The aim of this work was to determine advanced the oxidative protein products (AOPPs), total antioxidant capacity (TAC), and myeloperoxidase activity (MPO) in the saliva of patients undergoing orthodontic treatment with clear removable aligners in comparison with another group in treatment with fixed passive self-ligating brackets applying light forces, before treatment, after 30 days, and after 90 days of treatment. This non-randomized clinical trial recruited patients consecutively, all of which were over 18 years of age and due to undergo orthodontic treatment. They were divided into two groups according to treatment type: Group A, 48 patients treated with clear aligners (Invisalign®); and Group B, 19 patients treated with Damon System® 0.22” self-ligating brackets applying light forces. Saliva samples were collected by a single clinician following the same protocol and underwent three analyses—AOPPs, TAC, and MPO levels–at baseline before placing the apparatus, after 30 days, and after 90 days treatment. Orthodontic treatment, whether with clear aligners or fixed self-ligating brackets and light forces, increased AOPPs after the first 30 days of treatment. During the initial phases of orthodontic treatment, neither clear aligners nor fixed self-ligating brackets applying light forces showed changes in TAC and MPO. Orthodontic treatment with both clear aligners and fixed apparatus self-ligating brackets applying light forces increases oxidative stress (AOPPs) after the first 30 days of treatment. There are no differences in AOPP levels between treatment with clear aligners and self-ligating brackets during the first 90 days of treatment. The antioxidative capacity of saliva during the initial phases of orthodontic treatment, whether with self-ligating brackets or clear aligners, does not undergo significant changes. With either orthodontic technique, the patients’ salivary antioxidant capacity is similar.

The aim was to evaluate sentinel node detection capacity by means of a magnetic probe in 11 patients with oral squamous cell carcinoma at stages T1-T2 received submucosal injections of a superparamagnetic iron oxide contrast agent (SPIO). A magnetic probe was used for sentinel node biopsy. The use of SPIO and magnetic probes in the early stages of oral cancer may offer an alternative to conventional radioisotope techniques and/or elective neck dissection.

Resumen Objetivo Nuestro objetivo fue analizar si existen diferencias en los niveles de estres oxidativo salival de pacientes con DM2 en comparacion con sujetos sanos no diabeticos, y si dicho estres oxidativo se puede asociar a la presencia de enfermedad periodontal en pacientes con diabetes. Material y metodos Se realizo un estudio observacional que incluyo 70 pacientes, estableciendose 3 grupos de estudio en funcion del control metabolico: 19 pacientes sin diabetes (grupo control); 24 pacientes DM2 con buen control metabolico (HbA1c 7%). En todos ellos se midieron los siguientes parametros de estres oxidativo salival: glutation peroxidasa (GPx), glutation reductasa (GRd), glutation reducido (GSH) y glutation oxidado (GSSG). El estado de salud periodontal se determino mediante el indice periodontal comunitario (CPI), recomendado por la OMS. Objective The aim of this study was to determine whether there are differences in salivary oxidative stress between patients with diabetes mellitus type 2 (DM2) and healthy non-diabetic patients, and whether this oxidative stress is associated with the presence of periodontal disease in diabetic patients. Material and methods This observational study included 70 patients divided into three groups according to metabolic control levels: 19 non-diabetic patients (control group); 24 patients with good metabolic control (HbA1c 7%). The following oxidative stress parameters were measured in all subjects: glutathione peroxidase (GPx), glutathione reductase (GRd), reduced glutathione (GSH) and oxidized glutathione (GSSG). BACKGROUND Over the years, many materials have been used in orbital reconstruction and cranioplasty. Among the materials in current use, polymethyl methacrylate (PMMA) offers a high degree of compatibility with human tissue. OBJECTIVE This work describes a new, custom-made, 3D printing-aided, prosthetic fabrication process for orbital and/or cranial reconstruction using PMMA. METHODS On the basis of information obtained from computerized tomography scanning processed in Digital Imaging and Communication in Medicine format and exported to treatment planning software, a 3D prosthesis master model was designed and digitally printed. This was then used to create an injection mold from which the prosthesis was cast in PMMA and implanted in the patient. Five patients with cranial and/or orbital defects of tumoral or traumatic etiology were treated by this method. After 5 to 7 years follow-up, no complications occurred to cause the removal of the prostheses. CONCLUSION This novel method makes it possible to produce customized PMMA prostheses to treat orbital and/or cranial defects that are cost-effective and individualized to each case.

BACKGROUND Osteomas are infrequent benign bone tumors of uncertain etiology. They are usually situated at extremities and their occurrence in the jaws is relatively rare. There are 3 types of osteoma: central, peripheral, and extraskeletal. Preoperative presumptive diagnosis is performed by means of radiography using orthopantomographs, computerized tomographs, or cone beam scans. Treatment is surgical and consists of careful exeresis and curettage of the adjacent tissue. In recent years, piezoelectric surgery has been used to treat bone pathologies in the maxillofacial area, orthopedic surgery, and neurosurgery. When used for the exeresis of a bone lesion, the device minimizes risks and eliminates possible complications arising from affectation of the vascular and nerve structures close to the lesion. OBJECTIVE This clinical report describes a peripheral mandibular osteoma situated in the left mandibular basilar which recurred 45 years after surgical treatment at another center. METHODS For resection, the surgical approach followed the scar remaining from the earlier surgery and was performed using piezoelectric surgery. Surgery did not cause hemorrhagic complications, affect the sensitivity of the left labial menton, or cause paralysis of the lower lip. CONCLUSIONS Based on the present patient, it may be concluded that mandibular osteoma treatment by means of piezoelectric surgery makes precise exeresis possible with less affectation of the important surrounding structures such as the inferior alveolar nerve and the marginal mandibular branch of the facial nerve.

