Abstract

Background: While several scoring systems for the severity of anaphylactic reactions have been developed, there is a lack of consensus on definition and categorisation of severity of food allergy disease as a whole. Aim: To develop an international consensus on the severity of food allergy (DEfinition of Food Allergy Severity, DEFASE) scoring system, to be used globally. Methods: Phase 1: We conducted a mixed-method systematic review (SR) of 11 databases for published and unpublished literature on severity of food allergy management and set up a panel of international experts. Phase 2: Based on our findings in Phase 1, we drafted statements for a two-round modified electronic Delphi (e-Delphi) survey. A purposefully selected multidisciplinary international expert panel on food allergy (n = 60) was identified and sent a structured questionnaire, including a set of statements on different domains of food allergy severity related to symptoms, health-related quality of life, and economic impact. Participants were asked to score their agreement on each statement on a 5-point Likert scale ranging from “strongly agree” to “strongly disagree”. Median scores and percentage agreements were calculated. Consensus was defined a priori as being achieved if 70% or more of panel members rated a statement as “strongly agree” to “agree” after the second round. Based on feedback, 2 additional online voting rounds were conducted. Results: We received responses from 92% of Delphi panel members in round 1 and 85% in round 2. Consensus was achieved on the overall score and in all of the 5 specific key domains as essential components of the DEFASE score. Conclusions: The DEFASE score is the first comprehensive grading of food allergy severity that considers not only the severity of a single reaction, but the whole disease spectrum. An international consensus has been achieved regarding a scoring system for food allergy disease. It offers an evaluation grid, which may help to rate the severity of food allergy. Phase 3 will involve validating the scoring system in research settings, and implementing it in clinical practice.

La mastocytose systémique indolente (MSI), maladie clonale des mastocytes principalement induite par la mutation D816V du gène KIT, s’accompagne de symptômes invalidants à long terme, dont une anaphylaxie pouvant mettre la vie en danger, mais aussi une mauvaise qualité de vie (QdV) et une morbidité significative. La plupart des patients reçoivent des traitements axés sur les symptômes qui s’avèrent souvent inefficaces. L’avapritinib, un inhibiteur oral, puissant et sélectif de la mutation D816V de KIT, est le premier et le seul traitement approuvé pour les patients adultes atteints de MSI dans l’UE et aux États-Unis.

Abstract

Skin Biopsies as a Sensitive Non-Lethal Technique for the Ecotoxicological Studies of Great White Shark (Carcharodon carcharias) Sampled in South Africa Top-predators may be extremely vulnerable to environmental contaminants, such as organochlorines (OCs) and polycyclic aromatic hydrocarbons (PAHs), mostly because of their position in the trophic web. In this study, the use of skin biopsy is proposed as a sensitive non-lethal technique for the toxicological assessment of white shark (Carcharodon carcharias) living off the South African coasts. In 2012, 15 specimens of great white shark were sampled in the waters off Dyer Island and Geyser Rock. Then OCs and PAHs were extracted from muscle and biomarkers techniques for the evaluation of the cytochrome P4501A (CYP1A), Vitellogenin (Vtg) and Zona Radiata Proteins (Zrp) in the skin have been developed for the first time. The results showed levels of OCs higher than those found in the literature, ranging in ng/g dry weight (d.w.) from 6.80 to 21.26 for hexachlorobenzene (HCB), from 86.72 to 1416.97 for DDTs and from 379.76 to 11284.31 for polychlorinated biphenyls (PCBs).

Abstract

Objective To evaluate variations in laboratory parameters and diagnoses of selected clinical conditions up to 16 weeks after starting a new systemic psoriasis treatment for Psocare Registry enrollees. Design Prospective cohort study. Setting Italian public referral centres for psoriasis treatment. Patients First-time recipients (n = 10,539) of continuous systemic psoriasis treatment for at least 16 weeks. Main outcome measure Mean variations in (weeks 8 and 16) and proportions of patients reaching a clinically meaningful increase in serum levels (week 16) of total and low-density lipoprotein cholesterol, triglycerides, aspartate amino transferase, alanine amino transferase and creatinine, as well as week-16 cumulative incidences of new diagnoses of diabetes mellitus and arterial hypertension. Results Mean cholesterol and triglyceride levels significantly increased in patients treated with acitretin or cyclosporine. Mean triglyceride levels also increased in efalizumab- and etanercept-treated patients. Mean transaminase values increased in methotrexate-treated patients, and mean aspartate amino transferase levels increased in infliximab-treated patients. The average serum creatinine value increased in cyclosporine-treated patients. Acitretin and cyclosporine were associated with risk of hypercholesterolaemia (odds ratios 1.51 and 1.34) and acitretin with risk of hypertriglyceridaemia (odds ratio 1.43). Methotrexate and infliximab were associated with risk of more than doubling the upper normal aspartate amino transferase (odds ratios 2.06 and 1.87) and alanine amino transferase (odds ratios 2.38 and 1.74) values. The relative risk of developing arterial hypertension and diabetes was increased for patients receiving cyclosporine (odds ratios 3.31 and 2.88). Conclusion Systemic treatments for psoriasis resulted in heterogeneous effects on the parameters analysed.
