The aim of this study is to find more suitable induction therapy and consolidation therapy for elderly AML patients.Methods: A total of 149 consecutive newly diagnosed elderly AML patients (aged ≥ 60 years) who received induction chemotherapy in our medical center from January 2015 to December 2019 were retrospectively analyzed.Results: After the first induction treatment, the complete remission/or complete remission with incomplete hematologic recovery (CR/CRi) rates in the standard-intensity chemotherapy group was significantly higher than that in the low-intensity chemotherapy group (58.2% vs 32.9%, p = 0.003). Compared with the low-intensity chemotherapy, the incidence of severe infection in the standard-intensity chemotherapy was significantly increased (p < 0.001), but the early mortality was comparable. One hundred and seven patients received minimal residual disease (MRD) examination after the first induction treatment; and MRD was negative accounting for 51.9% in the standard-intensity chemotherapy group, while only 32.7% in the low-intensity group (p = 0.05). The 2-year-overall survival (OS) of patients in standard-intensity induction chemotherapy group (37.2%) was slightly higher than that in low-intensity induction chemotherapy group (23.4%) (p = 0.075). Eighty-one CR/CRi patients received intermediate or high dose cytarabine (n = 35) or sequential chemotherapy regimens (n = 46) as consolidation treatment. The 2-year OS and event-free survival (EFS) of patients in the intermediate or high-dose cytarabine group were significantly higher than those in the sequential chemotherapy regimens group (73.0% vs 38.5%, p = 0.002; 54.8% vs 35.0%, p = 0.035).Conclusion: Our results showed that standard-intensity induction chemotherapy can significantly improve the CR rate for elderly AML patients, and does not increase the early mortality; consolidation therapy with intermediate or high-dose cytarabine can significantly improve EFS and OS for elderly AML patients achieved CR.Keywords: acute myeloid leukemia, elderly patients, minimal residual disease, standard-intensity chemotherapy, intermediate or high dose cytarabine

Abstract

Background: Juvenile myelomonocytic leukemia (JMML) is a rare hematological malignancy in young children and can only be cured through the allogeneic stem cell transplantation. Procedure: We have retrospectively analyzed the outcomes of nine children with JMML after unrelated cord blood transplantation (UCBT). Results: Eight patients who have received a myeloablative conditioning regimen of fludarabine (FLU), busulfan (BU), and cyclophosphamide (CY) have gotten engraftment. None of the nine patients has relapsed following initial UCBT. Six patients are still alive and in complete remission after UCBT with a median observation time of 43 months (range: 10–80 months). Conclusions: This study shows that UCBT with FLU‐BU‐CY conditioning regimen can represent a suitable option for children with JMML. [ABSTRACT FROM AUTHOR]

Abstract

Prolonged isolated thrombocytopenia (PIT) is a common complication after umbilical cord blood transplantation (UCBT). However, data on PIT prediction and impacts on transplantation outcomes for UCBT patients are rare. We retrospectively analyzed 244 patients with hematological malignancies who received single‐unit UCBT at the First Affiliated Hospital of USTC between August 2018 and December 2019. Among them, PIT occurred in 49 recipients, with a crude incidence of 20.1%. In the PIT patients, the 2‐year cumulative incidence of transplant‐related mortality (TRM) was significantly higher, and the probabilities of 2‐year overall survival, leukemia‐free survival and graft‐versus‐host disease (GVHD)‐free relapse‐free survival were significantly poorer (57.1% vs. 88.6%; 53.1% vs. 81.9%; 22.4% vs. 59.8%; p < 0.001), without remarkable increases in the cumulative incidence of relapse or chronic GVHD. Importantly, the multivariate analysis revealed that lower high‐resolution HLA compatibility (≤6/10), lower infused CD34+ cell count (≤1.78 × 105/kg), grade II–IV acute GVHD preplatelet engraftment, a lower pretransplantation platelet count (≤100 × 109/L), and a longer neutrophil engraftment time (≥17 days) were independent risk factors for PIT after UCBT. These results demonstrate that PIT is common after UCBT, predicting inferior survival and the need for more monitoring during the early phase. [ABSTRACT FROM AUTHOR]

