Purpose: This study was conducted to show diverse clinical presentation of ocular surface squamous neoplasia with a rare presentation of caruncle squamous cell carcinoma (SCC). Methods: All cases that had suspicious ocular lesion were included in the treatment study. A detailed history including demographic data was obtained. Clinical features regarding the type of lesion, location, and the involvement of cornea were evaluated. For all patients, excisional biopsy following “Shield’s no-touch technique” with 3 to 4 mm margins of uninvolved tissue and cryotherapy at excisional margins was done. All specimens were sent for a histopathological evaluation. Topical chemotherapy (mitomycin C, 0.02%) was used as an adjunctive therapy following surgical excision for large and diffuse ocular surface tumors. All patients were subjected to long-term regular follow-ups. Results: Twenty-six patients (18 men and 8 women) with a mean age of 54.2 years were enrolled in this study. The results showed that the most common localization was bulbar conjunctiva (92%). Nodular lesions (46.5%) and SCC (57.5%) were the most common clinical and histopathological features, respectively. We observed one rare case of primary SCC of the left caruncle in a 68-year-old man who had an asymptomatic medial canthal mass. Recurrence was found in two patients with SCC, one of them having an orbital extension. Conclusion: The early suspicion of ocular surface neoplasia will be accomplished with a simple excision in most cases, leading to a favorable outcome except in severe progressive cases.

Exudative age-related macular degeneration is the most common cause of vision loss of people over the age 60 in the developed countries. The cause is not clear yet, but since Avastin is largely used for the treatment, therefore, this study was conducted to assess the influence of avastin.

Introduction: Diabetic retinopathy as an important complication of diabetes mellitus is a common cause of blindness in diabetic people. A cure or treatment is not available yet. This study was designed to investigate the effectiveness of the intravitreal injection of Avastin on patient’s visual satisfaction with diabetic retinopathy. Materials and Methods: This study included 30 eyes of thirty diabetic retinopathy patients (mean age: 60.47±8.94 years) showing no recovery with common treatments. After the intravitreal injection of 1.25mg/0.05ml of Avastin, the examination was performed after injection. In this examination, the resolution of neovascularization, clearance of vitreous and visual acuity were evaluated versus visual satisfaction. Results: 60% of patients were women (n=18) and 40% were men (n=12). All patients had type 2 diabetes for a period of 8. 47±5.28 years (range 2-25 years). OR.2014.6.009 353 was 0.22±0.63 log MAR in the best corrected visual acuity (BCVA) in right eye which was statistically significant (p=0.010). The BCVA in left eye was 0.31±0.74 log mar, also showing significant relation (p=0.020). BCVA Changes did not have a meaningful relationship with age and sex of patients, but these changes were negatively related to duration of diabetes retinopathy in both right and left eyes (p=0/048, p=0/006 respectively). Conclusion: Avastin showed short-term statistically significant visual benefits and also, improvements in ophthalmic pathology in clinical examination versus visual satisfaction, however it was not compatible to ask all of the patient's visual satisfaction. Therefore, further studies will be needed to determine the therapeutic effects of this treatment option.

We report of a 19-year-old man complaining of clumsy gait, drooling of saliva, difficulty in speech, and behavioral changes who underwent ophthalmic examination and was diagnosed with a rare case of Wilson disease: Kayser-Fleischer ring and unilateral hypertrophy of retinal pigment epithelium.

The current study was an attempt to report a case of subacute bilateral hypopionic uveitis and pan uveitis in a referral case of Behcet patient. Careful questioning of the patient and detailed examination of the eyes for other signs are necessary to guide the differential diagnosis and relevant investigations. In this study, a case of a patient with a 20 year history of Behcet disease and occular manifestation of bilateral hypopionic uveitis was presented in ophthalmic consultation. Hypopion resolved shortly after instating therapy but old scarified retinal lesion did not respond as well as long lasting posterior synchia. Hypopion uveitis is considered in the context of the associated ocular and systemic diseases causing it. If Steroid therapy, which leads to significant remission of symptoms, missed diagnosis, it could have serious consequences for the patient. A rational approach based on the understanding of the causes of hypopyon uveitis is imperative.

There is evidence of Helicobacter pylori infection that induces eye diseases. This study was done to find if there is any association between H. Pylori and ocular diseases, and if there is any difference in color of eye among them. Of all the 35 patients tested for biopsy with positive H. pylori, 23 (65.7%) were females and 12 (34.3%), males. The mean age of patients was 39.65±18.55 years. 20 (57.1%) patients were below 40 years old; 8 (22.9%) patients, 40 to 60 years and only 7 patients were more than 60 years old. Most of the patients were 20 years old and had no ophthalmic or systemic diseases in juvenile age group; most of them with the best correct vision were within normal measure, except the old patients who had normal tension glaucoma and two cataract cases. Brown eyes were the commonest but blue color was the least without any risk of ocular diseases or H. Pylori associated with more prevalent brown iris color in study group. Ophthalmic extragastric diseases do not show any association between eye diseases and H. pyloriinfection in juvenile age groups. Pesudoexfolitive glaucoma and two cases of cataract were among the old age related diseases which may be due to the late appearance of H. pylori effect. Best visual acuity, cup/disc character and brown color were the most associated finding. Key words: Helicobacter pylori, extragastric manifestations, ophthalmic diseases.

Trachoma is a keratoconjunctivitis that causes ocular infection with Chlamydia trachomatis (Ct). The chronicity and reinfection leads to increasingly severe inflammation that can damage the conjunctiva and cornea. The WHO (World Health Organization) is in alliance with the Global Elimination of Trachoma by 2020. This safe strategy includes: surgery for trichiasis, antibiotics for active trachoma, facial cleanliness, and environmental improvement. Although there is evidence which supports effectiveness of this approach, will it be eliminated by 2020? There are many old people who suffer from trachoma sequel in Southern Khorassan, Iran, thus discrepancy of clinical versus paraclinical diagnosis of active trachoma especially in a suspicious case remains unclear. Therefore, our negative result according to PCR approaches among 10 cases of clinically suspicious or definite trachoma will be an emphasis on clinical diagnosis.
