Background : One-anastomosis gastric bypass (OAGB) is a simpler procedure than Roux-en-Y gastric bypass (RYGB); however, biliary reflux can occur and impair outcomes. This study aimed to compare outcomes of OAGB and RYGB. Methods : Twenty patients with morbid obesity were randomized prospectively into two groups: OAGB (n=10) or RYGB (n=10). Quality of life (36-item short-form health survey [SF-36]), satisfaction (Visick scale), and body mass index (BMI) were evaluated before and 6 months after the operation. All patients underwent esophagogastroduodenoscopy with gastric and esophageal mucosal biopsies at 3 and 6 months after their operation. Results : The study found no significant difference in BMI before surgery (OAGB, 43.2 kg/m2; RYGB, 43.1 kg/m2; P=0.90) or at 6 months postoperative (OAGB, 32.1 kg/m2; RYGB, 31.8 kg/m2; P=0.91). There was no significant difference in improvement of quality of life (four SF-36 domains) or satisfaction (P=0.08) between groups at 6 months. There was no statistical difference between gastric (P=0.10) and esophageal (P=0.76) inflammation grade at three or 6 months between the two groups. Conclusion : OAGB and RYGB are equally effective in terms of weight loss, patient satisfaction, and quality of life improvement at 6 months after the procedures. Inflammation grade and cellular damage in the gastric pouch and in the esophagus were similar.

ABSTRACT Purpose: Simultaneous pancreas-kidney transplantation (SPKT) brings several benefits for insulin-dependent type-1 diabetic patients associated with end-stage renal disease (ESRD). However, data on psychological outcomes for the waiting list and the transplanted patients are still lacking. Methods: Using the psychological Beck inventories of anxiety (BAI) and depression (BDI), 39 patients on the waiting list were compared to 88 post-transplanted patients who had undergone SPKT. Results: Significant differences were found regarding depression (p = 0.003) but not anxiety (p = 0.161), being the pretransplant patients more vulnerable to psychological disorders. Remarkable differences were observed relative to the feeling of punishment (p < 0.001) and suicidal thoughts (p = 0.008) between the groups. It was observed that patients who waited a longer period for the transplant showed more post-transplant anxiety symptoms due to the long treatment burden (p = 0.002). Conclusions: These results demonstrated the positive impact of SPKT on psychological aspects related to depression when comparing the groups. The high number of stressors in the pretransplant stage impacts more severely the psychosocial condition of the patient.

Abstract Purpose: To analyze the use of this sponge in pediatric patients undergoing split-liver transplantation. Methods: Retrospective study, including 35 pediatric patients undergoing split-liver transplantation, divided into two groups according to the use of the sponge: 18 patients in Group A (no sponge) and 17 in Group B (with sponge). Results: The characteristics of recipients and donors were similar. We observed greater number of reoperation due to bleeding in the wound area in Group A (10 patients - 55.5%) than in Group B (3 patients - 17.6%); p = 0.035. The median volume of red blood cells transfused in Group A was significantly higher (73.4 ± 102.38 mL/kg) than that in Group B (35.1 ± 41.67 mL/kg); p = 0.048. Regarding bile leak there was no statistical difference. Conclusion: The use of the human fibrinogen and thrombin sponge, required lower volume of red blood cell transfusion and presented lower reoperation rates due to bleeding in the wound area.

Abstract Purpose: To evaluate the effects of mesenchymal stem cells on liver regeneration in rats following a 70% hepatectomy. Methods: Forty rats were subjected to 70% hepatectomy and then ~106 mesenchymal stem cells (test group), or saline solution (control group), were infused into their livers via the portal vein. Each treatment group was divided into early and late subgroups (euthanized 3 d and 5 d following the operation, respectively). Group comparisons of Albumin, aminotransaminases (AST, ALT), and Alcaline Phosphatase (AP) levels, proliferative index (ki-67+ straining), and mitotic cell counts were conducted. Abstract Purpose: To compare the effectiveness of anal and perianal condylomata treatment using argon plasma and electrofulguration. Methods: From January 2013 to April 2014, 37 patients with anal and perianal condylomata, who had been diagnosed through proctological examination, oncotic cytology, polymerase chain reaction (PCR) and histology, underwent treatment with argon plasma and electrofulguration. The perianal and anal regions were divided into two semicircles. Each semicircle was treated using one of the methods by means of simple randomization. Therapeutic sessions were repeated until all clinical signs of infection by HPV were eliminated. The patients were evaluated according to several variables like the genotype of HPV, HIV infection, oncological potential per genotype, oncotic cytology and histology. Results: Among all the variables studied, only immunosuppression due to HIV influenced the results, specifically when the fulguration method was used. There was no significant difference in effectiveness between argon and fulguration based on lesion relapse (p > 0.05). However, among HIV-positive patients, fulguration presented worse results, with a significant difference (p = 0.01). Conclusion: Regarding treatment of anal and perianal condylomata acuminata, comparison between applying fulguration and argon demonstrated that these methods were equivalent, but use of fulguration presented more relapses among HIV-positive patients.

