Purpose: Here we report a case of posterior chamber intraocular lens (IOL) dislocation into the subconjunctival space (pesudophacocele) following ocular trauma.Case summary: A 66-year-old male presented with ocular pain and decreased vision in the right eye following trauma with a metallic rod. The patient had a history of trabeculectomy, glaucoma drainage device implantation, transscleral cyclophotocoagulation, and cataract surgery for uveitic glaucoma and a cataract in the right eye. On examination, vision was hand movement, the intraocular pressure was 3 mmHg, and subconjunctival hemorrhage and hyphema were observed. After the resolution of hemorrhage, uveal tissue prolapse was seen nasally behind the corneal limbus and the IOL was found to be dislocated into the nasal subconjunctival space. There were no changes in the filtering bleb and tube compared to the pre-trauma status. The IOL was removed through a conjunctival incision because the patient refused any active treatment.Conclusions: Pesudophacocele developed in a patient who had a history of glaucoma and cataract surgery in the injured eye. The IOL could not be assessed immediately after the trauma because of subconjunctival hemorrhage and hyphema. When the status of IOL is unclear or suspected to be dislocated after trauma, the possibility of pseudophacocele should be considered, in addition to the dislocation into the vitreous cavity.

This paper presents a quick block transport system for scalable Hyperledger Fabric (HLF) blockchain over device-todevice (D2D)-assisted 5G networks. The objective of the proposed system is to increase the transaction throughput and further enable the application of the blockchain to delay-sensitive 5G services. To this end, the system rapidly disseminates the block data to all the peers on the blockchain network. The blockchain network is organized with mobile edge cloud (MEC) server and mobile peers in the 5G network. To ensure a scalable and reliable blockchain architecture, the core functionalities of the HLF blockchain are appropriately distributed among the mobile edge cloud and each mobile peer. The MEC server handles core functionalities for the blockchain network, such as block generation and membership management. In addition, it is responsible for gathering the network status of all peers and organizing the overlay structure to be utilized to rapidly propagate the block with a low network cost. On the other hand, the mobile peers handle the endorsing and validating functionalities to prevent block data tampering in the blockchain network. Furthermore, locality-aware device-to-device networking is utilized in 5G network coverage to achieve faster block dissemination. The proposed system is designed to work seamlessly in an existing HLF blockchain framework with consideration of scalability and compatibility. It is fully implemented using the open-source HLF and OpenAirInterface. To verify the effectiveness of the proposed block transport system, it was evaluated in an actual 5G network as well as 802.11 network. The experimental results verified its feasibility.

Type 2 diabetes mellitus (T2DM) and hypertension (HTN) are both relatively common systemic diseases and cause damage to the retina, such as inner retina reduction and microvascular impairment. The purpose of this study was to identify peripapillary retinal nerve fiber layer (pRNFL) damage by diabetic neurodegeneration and the effects of HTN on the pRNFL thickness in patients with T2DM without clinical diabetic retinopathy. Subjects were divided into three groups: healthy control subjects (group 1), patients with T2DM (group 2), and patients with both diabetes and HTN (group 3). The pRNFL thickness was measured using optical coherence tomography and compared among each group. Linear regression analyses were performed to identify factors associated with pRNFL thickness. A total of 325 eyes were included: 143 eyes in the group 1, 126 eyes in group 2, and 56 eyes in group 3. The mean pRNFL thicknesses of each group were 96.1 ± 7.7, 94.4 ± 8.6, and 91.6 ± 9.6 μm, respectively (P = 0.003). In multivariate linear analyses, diabetes duration (β = −0.236; P = 0.018) and HTN (β = −3.766; P = 0.008) were significant factors affecting the pRNFL thickness in groups 2 and 3. Additionally, the HTN duration was significantly correlated with pRNFL thickness in group 3 (R2 = 0.121; P = 0.008). In conclusion, patients with T2DM with HTN showed thinner pRNFL thickness than those with T2DM only. Additionally, the duration of HTN was significantly correlated with pRNFL thickness in patients with both diabetes and HTN.