Following the publication of our paper by Calvo Guirado et al, 2015 [1] in Clinical Oral Investigations, it came to light that a certain part of the text at the materials and methods and results sections were similar.

Objective: A wide range of surgical techniques are available for maxillary sinus augmentation. This review aimed to determine which techniques have achieved the highest success rates and so offer the greatest predictability. Materials and methods: A systematic literature review was performed using the PubMed, MEDLINE, and Scopus databases, identifying clinical trials that assessed different surgical techniques for maxillary sinus augmentation, and registered the success rates of subsequent implant placement. Results: A total of 40 articles described clinical studies involving different maxillary sinus augmentation procedures with follow-up periods of at least 6 months after dental implant placement. Implant success rates varied between 94% and 100% during the follow-up periods. Conclusion: A wide variety of clinical techniques are available for maxillary sinus augmentation; the choice of the technique will depend chiefly on the characteristics of the edentulous site, which will permit or prevent the placement of the implant at the moment of sinus augmentation surgery. Statins are considered the most effective drugs used in the treatment of dyslipidemias. Some of their adverse effects are related to muscle problems. Myalgias produced by statins appear more often during exercise. Mandibular advancement devices (MAD) force the propulsory and elevatory musculature of the mandible to exercise by making the jaw move forward. The aim of this study is to evaluate the incidence of muscular side effects (referred, spontaneous, or under palpation pain, myofascial pain, mandibular rigidity and fatigue, tension and sensitivity of the masticatory muscles) in a group of patients with a diagnosis of obstructive sleep apnea being treated with MAD. This was a prospective study, involving consecutively 104 patients with a diagnosis of OSAS, and who had begun treatment with a custom made oral device. Muscular side effects were collected by anamnesis (verbal request and questionnaires), psychological status and clinical assessment (manual muscle palpation in the masticatory and cervical muscle groups), before and during MAD treatment. Purpose: The aim of this study was to evaluate whether the drilling speed used during implant site preparation influences primary stability. Materials and Methods: Eighty tapered designed implants (3.8 × 10 mm) were inserted following osteotomies created in solid rigid polyurethane foam (simulating bone type II) and cellular rigid polyurethane foam (simulating bone type IV). Half were prepared using drilling speeds of 800 rpm (low speed), and the other half were prepared using speeds of 1,500 rpm (high speed). Following insertion, implant primary stability was measured using Periotest and Osstell (resonance frequency analysis [RFA]) devices. Results: Two-way analysis of variance (ANOVA) used for this study found that the drilling speed used to create the osteotomies appeared to have no significant impact on primary stability. Conclusion: The bone quality and not the osteotomy drilling speed seems to influence the implant primary stability.

Objective: This study assessed the bone formation around titanium surface incorporating the calcium–magnesium (CaMg) deposited by blasted in rabbit tibia bone to determine whether this surface would further enhance bone healing compared with commercially available implant surface. Material and methods: The deposition of CaMg on the titanium SLA surface were obtained by blasting formed the experimental group (EX group), while implants with traditional SLA surface were used as control group (CO group), in this study. Fifty cylindrical threaded implants with a length of 8 mm were used (P = 25 per group). Five implants of each group were used to surface characterization by scanning electron microscopy, energy dispersive spectrometry, and optical profilometry. Ten New Zealand white rabbits received 40 implants (n = 20 per group). Resonance frequency analysis was performed three times (0, 4, and 6 weeks). Histomorphometric analysis was performed 4 and 6 weeks after implantation. Statistical significance was set at P < 0.05. Results: Blasted CaMg deposition on SLA implant surface displayed almost identical surface morphologies and R(a) values at the micron scale. In comparing the implant stability quotient at the three time points, highly significant statistic differences were found (P < 0.001). Histomorphological analysis showed higher degrees of bone organization in the samples of test implant surfaces at both implantation times. Conclusion: Within the limitations of this study, the results indicate that the deposition of CaMg on the SLA titanium surface may be effective in enhancing the osseointegration of moderately rough grit-blasted implants by increasing the degree of bone–implant contact. The aim of this study is to assess the effectiveness of using non-titanium abutments for better estab- lishment of peri-implant biological width and to assess the stability of the soft tissue. Forty-eight tapered dental titanium implants with internal connection of 3.5 mm in diameter and 10 mm length were implanted in post extraction alveoli of 6 dogs. Twenty-four abutments made in a reinforced polyetheretherketone (PEEK) formed the test group, and 24 titanium abutments, the control group. The groups were randomized. Histological, histomorphometric, ISQ and radiological analyses were per- formed. Greatest differences (control group vs. test group) were found at PM-Lc (Mucosa to lingual bone contact) (2.91 ± 0.03 vs. 3.71 ± 0.18), and to PM Lingual-IS (2.65 ± 0.43 vs. 3.57 ± 0.38). Reinforced PEEK constitutes an effective alternative to conventional titanium abutments, given its high rate of biocom- patibility, preservation of bone height and soft tissue stability. Odontología

This study aimed at a histologic and histomorphometric analysis of the periimplant tissues and bone-titanium interface reactions in successfully osseointegrated, clinically stable, and immobile human titanium dental implants retrieved after a long loading period.In the last 30 months (2013-2015), 21 implants, retrieved from individuals for different causes after a loading period ranging from 4 to 20 years, were sent to the Implant Retrieval Center, University of Chieti-Pescara, Italy. In 4 cases, almost all the bone had been lost during the retrieval, whereas in the remaining 17 cases, mineralized bone was still present.Around the majority of the implants, mature compact bone with few marrow spaces was found. The trabecular bone, constituted by a few thin bone trabeculae, was in a periimplant location around only a few implants. A high percentage of bone-implant contact (BIC) (comprised between 32 ± 4.1% and 83 ± 2.9%) was present. In almost all implants, the space within the threads was almost completely filled by compact lamellar bone or by a thin layer of bone. Close and tight contact between bone and implant surface was observed in all specimens with no gaps or connective tissue at the interface.All implants appeared to be well integrated in the surrounding mineralized bone, and all of them showed adequate bone-to-implant contact percentages.