Abstract Purpose: To analyze aspects of the biomodulating effect of light in biological tissues, bone cells from surgical explants of the femur of rats were irradiated with low intensity laser. Methods: Bone cells were cultured and irradiated with LASER light (GaAlAs). Growth, cell viability, mineralized matrix formation, total protein dosage, immunostimulatory properties, cytochemical analysis, gene expression of bone proteins were examined using live cell imaging and cell counting by colorimetric assay. The gene expression of: alkaline phosphatase (ALP), type 1 collagen, osteocalcin and osteopontin through the real-time polymerase chain reaction. Results: At 8 days, the viability of the irradiated culture was 82.3% and 72.4% in non-irradiated cells. At 18 days, the cellular viability (with laser) was 77.42% and 47.62% without laser. At 8 days, the total protein concentration was 21.622 mg / mol in the irradiated group and 16, 604 mg / mol in the non-irradiated group and at 18 days the concentration was 37.25 mg / mol in the irradiated group and 24, 95 mg / mol in the non-irradiated group. Conclusion: The laser interfered in the histochemical reaction, cell viability, matrix mineralization, and maintained the cellular expression of proteins

The extraordinary technological development that occurred in the last fifty years, in which Medicine has evolved more than in the previous fifty centuries, has given rise to several questions of an ethical nature. Since the second half of the last century, there has been a growing perception that doctors are increasingly distant from their patients, prioritizing, at most, the technical aspect of the profession. Bioethics emerged to help humanity to act in a balanced way in the face of environmental and behavioral changes arising from technological evolution.

ABSTRACT PURPOSE: To evaluate the occurrence of seroma and surgical wound infection after surgery. METHODS: A total of 42 individuals with large incisional hernias were subjected toonlay mesh repair. Following the mesh placement, the participants were randomly allocated to two groups. In group 1, closed-suction drains were placed in the subcutaneous tissue, while progressive tension sutures were performed in group 2. The participants were subjected to clinical and ultrasound assessment to detect seroma and surgical wound infection at three time-points after surgery. RESULTS: The occurrence of seroma at the early, intermediate or late assessments was respectively 19.0%, 47.6%, 52.4% in group 1 and 28.6%, 57.1%, 42.9% in group 2 and was not significantly different between groups (p 0.469; 0.631; 0.619). Surgical wound infection occurred 19% in group 1 and 23.8% in group 2, without a significant difference between the groups (p>0.999). CONCLUSION: The frequency of seroma and infection did not exhibit significant differences between individuals subjected to onlay mesh repair of large incisional hernias with drains or progressive tension sutures without drainage.

PURPOSE: To investigate the deposition of collagen in the colon wall of patients with sigmoid diverticulitis. METHODS: Samples of sigmoid tissue from 15 patients (disease group), seven men and eight women aged 37-77 years who underwent surgery for the treatment of diverticulitis, were selected. For the control group, specimens from five patients, three men and two women aged 19-58 years undergoing emergency surgery for sigmoid trauma were selected. These subjects had no associated diseases. The histological study of the surgical specimens was performed by staining with hematoxylin-eosin and picrosirius and using a histochemical method for collagen quantification. RESULTS: Collagen deposition in the colon wall in terms of area (F), glandular epithelium (E) and total area was significantly higher in the disease group compared to control (p=0.003, p=0.026 and p=0.010, respectively). The collagen volume fraction (F fraction) and muscle tissue (M fraction) were also significantly higher compared to control (p=0.044 and p=0.026, respectively). The muscle (M area) and volume fraction of glandular epithelium (E fraction) did not differ significantly between the two groups, (p=0.074 and p=1.000, respectively). CONCLUSION: In this study, collagen deposition in the colon wall of the patients operated for sigmoid diverticulitis was higher compared to patients without the disease.

