Hintergrund: P��diatrische niedriggradige Gliome [US-engl. pediatric low-grade glioma: PLGG] sind die h��ufigsten soliden Tumore im Kindes- und Jugendalter. Ein relevanter Anteil der Erkrankten leidet an einer chronisch-progressiven PLGG-Form mit der Notwendigkeit wiederholter therapeutischer Interventionen. Das Mehrstadienmodell [MSM] erm��glicht eine erweiterte ��berlebenszeitanalyse ��ber ein erstes Ereignis hinaus. Der Einfluss von Kofaktoren sowie konkurrierender Ereignisse auf den nachfolgenden Krankheitsverlauf werden im MSM ��ber die gesamte Beobachtungszeit ber��cksichtigt. Die Arbeit von Goebel et al. 2019 ist die erste Mehrstadienanalyse im Bereich der p��diatrischen Onkologie. Methoden: Die Analyse umfasste die Daten der in Deutschland diagnostizierten und in der SIOP LGG 2004-Studie eingeschlossenen PLGG-Patienten. Das MSM beinhaltete ein initiales Stadium "Diagnose", transiente Zwischenstadien "therapeutische Intervention" in Form von ggf. aufeinanderfolgenden chirurgischen und/oder adjuvanten Therapien (Chemo- oder Radiotherapie) sowie ein absorbierendes Stadium "Tod" durch jede Ursache. Evaluiert wurde der Verlauf der Stadienwahrscheinlichkeiten f��r die gesamte Kohorte sowie f��r gr����ere Untergruppen. Auf Grundlage des Stadienverlaufs der ersten zwei Jahre nach Diagnose wurde ein prognostisches Modell f��r den Krankheitsverlauf der nachfolgenden Jahre entwickelt. Ergebnisse: Die Kohorte umfasste 1587 Patienten mit einem PLGG (medianer Beobachtungszeitraum 6,3 Jahre ab Diagnose). Es wurden insgesamt 22 Stadien identifiziert. F��r eine robuste Mehrstadienanalyse wurde das Modell auf 7 Stadien reduziert und 3 Progressionsstufen festgelegt: nicht-, niedriggradig und hochgradig progressive PLGG Erkrankung. F��nf Jahre nach Diagnose fand sich f��r die gesamte Kohorte die h��chste Wahrscheinlichkeit (0,60) f��r eine nicht-progressive Erkrankung: keine Intervention (0,11) bzw. eine alleinige chirurgische Intervention (0,49). Die 5-Jahres-Wahrscheinlichkeit f��r eine niedriggradig progressive PLGG-Erkrankung war 0,30 und f��r eine hochgradig progressive PLGG-Erkrankung 0,08. Die Wahrscheinlichkeit bis zu diesem Zeitpunkt zu versterben war mit 0,02 gering. Die Risikoanalyse ergab eine erh��hte 5-Jahres-Wahrscheinlichkeit f��r eine hochgradig progressive Erkrankung f��r die Altersgruppe < 1 Jahr bei Diagnose (0,30), f��r eine Tumorlokalisation in der supratentoriellen Mittellinie (0,17) und f��r die histologische Gruppe der diffusen Astrozytome Grad II (0,12). Erkrankte mit Neurofibromatose Typ-1 hatten eine hohe Wahrscheinlichkeit f��r keine Intervention (0,36) oder eine adjuvante Therapie (0,45) innerhalb der ersten 5 Jahre. Das prognostische Modell ergab eine zunehmende 5-Jahres Wahrscheinlichkeit f��r einen hochgradig progressiven Krankheitsverlauf oder f��r Versterben, je mehr Interventionen in den ersten zwei Jahren notwendig gewesen waren. Fazit: Mit Hilfe des MSM gelang erstmals eine verfeinerte Darstellung des Krankheitsverlaufes einer gro��en PLGG-Kohorte sowie gr����erer Untergruppen. Die Mehrstadienanalyse identifizierte Risikogruppen f��r einen hochgradig progressiven Verlauf bzw. mit einer h��heren Wahrscheinlichkeit zu versterben. Das Wachstumsverhalten in den ersten zwei Jahren nach Diagnose bestimmte den weiteren Krankheitsverlauf., Background: Pediatric low-grade glioma [PLGG] are the most frequent solid tumors in the pediatric age group. A relevant proportion of patients suffer from chronic progressive disease requiring multiple interventions. The multi-state model [MSM] allows an extended survival analysis beyond a first event. Influence of cofactors and competing events on the subsequent course of disease is considered in the MSM over the entire observation period. The study by Goebel et al. 2019 is the first multi-state analysis in the field of pediatric oncology. Methods: Data of PLGG-patients diagnosed in Germany and included in the SIOP-LGG 2004 study were analyzed. The MSM incorporated an initial state "diagnosis", transient intermediate states "therapeutic intervention", i.e. subsequent surgeries and/or adjuvant interventions (chemo- or radiotherapies), and one absorbing state "death by any cause". The course of state-probabilities was evaluated for the entire cohort as well as for larger subgroups. A prognostic model for the course of the