TbxGd1−xScO3 and TbxDy1−xScO3 solid-solution single crystals were grown by the Czochralski method to validate the practicality of solid solutions between neighboring RE scandates (REScO3). Within this material family, it is demonstrated that fine-tuning the pseudo-cubic lattice parameters between nearly all endmembers is now possible by adjusting the chemical composition of the mixed crystals. Film-tailored substrate lattice spacings enable fine adjustments of epitaxial strain or the growth of nearly strain-free heteroepitaxial films. Investigations of the chemical composition of the grown crystals revealed low segregation, which qualifies the solid-solutions as suitable substrate materials. The melting behavior was studied by differential thermal analysis. To calculate the thermal conductivity, λ(T), the heat capacity and thermal diffusivity were measured by differential scanning calorimetry and the laser flash technique, respectively. It was found that the thermal conductivity shows a minimum near 900 K and increases untypically at higher temperatures. [ABSTRACT FROM AUTHOR]

Bismuth ferrites, specifically perovskite-type BiFeO 3 and mullite-type Bi 2 Fe 4 O 9 , hold significant technological promise as catalysts, photovoltaics, and room-temperature multiferroics. However, challenges arise due to their frequent cocrystallization, particularly in the nanoregime, hindering the production of phase-pure materials. This study unveils a controlled sol-gel crystallization approach, elucidating the phase formation complexities in the bismuth ferrite oxide system by coupling thermochemical analysis and total scattering with pair distribution function analysis. We tune the crystallization pathways in the BiFeO 3 -Bi 2 Fe 4 O 9 system by adjusting the metal to complexing agent ratio and pH during precursor preparation with a fixed Bi/Fe ratio of 1:2. Although all precursors undergo an amorphization process during heating, our results demonstrate a consistent correlation between the crystallization pathway and the initial structural entities formed during gel formation. Pair distribution function analysis reveals structural differences in the intermediate amorphous structures, which preferentially crystallize into either BiFeO 3 or Bi 2 Fe 4 O 9 . This study offers mechanistic insights into the formation processes in the system and synthetic guidance for the controlled synthesis of pure Bi 2 Fe 4 O 9 and mixed BiFeO 3 /Bi 2 Fe 4 O 9 nanomaterials. Importance: For older adults with frailty syndrome, reducing polypharmacy may have utility as a safety-promoting treatment option., Objective: To investigate the effects of family conferences on medication and clinical outcomes in community-dwelling older adults with frailty receiving polypharmacy., Design, Setting, and Participants: This cluster randomized clinical trial was conducted from April 30, 2019, to June 30, 221, at 110 primary care practices in Germany. The study included community-dwelling adults aged 70 years or older with frailty syndrome, daily use of at least 5 different medications, a life expectancy of at least 6 months, and no moderate or severe dementia., Interventions: General practitioners (GPs) in the intervention group received 3 training sessions on family conferences, a deprescribing guideline, and a toolkit with relevant nonpharmacologic interventions. Three GP-led family conferences for shared decision-making involving the participants and family caregivers and/or nursing services were subsequently held per patient at home over a period of 9 months. Patients in the control group received care as usual., Main Outcomes and Measures: The primary outcome was the number of hospitalizations within 12 months, as assessed by nurses during home visits or telephone interviews. Secondary outcomes included the number of medications, the number of European Union list of the number of potentially inappropriate medication (EU[7]-PIM) for older people, and geriatric assessment parameters. Both per-protocol and intention-to-treat analyses were conducted., Results: The baseline assessment included 521 individuals (356 women [68.3%]; mean [SD] age, 83.5 [6.17] years). The intention-to-treat analysis with 510 patients showed no significant difference in the adjusted mean (SD) number of hospitalizations between the intervention group (0.98 [1.72]) and the control group (0.99 [1.53]). In the per-protocol analysis including 385 individuals, the mean (SD) number of medications decreased from 8.98 (3.56) to 8.11 (3.21) at 6 months and to 8.49 (3.63) at 12 months in the intervention group and from 9.24 (3.44) to 9.32 (3.59) at 6 months and to 9.16 (3.42) at 12 months in the control group, with a statistically significant difference at 6 months in the mixed-effect Poisson regression model (P = .001). After 6 months, the mean (SD) number of EU(7)-PIMs was significantly lower in the intervention group (1.30 [1.05]) than in the control group (1.71 [1.25]; P = .04). There was no significant difference in the mean number of EU(7)-PIMs after 12 months., Conclusions and Relevance: In this cluster randomized clinical trial with older adults taking 5 or more medications, the intervention consisting of GP-led family conferences did not achieve sustainable effects in reducing the number of hospitalizations or the number of medications and EU(7)-PIMs after 12 months., Trial Registration: German Clinical Trials Register: DRKS00015055.

