Abstract

PURPOSE The aim of this study was to evaluate interobserver agreement (IOA) on target volume definition for pancreatic cancer (PACA) within the Radiosurgery and Stereotactic Radiotherapy Working Group of the German Society of Radiation Oncology (DEGRO) and to identify the influence of imaging modalities on the definition of the target volumes. METHODS Two cases of locally advanced PACA and one local recurrence were selected from a large SBRT database. Delineation was based on either a planning 4D CT with or without (w/wo) IV contrast, w/wo PET/CT, and w/wo diagnostic MRI. Novel compared to other studies, a combination of four metrics was used to integrate several aspects of target volume segmentation: the Dice coefficient (DSC), the Hausdorff distance (HD), the probabilistic distance (PBD), and the volumetric similarity (VS). RESULTS For all three GTVs, the median DSC was 0.75 (range 0.17-0.95), the median HD 15 (range 3.22-67.11) mm, the median PBD 0.33 (range 0.06-4.86), and the median VS was 0.88 (range 0.31-1). For ITVs and PTVs the results were similar. When comparing the imaging modalities for delineation, the best agreement for the GTV was achieved using PET/CT, and for the ITV and PTV using 4D PET/CT, in treatment position with abdominal compression. CONCLUSION Overall, there was good GTV agreement (DSC). Combined metrics appeared to allow a more valid detection of interobserver variation. For SBRT, either 4D PET/CT or 3D PET/CT in treatment position with abdominal compression leads to better agreement and should be considered as a very useful imaging modality for the definition of treatment volumes in pancreatic SBRT. Contouring does not appear to be the weakest link in the treatment planning chain of SBRT for PACA.

Abstract

The study aimed to evaluate the immunomodulatory effects of ablative stereotactic body radiotherapy (SBRT) in patients with primary liver cancers (hepatocellular carcinoma or cholangiocarcinoma). Fifty-three patients with liver cancer treated with SBRT were included in the prospective LAPIS study. To evaluate the effects of SBRT, immune profiling of peripheral blood was performed at the first SBRT fraction (pre-treatment), during and at the end of SBRT, and at the first (FU1) and second (FU2) follow-up. Patients were treated in 3 to 12 fractions, aiming at a BED of 100 Gy. The relative CD8+ and CD4+ T cell fractions increased at the end of treatment up to the FU2, despite a significant drop in the absolute cell counts. The analysis confirmed the increased fraction of proliferating CD8+ and CD4+ T cells, including PD-1+ and PD-1– CD8+ and CD4+ T cells. Increased Ki67+ fractions in both, CD8+ and CD4+ cells may indicate an anti-tumoral T cell response. The T cells showed increased levels of exhaustion markers (PD-1, Tim-3, CTLA-4), and most T cells expressed PD-1. Tumor-reactive T cells have been found among PD-1 expressing T cells. At the end of treatment there was a significant increase in activated non-exhausted PD-1+Tim-3– CD8+ and CD4+ T cells, which lasted until the FU2 for CD4+ T cells. Expression of the activation marker ICOS on CD8+ and CD4+ T cells was accordingly increased at the end of treatment up to the FU2 for CD4+ T cells. There was a significant drop of CD8+ and CD4+ T cells expressing the chemokine receptor CXCR3, important for T cell trafficking, which was more pronounced in CD4+ T cells. Expression of IFNg was increased in CD8+ and CD4+ T cells at the end of treatment up to the FU2 and was much more prominent in CD4+ T cells. TNF-a expression in CD8+ T cells was strongly increased at the end of treatment up to FU2, and GrzB expression increased in CD8+ and CD4+ T cells at FU1. The relative and absolute numbers of NK and NKT cells were strongly reduced mainly at the end of treatment up to the FU2. Nevertheless, IFN-g and TNF-a expression was strongly increased at the end of treatment and FU2 in NK cells. GrzB expression in NK cells was slightly reduced at FU2. Finally, there was also a significant increase in immunosuppressive Treg and MDSC. SBRT induced lymphopenia, but also lymphocyte activation and proliferation in liver cancer patients. Although T cells and NK cells showed good functionality, as indicated by the strong expression as indicated by the strong expression of IFN-g and TNF-α, there was also an increase in immunosuppressive Tregs and MDSCs after SBRT. Abstract

