PURPOSE: Since 1911, when Dollinger first described surgical orbital decompression, many different techniques and approaches have been described, including 1-, 2-, and 3-wall decompressions with orbital fat removal. The indications for surgical treatment have been widened and include additional to optic neuropathy severe proptosis causing exposure keratopathy and disfigurement. Popular techniques for surgical decompression are the transantral or transpalpebral approach to the medial wall and orbital floor. Sometimes theses approaches are combined with lateral canthotomy incisions for access to the lateral wall ('3-wall decompression'). This report shows a compilation of different techniques using computer-assisted surgery for a 3-wall decompression with the introduction of the 'temporal cage' in patients suffering from Graves' disease and the combination with zygomatic bone osteotomy. PATIENTS AND METHODS: Twelve patients suffering from Graves' disease presented for surgical treatment in our clinic. For surgical planning, a computed tomographic scan with navigation markers was carried out. The navigation was planned with voxel-based navigation software (VoXim or BrainLab). RESULTS: Of the patients, 1 had unilateral and 11 had bilateral decompressions. Of the 11 patients with bilateral procedures, 1 patient underwent simultaneous and 10 underwent sequential procedures. The 3-wall decompression in all cases was performed by transconjunctival approach. Temporary double vision could be observed in all cases. There was a significant reduction in proptosis. CONCLUSION: It was concluded that this approach and the computer-assisted surgery would be the operation of first choice when there is an indication for orbital decompression. [ABSTRACT FROM AUTHOR]

Purpose: The aim of this international AO-study was to compare the functional outcome after open versus closed treatment of mandibular condylar neck fractures. Patients and Methods: A prospective comparative study with two follow-ups (FU) at 8-12 weeks and 1 year was undertaken in two clinics, which exclusively privileged either surgical or conservative treatment due to different therapeutic agendas. Patients from clinic 1 (ENDO group) received endoscope-assisted transoral open reduction and internal fixation, whereas patients from clinic 2 (CONS group) were treated conservatively without surgery. Patients with unilateral condylar neck fractures showing one or more of the following conditions were included: displacement of the condyle with an inclination >30° and/or severe functional impairment such as malocclusion or open bite, with or without dislocation of the condylar fragment; severe pain upon palpation or movement, and/or vertical shortening of the ascending ramus. High or intracapsular condylar neck fractures were excluded. Results: 75 patients (44 CONS and 31 ENDO patients) with condylar neck fractures were included in this study. The Asymmetric Helkimo Dysfunction Score (A-HDS) was slightly lower in the CONS group than in the ENDO group at the 8-12-week FU, corresponding to better function on the short-term. At the 1-year FU, however, there were slightly better values in the ENDO group. For the Clinical Dysfunction Index (Di) and the Anamnestic Dysfunction Index (Ai), CONS patients had a better outcome than ENDO patients at the 8-12 week FU, ie, a higher proportion of ENDO patients had severe symptoms due to the operative trauma. Yet these symptoms improved by one year, finishing with a significant higher proportion of symptom-free patients in the ENDO group. In addition, these patients had better values for the Index for Occlusion and Articulation Disturbance (Oi) at both FU examinations, ie, the proportion of patients without any occlusal disturbances was significantly higher in the ENDO group. On average, the duration of postoperative maxillo-mandibular fixation (MMF) was 3 times longer for the CONS group than for the ENDO group (33 vs. 11 days). Conclusion: Both treatment options may yield acceptable results for displaced condylar neck fractures. Especially in patients with severe malocclusion directly after trauma, however, endoscope-assisted transoral open reduction and fixation seems to be the appropriate treatment for prevention of occlusal disturbances during FU. Ablative surgery of the orbit is often associated with dramatic changes in facial geometry. Surgical intervention is often necessary to correct the functional and esthetic appearance in those patients who are anophthalmic, having an intact eyelid appearance and an orbital prosthesis. The outcome of the surgical correction depends on the shape of the orbital implants and their adequate placement. In the case of comparatively small rearrangements, the effect of implants on soft tissues can be estimated by surgeons on the basis of their experience. However, large deformities in complex cases (including large deformation of soft tissue or asymmetry) can be hardly predicted on the basis of simple empirical considerations. The purpose of the present technical note was to describe a new procedure of inverse design of customized orbital titanium meshes. To demonstrate this procedure, an anophthalmic patient with superior sulcus deformity and enophthalmos was enrolled. The volume and structure of the extraocular muscles, soft tissue, and bony structure of the orbital walls were examined using high-resolution multislice computed tomography. Next, a geometric model of the patient's anatomy was generated from the tomography data. Afterward, the orbital prosthesis was virtually relocated to a new position. Then, the desired correction of the particular soft tissue regions was performed using virtual sculpturing tools. Next, the deformation of the soft tissues and initial