- Abstract
MECP2 mutations cause Rett syndrome (RTT), a severe and progressive neurodevelopmental disorder mainly affecting females. Although RTT patients exhibit delayed onset of symptoms, several evidences demonstrate that MeCP2 deficiency alters early development of the brain. Indeed, during early maturation, Mecp2 null cortical neurons display widespread transcriptional changes, reduced activity, and defective morphology. It has been proposed that during brain development these elements are linked in a feed-forward cycle where neuronal activity drives transcriptional and morphological changes that further increase network maturity. We hypothesized that the enhancement of neuronal activity during early maturation might prevent the onset of RTT-typical molecular and cellular phenotypes. Accordingly, we show that the enhancement of excitability, obtained by adding to neuronal cultures Ampakine CX546, rescues transcription of several genes, neuronal morphology, and responsiveness to stimuli. Greater effects are achieved in response to earlier treatments. In vivo, short and early administration of CX546 to Mecp2 null mice prolongs lifespan, delays the disease progression, and rescues motor abilities and spatial memory, thus confirming the value for RTT of an early restoration of neuronal activity.

To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause.Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms.Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year.Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women agedor = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73).This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

OBJECTIVE To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

- Abstract
Nei Paesi dell'Europa occidentale le neoplasie sia per la loro natura cronica sia per la frequenza con cui si presentano nella popolazione sono tra le patologie che hanno un impatto più rilevante sul sistema sanitario. I pazienti oncologici prevalenti (ovverosia persone diagnosticate con un tumore e sopravviventi ad una certa data) sono in numero crescente, questo dato racchiude in sé una serie di aspetti, alcuni dei quali da interpretare positivamente (1). Il numero di diagnosi di tumore aumenta anche per effetto della diffusione sul territorio di programmi di screening e di un'accuratezza diagnostica crescente, inoltre la speranza di vita di chi scopre di avere un tumore aumenta di pari passo con la disponibilità crescente di trattamenti specifici, efficaci, con effetti collaterali in molti casi moderati o assenti. A tutto questo si accompagnano tuttavia l'aumento della popolazione anziana, più colpita dalle patologie neoplastiche, e il proliferare di farmaci sempre più mirati a curare singoli soggetti, con costi molto elevati. In questo panorama, comune anche all'Italia, diventa strategica l'individuazione dei bisogni sanitari relativi ai pazienti oncologici prevalenti, al fine di una corretta pianificazione dell'offerta sanitaria sul territorio. La conoscenza della popolazione prevalente, non solo del suo ammontare complessivo, ma anche della sua distribuzione rispetto al percorso di malattia, permette di valutarne le aspettative in termini di assistenza sanitaria diretta sul territorio e delle risorse necessarie per soddisfarla. In un sistema sanitario come quello italiano, caratterizzato da una centralità delle scelte strategiche di fondo e da un decentramento alle Regioni della gestione e del finanziamento dei servizi erogati al cittadino, è particolarmente importante che le autorità locali dispongano di strumenti per realizzare una corretta programmazione sanitaria. La linea di ricerca che qui viene illustrata ha per obiettivi: la valutazione dei percorsi diagnostico-terapeutici dei pazienti oncologici dalla diagnosi lungo l'intero percorso di malattia; la quantificazione delle risorse richieste ai sistemi sanitari locali per soddisfare i bisogni sanitari dei pazienti oncologici; la stima delle ricadute in termini di risorse impiegate e benefici ricevuti dai pazienti a seguito di specifici interventi di prevenzione (ad esempio programmi di screening) o di cura (ad esempio nuovi farmaci chemioterapici). L'idea è quella di fornire uno strumento utile all'analisi e alla pianificazione delle risorse sanitarie da destinare all'oncologia, di cui le autorità sanitarie si possano servire anche ai fini predittivi oltre che per la valutazione delle risorse correnti impiegate.