Purpose: To evaluate the dynamic range of retinal nerve fiber layer (RNFL) and optic nerve head (ONH) parameters measured using optical coherence tomography (OCT) in conditions ranging from nonglaucomatous status to advanced glaucoma by longitudinal observation.Methods: A total of 15 eyes from 12 participants with glaucoma progression from a nonglaucomatous status to advanced glaucoma were included. The RNFL and ONH parameters were compared between the nonglaucomatous and advanced stages within the same eye. The absolute and relative changes in OCT parameters were analyzed.Results: The median highest intraocular pressure was 42.5 mmHg (interquartile range, 37.5 to 54.5 mmHg), and the final mean deviation of the visual field test was –24.68 dB (interquartile range, –23.93 to –31.13 dB). The median relative changes in RNFL thickness were –40.6% in the overall area, and –51.9%, –21.4%, –51.1%, and –41.8% in the superior, nasal, inferior, and temporal quadrants, respectively (all p < 0.05). Relative changes in the rim area, disc area, average cup to disc ratio, vertical cup to disc ratio, and cup volume were –56.64%, 0.59%, 62.10%, 66.0%, and 337.90%, respectively (all p < 0.05, except for disc area with a p-value of 0.753).Conclusions: The dynamic range of the RNFL thickness ranged from 40.6% to 51.9%, and the dynamic range of the ONH parameters ranged from 56.64% to 337.90%. During the course of glaucoma progression, the cup volume showed the widest dynamic range. However, the disc area did not show significant changes.

This paper presents a video quality-aware traffic offloading system that provides seamless video streaming services to users by offloading part of the macrobase station traffic into small cells. The proposed system achieves a good balance between fairness and social welfare in terms of the video quality by allocating the radio resource of the macrobase station. The data flow is split into two parts toward the macrobase station and small cell AP, and the users simultaneously receive their data from both. In the proposed system, fountain code is employed to achieve dual connectivity enhancement by overcoming frequent packet loss in error-prone mobile networks and practical issues in the dual connectivity architecture. Furthermore, software-defined networking (SDN) technology is adopted not only to quickly react to time-varying network status but also to control network resources efficiently. The proposed resource allocation process is designed to require a low running time complexity. The proposed system is implemented using C/C++, Java, and a well-known open source, ONOS. The experimental results verify that the proposed system can achieve much better performance than other offloading algorithms.

To identify the effects of prolonged type 2 diabetes (T2DM) on changes in peripapillary retinal nerve fiber layer (pRNFL) thickness in patients without clinical diabetic retinopathy. Subjects were divided into two groups: controls and patients with T2DM (DM group). After the initial visits, the pRNFL thicknesses were measured three more times at 1-year intervals. Subgroup analyses were performed in patients with T2DM duration ≥ 10 years. The mean pRNFL thickness at each visit was 95.8 ± 8.1, 95.4 ± 8.3, 94.9 ± 8.1, and 94.5 ± 8.3 μm in the control group (P = 0.138) (n = 55); and 93.4 ± 9.1, 92.1 ± 9.3, 90.9 ± 9.3, and 89.5 ± 9.2 μm in the DM group (P P = 0.001) were significant factors associated with pRNFL reduction. In patients with T2DM duration ≥ 10 years, the estimated pRNFL reduction rate was − 1.61 μm/year, and hypertension was a significant factor affecting the pRNFL reduction (P = 0.046). We confirmed rapid pRNFL reduction over time in T2DM, and the reduction rate was higher in patients with T2DM ≥ 10 years. Additionally, BCVA and HbA1c levels were significantly associated with the change in pRNFL thickness in T2DM patients.

Objective To investigate the effects of axial length (AL) on the peripapillary microvascular density acquired from optical coherence tomography angiography (OCTA). Methods Retrospective observational study. A total of 111 eyes from 111 normal healthy subjects were examined. The subjects were divided into three groups according to the AL: Group 1 (AL: < 24.0 mm; 35 eyes), Group 2 (AL: 24.0–25.99 mm; 37 eyes), and Group 3 (AL: ≥ 26 mm; 39 eyes). Peripapillary OCTA images were acquired using 6× 6 mm angiography scans, and vessel density (VD) and perfusion density (PD) of the superficial capillary plexus were calculated automatically. VD and PD were compared among the three groups according to the distance from the optic disc (inner and outer rings). Linear regression analyses were also performed to identify clinical factors associated with average VD. Results The average ALs of Groups 1–3 were 23.33± 0.57, 25.05± 0.60, and 27.42± 0.82, respectively. Average VD (P = 0.009) and PD (P = 0.029) in the inner ring increased with increasing AL. However, average VD (P < 0.001) and PD (P < 0.001) in the outer ring decreased with AL increased; the same trends were found for the full areas (VD, p Conclusions Peripapillary VD and PD were significantly associated with AL. Depending on the distance from the disc, peripapillary VDs and PDs of the inner and outer rings were differentially affected by AL. Physicians should therefore consider the effects of AL in the analyses of peripapillary microvasculature.