PURPOSE: To evaluate the effects of copaiba oil on the correction of abdominal defect treated with the use of polypropylene/polyglecaprone mesh in rats. METHODS: A defect in the abdominal wall was created and corrected with polypropylene/polyglecaprone mesh in 36 rats. They were randomly distributed into three groups: control, copaiba by oral administration (gavage) and copaiba oil dip in the mesh. Objective: To assess the presence of microorganism contamination in the preservation solution for transplant organs (kidney/pancreas). Method: Between August 2007 and March 2008, 136 samples of preservation solution were studied prior to graft implantation. Variables related to the donor and to the presence of microorganisms in the preservation solution of organs were evaluated, after which the contamination was evaluated in relation to the “recipient culture” variable. Univariate and multivariate statistical analyses were performed. Results: The contamination rate of the preservation solution was 27.9%. Coagulase-negative Staphylococcus was the most frequently isolated microorganism. However, highly virulent agents, such as fungi and enterobacteria, were also isolated. In univariate analysis, the variable “donor antibiotic use” was significantly associated to the contamination of the preservation solution. On the other hand, multivariate analysis found statistical significance in “donor antibiotic use” and “donor's infectious complications” variables. Conclusions: In this study, 27.9% of the preservation solutions of transplant organs were contaminated. Infectious diseases and non-use of antibiotics by the donor were significantly related to the presence of microorganisms in organ preservation solutions. Contamination in organ preservation solutions was not associated with infection in the recipient. PURPOSE: To investigate the healing process of the aponeurosis of the anterior abdominal wall of rats, comparing two different materials for wound closure: 3-0 nylon suture and tissue adhesive N-butyl-2-cyanoacrylate. METHODS: Forty-four Wistar rats were randomly divided into four groups according to the type of material used (suture or adhesive) and the number of days until reoperation (seven or 14 days). After a 4 cm incision in the aponeurosis, 22 rats underwent wound closure using 3-0 nylon suture and the other 22, the tissue adhesive. After seven days, 11 rats from each group were weighed again, submitted to reoperation and then euthanized. The same procedure was carried out after 14 days with the remaining rats. The surgical wound was macroscopically examined, the tensile strength was measured and the tissue edges were histologically examined. The statistical analysis was performed using analysis of variance and Cox's proportional hazards model. Significance level was set at p < 0.05. RESULTS: The animals lost on average 20 g over the period between the two operations. Wound closure was faster using the tissue adhesive. Only one animal, from the tissue adhesive group, had a small abscess with wound dehiscence. With regard to tensile strength, the best results were obtained with the tissue adhesive 14 days after the first surgery. The results of the histological examination showed no significant difference between groups. CONCLUSIONS: Upon morphological evaluation, the two types of material analyzed in this study (3-0 nylon suture and N-butyl-2-cyanoacrylate, a tissue adhesive) were not significantly different with regard to the healing process of the aponeurosis of the anterior abdominal wall of rats. Wound closure using the tissue adhesive was faster. Higher tensile strength was observed in the tissue adhesive group 14 days after the first surgery.OBJETIVO: Investigar o processo de cicatrização da aponeurose da parede abdominal anterior em ratos, comparando dois diferentes materiais de sutura: fio de poliamida monofilamentar 3-0 e adesivo N-butil-2-cianoacrilato. MÉTODOS: Quarenta e quatro ratos Wistar, foram divididos aleatoriamente em quatro grupos, de acordo com o material de síntese (fio e adesivo cirúrgico) e o tempo de reoperação (7 e 14 dias). Após uma incisão de 4 cm na aponeurose, 22 animais foram submetidos à síntese com o fio de poliamida e os outros 22 animais com o adesivo proposto. Após o procedimento, aguardou-se um período de 7 e 14 dias, quando os animais, 11 de cada grupo, foram novamente pesados e submetidos à eutanásia, sendo realizada a avaliação macroscópica da ferida operatória, mensuração da força de ruptura da parede abdominal e estudo histológico das bordas da incisão. A análise estatística foi realizada através de um modelo de análise de variância e de riscos proporcionais de Cox, considerando significantes valores de p < 0,05. RESULTADOS: Os animais apresentaram uma perda média de 20 g, do dia da operação para o dia da reoperação. A síntese da aponeurose com o adesivo tecidual foi o método mais rápido. Apenas um animal, do grupo adesivo tecidual, apresentou pequeno abscesso local com deiscência de sutura. Com relação à força de ruptura da aponeurose, o melhor desempenho foi do grupo adesivo tecidual, com 14 dias de pós-operatório. Quanto ao estudo histológico, os resultados não mostraram diferença estatisticamente significante entre os dois grupos. CONCLUSÕES: O estudo do processo de cicatrização da aponeurose da parede abdominal anterior mostrou que tanto a síntese com o adesivo N-butil-2-cianoacrilato como com o fio de poliamida monofilamentar 3-0 não apresentou diferença estatisticamente significante na avaliação morfológica, enquanto que a síntese com o adesivo foi mais rápida e mais resistente ao teste de força de ruptura no 14° dia do que a síntese com o fio.

