9. Oral lichen sclerosus: a rare case and update

Abstract

Lichen sclerosus (LS) is a chronic inflammatory mucocutaneous disease that often affects the anogenital area; oral mucosal lesions are extremely rare. A 52-year-old woman presented for evaluation of an 8-year history of a persistent whitish plaque in the buccal mucosa. Intraoral examination revealed multiple elevated whitish plaques diffusely distributed in the buccal mucosa associated with an area of tissue atrophy. Although both leukoplakia and lichen planus were considered, incisional biopsy and later, full excision confirmed the histopathological diagnosis of oral LS. After 6 months of follow-up, there are no clinical signs of relapse. This case highlights the importance of clinical and histopathological findings for the correct diagnosis and treatment of oral LS.

Abstract

Objective: This retrospective study aimed to perform a clinicopathological analysis of oral lymphangioma cases diagnosed at a reference service in oral pathology. Materials and Methods: A total of 40 cases with reviewed and confirmed histopathological diagnoses for oral lymphangioma were included in the study. Results: It was found that Caucasian women represented most of the sample, with the tongue and lower lip being the most affected sites. The most frequent clinical presentation of the lesions was as an asymptomatic reddish nodule. As for morphological characteristics, there was a slight predominance of simple lymphangioma (55%). Surgical removal was the therapeutic approach adopted in most cases. Conclusions: Despite OL being rare, it is essential that dentists and other professionals recognize variations in the clinical presentation and histopathological aspects of these lesions, differentiating them from other vascular lesions, and implementing correct treatment in each case. Abstract

Background: Choristomas consist of the proliferation of normal cells or tissue in an abnormal location. Oral choristomas are considered very rare. Aims: The present study aims to report a case of dorsal tongue surface cartilaginous choristoma, with emphasis on its clinical and histopathological features. Case Report: A 72‐year‐old woman attended a dental office with a chief complaint of a persistent tongue lesion. The patient reported that the lesion grew slowly for about 3 months, with no trauma history in the region. An intraoral examination revealed a nodular lesion on the dorsal tongue surface, normal mucosa coloration, firm consistency and sessile implantation, measuring approximately 1.5 cm in diameter. Based on the clinical examination, a diagnosis hypothesis of a granular cell tumour was considered, and an excisional biopsy was performed for confirmation. Given the microscopic findings, the histopathological diagnosis confirmed a cartilaginous choristoma. The patient has been under follow‐up for 12 months, with no signs of relapse. Conclusion: This case highlights the importance of clinical and histopathological findings for the correct diagnosis and treatment of oral choristoma. [ABSTRACT FROM AUTHOR]

Abstract

Congenital granular cell tumor is an uncommon benign oral lesion of uncertain histogenesis that affects the alveolar ridge region of newborns. The present study aims to report an unusual congenital granular cell tumor case, emphasizing clinical, morphological, and immunohistochemical aspects. A 10-day-old newborn patient attended the stomatology service with a nodular pedunculated lesion of normal color located in the anterior region of the superior alveolar ridge. Thus, the diagnostic hypothesis was congenital epulis, and an excisional biopsy was performed. Histopathological analysis revealed features consistent with the diagnosis of congenital granular cell tumor. In addition, we performed immunohistochemical staining for vimentin, Ki-67, and S100. The patient has been under follow-up with no clinical signs of recurrence. Due to its rarity, the analysis of congenital granular cell tumor’s clinicopathological characteristics enables an adequate diagnosis and treatment.

Abstract

Solitary fibrous tumour (SFT) is an uncommon soft tissue lesion, and even rarer in the oral cavity, where it presents a similar clinical presentation to other lesions that affect the oral cavity. Due to its broad morphological spectrum, the histopathological diagnosis might be challenging and, for a definite diagnosis, the use of complementary tools is usually required, such as immunohistochemistry. We aimed to report a case of a 31‐year‐old man with an asymptomatic, slightly purplish nodule located on the dorsum of the tongue. The intraoral physical examination revealed the presence of a nodular exophytic lesion and fibrous consistency, measuring 0.9 cm. The histopathological examination revealed a proliferation of mesenchymal cells with variable morphology in a stroma consisting of dense fibrous connective tissue with areas of hyalinization, in addition to numerous vascular structures with a "staghorn‐shaped" appearance. Considering the unspecific histopathological features, we performed the immunohistochemistry technique, which revealed diffuse positivity for CD34, focal positivity for Bcl‐2 and negativity for S100. Therefore, combined clinical, morphological and immunohistochemical features enable the conclusive diagnosis of solitary fibrous tumour. We emphasize the importance of knowledge about the differential diagnosis of undifferentiated lesions that affect the soft tissues to distinguish SFT from other neoplasms and, thus, perform the most appropriate treatment. Abstract

