The academic community has been actively discussing and assessing the results of the reform of the Russian healthcare system, which began in the early 2000s. This study considers the results of the reform from the perspective of a concept of health-related behaviour. The paper determines the behaviour patterns of household members concerning personal expenditures on medical goods and services, depending on their monetary income and spending pattern. The main research method was an econometric (correlation and regression) analysis of time series. As a result, we discovered that household income dynamics significantly influence expenditures on medical goods and hygiene items (that vary according to the household income). Moreover, household expenditures on health care depend on its spending on food products. We conclude that, firstly, in 2005–2019, Russia has been showing a steady growth of paid medical services (in total medical care), which are available to less than half of the households. Secondly, health-related behaviour of different groups of households divided by income level significantly changes depending on the income dynamics and spending pattern. Thirdly, an increase in income decreases expenditures on medical services and goods, as well as in an increase in expenditures on health-improvement (preventive) services. We have revealed that spending on food products on average determines 55.9 % of a change in expenditures on medical services. Simultaneously, in the households’ budgets, a 1 % increase in food expenditures displaces 2 % of expenditures on health-improvement services. The obtained results extend the understanding of the features of health-related behaviour that is defined by households’ income, as well as by the spending pattern on food, medical goods and services. Public authorities can consider the revealed features of health-related behaviour of different groups of households divided by income when developing and improving the socio-economic policy in the field of health care. Оценка результатов реформирования системы здравоохранения России, начавшегося в 2000-х гг., стала предметом активной научной дискуссии. В нашем исследовании результаты оценки рассматриваются в контексте концепции самосохранительного поведения населения. Целью исследования являлось определение моделей поведения членов домохозяйств в отношении расходования личных средств на медицинские товары и услуги, в зависимости от уровня их денежных доходов и структуры расходов. Исследование проведено с использованием эконометрического (корреляционно-регрессионного) анализа временных рядов. В результате были выявлены существенное влияние динамики доходов домохозяйств на долю их расходов на медицинские товары и предметы гигиены (различающееся по характеру в зависимости от уровня доходов домохозяйств), а также зависимость расходов домохозяйств на здравоохранение от уровня их затрат на покупку продуктов питания. В работе были сделаны следующие выводы: во-первых, в 2005–2019 гг. в нашей стране наблюдается устойчивая тенденция роста доли платных медицинских услуг в общем объеме медицинских услуг, воспользоваться которыми могут менее половины всех домохозяйств России, во-вторых, самосохранительное поведение различных по уровню доходов групп домохозяйств существенно различается при изменении динамики их доходов и структуры расходов, в-третьих, по мере роста доходов населения сокращается доля расходов на медицинские услуги и товары, а доля расходов санаторно-оздоровительного (профилактического) характера увеличивается. Так, определено, что уровень расходов на продукты питания в среднем определяет 55,9 % изменения расходов населения на медицинские услуги. В то же время повышение на 1 % их бюджета расходов на продукты питания вытесняет из него 2 % расходов на санаторно-оздоровительные услуги. Полученные результаты расширяют представления об особенностях самосохранительного поведения населения, определяющегося, с одной стороны, уровнем доходов, с другой — структурой расходов на продукты питания, медицинские товары и услуги. Выявленные в работе особенности самосохранительного поведения различных по уровню доходов групп населения могут быть использованы представителями органов государственной власти при разработке и совершенствовании государственной социально-экономической политики в сфере здравоохранения. Today, the problem of improving the quality of short- and medium-term forecasting of socio - economic processes has become more urgent than ever. Classical forecasting methods based on rational expert thinking and analytical procedures often do not allow the creation of an accurate and adequate model of an economic phenomenon or process. The macroeconomic system is an interconnected system of links between the economy as a whole and its economic entities. The object of macroeconomic analysis is the economic system as a whole and its aggregate parameters. The following problems of macroeconomic policy are distinguished: ensuring the effective functioning of the economy; full employment; the minimum inflation rate; redistribution of income; ensuring sustainable economic growth of the state; regulation of macroeconomic processes of an open economy, etc. It has been substantiated that currently, the world has formed three leading planning and regulatory systems: North American (USA, Canada), Asian (Japan and South Korea), European (France and Sweden). The leader in forecasting is the United States. Forecasting in the United States is considered one of the most important forms of regulation of the economy. The main contemporary models of the dynamics of socio-economic processes and systems of different levels, theoretical principles of the construction and implementation of macroeconomic models of state policy have been researched. The possibilities of their application in studies of the global economy have been considered., Сегодня как никогда актуальна проблема повышения качества кратко- и среднесрочного прогнозирования социально-экономических процессов. Классические методы прогнозирования, основанные на рациональном экспертном мышлении и аналитических процедурах зачастую не позволяют создать точную и адекватную модель экономического явления или процесса. Макроэкономическая система - это взаимосвязанная система связей между экономикой в целом и ее хозяйственными субъектами. Объектом макроэкономического анализа выступает экономическая система в целом и ее агрегированные параметры. Были выделены следующие проблемы макроэкономической политики: обеспечение эффективного функционирования экономики; полная занятость; минимальный уровень инфляции; перераспределение доходов; обеспечение устойчивого экономического роста страны; регулирования макроэкономических процессов открытой экономики и тому подобное. Доказано, что в настоящее время в мире сформировалось три ведущие системы планирования и регулирования: северо американская (США, Канада), азиатская (Япония и Южная Корея), европейская (Франция и Швеция). Лидером в прогнозировании являются США. Прогнозирование в США считается одной из важнейших форм регулирования экономики. An analysis of legal acts and strategic documents made it possible to identify three main approaches to solving a problem of inequality: creating equal conditions for all economic agents, ensuring fair distribution of benefits and raising a standard of living of a population due to a quality of economic growth of subjects of the Federation. В настоящем исследовании методом контент-анализа были определены основные направления и инструменты реализуемой на федеральном и региональном уровнях государственной политики в области социально-экономической дифференциации населения. Проведенный анализ нормативно-правовых актов и стратегических документов позволил выявить три основных подхода к решению проблемы неравенства: создания равенства условий для всех экономических агентов; обеспечения справедливости распределения благ; повышения уровня жизни населения за счет качества экономического роста субъектов Федерации. Preterm human neonates, contrary to preterm piglets, obtain immunoglobulins from their mothers via the placenta during intrauterine development. However, one should note that the majority of trans-placental transfer of immunoglobulins in humans takes place during the last trimester of pregnancy. It is also known that the feeding of limited amounts of colostrum or systemic infusion of small amounts of serum improves the survival of preterm and full-term piglets. Full-term piglets deprived of their mother’s immunoglobulins exhibit strong apathy and develop watery diarrhoea, often resulting in death. The aim of the current study was to determine if provision of immunoglobulins using different approaches would be beneficial for survival outcomes. To reach the immunological sufficient level we infused immunoglobulins intravenously in amount mimicking the blood level in piglets fed with sow colostrum. Intravenous infusion of immunoglobulins in both preterm and full-term newborn piglets fully ensured their survival, growth and blood immunoglobulin G and protein levels similar to those observed in piglets fed colostrum. Piglets completely deprived of immunoglobulins exhibited significantly lower blood levels of immunoglobulins and protein compared to colostrum-fed animals. Piglets infused with only serum exhibited significantly lower blood immunoglobulin G level compared to those infused with immunoglobulins. In conclusion, based on the data obtained, we suggest that passive immune support provided by colostrum intake or early systemic infusion of Ig’s in sufficient amounts is key to ensuring the general well-being of preterm and full-term new born piglets, used as an animal model for the human infant.