PURPOSE: To evaluate the hepatic effect of Croton cajucara Benth (sacaca) herbal extract in rats. METHODS: 60 Wistar rats (Rattus novergicus albinus) were used, weighing between 250 and 350 g. The animals were distributed randomically in 2 groups: GS - animals which received daily 0,1 ml/ 100 g of sacaca herbal extract through gavage, and GA -animals which received daily 0,1 ml/100g of distilled water through gavage. These were distributed in 3 subgroups with 10 animals, according to theirs euthanasia dates, which were 14th, 28th and 56th day of treatment. RESULTS: Architectural alterations were not observed, however when it was analyzed the presence or absence of necrosis, it was observed in 50% of GS28 subgroup and 90% of subgroup. In 50% of the animals from GS28 subgroup and 90% of GS56 subgroup was observed vast degeneration areas and zonal necrosis, regarding center-lobular veins alterations, there were no alterations in any of the groups CONCLUSION: The Croton cajucara Benth (sacaca) herbal extract in this experiment caused degeneration and hepatic necrosis, suggesting dose-dependent action.OBJETIVO: Avaliar histologicamente os efeitos do infuso, por gavagem, do Extrato Bruto Aquoso Seco (EBAS) do Croton cajucara Benth no fígado de ratos. MÉTODOS: Foram utilizados 60 ratos (Rattus novergicus albinus Wistar), pesando entre 250 e 350g. Os animais foram distribuídos de maneira aleatória em 2 grupos: GS - Grupo de animais que receberam 0,1ml/100g do infuso de sacaca, pela via oral e GA - Grupo de animais que recebeu 0,1ml/100g de água destilada, pela via oral. Estes foram distribuídos em 3 subgrupos de acordo com o dia da eutanásia, sendo esta realizada no 14º, 28º e 56º dia, cada um com 10 animais. RESULTADOS: Os resultados obtidos demonstraram que, microscopicamente, não houve alterações arquiteturais, porém quando se analisou a necrose hepatocitária, esta esteve presente em 50% do subgrupo GS28 e 90% do subgrupo GS56; as alterações inflamatórias puderam ser observadas tanto no subgrupo GS28 como no subgrupo GS56, sendo que neste último se fez presente em 80%, enquanto que no primeiro em apenas 20% dos animais submetidos ao experimento; 50% dos animais do subgrupo GS28 e 90% dos do subgrupo GS56 apresentaram extensa área de degeneração e necrose focal do hepatócito; em 100% dos animais tanto do GA como do GS não apresentaram qualquer alteração da veia centro-lobular. CONCLUSÃO: O infuso de Croton cajucara Benth (sacaca), nas condições deste experimento, foi capaz de determinar degeneração e necrose hepatocitária, sugerindo que a hepatotoxicidade à sacaca é dose dependente.

OBJETIVO: Avaliar, de maneira quantitativa, as proteoglicanas na fáscia transversal e na bainha anterior do músculo reto abdominal de pacientes homens, adultos, portadores de hérnia inguinal tipo II e IIIA de NYHUS. MÉTODO: Foram constituídos três grupos de estudo: um grupo controle, composto por dez cadáveres com óbito até 24 horas e de dois grupos, cada um com vinte pacientes, portadores de hérnias tipo II e IIIA de NYHUS. Foram retiradas amostras da fáscia transversal e da bainha anterior do músculo reto abdominal que foram coradas com Alcian Blue, pH 2,5. As lâminas foram analisadas no programa IMAGELAB de avaliação histológica informatizada. RESULTADOS: Observou-se menor quantidade de proteoglicanas nos pacientes com hérnia inguinal, em relação ao grupo controle. Essa diferença foi estatisticamente significante. CONCLUSÃO: A concentração de proteoglicanas na matriz extracelular está diminuída na fáscia transversal e na bainha anterior do músculo reto abdominal de pacientes homens adultos, portadores de hérnia inguinal tipo II e IIIA de NYHUS, em relação ao grupo controle, constituído por cadáveres não portadores de hérnia inguinal.

