4. Prevalence of Autism Spectrum Disorders in Adult Outpatient Psychiatry

Abstract

Relatively little has been published about the prevalence of autism in adults with psychiatric disorders. In this study, all new patients referred to an adult psychiatric outpatient clinic in Sweden between November 2019 and October 2020 (n = 562) were screened for autism spectrum disorders using the Ritvo Autism and Asperger Diagnostic Scale Screen (RAADS-14). Out of the 304 (58%) responders, 197 who scored above the cut off (14) were invited to participate in an in-depth assessment. Twenty-six of the 48 that participated in the assessment met criteria for ASD and an additional eight had subthreshold ASD symptoms. We estimated the prevalence of ASD in this population to at least 18.9%, with another 5-10% having subthreshold symptoms.

5. Practice Patterns and Potential Barriers to Early Diagnosis of Autism in Bosnia and Herzegovina: A Preliminary Study

Abstract

The aim of this preliminary study was to identify the practice patterns and potential barriers to diagnosing autism in Bosnia & Herzegovina. 126 children aged 23 to 94 months with developmental concerns referred to treatment centers participated in the study. Although parents had reported developmental problems in their children usually around the age of 17 months, it took them 812 visits to professionals (> 6 visits per child) over several months (mean 16.8, range 2-52 months) to get diagnosed with any developmental disorder. Only 8 children (6.3%) received a diagnosis of autism. However, when we re-examined 110 of the 126 children, 90 of them (71%) were identified with autism. Implications of these findings are discussed.

Abstract

This study explored the physical and clinical phenotype of Bangladeshi children with autism spectrum disorder (ASD). A totally of 283 children who were referred for screening and administered Module 1 of the Autism Diagnostic Observation Schedule (ADOS) were included. Overall, 209 met the ADOS algorithmic cutoff for ASD. A trend for greater weight and head circumference was observed in children with ASD versus non-ASD. Head circumference was significantly (p < 0.03) larger in ASD males compared with non-ASD males. A trend was also observed for symptom severity, higher in females than males (p = 0.068), with further analyses demonstrating that social reciprocity (p < 0.014) and functional play (p < 0.03) were significantly more impaired in ASD females than males. The findings help understand sex differences in ASD.

23. DSM-5 ASD Moves Forward into the Past

Abstract

The fifth edition of the diagnostic and statistical manual of mental disorders (DSM-5) (APA in diagnostic and statistical manual of mental disorders, Author, Washington, 2013) has decided to merge the subtypes of pervasive developmental disorders into a single category of autism spectrum disorder (ASD) on the assumption that they cannot be reliably differentiated from one another. The purpose of this review is to analyze the basis of this assumption by examining the comparative studies between Asperger's disorder (AsD) and autistic disorder (AD), and between pervasive developmental disorder not otherwise specified (PDDNOS) and AD. In all, 125 studies compared AsD with AD. Of these, 30 studies concluded that AsD and AD were similar conditions while 95 studies found quantitative and qualitative differences between them. Likewise, 37 studies compared PDDNOS with AD. Nine of these concluded that PDDNOS did not differ significantly from AD while 28 reported quantitative and qualitative differences between them. Taken together, these findings do not support the conceptualization of AD, AsD and PDDNOS as a single category of ASD. Irrespective of the changes proposed by the DSM-5, future research and clinical practice will continue to find ways to meaningfully subtype the ASD.

27. Psychopharmacology of Autism Spectrum Disorders: A Selective Review

Abstract

While there is no cure for autism spectrum disorder, psychopharmacologic agents are often used with behavioral and educational approaches to treat its comorbid symptoms of hyperactivity, irritability, and aggression. Studies suggest that at least 50% of persons with autism spectrum disorder receive psychotropic medications during their life span. This selective review examines recent studies about the use of psychotropic medications in persons with autism spectrum disorder. The aim was to focus on randomized controlled trials conducted from 1990 to 2010 on this topic. A comprehensive literature search was performed using PubMed and Cochrane databases. Out of 105 studies identified for the review, only 24 were randomized controlled trials. Thus, despite the common use of these medications in autism spectrum disorder, more controlled studies are needed to determine their long-term efficacy and safety.

30. Violent Crime in Asperger Syndrome: The Role of Psychiatric Comorbidity

Abstract

Although several studies have suggested an association between violent crime and Asperger syndrome (AS), few have examined the underlying reasons. The aim of this review is to determine to what extent psychiatric factors contribute to offending behavior in this population. Online databases were used to identify relevant articles which were then cross-referenced with keyword searches for "violence," "crime," "murder," "assault," "rape," and "sex offenses." Most of the 17 publications which met the inclusion criteria were single case reports. Of the 37 cases described in these publications, 11 cases (29.7%) cases had a definite psychiatric disorder and 20 cases (54%) had a probable psychiatric disorder at the time of committing the crime. These findings underscore the role of psychiatric disorders in the occurrence of violent crime in persons with Asperger syndrome and highlight the need for their early diagnosis and treatment.

