. © 2018 The Authors. Abstract

Most supplier evaluation and selection models found in the literature evaluate candidate suppliers individually on various dimensions such as price, quality and delivery timing. Combinations of suppliers used together are modelled as linear combinations of the individual suppliers’ characteristics on each relevant dimension. The existing models are contextual in that they incorporate factors of importance to the buyer. However, they do not consider the interaction between co-suppliers and the interaction between the suppliers’ characteristics and the ordering policy used by the buyer. The present paper proposes a set of stochastic models that explicitly incorporate these interactions. Computational experiments are performed and the results reported. The results show that taking into account the ordering policy and the interaction effects it can induce can lead to co-supplier evaluations that can differ significantly from those obtained from existing models that do not consider these effects. En este artículo se muestra información acerca de un material natural biodegradable, como lo es la cera de candelilla, presentando aspectos sobre algunas características como lo son sus propiedades, método de obtención, características físicas, aplicaciones y algunos estudios realizados sobre este material. Esto con el fin de dar a conocer lo que representa la cera de candelilla, la cual es uno de los productos naturales más apreciados en diferentes industrias, desde la cosmética hasta la electrónica, por sus características únicas de alta calidad, color amarillo transparente, teniendo también una mayor dureza frente otras ceras naturales, así como el brillo y la maleabilidad entre otras propiedades interesantes que presenta este tipo de cera. This paper shows information about biodegradable natural material, such as candelilla wax, presenting aspects of some features such as its properties, method of production, physical characteristics, applications and some studies regard to this material. Stroke represents a public health enemy. Currently, and in spite of multiple clinical trials, thrombolysis remains as the only approved therapy.