OBJETIVO: Comparar a quantidade do glicosaminoglicano dermatam sulfato entre pacientes homens, portadores de hérnia inguinal tipo II de Nyhus e, indivíduos sem hérnia inguinal, com idade entre 20 e 40 anos. MÉTODOS: Foram constituídos dois grupos. Um de 15 pacientes do sexo masculino com hérnia inguinal tipo II de Nyhus e idade entre 20 e 40 anos, com risco ASA I e II, e um grupo controle com dez indivíduos, também do sexo masculino entre 20 e 40 anos, que morreram em período de até 24 h. Foram excluídos os pacientes do sexo feminino, diabéticos, portadores de doença do tecido conjuntivo, tabagistas e com risco cirúrgico ASA III e IV. Foi retirada uma amostra de 1cm² da fáscia transversal na parte intermediária do trígono inguinal, e 1cm² na bainha anterior do músculo reto abdominal na região inguinal correspondente e quantificados os glicosaminoglicanos dermatam sulfato por densitometria, após eletroforese em gel de agarose. RESULTADOS: A quantidade de dermatam sulfato não apresentou diferença estatisticamente significante entre os pacientes com hérnia inguinal e os indivíduos sem hérnia inguinal, tanto na fáscia transversal (p=0,108) quanto na bainha anterior do músculo reto abdominal (p=0,292). CONCLUSÃO: Não se encontrou diferença na quantidade do glicosaminoglicano dermatam sulfato entre os pacientes portadores de hérnia inguinal tipo II de Nyhus e indivíduos sem hérnia inguinal em homens adultos.

A técnica de Shouldice para o tratamento cirúrgico das hérnias inguinais, apesar dos excelentes resultados descritos na literatura, tem sido pouco difundida em nosso meio, No período de junho de 1992 a abril de 1995, 87 hérnias inguinais foram reparadas seguindo os princípios técnicos da Clínica Shouldice, sendo a grande maioria constituída de hérnias primárias (94,7%), A idade dos pacientes variou de 20 a 85 anos, com média de 54 anos e todos foram operados em caráter eletivo, Neste trabalho apresentamos a descrição técnica detalhada do procedimento e os resultados iniciais (taxa de recidiva global de 3,9%) obtidos em um período médio de 13,2 meses de acompanhamento, que sugerem tratar-se de uma boa técnica para o tratamento cirúrgico das hérnias inguinais.

OBJETIVO: Avaliar os resultados do Protocolo de Atendimento de pacientes com diagnóstico de pancreatite aguda grave. MÉTODOS: Foram analisados, consecutivamente, a partir de janeiro de 2002, idade, sexo, etiologia, tempo de internação, tipo de tratamento e mortalidade de 37 pacientes portadores de pancreatite aguda grave. RESULTADOS: A idade dos pacientes variou de 20 a 88 anos (média de 50 anos); 27% foram do sexo feminino e 73% do masculino. O tempo médio global de internação foi 47 dias. Treze pacientes foram tratados cirurgicamente; a média de operações realizadas foi duas por paciente. Ocorreram seis óbitos dentre os pacientes submetidos ao tratamento cirúrgico (46%) e dois óbitos no grupo submetido somente ao tratamento clínico (8,3%). A mortalidade global foi 21% CONCLUSÃO: Após a modificação na forma de abordagem dos pacientes com pancreatite aguda grave, houve diminuição da mortalidade e uma tendência para a conduta expectante.

