Abstract

We present the first spectroscopic confirmation of a dust-obscured dwarf galaxy, CEERS-14821. The analysis is performed combining JWST NIRCam broad-band photometry and NIRSpec/PRISM spectroscopic data. From the detection of multiple rest-frame optical lines, we derive that CEERS-14821 is located at $z=4.883\pm0.003$. Moreover, from a secure detection of the $H_{\alpha}$ and $H_{\beta}$ we derived that the galaxy has a dust extinction ranging from Av=2.2 to Av=3.3, depending on the assumed reddening law. This value is extremely large given that we estimated a low stellar mass around log(M/Mo)=8.0-8.2. Moreover, using different metallicity tracers, we verify that the galaxy is also metal-rich, with 12+log(O/H)>8.3. This is well above the expectation from both the mass-metallicity relation and the fundamental mass-metalliticy relation. CEERS-14821 is going through a burst of star formation, there are no indications of a strong contribution from an active galactic nuclei (f(AGN)<0.5 with respect to the total dust luminosity). Based on the rest-frame optical images, this source has a size compatible with galaxies of similar stellar mass and redshift. Recently, there has been an increasing interest in SIJ fusion using minimally invasive surgery (MIS) due to safety. Initially, devices designed for MIS were intended for lateral approaches. A minimally invasive sacroiliac fusion implant for use with a posterior approach has been developed and is regulated for clinical use under the regulatory framework required for human cells, tissues, or cellular or tissue-based products (HCT/Ps).Methods: A multi-center, prospective, single-arm study was launched after initial studies provided preliminary data to support safety, efficacy, and durability of this minimally invasive sacroiliac posterior fusion LinQ allograft implant (NCT04423120). Preliminary results were reported previously. Final results for the full participant cohort are presented here.Results: One-hundred and fifty-nine (159) participants were enrolled across 16 investigational sites in the US between January 2020 and March 2022. One-hundred and twenty-two (122) participants were implanted. At the 1-month follow-up, 82 participants satisfied all criteria for the composite responder endpoint, representing 73.2% of the study cohort. These results stayed consistent across the remaining study timepoints with 66.0%, 74.4%, and 73.5% of participants classified as responders at the 3-, 6- and 12-month follow-up visits, respectively. VAS scores were significantly reduced (p < 0.0001) and ODI scores were significantly improved (p < 0.0001). All domains of the PROMIS-29 were also significantly improved (all p’s < 0.0001). Only one procedure-related serious AE was reported in the study.Conclusion: These results suggest that the posterior approach LinQ Implant System is a safe and effective treatment for sacroiliac joint dysfunction at 12 months, with results that are favorable compared to outcomes reported for an FDA-cleared lateral approach.Keywords: sacroiliac joint disease, sacroiliac fusion, single point posterior fusion, back pain, minimally invasive spine surgery, sacroiliitis

Abstract

Aims. To explore the chemical pattern of early-type stars with planets, searching for a possible signature of planet formation. In particular, we study a likely relation between the lambda Bootis chemical pattern and the presence of giant planets. Methods. We performed a detailed abundance determination in a sample of early-type stars with and without planets via spectral synthesis. Results. We compared the chemical pattern of the stars in our sample (13 stars with planets and 24 stars without detected planets) with those of lambda Bootis and other chemically peculiar stars. We have found four lambda Bootis stars in our sample, two of which present planets and circumstellar disks (HR 8799 and HD 169142) and one without planets detected (HD 110058). We have also identified the first lambda Bootis star orbited by a brown dwarf (zeta Del). This interesting pair lambda Bootis star + brown dwarf could help to test stellar formation scenarios. We found no unique chemical pattern for the group of early-type stars bearing giant planets. However, our results support, in principle, a suggested scenario in which giant planets orbiting pre-main-sequence stars possibly block the dust of the disk and result in a lambda Bootis-like pattern. On the other hand, we do not find a lambda Bootis pattern in different hot-Jupiter planet host stars, which do not support the idea of possible accretion from the winds of hot-Jupiters, recently proposed in the literature. Then, other mechanisms should account for the presence of the lambda Bootis pattern between main-sequence stars. Abstract

