Abstract

Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Abstract

This paper presents a direct load control based demand side management (DSM) algorithm that performs peak shaving considering time-varying renewable generation, and thermal comfort of the buildings. The demand side operator of the microgrid (MG) uses the DSM algorithm for peak-shaving, and reducing the energy costs. The DSM controller has a hierarchical control architecture, wherein there is a central controller (CC) and numerous local controllers (LCs). The CC uses information on demand and renewable generation to compute the load to be curtailed. The LCs that supply the consumers, reduce the demand by curtailing heating loads in buildings without breaching thermal comfort limits. Building models, information on comfort margins, and an optimization routine are used by the LCs to implement the DSM algorithm. As the algorithm guarantees thermal comfort, reluctance among consumers to employ direct load control based DSM algorithm is eliminated. Further, in the proposed algorithm demand side operator controls the consumption by monitoring the temperature, therefore need to instal smart thermostats/controllers, and continuous monitoring of prices in buildings is eliminated. The working of the DSM algorithm is illustrated using simulations performed on data obtained from residential heating system with 50 buildings in Norwegian living lab in Steinkjer. Our results indicate that the algorithm performs peak-shaving considering information on renewable generation without breaching thermal comfort margins of the consumer.

Abstract

This is a contribution to the economic dispatch problem of combined electrical and heat power microgrids. A mixed integer linear microgrid model has been developed; the microgrid operations optimization problem has been formulated using Mixed-Integer Linear Programming and Model Predictive Control technique has been applied to take system uncertainties into account. The proposed optimization algorithm has been applied to a tertiary site microgrid, located in Finland; the obtained numerical results have been compared with a heuristic algorithm.

Abstract

This paper focuses on the optimal operations of an industrial power plant equipped with a Combined Cooling, Heat and Power (CCHP). The goal of the control strategy is to minimize the generation and maintenance costs of the power plant, scheduling the CCHP's operations, the usage of the auxiliary generators (used to meet the demand when the CCHP is turned OFF), the purchasing/selling phase from/to the main grid and integrating the renewable energy sources. The overall problem is stated as a constrained mixed integer linear optimization problem with both continuous and logical variables. A Model Predictive Control (MPC) approach is used to compensate forecasts uncertainties in the control strategy. Being the industrial work load related to the production lines consumption, an optimal load allocation algorithm is implemented to optimally schedule the production lines over the planning day. The simulation results are performed on an industrial plant layout located in the city of Benevento, in Italy.

Abstract

—In this paper we present a comparison between two different vehicle lateral dynamic control approaches: a novel Model Predictive Control (MPC) and a Linear Quadratic Regulator (LQR). Both strategies are aimed at controlling yaw rate and side slip of the vehicle through differential braking and are designed on the basis of a two-degrees of freedom planar model. On the basis of the estimate of tire longitudinal forces we estimate the range of lateral forces which the tire can exert; these bounds, together with the rate limiter of the braking system, represent the hard constraints of MPC controller and the saturations of LQR controller. The results show the benefits to use MPC controller; in particular, when drives in conditions near the physical constraints borders, the MPC controller can produce a better distribution of braking forces because of information is formalized in optimization problem.

Abstract

This report describes the use of the intraoperative parathyroid hormone (ioPTH) assay during parathyroidectomy in waiting list and transplanted patients. ioPTH levels were determined in 40 patients on the waiting list for kidney transplantation with secondary hyperparathyroidism who underwent subtotal parathyroidectomy and 9 transplanted patients with tertiary hyperparathyroidism who underwent removal of hyperplasic glands. Rapid PTH levels decreased significantly at each time period; the percentage decrease in rapid PTH levels was 61.3% among patients with IPT II and 70.2% in patients with IPT III at 10 minutes and 86.5% in patients with IPT II and 91% in patients with IPT III at 15 minutes after excision of hypersecreting parathyroid tissue. A decrease of 50% or more from baseline PTH levels at 10 minutes and/or a decrease of 85% or more at 15 minutes predicted successful removal of abnormal parathyroid glands. The application of this technique during subtotal parathyroidectomy has proved useful for correct excision of parathyroid glands among waiting list patients with IPT II, while in kidney transplant patients with IPT III it allowed removal of only the pathological glands with a limited surgical approach.