Objectives To compare the use of sonic powered or manual toothbrush in patients with intellectual disability (ID) in terms of plaque (PlI) and gingival (GI) indices and adverse effects. Material and Methods Subjects with ID were recruited for this cluster-randomized, single blinded (examiner), 6-month clinical trial, comparing powered versus manual toothbrushing. Outcome variables included PlI and GI, evaluated at baseline and 3 months after supervised toothbrushing and after 3 additional months of unsupervised used. Clinical outcome variables were analysed by repeated measures ANCOVA considering time and group as factors and respective baseline values as covariates (generalized linear model). Objectives: This study set out to make antibiograms of positive bacteria cultures in samples collected when performing maxillary sinus elevations to determine a specific and effective antibiotic in each case. Material and methods: A total of 174 patients (90 women and 84 men) with a mean age of 55.92 years underwent 227 sinus elevations. As the membrane was lifted, a sample was collected from the maxillary sinus floor with a cotton swab and placed on a blood agar and chocolate agar culture to incubate for 48 h at 37°; the samples then underwent microbiological analysis. Antibiograms were made for each positive culture to identify the most sensitive antibiotic, which were regrouped according to their mechanism of action as: beta-lactam (penicillins), beta-lactam (cephalosporins), macrolides, quinolones, fosfomycin, aminoglycosides, or trimethoprim-sulfamethoxazole. Results: Of 227 cultures, 18.1% were bacteria-positive. Of the germs, 45% were of the Streptococcus genus, most of which belonged to the Streptococcus viridans group (61.1%). The germs studied showed greater resistance to macrolides and greater sensitivity to penicillins, cephalosporins, and ciprofloxacin. The antibiotics that showed the greatest efficacy were as follows: ampicillin, amoxicillin-clavulanate, and ciprofloxacin. Conclusions: On the basis of antibiograms of positive cultures, the antibiotics presenting the greatest efficacy against possible complications were as follows: ampicillin, amoxicillin-clavulanate, and ciprofloxacin. Clinically, the antibiograms proved useful as they allowed the prescription of specific antibiotics to resolve possible postoperative sinus infections. Background: Periodontal diseases (PD), including periodontitis, are chronic inflammatory pathologies caused by bacteria in the subgingival biofilm which affect the periodontal tissues. PD is now considered a localized, chronic, oral infection that activates the host immuno-inflammatory responses both locally and systemically, and also constitutes a source of bacteraemia. It is a known fact that periodontal diseases exercise an important influence on the pathogenesis of numerous systemic diseases, including diabetes mellitus (DM). In the mid-nineties, sufficient scientific evidence emerged to confirm an association between DM and periodontitis, which then began to be regarded as the sixth complication of DM. Current scientific evidence points to a two-way relationship between DM and periodontal disease, whereby DM is associated with an increase in the incidence and progression of periodontitis, while periodontal infection is associated with worsening glycemic control in diabetic patients. This two-way relationship points to a need to promote oral health in DM patients, and to implement a joint management protocol between endocrinologist and dentist that aims to create adequate conditions for early diagnosis and the effective treatment of both diseases.

This study reports the characterization process and in vivo application of a new dicalcium silicate cement consisting of α ́H-dicalcium silicate doped phosphorous in solid solution within the Ca3(PO4)2–Ca2SiO4 system. Workable α ́H- Ca2SiO4 solid solution (α ́-C2Sss) paste with a liquid- to-powder (L/P) ratio of 0.35 was inserted into the critical size defect of 15 rabbits, using both tibiae, and examined at 60 days. Material was tested for their bone repairing capacity and osteoinductive potential in a New Zealand Rabbit model. α ́-C2Sss pastes with L/P ratio of 0.35 and 0.37 could be injected for 18–52 min (nozzle diameter 2.0 mm) and enabled initial setting times of 25–120 min. The compressive strength of the α ́-C2Sss ranged from 2.4 to 10.8 MPa. Histomorphometric analysis revealed that the material (66.4370.29) produced higher values of bone-to- implant contact (BIC) percentages (higher quality, closer contact); moreover, defect closure was significantly different (Po0.05) in relation with control group (64.1573.52). This combination of properties indicates that the novel α ́-Ca2SiO4ss cement might be suitable for potential applications in the biomedical field, preferentially as materials for bone/dental repair. It is estimated that over 190 million bisphosphonates have been prescribed worldwide. But this drug can produce adverse effects, of which osteonecrosis of the jaw and severe hypocalcemia are the most serious. It is evident that bisphosphonate administration affects multiple and diverse biochemical mediators related to bone metabolism. This review of literature investigates four basic parameters in patients treated with bisphosphonates - parathyroid hormone (PTH), bisphosphonates, vitamin D, calcium, and jaw osteonecrosis - which are fundamental for assessing bone metabolism and so the efficacy and correct use of the drug. The imbalances generated by vitamin D and calcium deficiencies, together with their multiple systemic repercussions, have been widely researched but the outcomes of these imbalances in relation to bisphosphonate administration are not well known, and some research has indicated that they may be associated with osteonecrosis of the jaw (ONJ). The present review set out to explain the functioning of bone metabolism, the importance of different chemical mediators, the imbalances produced by incorrect use of this drug, in order to forewarn against the possible relation of these parameters with ONJ, whose physiopathology remains unknown. Medical and dental clinics should keep detailed anamneses of the use of vitamin D and calcium supplements, as it is of vital importance to maintain their correct levels in blood, given that these are related to ONJ as well as other adverse effects; this procedure is also necessary in order to ensure the correct use of the drug. Materials and methods: Two defects were performed in each tibiae for a total of 180 defects: Group I HA granules (2000–4000 lm), Group II HA granules (1000–2000 lm), Group III HA granules (600–1000 lm), Group IV Ossceram" nano (Bredent medical GmbH & Co. KG, Senden, Germany), Group V 4Bone" granules (MIS Implants Technologies Ltd, Shlomi, Israel), and Group VI: empty defect acted as control. Comparison was performed by mean of material characterization, SEM, and EDX. Results: 4Bone: Pores between 300 lm and 100 lm with intra- and interparticle spaces. Ossceram show also interparticle and intraparticle pores, between 100 lm and 26 lm, but the proportion of interparticles in the 4Bone is clearly minor than in the Ossceram. Related with pure HA, Group III has the greatest porosity (69.3%). For Group I, the intraparticle pores (0.71 lm) are about 8 times larger than those of the other two samples. The analysis of the images of in vivo SEM shown as biphasic groups has presented a more gradual resorption of the material. Conclusion: HA-based biomaterials, both pure and biphasic, are an effective means for bone regeneration processes; of these, materials with higher initial and secondary porosimetry allow greater cell colonization and therefore more effective substitution by new bone. The two-phase materials have a higher ion release to the environment in the early stages and thus allow greater colonization by collagen fibers that can be matured into new bone. Odontología

