Abstract

Objective To describe demographic, clinical and laboratory features of systemic sclerosis sine scleroderma (ssSSc) in a large multicentre systemic sclerosis (SSc) cohort. Methods Data involving 1808 SSc patients from Italian Systemic sclerosis PRogression INvestiGation registry were collected. The ssSSc was defined by the absence of any cutaneous sclerosis and/or puffy fingers. Clinical and serological features of ssSSc were compared with limited cutaneous (lcSSc) and diffuse cutaneous (dcSSc) subsets. Results Among patients with SSc, only 61 (3.4%) were classified as having ssSSc (F/M=19/1). Time from Raynaud's phenomenon (RP) onset to diagnosis was longer in ssSSc (3 years, IQR 1-16.5) than lcSSc (2 years, IQR 0-7), and dcSSc (1 year, IQR 0-3) (p<0.001). Clinical ssSSc phenotype was comparable to lcSSc, except for digital pitting scars (DPS) (19.7% vs 42%, p=0.01), but significantly milder than dcSSc, particularly for digital ulcers (DU) (6.6% vs 35.7%, p<0.001), oesophagus (46.2% vs 63.5%, p=0.009), lung (mean diffusion capacity for carbon monoxide 72.2±19.6 vs 62.4±22.8, p=0.009; mean forced vital capacity 105.6±21.7 vs 89.2±20.9, p<0.001) and major videocapillaroscopic alterations (late pattern 8.6% vs 47.6%, p<0.001). Moreover, in ssSSc the percentages of anticentromere and antitopoisomerase were comparable to lcSSc (40% and 18.3% vs 36.7% and 26.6%), but divergent respect to dcSSc (8.6% and 67.4%, p<0.001). Conclusion The ssSSc is a quite rare disease variant characterised by clinico-serological features comparable to lcSSc, but significantly different from dcSSc. Overall, longer RP duration, low percentages of DPS and peripheral microvascular abnormalities, and increased anti-centromere seropositivity distinguish ssSSc. Further investigations based on national registries might provide useful insights on the actual relevance of the ssSSc within the scleroderma spectrum.

Abstract

A 65-year-old non-smoker female was diagnosed with lung adenocarcinoma clinically staged as IV M1a because of bilateral pulmonary lesions. After a differential response to chemotherapy, further analyses allowed us to re-stage the tumor as a synchronous bilateral local disease with unilateral ALK (Anaplastic lymphoma kinase) rearrangement. Combined treatment with chemotherapy, crizotinib and surgery, with clinical and pathological tumor-response to pre-operative crizotinib, obtained complete tumors remission, and the patient is still disease free after 11 months since the last tumor resection. As far as we know this is the first report of a clinical and pathological regression of an early-stage ALK-rearranged NSCLC treated with neo-adjuvant crizotinib. This report supports further studies to assess activity and efficacy of ALK–inhibitors in neoadjuvant setting.

Abstract

Cirrhosis has been long considered a risk factor for bleeding due to the co-existence of the so-called ‘coagulopathy’. More recently, however, compelling evidences have been provided on the occurrence of thrombotic events in the portal and systemic circulation.3–5 Portal vein thrombosis (PVT) is predominantly observed in patients with moderate to severe liver failure with a variable prevalence ranging from 0.6 to 25%. Only fewstudies have provided a longitudinal assessment of the PVT incidence and its sequelae, including recurrence and survival.9–14 Due to the variability of PVT incidence and the paucity of data regarding recurrence and survival,15–20 we prospectively analysed the incidence and the recurrence of PVT in the population of Portal vein thrombosis Relevance On Liver cirrhosis: ItalianVenous thromboticEventsRegistry (PROLIVER), a multi-centre study,8 which involved 43 enrolling centres in Italy (ClinicalTrials.gov Identifier: NCT01470547).

