Abstract

BackgroundAcute myeloid leukemia (AML) is fatal in elderly patients who are unfit for standard induction chemotherapy. The objective of this study was to evaluate the survival benefit of administering sapacitabine, an oral nucleoside analogue, in alternating cycles with decitabine, a low-intensity therapy, to elderly patients with newly diagnosed AML.MethodsThis randomized, open-label, phase 3 study (SEAMLESS) was conducted at 87 sites in 11 countries. Patients aged ≥70 years who were not candidates for or chose not to receive standard induction chemotherapy were randomized 1:1 to arm A (decitabine in alternating cycles with sapacitabine) received 1-hour intravenous infusions of decitabine 20 mg/m2 once daily for 5 consecutive days every 8 weeks (first cycle and subsequent odd cycles) and sapacitabine 300 mg twice daily on 3 consecutive days per week for 2 weeks every 8 weeks (second cycle and subsequent even cycles) or to control arm C who received 1-hour infusions of decitabine 20 mg/m2 once daily for 5 consecutive days every 4 weeks. Prior hypomethylating agent therapy for preexisting myelodysplastic syndromes or myeloproliferative neoplasms was an exclusion criterion. Abstract

BackgroundNRG Oncology/NSABP trial B-40 tested the impact of adding bevacizumab (bev) to neoadjuvant chemotherapy for operable breast cancer. Secondary endpoints included rates of surgical complications after surgery in patients who did or did not receive bev.MethodsA total of 1206 women with HER2-negative operable breast cancer were randomly assigned to receive one of three different docetaxel-plus-anthracycline-based regimens, without or with bev (15 mg/kg every 3 weeks) for the first 6 of 8 cycles and for 10 doses postoperatively. Surgical complications were assessed from date of surgery through 24 months following study entry.ResultsEarly surgical complications were significantly more frequent in the bev group (25.4 vs. 18.9%; trend test p = 0.008), but most were grade 1-2. Early noninfectious wound dehiscences were infrequent and not significantly different (5.4 vs. 3.1%; trend test p = 0.15). Long-term noninfectious wound complications were significantly higher for patients receiving bev (11.8 vs. 5.1%; trend test p = 0.0007), but the incidence of grade ≥3 wound dehiscence was low in both groups (

Abstract

In a developing country like India where 68.84% of people live in villages with insufficient sanitation and poor infrastructure for waterborne sanitation. Government institutions, private water agencies and research institutes are engaged in exploring all alternative domestic wastewater treatment options at the onsite level. Integrated settler-based anaerobic biofilm reactor (ISBABR) is a research initiative in the field of domestic wastewater treatment that this paper reflects the study of its performance on a pilot scale. The ISBABR is designed on the basic concept of conventional septic tanks and anaerobic biofilm reactors. The main motive of this research project to develop an electric-free, minimum capital cost, low operation and maintenance-based onsite treatment system. In this paper we discussed on only the treatment of grey water generated from the hostel with the help of ISBABR. The ISBABR was constructed based on the design of a laboratory-scale reactor (57 litters), which treated the actual sewage at 24 hours HRT. The reactor was started without inoculation of any biomass. Results of a reactor in terms TSS, BOD and COD was 81%, 83%, and 81% removal efficiency respectively. In bacterial analysis, the removal efficiency of the system was found to be higher than 84 percent, which indicates the good performance of the ISBABR system at pilot scale.

BACKGROUND: MYC gene rearrangement (MYC-R) is present in approximately 10% of aggressive B-cell lymphomas with approximately half harboring a BCL2 gene rearrangement (BCL2-R). Multiple retrospective studies of R-CHOP show an inferior outcome in patients with MYC-R, both alone and with BCL2-R and/or BCL6-R, and suggest better outcomes with more aggressive treatment. In the current study, we aimed to determine the outcome of DA-EPOCH-R, an aggressive infusional treatment regimen, in untreated MYC-R aggressive B-cell lymphomas. METHODS: Final analysis of a prospective multi-center study of DA-EPOCH-R (dose-adjusted treatment: etoposide, prednisone, vincristine, cyclophosphamide, doxorubicin and rituxiomab) in 53 patients with untreated MYC-R aggressive B-cell lymphomas. DAEPOCH-R was scheduled to be administered with central nervous system prophylaxis for 6 cycles. Primary endpoints included event-free and overall survival. The study was registered at ClinicalTrials.gov (NCT01092182). FINDINGS: Patient characteristics included median age 61 (range 29–80) years, stage III/IV disease in 43 (81%), and high-intermediate/high international prognostic score (IPI) in 26 (49%) patients. Characteristics were similar among patients with confirmed MYC-R single-hit (n=19) versus those with a BCL2-R and/or BCL6-R, termed double-hit, (n=24) lymphomas. With a median follow up of 55.6 (Interquartile range: 50.5–61.1) months, the 48-month EFS and OS for all patients was 71% and 77%, respectively, with no differences among patients with single versus double-hit tumors or age < versus ≥ 60 years. The EFS at 48-months for low/low-intermediate (0–2) versus high-intermediate/high (3–5) IPI was 89% versus 50%, respectively, for all patients, and 92% versus 55% for double-hit patients. Toxicity included grade 4 neutropenia and thrombocytopenia on 53% and 13% of cycles, respectively, and fever with neutropenia occurred on 19% of cycles. There were 3 treatment related deaths. INTERPRETATION: In this study, DA-EPOCH-R produced durable remissions in MYC-R aggressive B-cell lymphomas and should be considered for the treatment of these diseases. Abstract