Objective: There have been plenty of articles published in recent decades on patient care in the form of case management (CM), but conclusions regarding health outcomes and costs have often been discordant. The objective of this study was to examine previous systematic reviews and meta-analyses with a view to assessing and pooling the overwhelming amount of data available on CM-based health outcomes and resource usage.Methods: We conducted a review of reviews of secondary studies (meta-analyses and systematic reviews) addressing the effectiveness of CM compared with usual care (or other organizational models) in adult (18+) with long-term conditions. PubMed, the Cochrane Database of Systematic Reviews, and the Database of Abstracts of Reviews of Effects (DARE) were searched from 2000 to the end of December 2017. The outcomes of interest are related to process of care, health measures, and resource usage.Results: Twenty-two articles were ultimately considered: 4 meta-analyses and 18 systematic reviews. There is strong evidence of CM increasing adherence to treatment guidelines and improving patient satisfaction, but none of the secondary studies considered demonstrated any effect on patient survival. Based on the available literature, there is contrasting evidence regarding all the other health outcomes, such as quality of life (QOL), clinical outcomes, and functional status. Good-quality secondary studies consistently found nothing to indicate that CM prompts any reduction in the use of hospital resources.Conclusion: The source of variability in the literature on the consistency of the evidence for most outcomes is unclear. It may stem from the heterogeneity of CM programs in terms of what their intervention entails, the populations targeted, and the tools used to measure the results. Abstract

Background and purpose: Limited research has been dedicated to upper limb (UL) rehabilitation in progressive multiple sclerosis (PMS). The objective in this pilot study was to investigate the effect of task‐oriented UL rehabilitation in PMS and to perform explorative analyses of the magnetic resonance imaging (MRI) correlates of changes in motor performance. Methods: Twenty‐six PMS patients with mild UL impairment were prospectively enrolled and randomized into two groups: an active treatment group (ATG, n = 13) and a passive treatment group (PTG, n = 13). At baseline and after training, patients underwent MRI scans with structural and functional imaging and were evaluated with the action research arm test, the nine‐hole peg test, the ABILHAND scale and the modified fatigue impact scale (MFIS). Measures of motor finger performance were obtained by engineered glove measuring. Results: After rehabilitation, the ATG improved in several finger motor tasks (0.001 ≤ P ≤ 0.03, 0.72 ≤ Cohen's d ≤ 1.22) and showed reduced MFIS scores compared with the PTG (P = 0.03). The ATG showed increased functional connectivity within the cerebellar and thalamic resting state networks compared with the PTG (P < 0.05). Correlations were found between several measures of motor improvement and thalamic and sensorimotor networks (0.87 ≤ r ≤ 0.93, 0.001 ≤ P ≤ 0.03). No changes in cerebral volumes and diffusion tensor imaging derived measures were detected. Conclusions: Progressive multiple sclerosis patients with mild UL dysfunction benefit from task‐oriented UL rehabilitation, which seems to be more efficient than simple passive mobilization. Despite a high burden of disability and brain damage, functional adaptive capacities seem to be preserved, thus providing a rationale for the use of rehabilitative treatments in late PMS. Abstract We consider the following Cauchy problem for a wave equation with time-dependent damping term b (t) u t and mass term m (t) 2 u , and a power nonlinearity | u | p : u t t − Δ u + b (t) u t + m 2 (t) u = | u | p , t ≥ 0 , x ∈ R n , u (0 , x) = f (x) , u t (0 , x) = g (x). We discuss how the interplay between an effective time-dependent damping term and a time-dependent mass term influences the decay rate of the solution to the corresponding linear Cauchy problem, in the case in which the mass is dominated by the damping term, i.e. m (t) = o (b (t)) as t → ∞. Then we use the obtained estimates of solutions to linear Cauchy problems to prove the existence of global in-time energy solutions to the Cauchy problem with power nonlinearity | u | p at the right-hand side of the equation, in a supercritical range p > p ̄ , assuming small data in the energy space (f , g) ∈ H 1 × L 2. In particular, we find a threshold case for the behavior of m (t) with respect to b (t). Below the threshold, the mass has no influence on the critical exponent, so that p ̄ = 1 + 4 ∕ n , as in the case with m = 0. Above the threshold, p ̄ = 1 , global (in time) small data energy solutions exist for any p > 1 , as it happens for the classical damped Klein–Gordon equation (b = m = 1). Along the threshold, varying a parameter β ∈ [ 0 , ∞ ] which depends on the behavior of m (t) with respect to b (t) , we find a scale of critical exponents, namely, p ̄ = 1 + 4 ∕ (n + 4 β). This scale of critical exponents is consistent with the diffusion phenomenon, that is, it is the same scale of critical exponents of the Cauchy problem for the corresponding diffusive equation − Δ v + b (t) v t + m 2 (t) v = | v | p , t ≥ 0 , x ∈ R n , v (0 , x) = φ (x). Abstract

The QDF (Quality of life, Depression and cognitive Function) is a project elaborated and implemented by a Research Network of the nursing personnel of the cardiological services which promoted and run the GISSI-HF trial. It is a "companion study" which included up to 1/3 of all the GISSI-HF centres.To describe the distribution and evaluation of QDF variables measured with standardised instruments (Kansas City Cardiomiopathy Quality of Life questionnaire, KCCQ; Geriaric Depression scale GDS, Mini Mental State Examination MMSE), to verify whether and how far their knowledge could influence clinical decisions; to assess the prognostic impact of QDF values measured at entrance on mortality and morbidity. The study protocol included also a comprehensive evaluation of QDF variables independent from the questionnaires, by the nurses in charge of the study patients (the results of this part of the protocol are not included in this report).The QDF data were collected at baseline, 6 months, 1, 2 and 3 years through two self administered questionnaires (KCCQ, GDS), while the MMSE only for patients70 years. The questionnaires were administered to the patients who accepted to participate. A specific goal of the project focuses on the degree of independence between the results of the questionnaires and the judgement of the nurses. This contribution presents the operational implementation of the study protocol and general framework.