In relation to the article with reference “Carreno-Carreno J, Gomez-Moreno G, Aguilar-Salvatierra A, Menendez-Lopez-Mateos C, Piattelli A, Menendez-Nunez M. Effect of smoking habit on crestal bone height in maxillary sinus augmentation with bone grafts. Med Oral Patol Oral Cir Bucal. (2018), doi:10.4317/medoral.22439”, published in Oral Medicine Oral Pathology and Oral Surgery in the modality "AHEAD OF PRINT - article in press", the authors mention that "The study was conducted according to the STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) Statement". However, and based on the opinion of several expert reviewers who have re-evaluated the article, it is considered that although "there has not been plagiarism", these STROBE criteria are not adequately met. In addition, the mentioned article is very similar to two articles that have already been published by the same authors in Clinical Oral Implants Research. The sample is the same; many data are repeated; and the only difference is the inclusion of a variable (smoking), which does not exert a significant effect. This outcome therefore would not justify publication of the paper in the journal. Retraction of the article is therefore decided.

BACKGROUND Reconstruction of an oromandibular defect presents a considerable surgical challenge. But since the advent of microvascularized free flap reconstructive surgery, outcomes have improved significantly so that today almost any defect may be reconstructed. Recently, virtual surgical planning has reduced surgical time, the morbidity associated with surgery, and surgical precision. OBJECTIVE This article reports a complex patient with a composite oromandibular defect. METHODS The patient had undergone multiple surgeries with bad results. She presented an orostoma of 5 cm in diameter, osteoradionecrosis and fracture of the left mandibular body, and relapse in the ipsilateral posterior maxilla. Reconstruction was performed by means of a scapular/parascapular chimeric free flap. A customized reconstruction plate was designed virtually based on the mirror image of the contralateral mandibular body. Surgery and the postoperative phase passed without incident. The patient recovered vocal, swallowing, and respiratory function without any problems. At the end of a 1-year follow-up, no complications or tumor relapse had occurred. CONCLUSIONS Composite defects are a surgical challenge in which free flap reconstruction is an evolution that has facilitated treatment and led to improved outcomes. Virtual treatment planning produces more exact results and greater control of surgical techniques.

To date, no evidence exists in the literature as to the effects of inhaled corticosteroids (ICs) on salivary composition in patients with bronchial asthma. The aim of this study was to assess the effect of ICs on salival composition. Adult patients attending an outpatient respiratory clinic who were classified into two groups (controls and patients with bronchial asthma receiving ICs), were recruited in this cross-sectional study. For each participant, data of clinical records, baseline history of asthma, and regular IC dose were recorded. A sample of stimulated saliva was collected and processed for investigation of mucin 5B (MUC5B), lipoxygenase (LPO), total antioxidant capacity, and 8-hydroxydeoxyguanosine (8-OHdG) levels. Overall, 103 patients (49 controls and 54 patients receiving regular treatment with ICs) were recruited. No differences in comorbidities or smoking habits were observed. Patients treated with high-doses of ICs showed lower levels of salival MUC5B compared with those treated with medium IC doses or those not treated with ICs (1.60 vs. 2.20 vs. 2.53 ng/mL; p = 0.042). In patients with asthma, treatment with high-doses of ICs is associated with reduced levels of salivary MUC5B. This effect can explain some of the effects of ICs on oral health.

Objectives The aim of this study was to evaluate the incidence of bleeding complications after dental implant placement in patients in treatment by the anticoagulant oral rivaroxaban without interrupting its administration or modifying dosage. Materials and methods About 57 patients were divided into two groups: 18 had been in treatment by rivaroxaban for over 6 month before implant surgery and a control group consisted of 39 healthy subjects. All subjects received dental implants in different positions, without interrupting or modifying rivaroxaban dosage. Patients were treated in an outpatient setting. Non-absorbable sutures were used, and all patients were given gauze impregnated with tranexamic acid 5%, to bite on for 30–60 min. Results One rivaroxaban patient presented moderate bleeding the day after surgery, and two control patients presented moderate bleeding the day after and on the second day. Bleeding was managed with gauzes impregnated with tranexamic acid. No statistically significant differences (P = 0.688) were found in relation to bleeding episodes between the groups, with a relative risk = 0.919 based on the pooled groups and 95% confidence interval of 0.078–10.844. Conclusions Dental implant surgery in patients taking the anticoagulant oral rivaroxaban can be performed safely in outpatients departments applying local hemostatic measures without the need to modify or interrupt anticoagulant medication.

