3. Strategic basin and delta planning increases the resilience of the Mekong Delta under future uncertainty

Abstract

The climate resilience of river deltas is threatened by rising sea levels, accelerated land subsidence, and reduced sediment supply from contributing river basins. Yet, these uncertain and rapidly changing threats are rarely considered in conjunction. Here we provide an integrated assessment, on basin and delta scales, to identify key planning levers for increasing the climate resilience of the Mekong Delta. We find, first, that 23 to 90% of this unusually productive delta might fall below sea level by 2100, with the large uncertainty driven mainly by future management of groundwater pumping and associated land subsidence. Second, maintaining sediment supply from the basin is crucial under all scenarios for maintaining delta land and enhancing the climate resilience of the system. We then use a bottom-up approach to identify basin development scenarios that are compatible with maintaining sediment supply at current levels. This analysis highlights, third, that strategic placement of hydropower dams will be more important for maintaining sediment supply than either projected increases in sediment yields or improved sediment management at individual dams. Our results demonstrate 1) the need for integrated planning across basin and delta scales, 2) the role of river sediment management as a nature-based solution to increase delta resilience, and 3) global benefits from strategic basin management to maintain resilient deltas, especially under uncertain and changing conditions.

Abstract

Objective: To compare the effectiveness and cost of in‐vitro fertilization (IVF) with or without preimplantation genetic testing for aneuploidy (PGT‐A) when only one or two blastocysts are obtained. Methods: A dataset was gathered from 1829 patients including 368 non‐PGT‐A and 1461 PGT‐A cycles with one or two blastocysts obtained, between April 2013 and July 2022. Patients were matched 1:1 by propensity‐score matching for maternal age, number of metaphase‐II oocytes inseminated and number of blastocysts obtained, achieving a database of 242 patients per group. The non‐PGT‐A and PGT‐A groups were compared for differences in live birth rate (LBR) per embryo transfer (ET), cumulative LBR per patient, miscarriage rate (MR) per clinical pregnancy, number of days between oocyte retrieval and conclusion of the IVF cycle (primary outcome), mean expenses incurred at the clinic and incremental cost‐effectiveness ratio (ICER). Results: More than twice as many ETs were conducted in the non‐PGT‐A group compared with the PGT‐A group, yet the cumulative LBR per patient was similar between groups (23.6% (95% CI, 18.5–29.5%) vs 27.3% (95% CI, 21.9–33.4%)). This outcome was achieved with a higher LBR per ET (16.2% (95% CI, 12.6–20.5%) vs 41.5% (95% CI, 33.9–49.4%)) and lower MR per clinical pregnancy (30.1% (95% CI, 21.8–42.6%) vs 13.9% (95% CI, 7.5–24.0%)) in the PGT‐A group. The MR per patient was also lower in the PGT‐A group (9.5% (95% CI, 6.2–14.1%) vs 4.5% (95% CI, 2.4–8.2%)). The mean duration between oocyte retrieval and IVF cycle conclusion was 131 (95% CI, 113–150) days in the non‐PGT‐A group vs 74 (95% CI, 61–87) days in the PGT‐A group (P < 0.001; power = 99.8%). The ICER of PGT‐A for the months saved between oocyte retrieval and conclusion of the IVF cycle was €499 overall, ranging between €170 and €2065 according to the number of blastocysts obtained and/or maternal age. The ICER of PGT‐A for prevented miscarriages was €18 968, decreasing to €3525 when calculated among patients aged ≥ 35 years with two blastocysts obtained. Conclusions: When conducted in expert IVF clinics for patients indicated for the procedure, PGT‐A is clinically valuable even when only one or two blastocysts are obtained. PGT‐A reduces the number of ETs and miscarriages while the cumulative LBR per patient remains unaffected, and allows these outcomes to be achieved in a shorter timeframe. Abstract

When asked what differentiates the Milan Approach now from the period when Luigi Boscolo and Gianfranco Cecchin were alive, the teachers at Via Leopardi remain humble. This article aims to explore the current identity of the Milan Approach. What is now the specificity of the Milan model? Do we have particularities that differentiate us from other clinical or therapeutic models? We still work according to the notion of the observing system, and we apply the three directives of the 1980s: hypothesizing, circularity, and curiosity. Is there a specific direction in which our approach is heading? Rather than attempting to take a picture of a status quo, this article seeks to represent the questions and doubts that organize a constant process in progress-as clinical work usually is. In this article, we wish to underline some aspects that are important to us and that we hold dear. We assert that the premises we have in common allow us to be flexible enough to keep in tune with current events and to address social issues that are of political relevance, in order to adapt to cultural changes.

Abstract

Colloidal particles provide an efficient mean of building multiple scale structured materials from colloidal dispersions. In this Brief Report, we account for experimental evidence on the formation of a colloidal cluster array at a three-phase contact line. We study the influence of low frequency external alternating electric fields on a diluted colloidal dispersion opened to the air. We focus on the cluster formation and their evolution in the meniscus by measuring characteristic times and lengths. We observe that the clusters are separated by a well-defined length and that, in our experimental conditions, they survive between five a fifteen minutes. Abstract

Introduction Prevention of cardiovascular disease (CVD) is of key importance in reducing morbidity, disability and mortality worldwide. Observational studies suggest that digital health interventions can be an effective strategy to reduce cardiovascular (CV) risk. However, evidence from large randomised clinical trials is lacking. Methods and analysis The CV-PREVITAL study is a multicentre, prospective, randomised, controlled, open-label interventional trial designed to compare the effectiveness of an educational and motivational mobile health (mHealth) intervention versus usual care in reducing CV risk. The intervention aims at improving diet, physical activity, sleep quality, psycho-behavioural aspects, as well as promoting smoking cessation and adherence to pharmacological treatment for CV risk factors. The trial aims to enrol approximately 80 000 subjects without overt CVDs referring to general practitioners' offices, community pharmacies or clinics of Scientific Institute for Research, Hospitalization and Health Care (Italian acronym IRCCS) affiliated with the Italian Cardiology Network. All participants are evaluated at baseline and after 12 months to assess the effectiveness of the intervention on short-term endpoints, namely improvement in CV risk score and reduction of major CV risk factors. Beyond the funded life of the study, a long-term (7 years) follow-up is also planned to assess the effectiveness of the intervention on the incidence of major adverse CV events. Abstract

