Abstract

Background: Young women with breast cancer (BC) have an increased chance of carrying germline BRCA pathogenic variants (PVs). Limited data exist on the prognostic impact of tumor histology (i.e. ductal versus lobular) in hereditary breast cancer. Methods: This multicenter retrospective cohort study included women aged ≤40 years with early-stage breast cancer diagnosed between January 2000 and December 2020 and known to carry germline PVs in BRCA1/2. Histology was locally assessed in each center. The Kaplan-Meier method and Cox regression analysis were used to assess disease-free survival and overall survival. Results: Of 4628 patients included from 78 centers worldwide, 3969 (86%) had pure ductal, 135 (3%) pure lobular, and 524 (11%) other histologies. Compared with ductal tumors, lobular tumors were more often grade 1/2 (57.7% versus 22.1%), stage III (29.6% versus 18.5%), and luminal A-like (42.2% versus 12.2%). Lobular tumors were more often associated with BRCA2 PVs (71.1% BRCA2), while ductal tumors were more often associated with BRCA1 PVs (65.7% BRCA1). Patients with lobular tumors more often had mastectomy (68.9% versus 58.3%), and less often received chemotherapy (83.7% versus 92.9%). With a median follow-up of 7.8 years, no significant differences were observed in disease-free survival (adjusted hazard ratio 1.01, 95% confidence interval 0.74-1.37) or overall survival (hazard ratio 0.96, 95% confidence interval 0.62-1.50) between patients with ductal versus lobular tumors. No significant survival differences were observed according to specific BRCA gene, breast cancer subtype, or body mass index. Conclusions: In this large global cohort of young BRCA carriers with breast cancer, the incidence of pure lobular histology was low and associated with higher disease stage at diagnosis, luminal-like disease and BRCA2 PVs. Histology did not appear to impact prognosis.

Abstract

Background: This study aims to compare retrospectively generated gated internal target volumes (ITVs) and to evaluate whether gated ITVs can reduce planned target volumes (PTVs) compared with standard ITV expansions. Materials and Methods: In this study, we retrospectively generated respiratory-gated ITVs and PTVs for our cohort of patients who underwent four-dimensional computed tomography for thoracic radiotherapy in our department between August 2018 and February 2019. We calculated the standard ITVs and two gated ITVs to analyze the volumetric reduction. Further, we considered a PTV reduction of >10% to be significant, and we analyzed the role of the localization and the size of the gross tumor volumes. Results: We included 38 patients with a median age of 70 years (mean = 68, SD ± 13,4, range =43-89), of whom 18 (47%) were females and 20 (52%) were males. The two gated PTVs (PTV 30%-70% and PTV 80%-20%) were significantly smaller than the standard PTVs (p-value < 0,001 for both PTVs). Considering the volume of the gross target volume (GTV), we found a significant correlation between GTV30cc and ITV30%-70% (chi-square analysis, p:0,006) and between GTV5cc and ITV80%-20% (p:0,003). We also found a correlation between the localization of the target lesion (mediastinal/ central/peripheral lesion) for both the gated ITVs (p: 0,030 for ITV 30%-70% and p:0,018 for ITV 80%-20%). Conclusion: Gated ITV plans could be useful for the sparing of normal tissues. Our results show that this approach could be useful for small lesions and for certain localizations (island tumors).

38. Industrial policy and productivity growth in Fascist Italy

Abstract

The first chapter - Crisis? Which Crisis? - constructs a new series of industrial value added at constant (1938) prices for Italy, for the period between 1928 and 1938. The data employed are shown to be better indicators of the dynamic of the Great Depression than those used by Carreras and Felice (2010) and allow to substantially revise the profile of the Crisis. The contraction appears to be more pronounced and persistent, placing the Italian experience more in line with that of other industrialised countries.The second chapter - The Italian Climacteric - presents new estimates of total factor productivity growth for Italy over the Fascist era and compares them with analogous ones for the pre-World War One period and for Germany and Britain. Because of the absence of a fully reliable GDP series, a dual growth accounting framework is employed. This approach permits the incorporation of new data on land rents and of new evidence on the returns to human capital. Results show that during the interwar era Italy experienced a “climacteric", defined as a cessation of TFP growth, which compares poorly with the coeval performance of Britain and Germany. This disappointing result contrasts vividly with what occurred in the late liberal Italy, when TFP grew less quickly than in Germany, but faster than in Britain.The third chapter - A Tale of Two Fascisms - offers the first quantitative assessment of labour productivity dynamics within the Italian industrial sector and of their links with Fascist competition policy. We argue that the institutional context in which Italian firms operated and, in particular, changes in the level of product market competition had a significant effect in determining their productivity performance. By relying on a new dataset and on new labour productivity estimates, we show that the earlier more liberal period of the Fascist era was characterised by a true productivity boom, which ended following the switch to a more interventionist industrial policy. Panel data evidence shows that reductions in the level of competition in the industrial sector were associated with lower productivity growth, while changes in industrial structure were a less significant factor.
