3. Immune response in breast cancer surgery. A comparative and prospective study of different anesthetic techniques

The main reason for high mortality in breast cancer is local recurrence and metastasis, despite surgery as the first therapeutic option. The anesthesia used in the operation room can determine the immune response.A prospective, comparative and non- randomised study in patients undergoing breast cancer surgery was conducted in our hospital after obtaining approval from the Hospital's Institutional Review Board. Patients were divided in two groups: Group A received general anesthesia with propofol and opioids. Group B, in addition to general anesthesia, three interfascial blocks (Pec I, Pec II and BRILMA) were performed in all patients. Three blood samples were taken 1) previous anesthetic induction; 2) two hours after the end of the surgery and 3) 24-48 h after surgery. Leukocytes, CD3, CD4, CD8 and Natural Killer cells were determined at each time.103 patients were included. 59 (group A) received general anesthesia and 54 (group B) general anesthesia and interfascial blocks. Regarding baseline characteristics, age was significantly higher in the group that received general anesthesia and mastectomy was more frequent in the group that received interfascial blocks. We observed after surgery an increase in leukocytes level that returns close to baseline levels. On the other hand, a reduction in the immune response was observed that also returns to the previous level 48 h after surgery. Group A and B get similar results and also subgroups of hormonal receptors (HER+, PR and/or ER+).Interfascial blocks in chest wall added to general anesthesia in breast cancer surgery has not shown a significant difference in the inflammatory response or immunological depression compared to general anesthesia as the only anesthetic technique. It seems to trend less immunological depression in the interfascial block group.

The pancreas is a complex organ consisting of differentiated cells and extracellular matrix (ECM) organized adequately to enable its endocrine and exocrine functions. Although much is known about the intrinsic factors that control pancreas development, very few studies have focused on the microenvironment surrounding pancreatic cells. This environment is composed of various cells and ECM components, which play a critical role in maintaining tissue organization and homeostasis. In this study, we applied mass spectrometry to identify and quantify the ECM composition of the developing pancreas at the embryonic (E) day 14.5 and postnatal (P) day 1 stages. Our proteomic analysis identified 160 ECM proteins that displayed a dynamic expression profile with a shift in collagens and proteoglycans. Furthermore, we used atomic force microscopy to measure the biomechanical properties and found that the pancreatic ECM was soft (≤400 Pa) with no significant change during pancreas maturation. Lastly, we optimized a decellularization protocol for P1 pancreatic tissues, incorporating a preliminary crosslinking step, which effectively preserved the 3D organization of the ECM. The resulting ECM scaffold proved suitable for recellularization studies. Our findings provide insights into the composition and biomechanics of the pancreatic embryonic and perinatal ECM, offering a foundation for future studies investigating the dynamic interactions between the ECM and pancreatic cells.

Aims To evaluate whether the addition of hydrochlorothiazide (HCTZ) to intravenous furosemide is a safe and effective strategy for improving diuretic response in acute heart failure (AHF). Methods and results A prospective, double-blind, placebo-controlled trial, including patients with AHF randomized to receive HCTZ or placebo in addition to an intravenous furosemide regimen. The coprimary endpoints were changes in body weight and patient-reported dyspnoea 72 h after randomization. Secondary outcomes included metrics of diuretic response and mortality/rehospitalizations at 30 and 90 days. Safety outcomes (changes in renal function and/or electrolytes) were also assessed. Two hundred and thirty patients (48 women, 83 years) were randomized. Patients assigned to HCTZ were more likely to lose weight at 72 h than those assigned to placebo [2.3 vs. 1.5 kg; adjusted estimated difference (notionally 95 confidence interval) 1.14 (1.84 to 0.42); P 0.002], but there were no significant differences in patient-reported dyspnoea (area under the curve for visual analogue scale: 960 vs. 720; P 0.497). These results were similar 96 h after randomization. Patients allocated to HCTZ showed greater 24 h diuresis (1775 vs. 1400 mL; P 0.05) and weight loss for each 40 mg of furosemide (at 72 and at 96 h) (P 0.001). Patients assigned to HCTZ more frequently presented impaired renal function (increase in creatinine 26.5 moL/L or decrease in eGFR 50; 46.5 vs. 17.2; P 0.001), but hypokalaemia and hypokalaemia were similar between groups. There were no differences in mortality or rehospitalizations. Conclusion The addition of HCTZ to loop diuretic therapy improved diuretic response in patients with AHF. [ABSTRACT FROM AUTHOR]