A profilaxia antimicrobiana é uma das medidas de controle da infecção da ferida cirúrgica. Mesmo com os princípios básicos hoje bem estabelecidos, cerca de 40% das indicações habituais de profilaxia são inadequadas e um dos erros mais comuns está relacionado à duração, em geral superior a 48 horas. Ajustes na profilaxia, além de favorecer sua eficácia na prevenção da infecção cirúrgica, provavelmente contribuiriam para reduzir a pressão seletiva sobre a emergência de bactérias resistentes e custos hospitalares. A simples instituição de uma rotina de antibiótico-profilaxia não garante a adesão dos cirurgiões para adequação do uso de antimicrobianos. No presente estudo, uma intervenção foi realizada na Disciplina de Gastroenterologia Cirúrgica da Universidade Federal de São Paulo - Escola Paulista de Medicina, com a implantação de uma rotina de profilaxia, com a supervisão direta de um infectologista. Os objetivos deste estudo foram avaliar a adequação do uso do antibiótico profilático e seu efeito sobre a infecção cirúrgica pós-operatória. Foi considerada adequada a profilaxia com duração menor ou igual a 24 horas. Dos 318 procedimentos cirúrgicos realizados nos períodos pré e pós-intervenção, em 67,9% foi usado um antibiótico profilático. A intervenção reduziu o uso inadequado de antibiótico de 46,3% para 20,4% (χ² = 15,59; p < 0,05). Infecção do sítio cirúrgico ocorreu em 35,8% dos procedimentos, não se observando modificação deste índice com a adequação da antibiótico-profilaxia. A participação do infectologista é importante na difícil tarefa de racionalizar o uso dos antimicrobianos em nível hospitalar.

RESUMO Objetivo: comparar o resultado do transplante de fígado por hepatocarcinoma em pacientes submetidos ou não ao tratamento loco-regional e downstaging, em relação à sobrevida e risco de recidiva na fila de transplante. Métodos: estudo retrospectivo dos pacientes portadores de hepatocarcinoma submetidos a transplante hepático na região metropolitana de São Paulo, entre janeiro de 2007 e dezembro de 2011, a partir de doador falecido. A amostra foi constituída de 414 pacientes. Destes, 29 foram incluídos na lista por downstaging. Os demais 385 foram submetidos ou não ao tratamento loco-regional. Resultados: as análises dos 414 prontuários demonstraram um predomínio de pacientes do sexo masculino (79,5%) e com média de idade de 56 anos. O tratamento dos nódulos foi realizado em 56,4% dos pacientes em fila de espera para o transplante. O método mais utilizado foi a quimio-embolização (79%). OBJETIVO: Analisar o perfil dos principais centros de transplantes do Brasil, quanto às opções técnicas no transplante de pâncreas. MÉTODO: Foi encaminhado um questionário por correio eletrônico (email) para um membro de cada equipe de 12 centros de transplante do Brasil, com casuística mínima de um transplante de pâncreas. O questionário continha 10 perguntas, abordando aspectos controversos e não padronizados. RESULTADOS: A maioria dos centros (90,9%) utiliza incisão mediana. O órgão de escolha a ser implantado primeiro foi principalmente o rim, em 63% dos centros. Em relação à drenagem venosa, 90,9% utilizam a drenagem sistêmica. A ligadura da veia ilíaca interna é realizada em 54,5% dos centros. A maioria dos centros (90,9%) utiliza a drenagem entérica para transplante combinado pâncreas-rim. Para o transplante de pâncreas isolado, apenas cinco centros responderam, sendo que dois utilizam a drenagem entérica e três a vesical. A utilização de dreno na cavidade abdominal ocorre em 63% dos centros. Em 72,7% dos centros é realizada algum tipo de indução na imunossupressão para o transplante combinado pâncreas-rim, sendo a imunossupressão básica a associação de tacrolimus (FK506), micofenolato mofetil (MMF) e corticóide. A antibioticoprofilaxia é realizada por todos os centros e profilaxia para fungos é realizada por seis centros (54,5%). Oito centros (72,7%) utilizam algum tipo de profilaxia para trombose vascular, em esquemas diversos. CONCLUSÃO: Existem diversos caminhos técnicos na condução do transplante pancreático. A falta de padronização dificulta a análise e a comparação dos resultados. Apesar dessa heterogeneidade das equipes, observamos uma tendência para a realização de incisão mediana, drenagem venosa sistêmica e exócrina entérica, com a utilização de algum tipo de profilaxia para trombose vascular nos transplantes combinados pâncreas-rim.