Abstract

La polyarthrite rhumatoïde (PR) est une maladie auto-immune pour laquelle divers facteurs environnementaux précoces, tels que le tabagisme dans l'enfance, jouent un rôle crucial dans son développement. L'impact de l'environnement, en particulier lors de la période périnatale, est de plus en plus étudié. Certaines études ont suggéré que les paramètres de naissance, le tabagisme in utero pourraient influencer le risque de PR. L'objectif de cette étude est d'explorer l'association entre les facteurs périnataux avec le risque de PR dans la cohorte E3N. La cohorte E3N (« étude épidémiologique auprès de femmes de la mutuelle générale de l'éducation nationale ») comprend 98 995 femmes suivies depuis 1990. Parmi elles, 698 ont développé une PR validée. Les données périnatales (poids, taille, terme), le tabagisme in utero, l'allaitement et la privation alimentaire durant l'enfance ont été collectées à partir du questionnaire Q7 en 2002. Les Hazard Ratios (HR) et les intervalles de confiance (IC 95 %) pour le risque de PR ont été estimés via des modèles de Cox ajustés pour l'âge et d'autres facteurs de confusion identifiés par une analyse de graphique acyclique dirigé (DAG). Sur les 78 473 femmes étudiées, 698 cas incidents de polyarthrite rhumatoïde ont été identifiés. Sur l'ensemble de la population, aucune association significative n'a été identifiée entre les paramètres de périnatalité et le risque de PR. Cependant, une tendance non significative à un risque accru de PR a été observée chez les individus de grande taille à la naissance (HR = 1,24 [0,91–1,70]), et chez ceux nés prématurément (HR = 1,34 [0,92–1,95]). Chez les femmes ayant été allaitées on trouvait une tendance non significative vers un risque accru de PR (HR = 1,21 [0,99–1,47]), de même en cas d'exposition au tabac in utero (HR = 1,60 [0,88–2,93]. Enfin, la privation alimentaire durant l'enfance (pendant la seconde guerre mondiale) n'était pas associée au risque de PR (HR = 0,95 [0,78–1,15]). L'analyse stratifiée selon le statut tabagique montre que chez les fumeuses, la prématurité est associée à un risque accru de PR (HR = 1,63 [1,02–2,64]) et une tendance non significative pour le tabagisme in utero (HR = 1,83 [0,91–3,69]). Chez les non-fumeuses, un risque accru significatif a été observé pour les femmes de grande taille à la naissance (HR = 1,55 [1,02–2,35]). Nos résultats suggèrent que la prématurité et l'exposition au tabac in utero pourraient influencer le risque de PR, particulièrement chez les fumeuses, tandis que la taille à la naissance aurait une influence chez les non-fumeuses. Concernant l'allaitement, la littérature présente des résultats contradictoires, certains suggérant que la prolactine, sécrétée durant l'allaitement, pourrait augmenter le risque de PR. Abstract

Background: Radiotherapy (RT) is used in head and neck squamous cell carcinoma (HNSCC) with excellent effectiveness, but it is burdened by important side effects, which may negatively impact patients' quality of life (QoL). In particular when associated with chemotherapy (CT), that has a radiosensitising effect (and its own toxicities), it is responsible for several adverse events, causing social discomfort and lower QoL, in patients who are already experiencing several tumor-related discomforts. Prehabilitation is a healthcare intervention consisting of several specialist visits prior to the start of treatment, with the aim of improving the patient's health status, resolving symptoms that interfere with treatment and impact QoL, and finally to better avoid or overcome complications. Of all cancer patients, HNSCC patients are among those who could benefit most from prehabilitation, both because of the high number of symptoms and toxicities and their difficult management. Despite this and the emerging data, prehabilitation is not often considered for the majority of patients undergoing (C)RT. In this review, we tried to understand what are the main areas in which interventions can be made prior to the (C)RT start, the possible side effects of the treatment, the effectiveness in their prevention and management, and the impact that prehabilitation may have in adherence to therapy and on the principal survival outcomes, providing important guidance for the planning of future studies., Evidences and Conclusions: Although there is no strong data evaluating multidisciplinary prehabilitation strategies, evidence shows that optimizing the patient's health status and preventing possible complications improve the QoL, reduce the incidence and severity of adverse events, and improve treatment adherence. While cardiology prehabilitation is of paramount importance for all patients undergoing concomitant CRT in the prevention of possible side effects, the remaining interventions are useful independently of the type of treatment proposed. Geriatricians have a key role in both elderly patients and younger patients characterized by many comorbidities to comprehensively assess health status and indicate which treatment may be the best in terms of risk/benefit ratio. Collaboration between nutritionists and phoniatrics, on the other hand, ensures adequate nutritional intake for the patient, where possible orally. This is because optimizing both body weight and muscle mass and qualities has been shown to impact key survival outcomes. Finally, HNSCC patients have the second highest suicide rate, and the disease has side effects such as pain, dysfiguration, and sialorrhea that can reduce the patient's social life and create shame and embarrassment: A psychological intake, in addition to the usefulness to the patient, can also provide current support to caregivers and family members. Therefore clinicians must define a personalized pathway for patients, considering the characteristics of the disease and the type of treatment proposed, to optimize health status and prevent possible side effects while also improving QoL and treatment adherence., (© 2024. 13. Stressful life events in pregnancy: A risk factor for exclusive breastfeeding among high-income mothers.