31. Defining the Behavioral Phenotype of Asperger Syndrome

Abstract

Asperger syndrome (AS) is characterized by social deficits and restricted interests in the absence of mental retardation and language delay. However, it is unclear to what extent the social deficits of AS differ from those of traditional autism. To address this issue, 58 subjects with AS were compared with 39 subjects with autism. Social deficits were classified according to Wing and Gould's method. Forty-six (79%) subjects with AS were rated as active but odd, while 32 (82%) with autism were described as aloof and passive, while a few subjects showed mixed features. Thus, despite the overlap between AS and autism, subjects with AS showed a distinct pattern of social impairment. Implications of these findings are discussed.

33. A Family History Study of Asperger Syndrome

Abstract

Asperger syndrome (AS) is a childhood-onset disorder often described as a mild variant of autism. Although classified as a distinct disorder in the DSM-IV, its overlap with autism continues to be a matter of ongoing debate. While the family genetic origins of autism are well established, few studies have investigated this topic in AS using current operational criteria. In this report, we examined the family psychiatric history of 58 subjects with AS diagnosed according to DSM-IV criteria (48 males; mean age 13.34; mean full scale IQ 104.87). All subjects had a history of mild autistic social deficits; focused special interests; normal level of intelligence; and an odd and often pedantic manner of speaking. None had a previous diagnosis of autism. Of the 58 subjects with Asperger syndrome, three had first degree relatives with AS; nine (15%) had a family history of schizophrenia; and 35 (60%) had a family history of depression. Of the 64 siblings, four had a diagnosis of AS and none of autism. Compared with a group of 39 subjects with normal intelligence autism (high functioning autism, HFA; 33 males; mean age 15.34; mean full scale IQ 85.89) subjects with AS were more likely to have relatives with depression; schizophrenia; and the broader autistic phenotype. Possible reasons for and implications of these findings are discussed.

34. Defining the Intellectual Profile of Asperger Syndrome: Comparison with High-Functioning Autism

Abstract

Asperger syndrome (AS) is a disorder of early childhood characterized by autistic social deficits, subtle communication impairment, and excessive isolated interests. There is no history of language delay or of mental retardation. Despite its increasing popularity as a distinct condition, its diagnostic validity remains uncertain. It is still unclear to what extent AS differs from autism with normal intelligence sometimes referred to as high-functioning autism (HFA). However, some reports have suggested that persons with AS possess a distinct profile on tests of intelligence characterized by a high verbal IQ and a low performance IQ, whereas in most cases with HFA, the pattern is reversed. Since few studies have directly compared AS subjects with HFA controls using unmodified diagnostic criteria and standardized measures of assessment, in this report we compared 22 AS subjects with 12 HFA controls, matched on age, sex and level of intelligence. As a group, subjects with AS showed a higher verbal IQ and higher scores on information and vocabulary subtests than those with HFA. However, scores of several AS and HFA subjects showed a mixed pattern. Implications of these findings are discussed in the context of the validity of Asperger Syndrome.

Abstract

This study examined the syntactic characteristics of the speech of adolescent subjects with Asperger syndrome (N=14) or high functioning autism (N=13) using a modified version of syntactic analysis, Brief Syntactic Analysis. Asperger syndrome subjects tended to show more complex speech patterns and longer sentences than did subjects with high functioning autism. (Contains references.) (DB)

Abstract

This study compared the family histories of 13 children with autism or pervasive developmental disorder (PDD) with depression and 10 similar children without depression. Ten of the depressed children had a positive family history of depression compared to three of the nondepressed group. Findings support the validity of depression as a distinct condition in some children with autism/PDD. (Author/DB)

Abstract

Seventeen patients with Asperger syndrome (AS) and a mean IQ of 97 were compared with a control group of 13 patients with normal intelligence or high-functioning autism. Data were collected about unpleasant events in the lives of 11 children with pervasive developmental disorders (PDD) and depression, and were compared against 11 age- and gender-matched controls with PDD and no depression. 42. Thought Disorder in Asperger Syndrome: Comparison with High-Functioning Autism.

Abstract

This study compared Rorschach test performance of 12 subjects with Asperger syndrome (AS) and 12 subjects with high-functioning autism (HFA, mean age 12 years). AS subjects demonstrated a trend toward greater levels of disorganized thinking than the HFA group and were more likely to be classified as "introversive." The test did not differentiate the two groups on most variables. (Author/DB)

This paper examines how clumsiness has been defined and assessed in the literature on Asperger syndrome. Of 42 identified papers, 23 mentioned clumsiness as a feature of the syndrome and 4 stated that they had used tests specifically to assess clumsiness, but none of the publications defined clumsiness. This paper assesses to what extent the ICD-10 (International Classification of Diseases) criteria differ from other diagnostic systems for Asperger syndrome. The paper concludes that the variety of criteria in use makes interpretation of research findings difficult and that the ICD-10 criteria are the most stringent and closest to Hans Asperger's original description. 48. Brief Report: Should the DSM V Drop Asperger Syndrome?

Abstract

The DSM IV defines Asperger syndrome (AS) as a pervasive developmental (autistic spectrum) disorder characterized by social deficits and rigid focused interests in the absence of language impairment and cognitive delay. Since its inclusion in the DSM-IV, there has been a dramatic increase in its recognition both in children and adults. However, because studies have generally failed to demonstrate a clear distinction between AS and autism, some researchers have called for its elimination from the forthcoming DSM V. This report argues for a modification of its diagnostic criteria and its continued retention in the diagnostic manual.