[ES] Actualmente la esperanza de vida en la población mundial es mucho más elevada respecto a años atrás. Este fenómeno tiene diversas causas entre las que cabe citar los avances en el campo de la medicina, las mejoras de los hábitos alimenticios, así como un aumento en la práctica de deporte. Estos son factores fundamentales para prevenir la aparición de enfermedades, de las cuales, algunas de ellas, hace no mucho eran mortales para quienes las padecían. Toda la información existente en el siglo XXI respecto a la curación de patologías está ligada a la profesionalización en el dominio de la investigación científica y los constantes descubrimientos realizados por profesionales, expertos e investigadores de este sector, tales como médicos, genetistas o biólogos. Es esencial para el entendimiento de este trabajo valorar que un volumen considerable de los progresos biológicos en los últimos años ha ido íntimamente ligado a la informática y ciencias de la computación. En este sentido, biología e informática se han visto unidos en múltiples ocasiones, desde la posibilidad de secuenciar el genoma humano hasta la posibilidad de identificar precozmente ciertas enfermedades, hasta dar respuesta a preguntas vitales de la humanidad tales como: ¿de dónde venimos?, ¿quiénes son nuestros antepasados?, etc. Para poder llegar a conocer las respuestas a estas cuestiones que nos han acompañado millones de años, el poder de los datos y el conocimiento que aportan serán imprescindibles. Para alcanzar la posibilidad de manejar el gran volumen de datos genómicos es necesario utilizar técnicas de ciencias de datos, las cuales faciliten algunas tareas como la limpieza de datos, o la integración de los mismos. El fin último es obtener el conocimiento necesario, para llegar en un futuro a tener la opción de brindar a la población el tratamiento de las enfermedades mediante el uso de la Medicina de Precisión, la cual pretende adaptar el tratamiento médico a las características individuales de cada paciente. Llegar a este punto solo es posible si se consigue el gran valor de toda la información existente en el inmenso conjunto de fuentes genómicas. Todo lo mencionado previamente conforman los pilares fundamentales de este Trabajo Fin de Máster, cuyo objetivo principal es poder determinar aquellas variaciones genómicas que se asocian con el neuroblastoma (tipo de cáncer infantil), haciendo uso de la Metodología experimental SILE (Search, Identification, Load and Exploitation), así como del modelo conceptual del genoma humano para tener una visión global de todos los conceptos que abarcan este dominio. El factor innovador de este proyecto es el empleo de ciertas herramientas y técnicas de ciencias de datos para el manejo del gran volumen de información existentes. This phenomenon has several causes, including advances in the field of medicine, improvements in eating habits, as well as an increase in the practice of sport. These are key factors in preventing the onset of diseases, some of which were not long ago fatal to those suffering from them. All the information available in the 21st century regarding the cure of pathologies is linked to the professionalisation in the field of scientific research and the constant discoveries made by professionals, experts and researchers in this sector, such as doctors, geneticists or biologists. It is essential for the understanding of this work to appreciate that a considerable volume of biological progress in recent years has been intimately linked to computer science and computing. In this sense, biology and informatics have been united on multiple occasions, from the possibility of sequencing the human genome to the possibility of early identification of certain diseases, to answering vital questions of humanity such as: where do we come from, who are our ancestors, etc. In order to get to know the answers to these questions that have accompanied us for millions of years, the power of the data and the knowledge they provide will be essential. To achieve the possibility of handling the large volume of genomic data it is necessary to use data science techniques, which facilitate some tasks such as data cleansing, or the integration of them. The ultimate goal is to obtain the necessary knowledge, to reach in the future to have the option of offering the population the treatment of diseases through the use of the Precision Medicine, which aims to adapt the medical treatment to the individual characteristics of each patient. Reaching this point is only possible if the great value of all the existing information in the immense set of genomic sources is achieved. All of the above constitute the fundamental pillars of this End of Master Work, whose main objective is to determine those genomic variations associated with neuroblastoma (type of childhood cancer), making use of the SILE experimental methodology (Search, Identification, Load and Exploitation), as well as the conceptual model of the human genome to have a global vision of all the concepts that cover this domain. The innovative factor of this project is the use of certain data science tools and techniques for handling the large volume of existing information. On the other hand, there will also be a critical evaluation of the methodology used.