Purpose: This experimental study was designed to analyze the effect of different compressive forces on the bone regeneration around a particulate bone graft material. Material and methods: Eighty 6-mm-diameter defects were created in the calvaria of 20 New Zealand rabbits (4 defects per rabbit calvaria). All the defects were filled with particles of synthetic bone. Two standardized compressive forces were then applied, 4.1 g to half the defects (Test A) and 8.2 g to the other half (Test B), all for 1 min. The graft sites were allowed to heal for 6 weeks, after which the rabbits were euthanized. The calvarium vault of each animal was extracted, radiographed, and prepared for histomorphometric analysis. The percentage of defect fill, bone density, new bone formation, and residual bone graft material were recorded, and the results were subjected to statistical analysis. Results: Histological evaluation found that defect closure among the Test A (lower compression) group ranged from 38.34 (95% lower CI) to 55.8 (95% upper CI) (mean 47 8.5%), while among the Test B group (higher compression), it ranged from 81.26 (95% lower CI) to 95.32 (mean 88 7.3%). Significantly more closure was achieved for the Test B group (P < 0.05). Histomorphometric comparison of the two groups found significantly more new bone formation, higher bone density, and a higher percentage of defect fill in the defects subjected to the higher compression level (P < 0.05). Materials and methods: This experimental in vitro study used 62 zirconia implants and 130 zirconia disks. Two experimental groups were created for the implants: 31 non-microgrooved implants (Control) and 31 microgrooved implants (Test); two experimental groups were created for the disks: 65 non-microgrooved disks (Control) and 65 microgrooved disks (Test). The following evaluations of the implants were made: lateral surface area (LSA), cell morphology, and density of human fetal osteoblasts seeded on implant surfaces. On the disks, surface parameters (roughness and chemistry) and cell activity (alkaline phosphatase – ALP and alizarin red – ALZ) were evaluated at 7 and 15 days. Results: LSA was lower for control implants (62.8 mm) compared with test implants (128.74 mm) (P < 0.05). Cell bodies on control surfaces were flattened and disorganized, while in the test group, they were aligned inside the microgrooves. Control group cells showed few lamellipodia, which were attached mainly inside topographical accidents (surface cracks, valleys, and pits). Test group implants presented cells rich in lamellipodia prolongations, attached to the inner walls or to the borders of the microgrooves and in the flat areas between the microgrooves. Cell density was higher in the test group compared with controls (P < 0.05) Surface roughness and oxygen content increased in test disks samples compared with controls (P < 0.05). Carbon and aluminum were reduced in disks test samples compared with controls (P < 0.05), and ALP and ALZ levels were significantly increased on test surfaces (P < 0.05) at both study times. It is characterized by the presence of sensorimotor polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, skin changes, and other systemic manifestations. The pathogenesis of the syndrome is unknown but over-production of vascular endothelial growth factor is probably responsible for most of the more characteristic symptoms. There is no standard treatment for POEMS syndrome and no randomized controlled clinical trials of treatment exist in the available literature. High-dose melphalan with autologous hematopoietic stem cell transplantation should be considered for younger patients with widespread osteosclerotic lesions, and for patients with rapidly progressive neuropathy. Case Report: This is the case of a 62-year-old Caucasian man who was admitted to our center presenting pretibial edema accompanied by significant weight loss and difficulty walking. POEMS criteria were present and an immunofixation test confirmed the presence of a monoclonal plasmaproliferative disorder. After autologous hematopoietic stem cell transplantation, the monoclonal component disappeared and the patient’s clinical status improved markedly. Conclusions: Autologous hematopoietic stem cell transplantation following high-dose melphalan is an effective therapy for younger patients with widespread osteosclerotic lesions in POEMS syndrome.

Due to its antioxidant properties and its ability to detoxify free radicals, melatonin may interfere in the function of osteoclasts and thereby inhibit bone resorption. This inhibition of bone resorption may be enhanced by a reaction of indolamine in osteoclastogenesis and this may contribute to certain benefits in implantology.This systematic literature review on the use of melatonin in implant dentistry aims to provide guidelines for clinicians.PubMed, Science Direct, ISI Web of Knowledge, and the Cochrane base databases were used to identify articles published between 1999 and 2013 on melatonin use in implant dentistry. Ten articles were selected consisting of 9 animal research studies and 1 review article, involving 60 Beagle dogs, 57 rats, and 30 rabbits and a total of 352 implants.Melatonin, which is released into the saliva, has important implications in the oral cavity. To achieve dental implant stability, osseointegration involves a cascade of protein and cell apposition, vascular invasion, bone formation, and maturation. This process may be accelerated by local delivery of growth-promoting factors, as occurs with the topical application of melatonin over the implant surface.The experimental evidence suggests that topical applications of melatonin may be useful in oral surgery and implant dentistry, increasing bone-to-implant contact values and new bone formation, and so improving the success and long-term survival of implant treatments.

Liposarcoma is considered one of the most frequently occurring tumors of the soft tissues, representing 17% to 30% of all mesenchymal cell tumors. It is less common in the head and neck representing10% of tumors in this region. The reconstruction of defects derived from resection of these tumors presents a surgical challenge. New three-dimensional digital technologies allow more exact surgery, cause less morbidity, and achieve adequate aesthetic outcomes.The aim of this article was to describe the complex reconstruction of a defect caused by the resection of a liposarcoma in the temporal region.Three-dimensional technology allowed patient planning and a reconstruction that was as exact as possible. A made-to-measure polymethyl methacrylate prosthesis was used to correct the defect in the zygomatic arch. The temporal fossa was covered with a standard porous polyethylene prosthesis.Satisfactory esthetic and functional results were achieved using three-dimensional digital technology for treatment planning and to fabricate a made-to-measure polyethylene prosthesis and surgical guide.

