Abstract

A number of numerical experiments with a large-scale air pollution model (Unified Danish Eulerian Model, UNI-DEM) have been carried out to compute Sobol sensitivity measures. Sensitivity analysis of ozone concentrations according to variations of rates of chemical reactions is developed. The sensitivity study has been done for the areas of four European cities (Genova, Milan, Manchester, and Edinburgh) with different geographical locations. Sensitivity analysis of the output of UNI-DEM according to emission levels is also provided. The influence of emission levels on the concentrations of four important air pollutants (ammonia, ozone, ammonium sulphate and ammonium nitrate) over three European cities (Milan, Manchester, and Edinburgh) is considered. Variance-based methods are most often used for providing sensitivity analysis. Sobol variance-based approach for global sensitivity analysis has been applied to compute the corresponding sensitivity measures. Advanced stochastic methods based on modified Sobol sequences are used for computing the sensitivity measures. The numerical experiments show that the stochastic algorithms for the multidimensional integrals under consideration are one of the best stochastic techniques for computing the small in value sensitivity indices. Abstract

The present study is based on The Unified Danish Eulerian Model (UNI-DEM) as one of the most advanced large- scale mathematical models that describes adequately all physical and chemical processes. One of the most attractive features of UNI-DEM is its advanced chemical scheme the Condensed CBM IV, which consider a large number of chemical species and numerous reactions between them, of which the ozone is one of the most important pollutants for its central role in many practical applications of the results. The calculations are done in a large spatial domain, which covers completely the European region and the Mediterranean, for certain time period. A comprehensive experimental study of Monte Carlo algorithm based on modifications of the Latin Hypercube Sampling Edge and Random Algorithms for multidimensional numerical integration has been done. Samplings with different seeds has been analyzed. This comparison has been made for the first time for sensitivity analysis of UNI-DEM. The algorithms have been successfully applied to compute global Sobol sensitivity measures corresponding to the six chemical reactions rates and four different groups of pollutants. Abstract

In this paper we propose and analyse different stochastic methods for solving a class of integral equations, namely the second kind Fredholm integral equations. We study and compare different possible approaches to compute linear functionals of the integral under consideration. Error balancing of both stochastic and systematic errors has been discussed and applied during the numerical implementation of the algorithms. An almost optimal Monte Carlo algorithm for integral equations in a combination with the idea of balancing of both systematic and stochastic errors is analysed. Conclusions about the applicability and efficiency of the algorithms have been drawn. Meaningful numerical examples and experiments with experimental and theoretical relative errors are presented. It is shown that the balancing of errors reduce the computational complexity if the error is fixed. [ABSTRACT FROM AUTHOR]

Abstract

Recently Monte Carlo and quasi-Monte Carlo approaches have become a very attractive and necessary computational tools in finance. The field of computational finance is becoming more complicated with increasing number of applications. The pricing of options is a very important in financial markets today and especially difficult when the dimension of the problem goes higher. Monte Carlo and quasi Monte Carlo methods are appropriate for solving multidimensional problems, since their computational complexity increases polynomially, but not exponentially with the dimensionality. A comprehensive experimental study based on scrambling of the Sobol sequence is applied for the first time to evaluate European style options. The Sobol scrambling method is not only one of the best available algorithms for high dimensional integrals but also one of the few possible methods, because in this work we show that the deterministic algorithms need a huge amount of time for the evaluation of the multidimensional integral, as it was discussed in this paper. The numerical tests show that the method is efficient for multidimensional integration and especially for computing multidimensional integrals of a very high dimension. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: Tropheryma whipplei has been detected in 3.5% of the blood culture-negative cases of endocarditis in Spain. Experience in the management of T. whipplei endocarditis is limited. Here we report the long-term outcome of the treatment of previously reported patients who were diagnosed with infective endocarditis (IE) caused by T. whipplei from the Spanish Collaboration on Endocarditis-Grupo de Apoyo al Manejo de la Endocarditis Infecciosa en Espana (GAMES) and discuss potential options for antimicrobial therapy for IE caused by T. whipplei. Patients and methods: Seventeen patients with T. whipplei endocarditis were recruited between 2008 and 2014 in 25 Spanish hospitals. Patients were classified according to the therapeutic regimen: ceftriaxone and trimethoprim/sulfamethoxazole, doxycycline + hydroxychloroquine and other treatment options. Results: Follow-up data were obtained from 14 patients. The median follow-up was 46.5months. All patients completed the antibiotic treatment prescribed, with a median duration of 13months. Six patients were treated with ceftriaxone and trimethoprim/sulfamethoxazole (median duration 13months), four with doxycycline + hydroxychloroquine (median duration 13.8months) and four with other treatment options (median duration 22.3months). The follow-up after the end of the treatments was between 5 and 84months (median 24months). Conclusions: All treatment lines were effective and well tolerated. Therapeutic failures were not detected during the treatment. None of the patients died or experienced a relapse during the follow-up. Only six patients received antibiotic treatment in accordance with guidelines. These data suggest that shorter antimicrobial treatments could be effective.