The development and application of evolutionary algorithms (EAs) and other metaheuristics for the optimisation of water resources systems has been an active research field for over two decades. Research to date has emphasized algorithmic improvements and individual applications in specific areas (e.g. model calibration, water distribution systems, groundwater management, river-basin planning and management, etc.). However, there has been limited synthesis between shared problem traits, common EA challenges, and needed advances across major applications. This paper clarifies the current status and future research directions for better solving key water resources problems using EAs. Advances in understanding fitness landscape properties and their effects on algorithm performance are critical. Future EA-based applications to real-world problems require a fundamental shift of focus towards improving problem formulations, understanding general theoretic frameworks for problem decompositions, major advances in EA computational efficiency, and most importantly aiding real decision-making in complex, uncertain application contexts.

Abstract

During the first years of the HIV pandemic, the virus diffusion was responsible for discriminatory behavior from medical and dental care workers towards HIV-infected patients, as described by our research group in 2009. The aim of the current study was to provide an update on the previous data, investigating the presence of discriminatory behaviors, evaluating the level of dentists’ knowledge about the virus and proposing strategies to be implemented to avoid professional exposure and cross-infections. This study was a cross-sectional, online, national survey on dentists, members of the main national dental associations (ANDI and AIO). The questionnaire was divided into four major sections, with the same structure as the questionnaire administered in 2009. The differences between groups were analyzed using the software program IBM SPSS Statistic, version 21.0. A total of 1054 dentists filled out the questionnaire completely. Among them 0.04% revealed a discriminatory attitude towards HIV-infected patients. The univariate analysis showed that discrimination towards HIV + patients was statistically associated with personal experiences and the level of fear associated with treating them (p = 0.001) and with the type of dental treatments performed (p = 0.01). This cross-sectional study revealed the persistence of dentists who still discriminate against HIV + patients, and the percentage of these dentists was only slightly lower than that in the previous survey (4.3% in the present survey vs. 4.5%). The survey also depicted other information worthy of consideration, such as a perceived ability to detect HIV + patients based on their appearance, complaints about not having received adequate training for treating HIV + individuals, and a lack of scientific knowledge about the virus. Abstract

Across continental Africa, more than 300 new hydropower projects are under consideration to meet the future energy demand that is expected based on the growing population and increasing energy access. Yet large uncertainties associated with hydroclimatic and socioeconomic changes challenge hydropower planning. In this work, we show that only 40 to 68% of the candidate hydropower capacity in Africa is economically attractive. By analyzing the African energy systems' development from 2020 to 2050 for different scenarios of energy demand, land-use change, and climate impacts on water availability, we find that wind and solar outcompete hydropower by 2030. An additional 1.8 to 4% increase in annual continental investment ensures reliability against future hydroclimatic variability. However, cooperation between countries is needed to overcome the divergent spatial distribution of investment costs and potential energy deficits.

Abstract

Atmospheric Extreme Events (EEs) cause severe damages to human societies and ecosystems. The frequency and intensity of EEs and other associated events are increasing in the current climate change and global warming risk. The accurate prediction, characterization, and attribution of atmospheric EEs is therefore a key research field, in which many groups are currently working by applying different methodologies and computational tools. Machine Learning (ML) methods have arisen in the last years as powerful techniques to tackle many of the problems related to atmospheric EEs. This paper reviews the ML algorithms applied to the analysis, characterization, prediction, and attribution of the most important atmospheric EEs. A summary of the most used ML techniques in this area, and a comprehensive critical review of literature related to ML in EEs, are provided. A number of examples is discussed and perspectives and outlooks on the field are provided.

Abstract

In the Jewish Bible as well as in the Christian Old Testament books agriculture was the mainstay of the Israelite economy both in villages and cities. Working the land and herding was an anthropological given; nevertheless it was conceived on the one hand as a very positive value, in so far as it was imitation of the Divine work of creation; on the other hand, for its difficulties and risks, it was thought of as a sort of punishment for the transgression of the first human couple. The Books of Moses approach the many fatigues on the fields in a quite pragmatic way, fixing social rules and ethical norms, e.g.: a severe law concerning the payment of the workers’ salary, without any delay. Other books such as Psalms, Proverbs, Qohelet, and Ben Sira are full of more traditional sentences and moralistic admonitions about human working; overall, despite of the pessimistic tune of Qohelet, their attitudes remain positive and appreciative regarding the value of human effort, although through pain, to make a living out the struggle with nature.

Abstract

The Talmudic definition of work/labor (melakhah) is based on the activities for building the holy temple in Jerusalem. The list amounts to 39 works which are forbidden on the day of Shabbat. The rest of the seventh day is therefore the lent through which the Jewish tradition understands and gives sense to human activities made to transform the world. The main source for the rabbinical ethics of work are the Chapter of the Fathers (Pirqe Avot) where we find a substantially positive attitude towards working the land, commerce, and any other human activity. In the Talmud almost every rabbi (teacher) has a manual work in order to make a living, and studying the Torah is a religious duty, not an economic enterprise. In addition, working is considered by many masters of Israel a form of co-operation with God in the continuous process of the creation of the world, therefore a form of imitatio Dei.