Abstract

We performed a subtotal cystectomy on the second and third hepatic segments, with the aid of a radiofrequency dissector device (Ligasure), together with removal of daughter cysts from the peritoneal cavity and irrigation of the peri- toneum with iodine solution. The postoperative course was uneventful and complication-free. A three-year follow-up was carried out without any recurrence. Our review of the recent literature showed that an adequate surgical procedure, proper cleaning of the peritoneal cavity and postoperative medical treatment with albendazole are the most important factors for reducing the incidence of recurrence.

Abstract

We report what to the best of our knowledge is the first case of primary angiosarcoma of the adrenal gland. A 49-year-old woman, with blunt abdominal trauma, was admitted to our department. Computed tomography revealed a suspected rupture of a liver neoplasm and surgical treatment was performed. At laparotomy there was no evidence of liver tumour but a large mass was found in the right adrenal gland. The mass was removed along with the entire periadrenal fat tissue and locoregional lymph nodes. Microscopically, we found diffuse neoangiogenesis with large, hyperchromatic cells. This cellular proliferation, together with the widespread necrosis, distorted the normal appearance of the adrenal gland. No adjuvant therapy was administered. Six months and 1 year later thoracic- abdominal computed tomography and positron emission tomography were performed, and no signs of local recurrence or metastases were observed.

Abstract

Polyarteritis nodosa is a form of vasculitis that affects several organs. Gastrointestinal involvement is frequent, but cases in which the gastrointestinal tract is the only site of disease are rare. In this paper we report a case of a 70-year-old patient with polyarteritis nodosa restricted to the large intestine, who underwent a total colectomy. The patient had no other signs of chronic vasculitis and for this reason surgical treatment resolved the clinical symptoms.

Abstract

This report describes the use of intraoperative parathyroid hormone (ioPTH) assay during parathyroidectomy for patients with secondary hyperparathyroidism on the waiting list for renal transplantation. The levels of ioPTH were determined among waiting list patients undergoing subtotal parathyroidectomy and tertiary hyperparathyroidism patients undergoing procedures. The levels of ioPTH were significantly reduced at 10 minutes by 59.7,3% among with secondary hyperparathyroidism and 68.9% among tertiary hyperparathyroidism. A 15 minutes it was 85% in secondary hyperparathyroidism and 89.7% in tertiary hyperparathyroidism. A decrement of 50% in basal values at 10 minutes and 85% decrement or more at 15 minutes was predictive for the success of abnormal parathyroid gland removal. The application of this technique during subtotal parathyroidectomy results was useful to predict a correct excision of abnormal parathyroid glands among patients with secondary hyperparathyroidism on the waiting list and for tertiary hyperparathyroidism patients.

Abstract

The use of elderly donors has been advocated to expand the organ donor pool because of increased needs and the organ shortage. The aim of this study was to analyze whether the use of elderly donors and marginal kidneys affected the outcome of renal transplantations. Herein we presented data on 126 kidney transplantations performed from January 1996 to September 2003 using 32 marginal donors (group A) and 94 ideal donors (group B). We analyzed the medical and surgical complications and the graft survivals at a median follow-up of 18 months. Medical and surgical complications occurred in 22% and 5% versus 7% and 4% in groups A and B, respectively. The mean cold ischemia time and the mean age were greater for patients undergoing kidney transplantations from marginal donors. No differences were observed in graft survival in groups A and B. In conclusion, our data suggested that with an appropriate strategy and a correct selection of patients, marginal kidneys can be safely used to decrease the gap between demand and supply.

Abstract

In this work, we present a Hardware In the Loop (HIL) platform for testing control strategies designed and implemented in a commercial electronic control unit for industrial refrigerators. The architecture structure consists of four elements: (i) a real-time processor, in which a detailed model of the refrigerator is implemented; (ii) the industrial control unit device under test; (iii) an Input/Output (I/O) hardware interface which allows data communication between the real-time processor (model) and the industrial device (control) and (iv) a power PC in order to display and save experimental results. Introducing the HIL methodology in the refrigerator industry could reduce electronic control unit testing costs and save time in the production stage because it is possible to verify the control strategy performances in realtime for several operating conditions and different refrigerator models avoiding expensive and time-consuming test bench procedures.