Background The aim is to evaluate periodontal alteration and biochemical markers associated with bone turnover in chronic oral with dicoumarins anticoagulant treatment patients. Material and Methods 80 patients treated with oral anticoagulants were divided into 2 cohort: Group A (n=36) 6 month to 1 year with anticoagulant treatment and Group B (n=44) > 2 years with anticoagulant treatment. Clinical evaluation included: Clinical attachment level (CAL), plaque index (PI) and gingival index (GI). Analytically biochemical parameters of bone remodeling (calcium and phosphorus), formation (total acid phosphatase, alkaline phosphatase and osteocalcin) and resorption (tartrate-resistant acid phosphatase and beta-crosslaps) were evaluated. Results High values of PI (67-100%) especially in men and in Group B were observed. Men with anticoagulation treatment length showed an increased GI (49.167 vs 78.083) while Group B women showed a decreased GI in comparison with Group A (59.389 vs 42.120). Women presented a greater average CAL than men as well as Group B vs Group A but without statistical significance. All biochemical markers were decreased respect to values of general population. Osteocalcin in GroupB women showed a statistically significant outcome vs GroupA (p=0.004). Acid phosphatase (total and tartrate-resistant) has a slight increase in Group B women versus Group A, and Beta-crosslap showed lower values in Group A men than Group B and slightly lower in Group A women versus Group B, without statistical significance. Conclusions Patients showed a slight to moderate degree of periodontal affectation, especially gingivitis related to bacterial plaque. Periodontal disorders tended to be more severe in Group B. While bone remodeling showed an overall decrease with greater affectation of bone neoformation phenomena, bone destruction tended to recover and normalize in time. The objective of this study was to evaluate the osseointegration and crestal bone loss (CBL) in two implant designs with different diameters (Mini Sky® and Narrow Sky®) implants, placed at different vertical levels at healed canine ridges. The second, third, and fourth mandibular premolars of six Beagle dogs were extracted bilaterally. After 2 months healing, four implants divided into two groups according to their diameters (i.e., Narrow Sky® and Mini Sky®) were placed in each hemi-mandible at the level of the bone crest or 2 mm subcrestally. The animals were euthanized at 12 weeks and undecalcified samples were processed for histology. Histomorphometric analysis was carried out to compare bone-to-implant contacts (BIC) and crestal bone loss (CBL). Few studies have assessed the relationships between xerostomia and the use of inhaled corticosteroids (ICS). The main objective of this study was to investigate the prevalence of xerostomia in a respiratory outpatient clinic and its relationship with bronchial asthma and ICS use. A cross-sectional observational study of patients recruited in an outpatient setting divided them according to previous diagnoses of bronchial asthma. Data about pulmonary function, concomitant medication, medical comorbidities, Xerostomia Inventory test (XI test), and the degree of asthma control by ACT (asthma control test) were collected for each patient. A linear regression model was applied, using the XI score as dependent variable and the ACT score as independent variable. The 57 patients were divided into asthmatics (40 patients, 70.2%) and control group without asthma (17, 29.8%). The prevalence of xerostomia was 87.7% (50 patients), with no differences between the study groups or current dose of ICS. In the asthmatic group, patients with uncontrolled asthma had worse XI scores than those with partially or totally controlled asthma (30.43 ± 8.71 vs. 24.92 ± 8.08; P < 0.05). In a logistic regression model, the XI test was significantly associated to ACT scores with a moderately strong correlation (r = 0.55; P = 0.005) after adjusting for the current daily dose of ICS. Xerostomia is a common symptom in the ambulatory setting. There is a moderate relationship between the degree of asthma control and the severity of xerostomia.

Objectives This study evaluated the effect of implant macrodesign and position, related to the bone crest, on bone-to-implant contact (BIC) and crestal bone (CB) in immediate implants. Material and methods The study comprised of six foxhound dogs in which 48 immediate implants were placed. Three types of implants from the same manufacturer with similar surface characteristics but different macrodesigns were randomly placed: Group A (external hex with no collar microthreads), Group B (internal hex and collar microrings), and Group C (internal conical connection and collar microrings). Half of the implants were placed leveled with the bone crest (control) and the remaining, 2 mm subcrestally (test). Block sections were obtained after 12 weeks and processed for mineralized ground sectioning. Statistical analysis consisted of nonparametric Friedman and Wilcoxon test. Results All implants were clinically stable and histologically osseointegrated. Mean BIC percentage within the control group was as follows: A: 42.52 ± 8.67, B: 35.19 ± 18.12, and C: 47.46 ± 11.50. Within the test group: A: 47.33 ± 5.23, B: 48.38 ± 11.63, and C: 54.88 ± 11.73. Differences between each subgroup in the test and the control groups were statistically significant. BIC was statistically significantly higher in the test (50.588 ± 8.663) than in the control (43.317 ± 9.851) group. Within both groups, differences between group C and the other 2 were statistically significant. Distance from the implant shoulder to the buccal CB was statistically significantly larger in the control than in the test group and between subgroups B and C in the control and test groups. Within the test groups, relative bone gain was noticed. Conclusions Subcrestal immediate implant positioning may lead to a relatively reduced CB resorption and increased BIC. Implants macrodesign with crestal microrings may enhance BIC in post-extraction implants.