prosthesis boundaries were computed from the predefined displacements of the relocated tissue regions with the help of the Finite Element Method. The differential volume between the initial and designated position of the orbital prosthesis yielded the preferred implant shape required to effect the desired correction of soft tissue. During surgery, the preplanned position of the customized titanium meshes was guided using a navigation system. Although the inverse design of custom-tailored titanium meshes for precision treatment of severe enophthalmos in anophthalmic patients appears to be a promising approach, it has rarely been applied in the past because of the technological complexity and additional time required. With the present study, we have launched a series of clinical evaluations of this novel method. To date, scientific evidence and trials showing a predictable simulation using an inverse finite element approach in the correction of severe enophthalmos in anophthalmic patients with computer-assisted fabricated individual titanium meshes for reconstruction of orbital walls have been rare. Background: There are considerable differences in the shape and composition of military and civilian projectiles.Materials and Methods: Five different projectiles with the same kinetic energy were fired into the heads of freshly sacrificed pigs (n=30) through the submental region in the occipital direction. Computed tomography (CT) and 3D face scans of the animal skulls were obtained before and after firing. The image data sets were fused and provided the basis for a quantitative analysis of destruction patterns.Results: As a result of the destruction of the parietal bone at the potential exit site, there were significant volume difference between the Action 4 (6.45 +/- 3.42 ml) and the Hydra-Shok projectile (12.71 +/- 2.86 ml). The partial metal-jacketed projectile showed a minor increase in volume (4.89 +/- 1.47 ml) and a partial loss of soft projectile components. Radiology showed differences between the various projectiles in fragmentation and bone and soft-tissue destruction.Conclusions: Although the projectiles had the same kinetic energy, there were considerable differences in injury patterns between full metal-jacketed projectiles, which are the only projectiles permitted for military use under the Geneva Conventions, and the other investigated projectiles. Subperiosteal implants have experienced fluctuating popularity since their inception. Initially introduced in the mid-20th century, they were overshadowed by the development and success of endosteal implants, which are now the standard in dental implantology. Modern subperiosteal implants are typically custom-made using computer-aided design and computer-aided manufacturing technologies, which ensure a precise fit to the patient's bone anatomy. A meta-analysis was conducted to evaluate the success rates, complications, and patient satisfaction associated with subperiosteal implants. Trial Design: This controlled in vivo experimental study examines the impact of two periosteum preparation techniques on microcirculation during bone augmentation with isogenic bone grafts in rats., Methods: Twenty female Lewis rats were divided into two groups (n=10 each). In one group, the periosteum was prepared with a conventional periosteal elevator; in the other, a piezoelectric device was used. After graft implantation at calvarial sites, intravital microscopy was performed post-operation (day 0) and on days 3, 8, and 28 immediately to assess microvascular parameters: functional capillary density, blood flow velocity, and vessel diameter. Statistical analysis was conducted using ANOVA on ranks with p<0.05., Results: The piezoelectric device group showed higher mean values for functional capillary density, blood flow velocity, and vessel diameter than the conventional instrument group, though differences were not statistically significant., Conclusions: This study suggests that periosteum preparation with a piezoelectric device does not significantly differ from conventional methods regarding microcirculatory outcomes. Either method appears viable for preserving microcirculatory integrity during bone augmentation. Background: Condylar hyperplasia of the mandible is characterized by abnormal size and configuration of the condylar process. In Germany, the administrative prevalence of diagnosed and/or treated condylar hyperplasia is 2.4-9.6 cases per 100 000 persons. Misdiagnosis is common and can lead to severe esthetic and functional complications, including facial deformity that can progress into the patient's twenties, as well as dysfunctional speech and mastication., Methods: We conducted a systematic review of the literature and a structured consensus-finding process with the Delphi method., Results: The experts recommend timely diagnosis of condylar hyperplasia so that its progression can be prevented by appropriate treatment. The basic diagnostic evaluation consists of history taking, physical examination, and imaging with orthopantomography. If condylar hyperplasia is confirmed, three-dimensional imaging is performed, usually with SPECT. The treatment is mainly surgical: partial condylectomy techniques and orthognathic surgery are the most common methods. If left untreated, the disorder causes severe dysfunction of mastication as well as disfigurement. Most studies of treatments for condylar hyperplasia have been based on low case numbers and have yielded only level 4 or level 5 evidence., Conclusion: Condylar hyperplasia is a common, but not widely known problem. Its early diagnosis and treatment can prevent severe jaw asymmetry and further complications such as craniomandibular dysfunction and arthrosis of the temporomandibular joints.