Abstract

IntroductionInfective endocarditis (IE) still carries high morbidity and mortality and frequently requires surgery. The benefit of mitral valve repair (MVr) in the setting of IE is yet to be proven. The goal of this study was to assess the results of MVr in patients with IE after a minimum follow-up of 1year.MethodsThis study is based on a Spanish nationwide prospective registry that included patients operated on for native mitral valve IE. The collaborating Institutions pooled their pre-, peri-, and postoperative data into the database of the GAMES group [Grupo de Apoyo al Manejo de la EndocarditiS (Group for support and management of infective endocarditis)].ResultsData from 27 hospitals were recorded and 3524 cases of active IE identified between 2008 and 2016. There were 1513 cases of mitral IE, of which 898 involved native valves. Of these, 437 patients underwent surgical treatment, and 369 completed the 1-year follow-up. The valve was repaired in 68 cases (18.4%). Preoperative groups were comparable (EuroSCORE MVr 7.7 vs MVR 8.0; p=ns). Mortality in the repair group was inferior to that in the replacement group (16.2% vs 27.2%, p=0.058). At 1year, mortality remained higher in the replacement group: 3.7% vs 2.9%. Relapse of the infection was slightly more frequent in the repair group (7.1% vs 3.7%; p=ns), although this did not lead to higher rates of reintervention (MVr/MVR: 2.9% vs 4.9%).ConclusionMVr is an attractive option for specific patients with IE and does not seem to negatively impact on relapses.

Abstract

Background. Outpatient parenteral antibiotic treatment (OPAT) has proven efficacious for treating infective endocarditis (IE). However, the 2001 Infectious Diseases Society of America (IDSA) criteria for OPAT in IE are very restrictive. We aimed to compare the outcomes of OPAT with those of hospital-based antibiotic treatment (HBAT). Methods. Retrospective analysis of data from a multicenter, prospective cohort study of 2000 consecutive IE patients in 25 Spanish hospitals (2008-2012) was performed. Results. A total of 429 patients (21.5%) received OPAT, and only 21.7% fulfilled IDSA criteria. Males accounted for 70.5%, median age was 68 years (interquartile range [IQR], 56-76), and 57% had native-valve IE. The most frequent causal microorganisms were viridans group streptococci (18.6%), Staphylococcus aureus (15.6%), and coagulase-negative staphylococci (14.5%). Median length of antibiotic treatment was 42 days (IQR, 32-54), and 44% of patients underwent cardiac surgery. One-year mortality was 8% (42% for HBAT; P < .001), 1.4% of patients relapsed, and 10.9% were readmitted during the first 3 months after discharge (no significant differences compared with HBAT). Charlson score (odds ratio [OR], 1.21; 95% confidence interval [CI], 1.04-1.42; P = .01) and cardiac surgery (OR, 0.24; 95% CI, .09-.63; P = .04) were associated with 1-year mortality, whereas aortic valve involvement (OR, 0.47; 95% CI, .22-.98; P = .007) was the only predictor of 1-year readmission. Failing to fulfill IDSA criteria was not a risk factor for mortality or readmission. Conclusions. OPAT provided excellent results despite the use of broader criteria than those recommended by IDSA. OPAT criteria should therefore be expanded.