Objective This study reports the characterization process and in vivo application of a new high-porosity biphasic calcium phosphate (4Bone® – HA 60%/β-TCP 40%) inserted into the critical size defect of a rabbit tibiae. The aim of this article is to compare the influence of surface treatment on the integration (at 2, 4 and 8 weeks) of 120 dental implants inserted in 60 tibiae of rabbits. Four different surfaces were double-blind tested: blasted, acid-etched and discrete crystal deposition (DCD) (group A); blasted (group B); acid-etched (group C) and blasted and acid-etched (group D). Bone-to-implant contact plus reverse torque and bone level were measured at the time of implant insertion and at 14, 28 and 56 days of healing. Group A showed the highest early and late bone-to-implant contact (BIC) values: 40.8 ± 2.3 % at 14 days decreasing to 27.7 ± 1.1 % after 28 days and 39.4 ± 1.4 % at 56 days. For group B, the average BIC values at 14, 28 and 56 days were 23.34 ± 2.1, 23.77 ± 1.9 and 29.47 ± 1.7 %, respectively. Group C showed a value of 25.72 ± 2.3 % after 14 days of integration, 34.92 ± 2.2 % at 28 days and 32.91 ± 1.6 % at 56 days. Group D showed a BIC value of 32 ± 2.5 % at 14 days, 32.85 ± 1.4 % at 28 days and 34.04 ± 2.3 % at 56 days. In the scanning electron microscopy (SEM) analysis, no statistically significant differences were found. The Ca/P ratio values were 1.762 for surface A, 1.625 for surface B, 1.663 for surface C and finally 1.722 for surface D. Therefore, we conclude that even if there seems to be a tendency to obtain better BIC results with surface A (blasted-etched and covered with hydroxyapatite (HA)), no statistical differences were obtained in this study. The study shows the influence of different implant surfaces in increasing osseointegation for immediate loading implants.

The aim of this study was to analyze the changes produced in peri-implant tissues in type 2 diabetes mellitus patients with different glycemia levels, measured by monitoring glycated hemoglobin A1c (HbA1c), over a period of 3 years following dental implant placement.Sixty-seven patients were divided into four groups according to their HbA1c levels: 21 patients in Group 1 (6); 24 patients in Group 2 (6.1-8); and 11 patients in Group 3 (8.1-10) and Group 4 (10.1). Each patient received one implant. All implants were placed in the anterior zone of the maxilla. The variables selected to assess the general state of patients' peri-implant health were as follows: probing depth, bleeding on probing, and marginal bone loss.Marginal bone loss was found to increase in relation to increases in HbA1c levels. Bleeding on probing showed statistically significant differences between groups. When the peri-implant area was probed, mean levels of bleeding varied from 0.43 in Group 1 at 1 year after implant surgery to 0.62 in Group 4 (P = 0.042 between the four study groups). After the second year, greater bleeding on probing was observed in Group 4 (0.63) in comparison with groups with lower HbA1c levels. Peri-implant pocket depths presented values that were too low to be considered pathological and without statistically significant differences between the study groups.Implant therapies for diabetic patients can be predictable, providing these patients fall within controlled ranges of glycemia over time, assessed by monitoring HbA1c levels.

Journal Article; Bisphosphonates (BP) are a type of drug known to inhibit bone resorption through complex interventions. Their primary mechanism of action is aimed at the cellular level, inhibiting osteoclast activity and so bone resorption. BPs are widely used, with many patients receiving continuous treatment for years. But it is well known that these drugs can produce osteonecrosis of the jaw (ONJ). Zoledronic acid (ZA) is an intravenous BP used in the treatment and prophylaxis of bone disease in patients with malignant tumors with bone implication. ZA is the most potent BP in clinical development. This report describes the case of a 62-year-old woman with breast cancer antecedents which relapsed, who had received a maxillary dental implant two years before the start of therapy with zoledronic acid. She later developed osteonecrosis of the jaw (ONJ), which began in the peri-implant area, and was treated for stage 3 ONJ by sub-total maxillectomy. Purpose This study applied femtosecond laser technology to zirconia dental implants (Bredent GmbH & Co.KG, Senden, Germany) to generate a surface texture of microgrooves over the entire intraosseous surface, analyzing its behavior in an in vivo model in comparison with titanium implants with sandblasted and acid-etched surfaces. Materials and Methods The study used six American Fox Hound dogs. Each received four implants per hemi-mandible, making a total of eight implants per animal. The 48 immediate loaded implants were divided into two groups of 24 titanium implants (control group) and 24 zirconia implants (study/test group), distributed randomly. Bone-to-implant contact (BIC) values and crestal resorption were determined at 1 and 3 months, also measuring calcium, phosphorous, and carbon concentrations by means of energy dispersive x-ray. Results BIC percentages after 30 days were 51.36% for titanium implants and 44.68% for zirconia implants. After 90 days, values increased to 61.73% in titanium and 47.94% in zirconia implants. After 30 days, there was more crestal bone lost in the titanium group (0.77 mm) compared with the zirconia group (0.01 mm). After 90 days, zirconia implants showed greater marginal bone resorption (1.25 mm) compared with the titanium group (0.37 mm). Conclusions The present study shows that zirconia implants with modified surfaces can produce good osseointegration values when compared with titanium implants in terms of BIC and crestal bone resorption at 1 and 3 months.