Abstract

Objective: The aim of this study was to examine the association between prenatal stressful life events in pregnancy and breastfeeding initiation, exclusivity, and duration, defined according to the WHO., Study Design: Feeding practices were prospectively collected from 425 puerperae living in an industrialized area of Northeast Italy starting on the second day post-partum, when they filled out the Life Experience Survey (LES, 1978)., Results: In this sample, analysis revealed that at discharge 65/358 (18.16 %) puerperae presented with a negative LES score and 293/358 (81.84 %) with a positive LES score. Puerperae with negative LES scores were more likely to adopt formula at discharge, either to complement breast milk or to substitute breast milk altogether (16/65; 24.62 % vs 43/293; 14.68 %; RR = 1.64 and 95 % CI: 1.01-2.70), and they were also more likely to adopt exclusive formula feeding at the 3rd month of life (8/48; 16.66 % versus 18/242; 7.44 %; RR = 2.03 and 95%CI: 1.06-3.86). At the 6th month of life, there were significantly less puerperae with a negative LES score who were breastfeeding exclusively (1/48; 2.08 vs 44/249; 17.67 %; RR = 0.11 and 95 % CI: 0.01-0.84) and correspondingly there were significantly more puerperae with a negative LES score who were using formula feeding with weaning practices (20/48; 41.67 % vs 35/249; 14.06 %; RR =3.14 and 95 % CI: 1.82-5.04)., Conclusions: Identification of specific barriers to breastfeeding related to negative life events in pregnancy may help direct providers' anticipatory guidance to improve breastfeeding rates in high-risk populations., (Copyright © 2022. Abstract

The use of the cone-in-cone connection to support definitive restorations was previously evaluated in cases involving full-acrylic resin or hybrid acrylic-resin composite prostheses. The aim of this study was to evaluate the performance of definitive fixed partial prostheses made with monolithic zirconia and supported by cone-in-cone abutments and integrated implants. Implants were placed into healed sites and fresh extraction sockets. The prostheses were placed in the posterior regions of partially edentulous patients after healing periods of 3 months. A total of 76 patients received fixed monolithic zirconia restorations splinted with cone-in-cone connections to two implants that were followed up yearly for 5 years. At each follow-up visit, peri-implant bone levels and pocket depths were recorded. Esthetic, functional, and biologic United States Public Health Services parameters modified by the World Dental Federation study design were assessed at the last follow-up appointments. The opposing dentition was categorized by type of restoration and supporting structure at the time of placement of the definitive zirconia partial restoration and at the 5-year follow-up. The treatment achieved an 88.2% success rate and a 97.4% survival rate at the 5-year follow-up. None of the prostheses became loose or detached. One fixed prosthesis (0.76%) fractured 41 months after placement. No significant difference involving peri-implant bone and probing levels between the experimental times was found. The results of this research indicated that abutment-prosthesis cone-in-cone connections were successful within the 5-year study period.