Membrane Bio Reactor (MBR) system, designed to achieve high-quality effluent within a small overall footprint. The MBR process utilizes the well-proven activated sludge process but replaces conventional final settlement with an ultrafine membrane which effectively filters the final effluent. Membrane bioreactors (MBRs) can be broadly defined as systems integrating biological degradation of waste products with membrane filtration. They have proven quite effective in removing organic and inorganic contaminants as well as biological entities from wastewater. Advantages of the MBR include good control of biological activity, high-quality effluent free of bacteria and pathogens, smaller plant size, and higher organic load rates. The quality of the MBR permeates for hostel wastewater conforms largely to the microbiological standards for reusable (e.g. flushing, cleaning, and agricultural use). This study was undertaken to determine the efficiency of the MBR treatment plant in terms of its physical, chemical and biological characteristics of influent and effluent wastewater. The removal efficiencies of the treatment plant (Turbidity>99%, TSS>98%, BOD>96%, COD>93% and MPN>99%) are very high compared to another treatment plant. Membrane technology is the most important method in achieving the objective of the reuse of wastewater in an era of water scarcity in many parts of the world.

Abstract

Background: Several registry‐based analyses suggested a survival advantage for married versus single patients with pancreatic cancer. The mechanisms underlying the association of marital status and survival are likely multiple and complex and, therefore, may be obscured in analyses generated from large population‐based databases. The goal of this research was to characterize this potential association of marital status with outcomes in patients with resected pancreatic cancer who underwent combined modality adjuvant therapy on a prospective clinical trial. Materials and Methods: This is an ancillary analysis of 367 patients with known marital status treated on NRG Oncology/RTOG 97‐04. Survival analysis was performed using the Kaplan‐Meier method and compared using the log‐rank test. Multivariate analysis was performed using the Cox proportional hazards regression model. Results: Of 367 patients, 271 (74%) were married or partnered and 96 (26%) were single. Married or partnered patients were more likely to be male. There was no association between marital status and overall survival (OS) or disease‐free survival (DFS) on univariate (hazard ratio [HR], 1.09 and 1.01, respectively) or multivariate analyses (HR, 1.05 and 0.98, respectively). Married or partnered male patients did not have improved survival compared with female or single patients. Conclusion: Ancillary analysis of data from NRG Oncology/RTOG 97‐04 demonstrated no association between marital and/or partner status and OS or DFS in patients with resected pancreatic cancer who received adjuvant postoperative chemotherapy followed by concurrent external beam radiation therapy and chemotherapy. Implications for Practice: Several population‐based studies have shown an epidemiological link between marital status and survival in patients with pancreatic cancer. A better understanding of this association could offer an opportunity to improve outcomes through psychosocial interventions designed to mitigate the negative effects of not being married. Based on the results of this analysis, patients who have undergone a resection and are receiving adjuvant therapy on a clinical trial are unlikely to benefit from such interventions. Further efforts to study the association between marital status and survival should be focused on less selected subgroups of patients with pancreatic cancer. Evidence suggests a survival advantage for married compared with single patients with pancreatic cancer. Abstract

Targeting a single pathway in pancreatic adenocarcinoma (PC) is unlikely to affect its natural history. We tested the hypothesis that simulataneous targeting of the epidermal growth factor receptor (EGFR) and insulin-like growth factor receptor-1 (IGF-1R) pathways would significantly improve progression-free survival (PFS) by abrogating reciprocal signaling that promote drug resistanceThis was a phase Ib/II study testing cixutumumab, combined with erlotinib and gemcitabine (G) in patients with untreated metastatic PC. The control arm was erlotinib plus G. The primary end point was PFS. Eligibility included performance status 0/1 and normal fasting blood glucose. Polymorphisms in genes involved in G metabolism and in the EGFR pathway were also studiedThe phase I results (n = 10) established the safety of cixutumumab 6 mg/kg/week intravenously, erlotinib 100 mg/day orally, and G 1000 mg/m(2) intravenously on days 1, 8, and 15 of a 28-day cycle. In the RP2 portion (116 eligible patients; median age, 63), the median PFS and overall survival (OS) were 3.6 and 7.0 months, respectively, on the cixutumumab arm, and 3.6 and 6.7 months, respecively, on the control arm. Major grades 3 and 4 toxicities with cixutumumab and control were elevation of transaminases, 12% and 6%, respectively; fatigue, 16% and 12%, respectively; gastrointestinal, 35% and 28%, respectively; neutropenia, 21% and 10%, respectively; and thrombocytopenia, 16% and 7%, respecively. Grade 3/4 hyperglycemia was seen in 16% of patients on cixutumumab. Grade 3 or 4 skin toxicity was similar in both arms of the study (5%). No significant differences in PFS by genotype were seen for any of the polymorphisms.Adding the IGF-1R inhibitor cixutumumab to erlotinib and G did not lead to longer PFS or OS in metastatic PC.