- Abstract
Two thousand one hundred and thirteen strains of enterococci isolated at Pisa General Hospital in 1998 were analysed retrospectively to determine their glycopeptide resistance. Of all the microorganisms isolated in this period, 14.7% were enterococci (1405 Enterococcus faecalis, 19 Enterococcus faecium, six Enterococcus avium and 683 Enterococcus spp.). Two hundred and thirty (10.8%) of these enterococci were resistant or demonstrated reduced susceptibility to vancomycin and/or teicoplanin. The highest rate of resistance was found in outpatient enterococcal strains isolated from the urogenital tract. The frequency of enterococcal glycopeptide resistance at Pisa Hospital is higher than that reported from other areas of Italy. [ABSTRACT FROM AUTHOR]

The authors report their experience in the arthroscopic treatment of joint-related stiffness of the knee. Arthroscopic arthrolysis is the treatment of choice for numerous conditions (stiffness subsequent to inflammatory processes, stiffness associated with marked osteoporosis, etc.) in which open arthrolysis is contraindicated. The totally atraumatic nature of arthroscopy and the more accurate lysis of the adhesions allow for postoperative management without complications or pain. This is of particular importance as it permits passive and active kinesitherapy from the moment the patient recovers from anaesthesia, without pain and therefore with greater commitment on his part. Furthermore, arthroscopic arthrolysis can be used successfully to treat extensor muscle stiffness where endoscopic study has revealed the presence of adhesions or calcification in the anterior compartments.

MECP2 mutations cause Rett syndrome (RTT), a severe and progressive neurodevelopmental disorder mainly affecting females. Although RTT patients exhibit delayed onset of symptoms, several evidences demonstrate that MeCP2 deficiency alters early development of the brain. Indeed, during early maturation, Mecp2 null cortical neurons display widespread transcriptional changes, reduced activity, and defective morphology. It has been proposed that during brain development these elements are linked in a feed‐forward cycle where neuronal activity drives transcriptional and morphological changes that further increase network maturity. We hypothesized that the enhancement of neuronal activity during early maturation might prevent the onset of RTT‐typical molecular and cellular phenotypes. Accordingly, we show that the enhancement of excitability, obtained by adding to neuronal cultures Ampakine CX546, rescues transcription of several genes, neuronal morphology, and responsiveness to stimuli. Greater effects are achieved in response to earlier treatments. In vivo, short and early administration of CX546 to Mecp2 null mice prolongs lifespan, delays the disease progression, and rescues motor abilities and spatial memory, thus confirming the value for RTT of an early restoration of neuronal activity., Neuronal activity drives transcriptional and morphological changes that ensure maturation. Such mechanism is affected by Mecp2 absence. We show the rescue effects produced by enhancing Mecp2 null neurons activity and propose new therapeutic time windows for the treatment of Rett syndrome.

Le indagini archeologiche ne ipotizzano altresì l'abbandono, avvenuto presumibilmente alla metà/fine del XVI secolo.

In order to assess the usefulness of amniotic fluid α-fetoprotein (AFP) levels at the time of midtrimester genetic amniocentesis, 4,430 cases were retrospectively studied to compare the high, normal or low AFP values with the karyotype characteristics and fetal anatomy using ultrasound (US) scanning and confirmed by postnatal evaluation or necroscopy in the case of termination of pregnancy. All the cases presenting malformations were correctly diagnosed by US examinations. AFP levels over the 2nd standard deviation (SD) were found in 112 cases (2.52%) and below the 2nd SD in 11 cases (0.24%). The characteristics of these cases are presented and discussed. According to our results, it is concluded that routine assessment of AFP at the time of midtrimester genetic amniocentesis, if coupled with optimal US scanning, is no longer justified.

We demonstrate optically tunable control of second-harmonic generation in all-dielectric nanoantennas: by using a control beam that is absorbed by the nanoresonator, we thermo-optically change the refractive index of the radiating element to modulate the amplitude of the second-harmonic signal. For a moderate temperature increase of roughly 40 K, modulation of the efficiency up to 60% is demonstrated; this large tunability of the single meta-atom response paves the way to exciting avenues for reconfigurable homogeneous and heterogeneous metasurfaces.