The study aimed to evaluate the influence of ablative SBRT fractionation on radiotherapy-induced lymphopenia, T cell proliferation and effector T cell function in patients with early-stage NSCLC or oligoprogressive or oligometastatic lung disease, using different dose and fractionation regimes (in 3, 5, 8 or 12 fractions) aiming at a BED of 100 Gy, with or without systemic treatment (chemotherapy or immunotherapy). Patients with lung SBRT, either in the primary or the metastatic setting with or without concurrent systemic treatment, were included in the prospective LAPIS studies. To evaluate the effects of SBRT, immunoprofiling of peripheral blood was performed at first SBRT fraction (pre-treatment), during and at the end of SBRT as well as at first (FU1) and second (FU2) follow-up by flow cytometry. The LAPIS studies accrued 156 patients between 2016-2021, of whom 130 were evaluable by flow cytometric analysis. With a median follow-up of 30 months, the median overall survival (OS) was 38 months from SBRT, and the median progression-free survival (PFS) was 14 months, while the 3– and 5-year PFS were 33% and 23%, respectively. For patients with an increase (yes vs no) in circulating CD8+ CTLs at FU1 (the primary study endpoint), the median PFS from FU1 was favorable (not reached versus 21 months), although the difference was not statistically significant (HR = 0.653, 95% CI: 0.25-1.74, p = 0,4). For patients treated with 8-12 fractions, SBRT induced stronger lymphopenia compared to 3-5 fractions at the end of treatment, but a significant increase in proliferation of CD8+ and CD4+ T cells (at the end of treatment. PD-1+/- T cell proliferation was also higher in the 8-12 fraction group. Effector T cell function, indicated by IFN-g expression, was comparable in both fractionation groups. These increases were durable and were observed at follow-up, especially in the subgroup of patients with metastatic disease who had not received concurrent systemic treatment. These data show that prolonged regimes given in moderate RT doses seem to better induce a polyfunctional T cell response compared to 3-5 fractions given in higher RT doses per fraction. Patients with non-small-cell lung cancer (NSCLC) were enrolled in two prospective trials (STRIPE, PET-Plan). We evaluated 48 patients who underwent static (3D) and retrospectively-respiratory-gated 4D PET/CT scans before treatment and a 3D scan during or after treatment. Our proposed method rejects IF changes due to intrinsic variability. The IF variability observed across 4D PET is employed as a patient individualized normalization factor to emphasize statistically relevant IF changes during treatment. Predictions of overall survival (OS), local recurrence (LR) and distant metastasis (DM) were evaluated. From 135 IFs, only 17 satisfied the required criteria of being normally distributed across 4D PET and robust between 3D and 4D images. Changes during treatment in the area-under-the-curve of the cumulative standard-uptake-value histogram (d AUC CSH ) within primary tumor discriminated (AUC = 0.87, Specificity = 0.78) patients with and without LR. The resulted prognostic model was validated with a different segmentation method (AUC = 0.83) and in a different patient cohort (AUC = 0.63). The quantification of tumor FDG heterogeneity by d AUC CSH during chemoradiation correlated with the incidence of local recurrence and might be recommended for monitoring treatment response in patients with NSCLC.

Abstract

Identifying patients with locally advanced head and neck carcinoma on high risk of recurrence after definitive concurrent radiochemotherapy is of key importance for the selection for consolidation therapy and for individualized treatment intensification. In this multicenter study we analyzed recurrence-associated single-nucleotide polymorphisms (SNPs) in DNA repair genes in tumor DNA from 132 patients with locally advanced head and neck carcinoma (LadHnSCC). Patients were treated with definitive radiotherapy and simultaneous cisplatin-based chemotherapy at six partner sites of the German Cancer Consortium (DKTK) Radiation Oncology Group from 2005 to 2011. For validation, a group of 20 patients was available. Score selection method using proportional hazard analysis and leave-one-out cross-validation were performed to identify markers associated with outcome. The SNPs rs1799793 and rs13181 were associated with survival and the same SNPs and in addition rs17655 with freedom from loco-regional relapse (ffLRR) in the trainings datasets from all patients. The homozygote major rs1799793 genotype at the ERCC2 gene was associated with better (Hazard ratio (HR): 0.418 (0.234-0.744), p = 0.003) and the homozygote minor rs13181 genotype at ERCC2 with worse survival (HR: 2.074, 95% CI (1.177-3.658), p = 0.017) in comparison to the other genotypes. At the ffLRR endpoint, rs1799793 and rs13181 had comparable prognostic value. The rs1799793 and rs13181 genotypes passed the leave-one-out cross-validation procedure and associated with survival and ffLRR in patients with LadHnSCC treated with definitive radiochemotherapy. While findings were confirmed in a small validation dataset, further validation is underway within a prospective biomarker study of the DKTK.

Moreover, it highlights ongoing studies for treatment individualisation and per-treatment tumour response monitoring for various primary tumours. Novel tracers and image analysis methods are discussed. The authors believe this contribution to be of crucial value for experts in the field as well as for policy makers deciding on the reimbursement of this powerful imaging modality.

Abstract

The European SocieTy for Radiation and Oncology -Advisory Committee on Radiation Oncology Practice (ESTRO-ACROP) endorsed a project to provide guidelines (GL) for the identification and delineation of clinically negative lymph-nodal stations (LNs) involved in upper gastrointestinal clinical scenarios. The presented GL is focused on preoperative (or definitive) setting. The project aim is to improve the consistency of clinical target volume (CTV) delineation by providing: a description of the anatomical boundaries of the LNs; a radiological computed tomography-based atlas depicting the LNs areas; a free, web-based, interactive example case for independent training of radiation oncologists on LNs delineation according to the presented GL, by both qualitative and quantitative analysis (through the FALCON EduCase platform). This project was carried out with the intention to facilitate and improve uniformity of future upper gastrointestinal guidelines on nodal CTV delineation. We report methodology and results from the collaboration of a working group panel selected by the ESTRO-ACROP.