OBJETIVO: Analisar os resultados de colangiografias peroperatórias (CPO), realizadas sistematicamente durante colecistectomias videolaparoscópicas (CVL). MÉTODO: No período de janeiro de 1998 a dezembro de 1999, foram realizadas CVL em 250 pacientes não selecionados, 65 (26%) do sexo masculino e 185 (74%) do sexo feminino, independente da presença ou não de sinais clínicos, laboratoriais, ultra-sonográficos ou de outros métodos diagnósticos, indicativos de coledocolitíase ou outras doenças da via biliar principal. RESULTADOS: A CPO pôde ser realizada em 237 (94,6%) pacientes. Foram encontradas 31 (13%) alterações, entre as quais seis (2,5%) casos de coledocolitíase e 25 (10,5%) casos de anomalias anatômicas. CONCLUSÃO: A CPO, durante a CVL, mostrou-se um procedimento seguro, sem complicações relacionadas ao exame, definindo a anatomia, prevenindo as lesões biliares e permitindo a detecção de coledocolitíase insuspeitada.

OBJETIVO: Avaliar a participação do colágeno na etiopatogenia das hérnias inguinais, através da análise quantitativa histoquímica do colágeno na bainha anterior do músculo reto abdominal e na fascia transversalis MÉTODO: Foram operados quarenta doentes do sexo masculino, na faixa etária entre 20 e 60 anos, portadores de hérnia inguinal tipo II e IIIA de Nyhus e realizadas autópsias em dez cadáveres frescos, na mesma faixa etária. Em ambos os grupos, amostras da bainha anterior do músculo reto abdominal e da fascia transversalis foram colhidas, coradas pela técnica histoquímica do picrocírius e submetidas a análise morfométrica quantitativa através de sistema computadorizado com a finalidade de identificar possíveis alterações quantitativas do colágeno, em cada um dos dois tipos de hérnias em comparação com indivíduos sem hérnias. RESULTADOS: Encontramos maior concentração do colágeno na bainha anterior do músculo reto abdominal, em comparação a fascia transversalis em todos os grupos estudados, embora esta diferença tenha sido estatisticamente significante apenas no grupo controle. Não encontramos diferenças na concentração do colágeno, na bainha anterior do músculo reto abdominal e na fascia transversalis, entre os doentes com hérnias tipo II e tipo IIIA de Nyhus. Não encontramos diferenças na concentração do colágeno quando comparamos os grupos com hérnias com o grupo controle. CONCLUSÃO: Não encontramos diferenças na quantidade do colágeno na bainha anterior do músculo reto abdominal e na fascia transversalis entre os pacientes portadores de hérnia inguinal tipo II e IIIA de Nyhus em comparação com indivíduos sem hérnias.

OBJECTIVE: to evaluate the role of fibrillar extracellular matrix components in the pathogenesis of inguinal hernias. METHODS: samples of the transverse fascia and of the anterior sheath of the rectus abdominis muscle were collected from 40 men aged between 20 and 60 years with type II and IIIA Nyhus inguinal hernia and from 10 fresh male cadavers (controls) without hernia in the same age range. The staining technique was immunohistochemistry for collagen I, collagen III and elastic fibers; quantification of fibrillar components was performed with an image analysis processing software. RESULTS: no statistically significant differences were found in the amount of elastic fibers, collagen I and collagen III, and the ratio of collagen I / III among patients with inguinal hernia when compared with subjects without hernia. CONCLUSION: the amount of fibrillar extracellular matrix components did not change in patients with and without inguinal hernia.

Malignant schwannoma or neurofibrosarcoma usually grows from the peripheral nerves and rarely from the retroperitoneum. When found in the retroperitoneum, it usually has a worse prognosis. This kind of tumor seems to be resistant to adjuvant therapy and the best treatment is radical surgery. We report a case of a primary retroperitoneal malignant schwannoma, in a 64 year-old man, treated surgically.

The authors report a case of a 25 year old Brazilian man with a history of crampy abdominal pain in the left iliac fossa for 2 weeks, abdominal distention, mucous diarrhea and anorexia. The patient presented signs of hemodynamic instability and a hard mass palpated in the left iliac fossa presented peritoneal irritation. At laparotomy, fecal peritonitis and a punched-out perforation of the midsigmoid colon were found. A left hemicolectomy was performed with terminal colostomy. Specimen examination revealed a thickened rectosigmoid wall, narrow lumen and multiple mucosal polyps. Microscopically, chronic granulomatous colitis with Schistosoma mansoni eggs confirmed the etiology. To the authors' knowledge, this is the first case of obstruction complicated with perforation due to mansoni schistosomiasis reported in the literature.