45. Wave optical model for tomographic volumetric additive manufacturing.

- Abstract
Tomographic Volumetric Additive Manufacturing (TVAM) allows printing of mesoscopic objects within seconds or minutes. In TVAM, tomographic patterns are illuminated onto a rotating glass vial which contains a photosensitive resin. Current pattern optimization is based on a ray optical assumption which ultimately leads to limited resolution around 20 µm and varying throughout the volume of the 3D object. In this work, we introduce a rigorous wave-based optical amplitude optimization scheme for TVAM which shows that high-resolution printing is theoretically possible over the full volume. The wave optical optimization approach is based on an efficient angular spectrum method of plane waves with custom written memory efficient gradients and allows for optimization of realistic volumes for TVAM such as (100 μ m)
3 or (10 mm)3 with 5503 voxels and 600 angles. Our simulations show that ray-optics start to produce artifacts when the desired features are 20 µm and below and more importantly, the amplitude modulated TVAM can reach sub 20 µm features when optimizing the patterns using a full wave model.- Published

- Abstract
Pioneer transcription factors are thought to play pivotal roles in developmental processes by binding nucleosomal DNA to activate gene expression, though mechanisms through which pioneer transcription factors remodel chromatin remain unclear. Here, using single-cell transcriptomics, we show that endogenous expression of neurogenic transcription factor ASCL1, considered a classical pioneer factor, defines a transient population of progenitors in human neural differentiation. Testing ASCL1's pioneer function using a knockout model to define the unbound state, we found that endogenous expression of ASCL1 drives progenitor differentiation by cis -regulation both as a classical pioneer factor and as a nonpioneer remodeler, where ASCL1 binds permissive chromatin to induce chromatin conformation changes. ASCL1 interacts with BAF SWI/SNF chromatin remodeling complexes, primarily at targets where it acts as a nonpioneer factor, and we provide evidence for codependent DNA binding and remodeling at a subset of ASCL1 and SWI/SNF cotargets. Our findings provide new insights into ASCL1 function regulating activation of long-range regulatory elements in human neurogenesis and uncover a novel mechanism of its chromatin remodeling function codependent on partner ATPase activity., (© 2023 Păun et al.; Published by Cold Spring Harbor Laboratory Press.)

- Abstract
The prospect of massive parallelism of optics enabling fast and low energy cost operations is attracting interest for novel photonic circuits where 3-dimensional (3D) implementations have a high potential for scalability. Since the technology for data input-output channels is 2-dimensional (2D), there is an unavoidable need to take 2D-nD transformations into account. Similarly, the 3D-2D and its reverse transformations are also tackled in a variety of fields such as optical tomography, additive manufacturing, and 3D optical memories. Here, we review how these 3D-2D transformations are tackled using iterative techniques and neural networks. 48. Retrospective analysis of the diagnostic accuracy of lung ultrasound for pulmonary embolism in patients with and without pleuritic chest pain.