Abstract

Aim: To evaluate the effect of the type of surgical indication on mortality in infective endocarditis (IE) patients who are rejected for surgery. Methods and results: From January 2008 to December 2016, 2714 patients with definite left-sided IE were attended in the participating hospitals. One thousand six hundred and fifty-three patients (60.9%) presented surgical indications. Five hundred and thirty-eight patients (32.5%) presented surgical indications but received medical treatment alone. The indications for surgery in these patients were uncontrolled infection (366 patients, 68%), heart failure (168 patients, 31.3%) and prevention of embolism (148 patients, 27.6%). One hundred and thirty patients (24.2%) presented more than one indication. The mortality during hospital admission was 60% (323 patients). The in-hospital mortality of patients whose indication for surgery was heart failure, uncontrolled infection or risk of embolism was 75.6%, 61.4% and 54.7%, respectively (p < 0.001). Surgical indications due to heart failure (OR: 3.24; CI 95%: 1.99-5.9) or uncontrolled infection (OR: 1.83; CI 95%: 1.04-3.18) were independently associated with a fatal outcome during hospital admission. Mortality during the first year was 75.4%. The mortality during the first year in patients whose indication for surgery was heart failure, uncontrolled infection or risk of embolism was 85.9%, 76.7% and 72.7%, respectively (p = 0.016). Surgical indication due to heart failure (OR: 3.03; CI 95%: 1.53-5.98) were independently associated with fatal outcome during the first year. Bairakova Recently, for purposes of radiation safety, the validation of the linear non-threshold (LNT) hypothesis at low doses of radiation was actively discussed. New knowledge, especially on subcellular level, might contribute to the clarification of this problem. The aim of our study was to describe the patterns of spontaneous (rs_sp) and induced (rs_ind) leukocyte DNA repair in workers from the Nuclear Power Plant in Kozlodui (Bulgaria). An epidemiological cross-sectional survey was carried out. Retrospective information about demographic characteristics, health status and occupational exposure (including age at initial exposure, age at sampling, duration of exposure, radiation doses, etc.) was collected. The personnel gave informed consent to participate in the study and were assured of complete anonymity. Blood samples were taken from 342 workers (286 exposed workers in the 'strict regimen' area and 56 non-exposed NPP administration staff). The annual doses of irradiation in the exposed group were within the occupational safety limits. Both the basal and repair DNA syntheses (so called unscheduled DNA synthesis) were defined in short-term culture of whole blood (before and after in vitro 2 Gy I[sup.3]-irradiation). The incorporation of 3H-thymidine in the leukocyte population (in the presence or absence of oxyurea, respectively) was measured radiometrically. Data was analysed by methods of descriptive statistics, percentage distribution, ANOVA, non-parametric tests, parametric and non-parametric correlation and regression analysis. The results from the initial analyses indicated that probability distribution of most variables, although similar in exposed and control groups, was asymmetrical which required a procedure of normalisation of the parameters by log-transformation. Higher levels of spontaneous and induced DNA repair were found in the exposed than in the control group. Low, but significant positive correlation (p < 0.05) was found between the cumulative dose and spontaneous DNA repair patterns. At low levels of the cumulative dose (< 20 mSv) this disappeared and only positive associations between the age (at initial exposure and at sampling) and spontaneous DNA repair were found to be significant (p < 0.05). The present study substantiated the hypothesis that low doses of occupational ionising radiation might have elevated the DNA repair capacity of human leukocytes in the view of eventual subsequent exposure to radiation at higher doses. It could be suggested, therefore, that low levels of such occupational radiation might have stimulated the adaptive mechanisms of the complex system of DNA-damage control. However, the interpretation of our results in the sense of beneficial effects of low doses of ionising radiation (radiation hormesis) should be done with caution because it is not clear whether the cumulative errors of DNA repair might lead to malignant transformation.

Abstract

In this article, a non-chemical crosslinking method is used to produce pure protein microparticles with an innovative approach, so-called protein activation spontaneous and self-assembly (PASS). The fabrication of protein microparticles is based on the idea of using the internal disulfide bridges within protein molecules as molecular linkers to assemble protein molecules into a microparticle form. The assembly process is triggered by an activating reagent-dithiothreitol (DTT), which only involved in the intermediate step without being incorporated into the resulting protein microparticles. Conventional protein microparticle fabrication methods usually involve emulsification process and chemical crosslink reactions using amine reactive reagents such as glutaraldehdye or EDC/NHS. The resulting protein microparticles are usually having various size distributions. Most importantly crosslinking reactions using amine reactive reagents will result in producing protein microparticles with undesired properties such as auto-fluorescence and high toxicity. In contrast to the conventional methods, our technology provides a simple and robust method to produce highly homogeneous, stable and non-fluorescence pure protein microparticles under mild conditions at physiological pH and temperature. The protein microparticles are found to be biodegradable, non-toxic to MDCK cells and with preserved biological activities. Results on the cytotoxcity study and enzyme function demonstrate the potential applications of the protein microparticles in the area of pharmaceutics and analytical chemistry. © 2010 WILEY-VCH Verlag GmbH & Co.
