International audience; Objectives: Vocal morbidity resulting from damage to the motor branch of the superior laryngeal nerve (SLN) after endocrine surgery is well known, but diagnosis is often delayed. The present study aimed to quantify these vocal changes acoustically (main objective), and correlate this with the vocal complaints of patients with suspected SLN motor impairment (secondary objective).Material and methods: Thirty females patients with suspected injury of the SLN cricothyroid branch (CT−) were compared to 30 patients without postoperative vocal impairment (CT+) and to 30 control subjects. Mean, minimal and maximal fundamental frequencies (F0mean, F0min and F0max) and vocal range were measured on /e/ at high frequency, sirens (glissandi), a reading text, and minimal intonation pairs. Subjective vocal impairment was evaluated on the Voice Handicap Index (VHI).Results: A lowering of F0mean associated with vocal range reduction by one fifth (in the reading text) seemed to be specific to CT− patients. Production of questions was affected, with differences in melodic curve and attack. Thyroidectomy within 2 months in itself (without suspected SLN cricothyroid branch injury) also affected these parameters, but to a lesser degree. CT− patients reported greater voice impairment than CT+ patients or controls (P = 0.0004).Conclusion: Alterations in speech intonation, quantified on minimal pair test, and self-assessed vocal handicap (VHI) are tools that can easily be used in daily practice to screen for SLN motor branch lesion.

International audience; Chimeric-antigen-receptor T cells directed against CD19 (CAR-T) are emerging hematological therapeutics with scarce data on its overall safety profile spectrum. To determine the clinical features and incidence of adverse-drug reactions (ADR) associated with CAR-T. This observational, cross-sectional, pharmacovigilance cohort study examined individual case safety reports from the World Health Organization database VigiBase and meta-analysis of data from CAR-T trials and cohorts in the literature was also performed through March, 2020. The primary objective was to identify ADR associated with approved CAR-T (axicabtagene-ciloleucel; tisagenlecleucel). We conducted a Bayesian disproportionate analysis with the 95% lower credibility-interval of information component (IC025 , significance > 0). We also performed a systematic-review and meta-analysis of CAR-T trials and cohorts in the literature to evaluate ADR incidence. Nine ADR classes were associated with CAR-T: Cytokine release syndrome (CRS, n = 1378, IC025 = 4.24), neurological disorders (n = 963, IC025 = 2.42), hematological disorders (n = 532, IC025 = 3.32), infections (n = 287, IC025 = 2.38), cardiovascular disorders (n = 256, IC025 = 2.81), pulmonary disorders (n = 186, IC025 = 3.80), reno-metabolic disorders (n = 123, IC025 = 1.89), hemophagocytic-lymphohistiocytosis (n = 36, IC025 = 5.01) and hepatic disorders (n = 32, IC025 = 2.49). ADR-related fatalities accounted for 99/1783 (5.5%) of the reports and 262/1783 (14.7%) for all-cause mortality. These ADR-related fatalities were associated with hemophagocytic-lymphohistiocytosis, cerebral vascular disorder, infections, and respiratory failure. In meta-analyses, the most frequent any-grade ADRs were CRS, hematological disorders, and neurological disorders. Fatal ADR were most found with neurological disorders, CRS, and infections. Note, CAR-T infusion may be associated with severe ADR mainly following the week of administration, though rarely fatal. Infections, hemophagocytic-lymphohistiocytosis and end organ failures including neurological or lung involvements require scrutiny.

Le but de cette étude rétrospective a été de les évaluer par des paramètres acoustiques et par leurs retentissements sur la modulation vocale en corrélant les résultats aux ressentis des pa-tients. Matériel et Méthodes: Les voix de 30 femmes avec suspicion d’atteinte du NLS ont été comparées à 30 autres sans dommage post-chirurgical, et à celle de témoins. La fréquence fondamentale (F0) maximale (F0max), minimale (F0min), l’étendue vocale et la durée des groupes significatifs d’une phrase, dits groupes rythmiques, ont été mesurées sur des épreuves vocales et de parole. National audience; But : Les paramètres acoustiques spécifiques à l’expression de la joie, la colère et la tristesse sont-ils modifiés chez des patients avec suspicion d’atteinte du nerf laryngé supérieur (NLS) post thyroïdectomie ? Ces modifications sont-elles en lien avec leur ressenti au quotidien ainsi que leur perception de ces émotions ?Méthode : 34 patients avec suspicion d’atteinte du NLS après une thyroïdectomie totale datant de plus de six mois et moins de trois ans, 34 patients ayant bénéficié de cette chirurgie, dans les mêmes conditions, mais sans atteinte peropératoire, et 34 témoins ont été comparés, avec un protocole commun. Une partie objective, composée d’épreuves classiques d’évaluation vocale et d’autres spécifiques à la communication émotionnelle, a permis d’analyser les paramètres acoustiques (intensité, durée, fréquence).