It is well known that during the suckling period pancreatic secretion is low. The aim of the study was to investigate the effects of dietary supplementation with pancreatic-like enzymes of microbial origin (PLEM) and SiO
2 , as an antibacterial factor, on the performance of piglets fed creep feed during weaning and thereafter. The experiment was performed on piglets from 30 litters. The piglets were divided into 5 groups (n = 63-66) and weaned at the age of 28 d old. The control group received standard creep feed (Days 7-28), and groups P1 and P2 and groups PA1 and PA2 received creep feed with SiO2 supplemented with protease (P1 and P2) or with protease+amylase (PA1 and PA2) during Days 7-13 (P1 and PA1) or Days 14-20 (P2 and PA2) before weaning. Dietary supplementation with PLEM and SiO2 led to a significant (P < 0.05) increase in ADG in the treated piglets compared to the control with the best results obtained in group P2. To summarize, creep feed supplemented with PLEM improves productivity parameters of piglets under standard rearing conditions. [ABSTRACT FROM AUTHOR]- Published

- Abstract
This review highlights modern aspects of the pathogenesis, diagnosis and treatment of bladder leukoplakia. The search was conducted using Medline, PubMed, and EMBASE databases. Data from a literature search indicate that leukoplakia of the bladder (LMP) is one of the controversial diseases of the urinary system, which is characterized by dystrophic and proliferative changes in the urothelium of Lietos triangle. The authors of various studies focus on the involvement of infectious and inflammatory diseases of the bladder in the development of this condition in the presence of predisposing factors.

