Introduction: phacolytic ocular hypertension is a condition where an increase in intraocular pressure is observed; which is secondary to the obstruction of the trabecular meshwork due to the deposition of high molecular weight crystalline proteins and macrophages in patients with hyper-mature cataract. Case report: a 68-year-old male patient with facolytic ocular hypertension in the left eye was examined. During the extraction of the lens and the aspiration of cortical remains, it was found that white flocculent material persisted between the posterior capsule of the lens and the anterior hyaloids, a posterior capsulotomy with anterior vitrectomy and IOL implantation in sulcus had to be performed. Despite the risks involved, satisfactory surgical results were achieved. Conclusions: the definitive behavior in any case with ocular facolytic hypertension involves the extraction of the lens; however, the patient’s own ocular characteristics must be assessed in such a way that the behavior to be followed can be individualized in order to obtain a surgical result and a favorable evolution.

Introduction: there is currently a tendency toward phacoemulsification in the primary angular closure due to the crucial role of the lens in the pathophysiology of the disease. Objective: to describe the results of phacoemulsification in patients with primary angular closure in Pinar del Río. Method: a prospective longitudinal descriptive study was carried out in Pinar del Río, between January 2013 and December 2015, which included 43 patients with primary angular closure not controlled by laser peripheral iridotomy or medical treatment, who underwent phacoemulsification. Statistical analysis was performed using the SPSS program. Results: mean age was 54.67 ± 4.22 years and 83.7% were women. Introducción: la trabeculectomía continúa siendo la cirugía incisional más utilizada para controlar la presión intraocular en pacientes glaucomatosos. Objetivo: describir los factores que influyen en el control de la presión intraocular postrabeculectomía. Método: se realizó un estudio descriptivo, longitudinal prospectivo en 91 trabeculectomizados del Hospital General Docente “Abel Santamaría Cuadrado” durante los años 2012 a 2015 y se describió control de la presión intraocular según variables que pudieron influir en este resultado, a través de la estadística descriptiva y el test de chi cuadrado. Resultados: los casos con control adecuado presentaron ausencia de antecedentes oculares (88,6 %), estadio moderado de la enfermedad (65,7 %), presión intraocular previa ≤ 30 mmHg (72,9 %) y ausencia de complicaciones posoperatorias (84,3 %). Con control relativamente adecuado o no adecuado predominaron los pacientes masculinos (92,9 % y 57,1 % respectivamente), con color de piel no blanca (85,7 %), antecedentes de trabeculectomía (57,1 %), daño avanzado y presión intraocular previa > 30 mmHg (85,7 %). Conclusiones: entre los factores que influyen en el control adecuado de la presión intraocular estuvieron los antecedentes oculares negativos, presión intraocular previa menor o igual a 30 mmHg y ausencia de complicaciones posoperatorias mientras que para un control relativamente adecuado o inadecuado influyeron el sexo masculino, color de piel no blanca, antecedente de trabeculectomía previa, daño glaucomatoso avanzado y presión intraocular previa mayor de 30 mmHg.