- Abstract
Background: The prevalence of childhood overweight is increasing worldwide. The human intestinal microbiota is composed of about 100 trillion cells essential for proper body growth, the development of immunity and nutrition. The pur-pose of this study was to evaluate the possible relationship between the risk factors that alter the microbiota (Cesarean delivery, breastfeeding in the first six months of life and taking antibiotics during the first three years) and the development of overweight child in children born in 2006 at the hospital Bishop Polanco in Teruel.Methods: We designed case-control study with a sample of 118 children of 12 years of age who fulfilled the inclusion criteria.Results: Not taking antibiotics before three years of age is a risk factor for the development of childhood overweight in the studied popu-lation (X2=4,374. p=0,036. OR=2,764 IC95%: 1,042- 7,334). Showed no positive association the birth by caesarean section (X2=6,020. p=0,014. OR=0,272 IC95%: 0,092-0,806) and breastfeeding in the first six months of life (X2=0,160. p=0,689. OR=0,852 IC95%:0,389-1,868).Conclusion: We conclude that the three pos-sible analyzed factors that alter the microbiota, they don’t show relationship association with overweight in children born in 2006 in the hospi-tal Bishop Polanco in Teruel. It would be neces-sary to develop similar studies with larger sample sizes, to assess whether there is an association between the three factors analyzed and childhood overweight., Introducción: La prevalencia de sobrepe-so infantil está aumentando a nivel mundial. La microbiota intestinal humana está formada por unos 100 billones de células indispensables para el correcto crecimiento corporal, el desarrollo de la inmunidad y la nutrición. El objetivo de este es-tudio es evaluar la posible relación entre los fac-tores de riesgo que alteran la microbiota (parto por cesárea, tipo de lactancia en los seis primeros meses de vida y toma de antibióticos durante los tres primeros años) y el desarrollo de sobrepeso infantil en los niños nacidos en el año 2006 en el hospital Obispo Polanco de Teruel.Metodología: Se diseñó un estudio casos-controles con una muestra de 118 niños de 12 años de edad que cumplían los criterios de inclusión establecidos.Resultados: No tomar antibióticos antes de los tres años de edad constituye un factor de ries-go para el desarrollo de sobrepeso infantil en la población estudiada (X2=4,374. p=0,036. OR=2,764 IC95%: 1,042- 7,334). No mostraron asociación po-sitiva el nacimiento por cesárea (X2=6,020. p=0,014. OR=0,272 IC95%: 0,092-0,806) y la lactancia mater-na en los seis primeros meses de vida (X2=0,160. p=0,689. OR=0,852 IC95%:0,389-1,868).Conclusión: Podemos concluir que los tres posibles factores analizados que alteran la micro-biota, no muestran relación de asociación signi-ficativa con el sobrepeso en los niños nacidos en el 2006 en el hospital Obispo Polanco de Teruel. Sería necesario desarrollar estudios similares con tamaños de muestra más grandes, para evaluar si existe una asociación entre los tres factores ana-lizados y el sobrepeso infantil

Trabajo de Fin de Máster del Máster en Estudios Avanzados sobre el Lenguaje, la Comunicación y sus Patologías. Curso 2016-2017., [ES] En el presente trabajo se ha llevado a cabo el diseño de un programa de intervención el cual ha sido implantado posteriormente a modo de estudio piloto, utilizando para ello una metodología de tipo cualitativa. Dicho programa busca obtener mejoras sobre la conciencia morfológica, así como en otras habilidades metalingüísticas como son la conciencia fonológica o la semántica en un escolar concreto con dificultades de aprendizaje de la lectura. El alumno tiene una edad cronológica de 9 años y se encuentra escolarizado en 3º de Educación Primaria en un colegio de la provincia de Salamanca. Este proceso ha sido evaluado a través de distintas pruebas estandarizadas como son el Tale, la Prueba de Eficiencia lectora (PEL) y pruebas que evalúan la conciencia morfológica, la conciencia fonológica y la silábica. Los resultados obtenidos evidencian una ligera mejora en las habilidades metalingüísticas mencionadas, ya que se constatan un menor número de errores de tipo natural en la evaluación post-test. [EN]In the present work has been carried out the design of an intervention program which has been subsequently implemented as a pilot study, using a qualitative type methodology. This program seeks to obtain improvements on morphological awareness, as well as other metalinguistic abilities such as phonological awareness or semantics in a specific pupil with learning difficulties in reading. The student has a chronological age of 9 years and he is enrolled in 3º of Primary Education in a school in the province of Salamanca. This process has been evaluated through different standardized tests such as Tale, Reading Efficiency Test (PEL) and tests that evaluate morphological awareness, phonological awareness and syllabic awareness. The obtained results evidenced a slight improvement in the metalinguistic abilities mentioned, due to the fact that a smaller number of natural type errors are verified in the post-test evaluation.