Background: Anticholinergic burden has been associated with adverse outcomes such as falls. To date, no gold standard measure has been identified to assess anticholinergic burden, and no conclusion has been drawn on which of the different measure algorithms best predicts falls in older patients from general practice. This study compared the ability of five measures of anticholinergic burden to predict falls. To account for patients' individual susceptibility to medications, the added predictive value of typical anticholinergic symptoms was further quantified in this context., Methods and Findings: To predict falls, models were developed and validated based on logistic regression models created using data from two German cluster-randomized controlled trials. The outcome was defined as "≥ 1 fall" vs. "no fall" within a 6-month follow-up period. Data from the RIME study (n = 1,197) were used in model development, and from PRIMUM (n = 502) for external validation. The models were developed step-wise in order to quantify the predictive ability of anticholinergic burden measures, and anticholinergic symptoms. In the development set, 1,015 patients had complete data and 188 (18.5%) experienced ≥ 1 fall within the 6-month follow-up period. The overall predictive value of the five anticholinergic measures was limited, with neither the employed anticholinergic variable (binary / count / burden), nor dose-dependent or dose-independent measures differing significantly in their ability to predict falls. The highest c-statistic was obtained using the German Anticholinergic Burden Score (0.73), whereby the optimism-corrected c-statistic was 0.71 after interval validation using bootstrapping and 0.63 in the external validation. Previous falls and dizziness / vertigo had the strongest prognostic value in all models., Conclusions: The ability of anticholinergic burden measures to predict falls does not appear to differ significantly, and the added value they contribute to risk classification in fall-prediction models is limited. Unique eleven-membered rings containing silicon, germanium, and tin were synthesized in good yields by the reactions of the corresponding 1,2-bis((2-bromothiophen-3-yl)methoxy)benzenes with (C 6 H 5 ) 2 ECl 2 where E = Sn, Ge, Si. The Sn and Ge congeners were crystallized, but the conformers that these rings crystallized in, were quite different. As confirmed by Density Functional Theory (DFT) calculations, (C 28 H 22 O 2 S 2 Sn) assumes a unique crystal structure that leaves more room around the tetrel atom as compared to the crystal structure of the corresponding Ge compound. In the latter, the central cavity is quite open, whereas in the former, one of the methylene groups can fold inwards. Another consequence is the influence on the planes of the aromatic rings flanking the heterocycle. In the Ge case, the benzene ring is folded away from the central cavity, whereas in the Sn case, it is almost parallel to the imaginary axis through the center of the ring. Thermal analysis investigations (TGA and DSC methods) of these eleven-membered rings suggested the loss of a phenyl group in the first decomposition step. The decomposition temperature decreased from the Si containing heterocycle to Ge and was lowest for the Sn containing heterocycle.

Novel Pb7O6Br2 (1) lead oxybromide was prepared from Pb oxybromide melt by the "rapid quenching" route. Bonding scheme, thermal expansion, and structural properties were studied. The structural features of this unexpectedly complex phase are described on the basis of lone electron pair stereochemical activity and Pb-Br versus Pb-O bonding scheme. The structure of 1 contains a number of cavities, which can be assigned to the self-containments of the lone electron pairs on Pb(2+) cations. "Empty" □Pb4 chains are observed in between of the folding sides of the adjacent strongly corrugated oxocentered [Pb7O6](2+) layers. Highly isotropic thermal expansion of 1 appeared to be unexpected. The possible explanations of such a behavior in 1 are given. The structure of 1 is an interesting example of tetrahedral framework with mixed chemical bonding and is the densest known among Pb oxyhalides with the density of 18.4 tetrahedra/1000 Å(3). Current study shows that oxocentered layers derivatives from α-PbO can be very flexible and form rather dense three-dimensional structural topologies. The properties and structure are compared to other phases crystallizing in the anhydrous PbO-PbX2 (X = F, Cl, Br, I) systems, illustrate the complexity of lead oxyhalides, and reveal new and general pathways for the targeted synthesis of new phases with the Pb-O units of desired dimensionality. The indirect gap value of ∼ 2.04 eV obtained from generalized gradient approximation calculations demonstrates potentially good photocatalytic properties of 1.