- Abstract
Background: Lung ultrasound (LUS) has a role in the diagnosis of pulmonary embolism (PE) mainly based on the visualization of pulmonary infarctions. However, examining the whole chest to detect small peripheral infarctions by LUS may be challenging. Pleuritic pain, a frequent presenting symptom in patients with PE, is usually localized in a restricted chest area identified by the patient itself. Our hypothesis is that sensitivity of LUS for PE in patients with pleuritic chest pain may be higher due to the possibility of focusing the examination in the painful area. We combined data from three prospective studies on LUS in patients suspected of PE and extracted data regarding patients with and without pleuritic pain at presentation to compare the performances of LUS., Results: Out of 872 patients suspected of PE, 217 (24.9%) presented with pleuritic pain and 279 patients (32%) were diagnosed with PE. Pooled sensitivity of LUS for PE in patients with and without pleuritic chest pain was 81.5% (95% CI 70-90.1%) and 49.5% (95% CI 42.7-56.4%) (p < 0.001), respectively. Specificity of LUS was similar in the two groups, respectively 95.4% (95% CI 90.7-98.1%) and 94.8% (95% CI 92.3-97.7%) (p = 0.86). In patients with pleuritic pain, a diagnostic strategy combining Wells score with LUS performed better both in terms of sensitivity (93%, 95% CI 80.9-98.5% vs 90.7%, 95% CI 77.9-97.4%) and negative predictive value (96.2%, 95% CI 89.6-98.7% vs 93.3%, 95% CI 84.4-97.3%). Efficiency of Wells score + LUS outperformed the conventional strategy based on Wells score + d-dimer (56.7%, 95% CI 48.5-65% vs 42.5%, 95% CI 34.3-51.2%, p = 0.02)., Conclusions: In a population of patients suspected of PE, LUS showed better sensitivity for the diagnosis of PE when applied to the subgroup with pleuritic chest pain. In these patients, a diagnostic strategy based on Wells score and LUS performed better to exclude PE than the conventional strategy combining Wells score and d-dimer., (© 2022. The Author(s).)

- Abstract
Purpose: Diverticulitis is a common cause of abdominal pain and CT scan is commonly used for its diagnosis in the emergency department (ED). The diagnostic performance of point-of-care ultrasound (POCUS) integrated into a clinical exam for diverticulitis is still not established. We evaluate the accuracy of clinical-sonographic assessment for the diagnosis of diverticulitis and whether POCUS could improve the selection of patients needing CT scan for complicated diverticulitis., Materials and Methods: This is a multicentric observational study involving adult patients suspected of having diverticulitis presenting at 4 EDs. 21 sonographer physicians were asked to diagnose diverticulitis and complicated diverticulitis based on clinical-sonographic assessment. The final diagnosis was established by two reviewers, blinded to POCUS, based on data collected during the one-month follow-up comprehensive CT scan., Results: Among 393 enrolled patients, 218 (55.5 %) were diagnosed with diverticulitis and 33 (8 %) had complicated diverticulitis. The time to diagnosis by the sonographer physicians was shorter compared to standard care (97 ± 102 vs. 330 ± 319 minutes, p < 0.001). Clinical-sonographic assessment showed optimal sensitivity (92.7 %) and specificity (90.9 %) for diverticulitis. However, the sensitivity (50 %) for complicated diverticulitis was low. The sonographer physician would have proceeded to CT scan in 194 (49.4 %) patients and the CT scan request compared to the final diagnosis of complicated diverticulitis demonstrated 94 % sensitivity., Conclusion: Clinical-sonographic assessment is rapid and accurate for the diagnosis of diverticulitis. 50. Opto-thermally controlled beam steering in nonlinear all-dielectric metastructures.

- Abstract
Reconfigurable metasurfaces have recently gained a lot of attention in applications such as adaptive meta-lenses, hyperspectral imaging and optical modulation. This kind of metastructure can be obtained by an external control signal, enabling us to dynamically manipulate the electromagnetic radiation. Here, we theoretically propose an AlGaAs device to control the second harmonic generation (SHG) emission at nanoscale upon optimized optical heating. The asymmetric shape of the used meta-atom is selected to guarantee a predominant second harmonic (SH) emission towards the normal direction. The proposed structure is concurrently excited by a pump beam at a fundamental wavelength of 1540 nm and by a continuous wave (CW) control signal above the semiconductor band gap. The optical tuning is achieved by a selective optimization of meta-atoms SH phase, which is modulated by the control signal intensity. We numerically demonstrate that the heating induced in the meta-atoms by the CW pump can be used to dynamically tune the device properties. In particular, we theoretically demonstrate a SH beam steering of 8° with respect to the vertical axis for an optimized device with average temperature increase even below 90° C.