- Abstract
Introduction: SARS-CoV-2 viral load has been related to COVID-19 severity. The main aim of this study was to evaluate the relationship between SARS-CoV-2 viremia and SNPs in genes previously studied by our group as predictors of COVID-19 severity., Materials and Methods: Retrospective observational study including 340 patients hospitalized for COVID-19 in the University Hospital La Princesa between March 2020 and December 2021, with at least one viremia determination. Positive viremia was considered when viral load was above the quantifiable threshold (20 copies/ml). A total of 38 SNPs were genotyped. To study their association with viremia a multivariate logistic regression was performed., Results: The mean age of the studied population was 64.5 years (SD 16.6), 60.9% patients were male and 79.4% white non-Hispanic. Only 126 patients (37.1%) had at least one positive viremia. After adjustment by confounders, the presence of the minor alleles of rs2071746 ( HMOX1 ; T/T genotype OR 9.9 p < 0.0001), rs78958998 (probably associated with SERPING1 expression; A/T genotype OR 2.3, p = 0.04 and T/T genotype OR 12.9, p < 0.0001), and rs713400 (eQTL for TMPRSS2 ; C/T + T/T genotype OR 1.86, p = 0.10) were associated with higher risk of viremia, whereas the minor alleles of rs11052877 ( CD69 ; A/G genotype OR 0.5, p = 0.04 and G/G genotype OR 0.3, p = 0.01), rs2660 ( OAS1 ; A/G genotype OR 0.6, p = 0.08), rs896 ( VIPR1 ; T/T genotype OR 0.4, p = 0.02) and rs33980500 ( TRAF3IP2 ; C/T + T/T genotype OR 0.3, p = 0.01) were associated with lower risk of viremia., Conclusion: Genetic variants in HMOX1 (rs2071746), SERPING1 (rs78958998), TMPRSS2 (rs713400), CD69 (rs11052877), TRAF3IP2 (rs33980500), OAS1 (rs2660) and VIPR1 (rs896) could explain heterogeneity in SARS-CoV-2 viremia in our population., Competing Interests: FA, has been consultant or investigator in clinical trials sponsored by the following pharmaceutical companies: Abbott, Alter, Aptatargets, Chemo, FAES, Farmalíder, Ferrer, Galenicum, GlaxoSmithKline, Gilead, Italfarmaco, Janssen-Cilag, Kern, Normon, Novartis, Servier, Teva and Zambon. - Abstract
Aims: To evaluate whether the addition of hydrochlorothiazide (HCTZ) to intravenous furosemide is a safe and effective strategy for improving diuretic response in acute heart failure (AHF)., Methods and Results: A prospective, double-blind, placebo-controlled trial, including patients with AHF randomized to receive HCTZ or placebo in addition to an intravenous furosemide regimen. The coprimary endpoints were changes in body weight and patient-reported dyspnoea 72 h after randomization. Secondary outcomes included metrics of diuretic response and mortality/rehospitalizations at 30 and 90 days. Safety outcomes (changes in renal function and/or electrolytes) were also assessed. Two hundred and thirty patients (48 women, 83 years) were randomized. Patients assigned to HCTZ were more likely to lose weight at 72 h than those assigned to placebo [2.3 vs. 1.5 kg; adjusted estimated difference (notionally 95 confidence interval) 1.14 (1.84 to 0.42); P 0.002], but there were no significant differences in patient-reported dyspnoea (area under the curve for visual analogue scale: 960 vs. 720; P 0.497). These results were similar 96 h after randomization. Patients allocated to HCTZ showed greater 24 h diuresis (1775 vs. 1400 mL; P 0.05) and weight loss for each 40 mg of furosemide (at 72 and at 96 h) (P 0.001). Patients assigned to HCTZ more frequently presented impaired renal function (increase in creatinine 26.5 moL/L or decrease in eGFR 50; 46.5 vs. 17.2; P 0.001), but hypokalaemia and hypokalaemia were similar between groups. - Abstract
The aim of the present study is to confirm that being born SGA is a serious risk for a negative neurocognitive development. 233 cases have been controlled yearly and longitudinally by the same investigator, some of them 11 times, showing 25,8 % an IQ less than 2 SD, being less affected the catch-up + group (15 %), compared to the catch-up - group (31,4 %). The GH therapy (n 64) started before the age of 6 (n 38) or after 6 (n 26), doesn't improve the negative outcome.

- Abstract
The cardiovascular diseases represent the chief cause of morbidity and mortality in Spain. The development of renal insufficiency has a definite prognostic value and modifies therapeutic options. Cardio-renal insufficiency (IC-R) is a differentiated entity, underestimated in our population, in which both cardiac and renal failure coexist. Patients with IC-R are genuine survivors of the application of diverse technologies and have a different hemodynamic and hydrosaline balance compared to that seen in patients with isolated cardiac or renal insufficiency. Early diagnosis and careful follow-up, both in hospital and at ambulatory level, along with a specific management, determine the prognosis of this entity. A key aspect in the control of these patients resides in the preservation of a delicate fluid balance, which is always individualized and personalized. This type of approach requires a particular type of training and mental attitude by healthcare professionals, with an integrated view of the multifactorial nature of IC-R. The design of a plan structured according to objectives and the full understanding of the individual equilibrium constitute the keystone in the management of IC-R. The present review, which is based on traditional concepts updated under a practical view, is aimed to call attention on a rapidly expanding clinical entitiy.