Abstract

Background: The severity of complications of transplantation is governed mainly by the status of immune reconstitution. Compared to other stem cell sources, cord blood transplantation (CBT) is characterized by an impairment of early T cell immune reconstitution but rapid B-cell neogenesis. In this study, we investigated the kinetics of early B-cell immune reconstitution and its relation with clinical outcome after CBT using myeloablative regimens without antithymocyte globulin (ATG). Methods: (1) Between July 2011 and Feb. 2016, 199 patients with malignant hematological diseases who received an unrelated CBT at Anhui provincial hospital were included in this study. The patients received myeloablative conditioning without ATG and cyclosporin A (CSA) and mycophenolate mofetile as graft-versus-host disease (GVHD) prophylaxis in CBT protocols. (2) Dynamic monitoring of blood cells and lymphocyte recovery after CBT. (3)Lymphocyte subsets were assessed by flow-cytometry after fluorochrome-conjugated monoclonal antibodies for T-cells (CD3, CD4, and CD8), B-cells (CD19), and NK-cells (CD56, CD16). Immunoglobulin levels were measured by immunoturbidimetry assay. These analyses were done approximately at 1-6, 9, 12, 18 and 24 months post-transplantation.(4)The relationship between immune reconstitution and clinical outcome was analyzed. Results: (1) From 1 month after CBT, the absolute monocyte count (AMC) and absolute lymphocyte count (ALC) sustained to increase until month 18, the median ALC, CD3+-cell, CD3+CD4+-cell, and CD3+CD8+-cell counts achieved normal at month 2, 3, 5, and 2, respectively. The median NK-cell counts achieved normal at month 1 after CBT. (2) Early B-cell recovery was delayed with gradual increase in the number of cells, starting around month 3 after CBT. The median CD19+-cell count at days 30, 60 and 90 was 1/µl, 2/µl, and 7/µl, respectively. The median CD19+-cell count reached normal at month 5, the counts continue to increase beyond the normal range until month 18 post CBT. The median serum IgM, IgG and IgA levels reached the normal range at month 3, 4, and 9 after CBT, respectively. (3)Thirty-one cases died of severe infection and the median time of death from infection was 91 days after CBT (range, 31-413 days). By assessment of the rate of readmission within 30 days of hospital discharge, the patients with lower counts of CD19+ population after CBT has been associated with a higher readmission rates because of infectious episodes and GVHD. Onset of B-cell robust recovery ,which characterized by CD19+-cell count greater than 45/µl at month 3, 90/µl at month 4, and 280/µl at month 6, the patients' subsequently readmission-rate was decreased significantly. The 30-day readmission rate was 6.9%,11% and 24% after CBT when the patients achieved robust B-cell recovery at month 3, 4, and 6, respectively.Assessment of B-cell counts within 83 graft samples, 32 of 83 patients had robust B cell recovery at month 3 who received comparable total B-cell and naive B-cell numbers in the grafts. The patients with or without rapid B-cell recovery showing a similar relapse rate (18.56% vs 15.34%, respectively). The 3-year probabilities of overall survival (OS) and disease-free survival (DFS) were 59%, respectively. Conclusion: Patients with rapid B-cell recovery are less likely to develop post-transplant GVHD, infections, and readmitted to hospital. The CD19+ B-cell counts may serve as useful predictor of GVHD-free survival, the 30-day readmission rate and quality-of-life after CBT. Disclosures No relevant conflicts of interest to declare.

Abstract

This retrospective study examined risk factors for cytomegalovirus (CMV) infection after umbilical cord blood transplantation (UCBT) and the impact of CMV infection on patient survival.In all 176 patients, plasma CMV DNA was negative prior to the transplantation, and examined twice a week for 100 d, and then once weekly for additional 300 d. Preemptive antiviral therapy (ganciclovir or foscarnet) was started in patients with1,000/mL copies of CMV DNA but no full-blown CMV disease, and was discontinued upon two consecutive negative reports of blood CMV DNA test. The survival and risk factors for CMV infection or disease were examined using logistic regression.CMV infection developed in 71% (125/176) of the patients, with a median onset of 32 d. Four patients (2.3%) developed CMV disease. Neither the 5-year overall survival (OS) nor event-free survival (EFS) differed significantly in infected patients vs. those with no infection (59.4% vs. 64.8%, P=0.194; 53.4% vs. 59.1%, P=0.226). A stepwise multivariate analysis indicated an association of CMV infection with age, high-dose glucocorticoids, the number of transplanted CD34(+) cells, and the number of platelet transfusion, but not with gender, the conditioning regimen, and the day of neutrophil recovery and chronic graft-versus-host disease (cGVHD).CMV infection is very common after UCBT, but does not seem to affect long-term survival with preemptive antiviral treatment.

Abstract

Background Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is an effective treatment for severe aplastic anemia (SAA). However, graft failure and graft-versus-host disease (GVHD) are major causes of the early morbidity in Allo-HSCT. Methods To reduce graft failure and GVHD, we treated fifteen patients with SAA using high- dose of HSCT with both G-CSF mobilized PB and BMSCs from HLA-identical siblings to treat patients with SAA. Results All patients had successful bone marrow engraftment. Only one patient had late rejection. Median time to ANC greater than 0.5 × 109/L and platelet counts greater than 20 × 109/L was 12 and 16.5 days, respectively. No acute GVHD was observed. The incidence of chronic GVHD was 6.67%. The total three-year probability of disease-free survival was 79.8%. Conclusion HSCT with both G-CSF mobilized PB and BMSCs is a promising approach for heavily transfused and/or allo-immunized patients with SAA.