Abstract

To document the degree of reliability of validated and mainly qualitative questionnaires most often used in research settings and/or projects, in a broad network of unselected cardiological centres; to assess whether the quality of data could be compatible with their use not only as descriptive but also in prognostic scores including mainly "hard" clinical variables.Eighty-three patients were included. Compliance with the self-administration of the Kansas City Cardiomiopathy Questionnaire (KCCQ) and the Geriatric Depression Scale (GDS) was very high throughout the study (KCCQ 97.7% baseline and 85.8% at three years). As expected more difficulties were encountered with the Mini Mental State Examination (administered to 80.6% patients baseline and to 54.3% at three years). Main reasons for not administering questionnaires was ascribed to organizational and patients problems.It is worth noticing that the study was conducted by nurses without economical incentives. The questionnaires were well completed with minimal missing data. If questionnaires are well presented to patients, a routine assessment of quality of life and depression could be feasible in everyday care.

Abstract

Aim of this paper is to explore and quanti-quantitatively assess whether QoL as measured with the Kansas City Cardiomiopathy Questionnaire (KCCQ) summary score could be considered as an independent relevant component of clinical prognostic score of morbidity and mortality and identify patients at risk for death or admissions.Sixty-three per cent NYHA II and 39.4% NYHA III-IV patients experience a good Qol (score75). Risk factors for "not good" (75) QoL are age, NYHA class III-IV, diabetes, COPD and previous hospitalizations. NYHA II and III-IV patients with not good QoL experience an higher mortality than patients with a good QoL in the same classes. NYHA II patients with not good QoL experience the same 1 year readmission rates as NYHA III-IV patients with good QoL.Quality of life scores identify patients with different risk of mortality and readmissions within the same NYHA class. The prognostic value of KCCQ summary scores could identify candidates for disease management in whom better targeted care strategies may reduce hospitalizations and prevent deaths.

Abstract

This contribution describes the patients included for the QDF (Quality of life, Depression and Cognitive functions) study and explores the distribution and time trends of scores of the three dimensions.The large majority of patients are males (78.3%), and mean age is 67.2 +/- 10.7 years (9.2%80 years). Patients in NYHA class III-IV experience more depression and a worse QoL as compared to the less severly clinically affected counterparts in NYHA class. Although QoL and depression are influenced by the same variables, not all patients with bad QoL are depressed and viceversa. Mean scores of cognitive functioning are not influenced by the same variables that influence QoL and depression.The regular measurement of QoL, depression and cognitive function is recommended by some authors. It seems to be confirmed that the three dimensions need to be explored to disentangle the specific informative value that each of them could add to clinical measures.

Abstract

The prevalence of cognitive impairment (CI) is reported to be higher in heart failure patients than in the general population.To describe the prevalence of CI and to assess its prognostic value on the increased risk of hospitalizations and death over the medium term (3 years).MMSE was administered to 620/745 heart failure patients70 years. Up to 35% of the tested population were classified as mild CI and 12% as moderate. The severity of baseline clinical conditions, as expressed as NYHA classes, do not appear to be associated to different degrees of CI. The subgroup of patients with moderate CI includes higher proportion of people with the lowest educational level, of feminine gender, of higher age (80). Even in a population closely monitored as the one included in the GISSI-HLF trial, it is worth to be underlined that the presence of moderate CI appears to be strongly prognostic of an increased burden of care (hospitalizations), and of death over the whole observation period of the study (3 years).The presence of even mild or moderate CI is prognostic for increased mortality and readmissions for patients of the same NYHA class. In the absence of effective intervention strategies, research should concentrate on the yield, if any, of a systematic screening for CI in heart failure patients.

Abstract

Depression is common in patients with heart failure but its causal and/or prognostic role with respect to the cardiac condition in still a matter of research.To describe the prevalence of depression and to assess its specific prognostic value in terms of increased risk of hospitalizations or death.Overall, 27.5% of 1495 patients experience symptoms of depression. Risk factors for depression are female sex, age70, higher NYHA class, previous hospitalizations and comorbidities. Depressed patients of the same NYHA class are at higher risk for 1 and 3 years mortality and all causes hospital admissions.Depression, assessed with a simple and easy to administer instrument, can add information to clinical variables in the identification of patients at increased risk of mortality and readmissions within the same NYHA class.