disease in the subsequent years was developed based on the state-probabilities from the first 2 years after diagnosis. Results: A total of 1587 patients with PLGG (median follow up 6.3 years from diagnosis) and 22 states were identified. For a robust multi-state analysis, the model was reduced to 7 states and 3 levels of disease-progressiveness were determined: non, low and highly progressive. Five years after diagnosis the highest probability (0.60) for the entire cohort was to have had a non-progressive disease, i.e. no intervention (0.11) or one surgical intervention-only (0.49), whereas probability for low progressive PLGG was 0.30 and for highly progressive PLGG 0.08. Five-year probability of death was low (0.02). Risk analysis revealed an increased 5-year probability for highly progressive disease in the age group < 1 year at diagnosis (0.30), supratentorial midline location (0.17), and diffuse astrocytoma WHO grade II histology (0.12). Patients with neurofibromatosis type-1 had a high 5-year probability for no intervention (0.36) and one adjuvant therapy (0.45). The need for more interventions in the first 2 years after diagnosis predicted an increasing 5-year probability of highly progressive disease and death. Conclusion: This first MSM of a large cohort of PLGG-patients obtained a refined description of the course of disease including analysis of larger subgroups. The multi-state analysis identified risk groups for a highly progressive disease-course and a higher probability of death. Growth behavior in the first 2 years after diagnosis predicted further course of the disease and the probability of death.

Magnetic Pulse Welding (MPW) is a joining technique favorable for the generation of strong atomic bonded areas between different metals, e.g. aluminum and steel. Brittle intermetallic phases can be avoided due to the high-speed collision and the absence of external heat. The demand for the use of this technique in industries like automotive and plant engineering rises. However, workpieces used in these fields are often coated, e.g. in order to improve the corrosion resistance. Since the weld quality depends on the material’s behavior at the collision zone, surface layers in that region have to be taken into account as well. This work investigates the influences of different coating types. Aluminum to steel welding is used as an example system. On the inner steel part (C45) coatings like zinc, nickel and chrome are applied, while the aluminum flyer tubes (EN AW-6060) are anodized, chromated and passivated. Welding tests are performed using two different welding systems with varying discharging frequencies and four geometrical part setups. For all combinations, the flyer velocity during the process is measured by Photon Doppler Velocimetry (PDV). By using the uncoated material combination as a reference, the removal of surface layers due to jetting is analyzed. Finally, the weld quality is characterized in peel tests, shear-push tests and by the help of metallographic analysis. It is found that certain coatings improve the joint formation, while others are obstructive for the performance of MPW. Some coatings have no influence on the joining process at all.

Magnetic pulse welding (MPW) enables the fabrication of joints via the harnessing of Lorentz forces, which result from discharging a current pulse through a coil. In the process, an outer piece (flyer) is accelerated onto an inner piece (parent), and welding is achieved using propagating impact fronts. The working length of the experimental setup allows for various shapes of the deformation front, and each configuration has its own advantages and drawbacks. The objective of this work is to show how the working length of tubular MPW specimens affects the front propagation as well as to indicate ways to optimize the front propagations, which are vital to the welding result. It is shown that for steel-aluminum joints, three different front regimes exist, which are related to geometrical factors. These results may be used to avoid seemingly favorable but nevertheless suboptimal conditions for flyer movement, which reduce the weld quality and energy efficiency of the process. Additionally, the methodology presented here may allow for faster process optimization without the need for time-consuming metallographic analyses.