This paper presents a game-theoretic scalable offloading system that provides seamless video streaming services by effectively offloading parts of video traffic in all video streaming services to a WiFi network to alleviate cellular network congestion. The system also consolidates multiple physical paths in a cost-effective manner. In the proposed system, the fountain encoding symbols of compressed video data are transmitted through long term evolution (LTE) and WiFi networks concurrently to flexibly control the amount of video traffic through the WiFi network as well as mitigate video quality degradation caused by wireless channel errors. Furthermore, the progressive second price auction mechanism is employed to allocate the limited LTE resources to multiple user equipment in order to maximize social welfare while converging to the ϵ -Nash equilibrium. Specifically, we design an application-centric resource valuation that explicitly considers both the realistic wireless network conditions and characteristics of video streaming services. In addition, the scalability and convergence properties of the proposed system are verified both theoretically and experimentally. The proposed system is implemented using network simulator 3. Simulation results are provided to demonstrate the performance improvement of the proposed system.

In this work, we present a novel fountain code-based hybrid P2P storage system that combines cloud storage with P2P storage. The proposed hybrid storage system minimizes data transmission time while guaranteeing high data retrieval and data privacy. In order to guarantee data privacy and storage efficiency, the user transmits encoded data after performing fountain code-based encoding. Also, the proposed algorithm guarantees the user's data retrieval by storing the data while considering each peer's survival probability. The simulation results show that the proposed algorithm enables fast completion of the upload transmission while satisfying the required data retrieval and supporting the privacy of user data under the system parameters.

IMPORTANCE: Type 2 diabetes is expected to accelerate age-related peripapillary retinal nerve fiber layer (pRNFL) loss, but limited information on the rate of reduction in pRNFL thicknesses in patients with type 2 diabetes is available. OBJECTIVE: To investigate longitudinal changes in pRNFL thickness in patients with type 2 diabetes, with or without diabetic retinopathy (DR). DESIGN, SETTING, AND PARTICIPANTS: A total of 164 eyes of 63 healthy individuals and 101 patients with type 2 diabetes (49 patients without DR [non-DR group] and 52 patients with mild to moderate nonproliferative DR [NPDR group]) were enrolled in this prospective, longitudinal, observational study from January 2, 2013, through February 27, 2015. Participants were followed up for 3 years, and the peripapillary mean and sector RNFL thicknesses were measured at 1-year intervals. The mean rate of pRNFL loss was estimated using a linear mixed model and compared among the 3 groups. Follow-up was completed on March 16, 2018, and data were analyzed from April 2 through July 27, 2018. EXPOSURE: Type 2 diabetes. MAIN OUTCOMES AND MEASURES: The rate of reduction in pRNFL thickness in patients with type 2 diabetes. RESULTS: A total of 164 participants (88 women [53.7%]; mean [SD] age, 58.2 [8.7] years) were included in the study analysis. The mean (SD) age of the control group was 56.5 (9.3) years (39 women [61.9%]); the non-DR group, 59.1 (9.4) years (26 women [53.1%]); and the NPDR group, 59.4 (11.0) years (23 women [44.2%]). Mean (SD) duration of type 2 diabetes was 7.1 (4.4) years in the non-DR group and 13.2 (8.4) years in the NPDR group. The baseline mean (SD) pRNFL thickness was 96.2 (11.0) μm in the control group, 93.5 (6.4) μm in the non-DR group, and 90.4 (7.9) μm in the NPDR group. During 3 years of follow-up, these values decreased to 95.0 (9.2) μm in the control group, 90.3 (6.4) in the non-DR group, and 86.6 (7.9) μm in the NPDR group. Methods Participants without any ophthalmic disease were divided into two groups: an HTN group (50 eyes) that included patients with HTN for ≥5 years and a control group. After the initial visit, pRNFL thicknesses were measured four more times at 1-year intervals using spectral-domain optical coherence tomography. The pRNFL thickness was fitted using linear mixed models. Univariate and multivariate generalized linear mixed models were used to determine factors associated with pRNFL reductions over time. Results The mean ages of the HTN and control groups were 62.9 ± 6.2 and 60.7 ± 6.1 years, respectively, and they were not significantly different (P = 0.089).
