Abstract

Background: There is currently no staging system for cutaneous squamous cell carcinoma (cSCC) that is adapted to decision-making and universally used. Experts have unconscious ability to simplify the heterogeneity of clinical situations into a few relevant groups to drive their therapeutic decisions. Therefore, we have used unsupervised clustering of real cases by experts to generate an operational classification of cSCCs, an approach that was successful for basal cell carcinomas. Objective: To generate a consensual and operational classification of cSCCs. Method: Unsupervised independent clustering of 248 cases of cSCCs considered difficult-to-treat. Eighteen international experts from different specialties classified these cases into what they considered homogeneous clusters useful for management, each with freedom regarding clustering criteria. Convergences and divergences between clustering were analysed using a similarity matrix, the K-mean approach and the average silhouette method. Mathematical modelling was used to look for the best consensual clustering. The operability of the derived classification was validated on 23 new practitioners. Results: Despite the high heterogeneity of the clinical cases, a mathematical consensus was observed. It was best represented by a partition into five clusters, which appeared a posteriori to describe different clinical scenarios. Applicability of this classification was shown by a good concordance (94%) in the allocation of cases between the new practitioners and the 18 experts. An additional group of easy-to-treat cSCC was included, resulting in a six-group final classification: easy-to-treat/complex to treat due to tumour and/or patient characteristics/multiple/locally advanced/regional disease/visceral metastases. Conclusion: Given the methodology based on the convergence of unguided intuitive clustering of cases by experts, this new classification is relevant for clinical practice. Abstract

Basal cell carcinoma (BCC) is the most common malignant tumour in white populations. Multidisciplinary experts from European Association of Dermato-Oncology (EADO), European Dermatology Forum, European Society for Radiotherapy and Oncology (ESTRO), Union Européenne des Médecins Spécialistes, and the European Academy of Dermatology and Venereology developed updated recommendations on diagnosis and treatment of BCC. BCCs were categorised into ‘easy-to-treat’ (common) and ‘difficult-to-treat’ according to the new EADO clinical classification. Diagnosis is based on clinico-dermatoscopic features, although histopathological confirmation is mandatory in equivocal lesions. The first-line treatment of BCC is complete surgery. Micrographically controlled surgery shall be offered in high-risk and recurrent BCC, and BCC located on critical anatomical sites. Topical therapies and destructive approaches can be considered in patients with low-risk superficial BCC. Photodynamic therapy is an effective treatment for superficial and low-risk nodular BCCs. Management of ‘difficult-to-treat’ BCCs should be discussed by a multidisciplinary tumour board. Hedgehog inhibitors (HHIs), vismodegib or sonidegib, should be offered to patients with locally advanced and metastatic BCC. Immunotherapy with anti-PD1 antibodies (cemiplimab) is a second-line treatment in patients with a progression of disease, contraindication, or intolerance to HHI therapy. Radiotherapy represents a valid alternative in patients who are not candidates for or decline surgery, especially elderly patients. Electrochemotherapy may be offered when surgery or radiotherapy is contraindicated. In Gorlin patients, regular skin examinations are required to diagnose and treat BCCs at an early stage. Long-term follow-up is recommended in patients with high-risk BCC, multiple BCCs, and Gorlin syndrome.

Notre base recueille les cas de carcinomes épidermoïdes cutanés (CEC) traités par voie systémique depuis 2020. Nous avions montré chez les 142 premiers patients que ceux-ci étaient caractérisés par une fréquence élevée de patients immunodéprimés et de patients avec un PS altéré et que les anti-PD-1 étaient administrés en 1religne dans la majorité des cas. Nous actualisons les données et présentons pour la première fois les données de réponse et de survie des patients sous anti-PD(L)-1.

La durée optimale n’est pas encore établie. L’obtention de réponses durables et le faible risque de rechute après arrêt permettent d’envisager une interruption avant progression/toxicité. L’objectif était d’évaluer la durée de l’immunothérapie grâce à l’émulation d’essais cibles.

Abstract

PIVOTAL (NCT02938299) est un essai ouvert, randomisé, multicentrique de phase 3 évaluant le daromun comme traitement (Tx) intralésionnel néoadjuvant pour le mélanome localement avancé, résécable, de stade III. Le daromun, une combinaison de deux fusions anticorps-cytokines (L19IL2, L19TNF) a montré son efficacité dans une étude de phase 2 (NCT02076633) chez des patients (pts) atteints de mélanome non résécable.

Abstract

Les résultats de l’essai pivot multicentrique randomisé de phase 3 CheckMate 067 chez des patients atteints de mélanome avancé ont montré des améliorations de la survie sans progression et de la survie globale (OS) avec nivolumab (NIVO) plus ipilimumab (IPI) ou NIVO en monothérapie par rapport à IPI en monothérapie. Nous présentons maintenant les résultats finaux de l’étude CheckMate 067, avec un suivi minimum de 10ans, représentant la durée de suivi la plus longue jamais rapportée pour une étude de phase 3 impliquant un traitement à base d’anti-PD-1 (NIVO), toutes tumeurs confondues.

Abstract

Background No simple classification system has emerged for `advanced basal cell carcinomas', and more generally for all difficult-to-treat BCCs (DTT-BCCs), due to the heterogeneity of situations, TNM inappropriateness to BCCs, and different approaches of different specialists. Objective To generate an operational classification, using the unconscious ability of experts to simplify the great heterogeneity of the clinical situations into a few relevant groups, which drive their treatment decisions. Method Non-supervised independent and blinded clustering of real clinical cases of DTT-BCCs was used.