A busca por metodologias que atraiam o interesse dos alunos de Odontologia e cirurgiões dentistas, em processo de especialização, devem ser uma das preocupações no ensino superior. Estudantes estrangeiros tem peculiaridades em relação a língua e cultura diversa, fazendo com que os docentes se empenhem em adaptar melhor suas metodologias. Nesse sentido, as experiencias práticas no ensino podem contribuir significativamente para isto, pois estas tendências servem de base para que as instituições procurem flexibilizar seu currículo. OBJETIVO: este relato de experiência pretende discorrer sobre a formação dos cirurgiões dentistas especialistas, oriundos de diversas nacionalidades (da américa latina), enfatizando os pontos mais relevantes da experiência do ensino. METODOLOGIA: Trata-se de um estudo descritivo, retrospectivo, transversal e qualitativo, do tipo relato de experiência, realizados com os alunos estrangeiros da Equipe Quixadaense de Pesquisa e Pós-Graduação [EQUIP] (Quixadá-CE) e os docentes sobre a reflexão acerca do trabalho desenvolvido na formação dos estudantes e seu aproveitamento. Os resultados evidenciam que o espaço da EQUIP permite aos alunos o aprendizado prático, baseado na diversidade de atividades, nas reflexões sobre a importância do curso, nos atendimentos e no uso das quatro habilidades do ensino que são ler, ouvir, falar e fazer durante a formação. Além disso, a instituição de ensino possui um histórico de trabalho com o público estrangeiro em outras áreas, facilitando dessa forma a interdisciplinaridade, promovendo grande envolvimento dos profissionais deste estabelecimento de ensino. Os resultados obtidos são percebidos no dia-a-dia dos alunos.

Abstract

As desordens potencialmente malignas (DPM) são lesões que podem acometer a cavidade oral e que apresentam um potencial de transformação maligna. Biomarcadores moleculares têm sido estudados com o objetivo de auxiliar na predição do risco de transformação maligna dessas lesões. Dentre eles, destacam-se as proteínas ING (1-5) (do inglês, inhibitor of growth). O objetivo deste trabalho é avaliar a expressão das proteínas ING-1 e ING-2 em lesões diagnosticadas como hiperceratose e displasia epitelial (DE) (leve, moderada e severa), e correlacionar o padrão de expressão observado com o grau de displasia epitelial. Trata-se de um estudo transversal retrospectivo, de caráter semi-quantitativo e comparativo. A amostra consistiu de 60 espécimes de hiperceratose e DE, os quais foram avaliados morfologicamente e reclassificados de acordo com o sistema binário de classificação das DE proposto por Kujan et al. (2006). A imunoexpressão para ING-1 e ING-2 nas lesões estudadas foi avaliada de forma semi-quantitativa a partir da atribuição de escores, que variavam de 0 a 4, de acordo com o percentual de células epiteliais positivas para aqueles marcadores. Para a análise estatística, foram realizados os testes de Mann-Whitney e de Spearman (p ≤ 0,05). Dos 60 casos analisados, 37 (61,7%) apresentaram-se como lesões de baixo risco e 23 (38,3%) como de alto risco. Tanto para ING-1 quanto para ING-2, 93,3% das lesões displásicas estudadas apresentaram marcação citoplasmática e nuclear nas células epiteliais, com predominância de positividade nas camadas basal e suprabasal. Em relação aos escores de marcação imunohistoquímica, houve predominância de casos com elevada expressão (escore 4) para ambos os marcadores. No entanto, ao se comparar estes escores (nuclear, citoplasmática e geral) com a gradação histológica das DE, tanto para ING-1 quanto para ING-2, não foi observada diferença estatisticamente significativa. Dessa forma, os resultados do presente estudo sugerem que a imunoexpressão de ING-1 e ING-2 não têm relação com o grau de DE oral. No entanto, a alta expressão observada nessas lesões sugere que estas proteínas estariam envolvidas no processo da carcinogênese oral. Potentially malignant disorders (PMD) are lesions that can affect the oral cavity and present a potential for malignant transformation. Molecular biomarkers have been studied to help predict the risk of malignant transformation of these lesions. Among them, the ING (inhibitor of growth) proteins (1-5) are highlighted. The objective of this study is to evaluate the expression of ING-1 and ING-2 proteins in lesions diagnosed as hyperkeratosis and ED (mild, moderate and severe), and to correlate the expression pattern observed with the degree of epithelial dysplasia. This is a cross-sectional, retrospective, semi-quantitative and comparative study. The sample consisted of 60 specimens of hyperkeratosis and epithelial dysplasia (ED), which were morphologically evaluated and reclassified according to the binary classification system of ED proposed by Kujan et al. (2006). The immunoexpression for ING-1 and ING-2 in the studied lesions was evaluated semi-quantitatively from the assignment of scores ranging from 0 to 4, according to the percentage of epithelial cells positive for those markers. For the statistical analysis, the Mann-Whitney and Spearman tests (p ≤ 0.05) were performed. Of the 60 cases analyzed, 37 (61.7%) presented as low-risk lesions and 23 (38.3%) as highrisk lesions. For both ING-1 and ING-2, 93.3% of the dysplastic lesions studied presented cytoplasmic and nuclear marking on epithelial cells, with predominance of positivity in the basal and suprabasal layers. Regarding the immunohistochemical marking scores, there was a predominance of cases with high expression (score 4) for both markers. However, when comparing these scores (nuclear, cytoplasmic and general) with the histological gradation of ED, for both ING-1 and ING-2, no statistically significant difference was observed. Thus, the results of the present study suggest that immunoexpression of ING-1 and ING-2 is not related to the degree of oral ED. However, the high expression observed in these lesions suggests that these proteins would be involved in the oral carcinogenesis process.
