Gastric cancer is the fifth most common cancer and fourth leading cause of cancer-related death in the world. The estimated numbers of new gastric cancer cases and deaths in 2020 [...], * Context.--A standardized detailed surgical pathology report is the cornerstone of gastric cancer management. Objective.--To guide management and prognostication for patients with gastric carcinomas globally, the International Collaboration on Cancer Reporting aimed to produce an evidence-based international pathology reporting data set with a panel of globally recognized expert pathologists and clinicians. Design.--Based on published guidelines/data sets for gastric carcinomas, a working draft was developed by the chair of the expert panel of pathologists and clinicians. The draft was then circulated to the panel and discussed in a series of teleconferences and email communications until consensus was achieved. The draft data set was uploaded on the International Collaboration on Cancer Reporting Web site for public comment. The data set was reviewed in consideration of the feedback, and a final version was approved by the panel. Results.--This data set was developed for gastrectomy specimens for primary gastric carcinomas, including neuroendocrine carcinomas and mixed neuro-endocrine-non-neuroendocrine neoplasms. Well-differentiated neuroendocrine tumors, nonepithelial malignancies, and secondary tumors were excluded from this data set. The final data set contains 15 core (required) elements and 8 noncore (recommended) elements. A commentary is provided for each element. Conclusions.--The International Collaboration on Cancer Reporting has published freely available, evidence-based data sets for gastric cancer reporting. Standardized reporting has been shown to improve patient care and facilitates data exchange and analysis for quality assurance, cancer epidemiology, and clinical and basic research. It is a referred type of pain resulting from irritation of phrenic nerve endings. Multiple manoeuvres were used to decrease its incidence with varying success rates. In this study, we tested the use of mild intraoperative hyperventilation to reduce PLSP in patients undergoing laparoscopic sleeve gastrectomy surgery (LSG). Methods: Consenting American Society of Anesthesiologists-I and II patients undergoing LSG under general anaesthesia were randomly assigned to two groups. Group A (53 patients) received intraoperative mild hyperventilation with target end-tidal carbon dioxide (ETCO[sub.2]) of 30-32 mmHg. Group B (51 patients) received conventional ventilation (ETCO[sub.2] of 35-40 mmHg). Incidence and severity of PLSP, cumulative analgesic requirements and incidence of nausea and vomiting were recorded at 12 and 24 hours postoperatively and then followed up after discharge over the phone at 48 hours, 1 week, 1 month and 3 months. Statistical significance of differences between the two groups was defined at P < 0.05. Results: Incidence of PLSP was comparable between the two groups in the first 24 hours. The intervention group had a significantly lower incidence of PLSP throughout the remaining assessment points (56.6 vs. 80.4, 30.2 vs. 78.4, 15.1 vs. 70.6, 3.8 vs. 35.3 at 36 hours, 48 hours, 1 week and 1 month, respectively, P < 0.05). The average PLSP pain score was significantly lower in the mild hyperventilation group at all assessment time points. Nausea and vomiting were non-significantly lower in the mild hyperventilation group. Conclusion: Mild intraoperative hyperventilation could be beneficial in reducing the incidence and severity of PLSP after LSG surgery. Laparoscopic sleeve gastrectomy (LSG) is a relatively new and effective procedure for weight loss. Owing to an increase in the number of bariatric surgical procedures, general surgeons should have an understanding of the complications associated with LSG and an approach for dealing with them. Early postoperative complications following LSG that need to be identified urgently include bleeding, staple line leak and development of an abscess. Delayed complications include strictures, nutritional deficiencies and gastresophageal reflux disease. We discuss the principles involved in the management of each complication. L'obesite est une maladie fort repandue, tant chez les adultes que chez les enfants, et son incidence est en hausse au Canada. La gastrectomie longitudinale laparoscopique (GLL) est une intervention relativement nouvelle et efficace pour la perte de poids. Compte tenu de l'augmentation du nombre d'interventions chirurgicales bariatriques, les chirurgiens generalistes doivent se familiariser avec les complications associees a la GLL et avec leur prise en charge. Les complications postoperatoires immediates de la GLL qu'il faut savoir reconnatre sans retard sont l'hemorragie, les fuites le long de la ligne d'agrafes et la formation d'abces. Parmi les complications plus tardives, mentionnons les stenoses, les carences alimentaires et le reflux gastro-oesophagien. Nous presentons les principes qui sous-tendent la prise en charge de chaque complication.