Purpose To describe contact, thickness, density, and orientation of connective tissue fibers around healing abutments of different geometries by means of a new method using coordinates. Materials and Methods Following the bilateral extraction of mandibular premolars (P2, P3, and P4) from six fox hound dogs and a 2-month healing period, 36 titanium implants were inserted, onto which two groups of healing abutments of different geometry were screwed: Group A (concave abutments) and Group B (wider healing abutment). After 3 months the animals were sacrificed and samples extracted containing each implant and surrounding soft and hard tissues. Histological analysis was performed without decalcifying the samples by means of circularly polarized light under optical microscope and a system of vertical and horizontal coordinates across all the connective tissue in an area delimited by the implant/abutment, epithelium, and bone tissue. Results In no case had the connective tissue formed a connection to the healing abutment/implant in the internal zone; a space of 35 ± 10 μm separated the connective tissue fibers from the healing abutment surface. Objective The aim of this study was to assess the performance of zirconia implants under immediate loading compared with delayed loading in dogs over 30–90 days. Materials and Methods Tooth extractions were made for the subsequent placement of dental implants at premolars P2, P3, P4 and molars M1 bilaterally in the dog mandible. Forty eight zirconia implants, 4 mm in diameter and 10 mm in length, were inserted, whose neck and body surfaces had been treated with femtosecond laser texturing; 24 implants were loaded immediately and 24 remained unloaded during the osseointegration period. Final radiographs were taken recording stability values before sacrifice at 30 or 90 days when sample removal for histological and histomorphometric analysis was performed. Bone-to-implant contact, crestal bone loss and implant stability were evaluated at these two study times. Results Bone-to-implant contact values after 30 days were 38.9% for immediately loaded implants and 32% for non-loaded implants. After 90 days, values increased to 65% for immediately loaded and 57.6% for non-loaded implants. After 30 days, there was more crestal bone lost in the non-loaded group (0.58 ± 0.28 mm) compared with the immediately loaded group (0.5 ± 0.3 mm). After 90 days, there was improved stability in both groups, but was lower in the immediately loaded group (0.5 ± 0.23 mm) compared with the non-loaded group (0.56 ± 0.28 mm). Initial implant stability values were −3.5 PTV at the moment of implant placement. Stability values after the first 30 days were −4 for immediately loaded implants and −3 for non-loaded. At 90 days, non-loaded implants showed −4.9, whereas immediately loaded showed −7.1. Conclusions For the different parameters studied, zirconia implants treated with femtosecond laser and subjected to immediate loading showed better results compared with non-loaded implants in terms of BIC, crestal bone loss and implant stability.

Objectives The aim of the present study was to evaluate bone-to-implant contact (BIC) and bone remodeling dynamics after immediate implant placement at different levels in relation to the crestal bone. Materials and methods The mandibular second, third, and fourth premolars of six Foxhound dogs were extracted bilaterally. Randomly, three implants were immediately placed in the hemi-arches of each dog, crestally or 2 mm subcrestally. Three dogs were allowed an 8-week submerged healing period, and the other three at 12-week submerged healing period. The animals were sacrificed at 12 weeks and samples were obtained. Biopsies were processed for ground sectioning. Histomorphometric analysis was carried out to compare BIC, bone neoformation, and bone remodeling. Results All implants osseointegrated clinically and histologically. Healing patterns examined microscopically at eight and twelve weeks for both groups (crestal and subcrestal) yielded similar qualitative bone findings. The Total BIC mean value for the crestal group 44.52% at 8 weeks and 39.50% at 12 weeks, and for the subcrestal group was 47.33% at 8 weeks and 53.85% at 12 weeks%. There was less bone resorption in the subcrestal group (test) than in crestal group (control). Conclusions Within the limitations of this study, our findings suggest that apical positioning of the top of the implant does not jeopardize bone crest and peri-implant tissue remodeling. However, less resorption of the lingual crest may be expected when implants are placed 2 mm subcrestally. Moreover, higher BIC values were found in implants placed subcrestally.

Objectives: The aim of this study is to evaluate the effect of the surface modification and cervical implant design on the bone remo- deling in implants installed at the crestal and subcrestal bone level. Methods: Ten American Fox Hound of approximately 1 year of age, each weighing approximately 14 to 15 kg, were used for this study. Two different dental implant macrodesign were used: cylindric- conical with 3.5mm of diameter and 9 in length (implant A) and conical with 2.9 mm of diameter and 9 mm in length (implant B). Two surfaces were used: sandblasting and acid etching (surface 1) and sandblasting and acid etching, then maintained in an isotonic solution of 0.9% sodium chloride (surface 2). Four groups were performed (n 1⁄4 20 implants): Group A1 (implant A with the surface 1), Group A2 (implant A with surface 2), Group B1 (implant B with surface 1), and Group B2 (implant B with surface 2). The mandibular premolars and molars (P1, P2, P3, M1) were removed and, after 2 months of healing, implants were inserted at the crestal and 2 mm subcrestal position related to the buccal bone level. Analysis was performed at 4 and 8 weeks. Histomorphometry with longitudinal measurements and bone implant contact, bone remodeling and implant stability quotient analysis were realized. Results: The surface 2 showed to get more close contact between implant and new bone formed after implant placement and more stability surrounding platform both at 4 and 8 weeks. Surface 2 groups and subrestally placed showed to have better results in terms of linear measurements, with less bone loss and soft tissue distance to the IS. The data showed significant differences among the groups (P < 0.001). Conclusions: Surface modification (surface 2) has shown to be an effective alternative to conventional surface with better results in situations placed subcrestally and combined with implant design. Key Clinical Message Aneurysmal bone cyst (ABC) is a benign osteolytic lesion that is fast‐growing, expansile, and locally destructive. The present case is of a young girl with facial asymmetry, which had become accentuated during the previous months. A conservative treatment was performed to reduce morbidity and affectation of the lower dental nerve.

Objectives Different studies have documented an association between periodontal disease and low birth-weight delivery. Hence, knowledge of periodontal status during pregnancy and postpartum is important in order to reduce the risks of both diseases. This study aimed to analyze periodontal status at successive stages of pregnancy and 3–6 weeks postpartum in women with initial periodontal alterations.