Goldenhar syndrome is a rare congenital disease, characterized by the malformation of craniofacial features, spine, and other organs. Its pathogenesis and impact on patients' quality of life (QoL) have not been fully elucidated as yet. Therefore, data of all patients with Goldenhar syndrome, who were treated at Hannover Medical School between 1997 and 2020, were analyzed. Forty-three patients were identified. Their medical records were carefully reviewed for data regarding their age, sex, demographics, Pruzansky classification, surgical history, and affected side. Goldenhar syndrome-associated medical conditions, therapeutic procedures, and the impact of disease on QoL were assessed. Their QoL was assessed using the Oral Health Impact Profile (OHIP)-14 questionnaire. The χ 2 test and Pearson correlation were performed for statistical evaluation. Female sex was positively correlated with OHIP outcomes, whereas male sex had a negative correlation with OHIP outcomes. Therefore, females had significantly worse QoL than males. No significant correlation was observed between the Pruzansky classification and QoL or between surgical history and QoL. Goldenhar syndrome is a rare disease that presents with varying severity. Introduction: Dental rehabilitation in oral cancer patients is essential for good oral health-related quality of life (OHRQoL). Patient-specific dental implants are suitable for treating tumor-related bony defects, resulting in satisfactory OHRQoL. However, knowledge concerning the clinical outcome and OHRQoL following tumor irradiation is lacking., Material and Methods: A retrospective analysis was carried out to evaluate clinical outcomes and OHRQoL in eight patients who received patient-specific dental implants and implant-supported dentures after surgical treatment for oral cancer with additional irradiation. OHRQoL assessment was performed using the German long version of the oral health impact profile (OHIP) questionnaire (OHIP-G53)., Results: Clinical examination revealed successful dental rehabilitation in all the patients with only minor impairments. Restricted stability and function of implants were not observed. OHIP sum-scores of all the patients indicated acceptable OHRQoL, but this varied between patients treated in the upper or lower jaw. Single-item sum-scores concerning the adverse events "difficulty in chewing," "food catching," "sore jaw," "sore spots," and "unclear speech" were detected to be the worst, and pain-related OHIP dimensions demonstrated the highest scores (followed by functional limitation, physical disability, and psychosocial impact) with a worse OHRQoL following lower jaw treatment. Other dimension sum-scores were overall lower and nearly equally distributed in patients., Conclusions: Dental rehabilitation of irradiated oral cancer patients using patient-specific dental implants may be suitable, leading to acceptable OHRQoL. However, implant insertion in the upper jaw seems to be more favorable. Defects and bone loss in the maxilla and mandible pose significant challenges for dental rehabilitation. This paper focuses on complex cases of bimaxillary dental rehabilitation, where traditional dental implant protocols were not feasible in at least one jaw. Four patients were examined conceptually, where conventional dental implant placement (n = 20) was combined in either the same or opposite jaw with a patient-specific subperiosteal implant (n = 5). This study evaluates aspects such as primary stability, prosthodontic restoration, complications, and soft tissue management over the observation period. None of the five patient-specific subperiosteal implants (IPS Implants ® Preprosthetic; KLS-Martin Group, Tuttlingen, Germany) experienced failure or showed any loosening of screws, with the longest observation period extending to 68 months. These implants were securely fixated away from the posts, without any biomechanical restrictions on loading from the time of insertion. Planning and manufacturing, including the initial suprastructure, followed a fully digital workflow. The number of screws required for multivector fixation ranged from 13 to 22. All dental implants placed remain functional, definitive prosthodontic restoration has been performed, and no stability loss or peri-implantitis has been observed. The IPS Implants ® Preprosthetic emerges as a valuable consideration when conventional implant dentistry protocols encounter limitations.

Background: During articulation the velopharynx needs to be opened and closed rapidly and a tight closure is needed. Based on the hypothesis that patients with cleft lip and palate (CLP) produce lower pressures in the velopharynx than healthy individuals, this study compared pressure profiles of the velopharyngeal closure during articulation of different sounds between healthy participants and patients with surgically closed unilateral CLP (UCLP) using high resolution manometry (HRM)., Materials and Methods: Ten healthy adult volunteers (group 1: 20-25.5 years) and ten patients with a non-syndromic surgically reconstructed UCLP (group 2: 19.1-26.9 years) were included in this study. Pressure profiles during the articulation of four sounds (/i:/, /s/, /ʃ/ and /n/) were measured by HRM. Maximum, minimum and average pressures, time intervals as well as detection of a previously described 3-phase-model were compared., Results: Both groups presented with similar pressure curves for each phoneme with regards to the phases described and pressure peaks, but differed in total pressures. An exception was noted for the sound /i:/, where a 3-phase-model could not be seen for most patients with UCLP. Differences in velopharynx pressures of 50% and more were found between the two groups. Maximum and average pressures in the production of the alveolar fricative reached statistical significance., Conclusions: It can be concluded that velopharyngeal pressures of patients with UCLP are not sufficient to eliminate nasal resonance or turbulence during articulation, especially for more complex sounds. Secondary mandibular reconstruction using fibular free flaps (FFF) is a technical challenge for surgeons. Appropriate operation planning is crucial for postoperative quality control and is notably necessary for the (re-) achievement of a physiological condylar position, and the sensible expansion and shaping of the transplant. Computer-assisted planning may help to reconstruct mandibular defects in a patient-specific and precise manner. Herein, we present a newly-developed workflow for secondary mandibular reconstruction using FFF; it comprises digital planning and in-house manufacturing to perform precise secondary mandible reconstruction. This method utilizes a newly designed positioning device to ensure the precise positioning of the fibula segments in relation to each other and the mandibular stumps. The presented in-house-printed positioning device made it possible to achieve digital planning with high precision during surgery.