15. Evidence of Signs and Symptoms of Craniomandibular Disorders in Fibromyalgia Patients.

Abstract

Objective: The purpose of this study is to highlight the evidence of signs and symptoms of craniomandibular disorders (CMD) in patients suffering from fibromyalgia., Materials and Method: The study has been carried out from May 2011 to May 2015, recruiting a sample of fibromyalgia patients at the Department of Neurophysiopathology at the hospital Policlinico in Bari. Among the 150 examined patients, 60 of them have been diagnosed to suffer from fibromyalgia and 27 accepted to be investigated with a gnathologic examination at the Dental School at the University of Bari., Results: 24 patients (88.9%) were women and 3 (11.1%) men; from 26 to 66 years old (average age, 39). 14 patients (51.9%) were affected by primary fibromyalgia, the remaining 13 (48.1%) by secondary fibromyalgia, mainly associated with hypothyroidism (29.6%). VAS average score was about 8 ± 1.85. The frequency of pain was daily in 15 patients (55.6%); twice a week in 10 patients (37.03%) and a few times a month in 2 patients (7.4%). 11 patients (40.7%) attributed the onset of fibromyalgia to a specific instigating event. In addition, from the gnathologic anamnesis, 11 patients (40,7%) reported a painful symptom in the head-neck region, especially in the frontal region, in the neck, in the masseter muscle and ATM. VAS average score was 3.4 ± 2.8, significantly lower than the one referring to the fibromyalgia pain. The gnathological examination found CMD signs and symptoms in 18 patients (66.7%). Concerning the prevalence of CMD, in type I fibromyalgia, myofascial pain was more frequent (5 patients), whereas in type II fibromyalgia, what was more frequent was a dislocation with reduction (3 patients)., Conclusion: Based on clinic experience, we can affirm that some patients with CMD report pain in other regions. It is difficult to distinguish the CMD forms directly correlated to fibromyalgia from those engendered by parafunctional activities; hence the need is to resolve the fibromyalgia syndrome adopting a multidisciplinary approach.

Abstract

Introduction: An educational activity, aimed at highlighting the benefits of Mediterranean Diet, compared to less healthy eating patterns, can encourage the adoption and maintenance of a mindful approach to food choice. This is especially important when a progressive shift towards a non-Mediterranean dietary pattern can be observed, even in Mediterranean countries., Aim: To test a protocol aimed at increasing knowledge and motivation to embrace healthy eating habits and, engendering conscientious food choices, improve the prevention of non-communicable diseases., Methods: Employees were involved in educational activities focusing on a healthy Mediterranean diet and on the role played by extra-virgin olive oil, one of its key components. Food questionnaires were completed both before and after the educational and information activities, in order to assess changes in personal knowledge of and attitudes towards fat consumption., Results: Answers on dietary guidelines and fat properties were more accurate after the seminars. The results showed increased understanding of the properties of extra-virgin olive oil versus seed oil and a stronger tendency towards healthy food choices., Conclusions: Implementing preventive information and training strategies and tools in the workplace, can motivate a more mindful approach to food choice with the long-term goal of contribute to reducing non-communicable diseases.

Abstract

Hemophilia B is a rare X-linked recessive disorder with plasma factor IX (FIX) deficiency. 1-3% of patients treated with exogenous FIX-containing products develop inhibitors (i.e. polyclonal high affinity immunoglobulins) that neutralize the procoagulant activity of a specific coagulation factor. Although the incidence of inhibitors in hemophilia B patients is low, most are "high titer" and frequently associated with the development of severe allergic or anaphylactic reactions. Immune tolerance induction as a strategy for inhibitor eradication was first described in 1984. Unfortunately, the overall reported success of immune tolerance induction in FIX deficiency with inhibitors is approximately 25-40%.We report the case of a 2-year-old boy with hemophilia B severe FIX deficiency (<1%), inhibitor antibodies to FIX development, and a history of adverse reactions to FIX infusions, who underwent a successful desensitization and immune tolerance induction with a daily FIX infusion. With this regimen the inhibitor titer decreased with effective bleeding prevention.

18. Leiomyoma of oral cavity: case report and literature review.

Abstract

Leiomyoma is a benign smooth muscle tumour, that occurs most frequently in the uterine myometrium, gastrointestinal tract, skin and lower extremities of middle-aged women. Leiomyomas are uncommon in the oral cavity, but in this location are usually localized on the tongue, lips and palate. Most lesions are asymptomatic, although occasional tumours can be painful. The diagnosis is mainly determined by histological studies due to its unspecific clinical appearance.The purpose of this article is to present a case report of 49-year-old male patient with a lesion of the lower lip. After surgical resection hematoxylin-eosin staining confirmed the diagnosis of leiomyoma.