- Abstract
The studies presented were designed to highlight the impact of pancreatic enzymes on glycemic control and insulin response. Blood glucose and plasma insulin levels were monitored after intravenous, oral or direct gut glucose tolerance tests (GTT) in 6 pigs with an intact gastrointestinal tract and in 12 pigs following duodenal-jejunal bypass (DJB) surgery. In the intact pigs, pancreatic enzymes (Creon®) given orally 1 h prior to the GTT, lowered the blood glucose levels during the oral and meal GTT and reduced the plasma insulin response during the intravenous and meal GTT. In DJB pigs, blood glucose and plasma insulin levels were higher following glucose loading into the by-passed biliopancreatic limb as compared to that following glucose loading orally or into the common intestinal limb. Infusion of amylase or amylase peptides together with glucose into the biliopancreatic limb lowered blood glucose levels in DJB pigs. These preliminary data suggest new, extra-digestive, actions of enteral pancreatic enzymes - probably amylase or its peptides - on glucose homeostasis, with an reduction in net glucose absorption into the blood and in insulin response. This ability of digestive enzymes (amylase) to reduce post-prandial hyperglycaemia in an insulin-independent manner could aid in preventing the development of obesity and diabetes.

- Abstract
Neurological disorders are among the main clinical problems affecting preterm children and often result in the development of communication and learning disabilities later in life. Several factors are of importance for brain development, however the role of immunoglobulins (passive immunity transfer) has not yet been investigated. Piglets are born agammaglobulinemic, as a result of the lack of transfer of maternal immunoglobulins in utero, thus, they serve as an ideal model to mimic the condition of immunoglobulin deficiency in preterm infants. Thirty six, unsuckled newborn piglets were fed an infant formula or colostrum and supplemented orally or intravenously with either species-specific or foreign immunoglobulin and then compared to both newborn and sow-reared piglets. Two days after the piglets were born behavioural tests (novel recognition and olfactory discrimination of conspecifics scent) were performed, after which the piglets were sacrificed and blood, cerebrospinal fluid and hippocampi samples were collected for analyses. Both parameters of neuronal plasticity (neuronal maturation and synapse-associated proteins) and behavioural test parameters appeared to be improved by the appearance of species-specific porcine immunoglulin in the circulation and cerebrospinal fluid of the piglets. In conclusion, we postulate possible positive clinical effects following intravenous infusion of human immunoglobulin in terms of neuronal plasticity and cognitive function in preterm infants born with low blood immunoglobulin levels.

- Abstract
An elevated level of serum uric acid-hyperuricemia, is strongly associated with the development of gout and chronic kidney disease (CKD) which is often accompanied by a significantly reduced glomerular filtration rate (GFR). In the present study, we investigated the extra-renal elimination of uric acid via the intestine in a healthy pig model and the effect of oral uricase therapy on plasma uric acid concentrations in pigs with induced hyperuricemia and CKD. The experiment was conducted on eleven, ten-week-old pigs (n = 11). The porcine model of CKD was developed by performing 9/10 nephrectomy surgery on eight pigs. A stable model of hyperuricemia was established in only five of the eight nephrectomized pigs by frequent injections of uric acid (UA) into the jugular vein. All pigs (three healthy pigs and five CKD pigs) were operated for implantation of jugular vein catheters and the three healthy pigs also had portal vein catheters inserted. Blood uric acid concentrations were measured spectrophotometrically, using the Uric Acid Assay Kit (BioAssay Systems, Hayward, USA). The piglets with CKD received orally administered uricase (treatment) and served as their own controls (without uricase supplementation). Oral uricase therapy significantly decreased plasma uric acid concentrations in pigs with CKD, whereas hyperuricemia was observed in the pigs whilst not being treated with uricase. Urinary uric acid excretion was similar during both the treatment and control periods during the first 8 h and 24 h after UA infusions in the CKD pigs. To demonstrate the elimination of UA via the intestine, the healthy pigs were infused with UA into the jugular vein. The blood collected from the jugular vein represents circulating UA concentrations and the blood collected from the portal vein represents the concentration of UA leaving the intestine. The final (after 2 h) concentration of UA was significantly lower in blood collected from the portal vein compared to that collected from the jugular vein (3.34 vs. 2.43 mg/dL, respectively, p = 0.024). The latter allows us to suggest that UA is eliminated from the blood via the gut tissue.

The maldigestion and malabsorption of fat in infants fed milk formula results due to the minimal production of pancreatic lipase. Thus, to investigate lipid digestion and absorption and mimic the situation in newborns, a young porcine exocrine pancreatic insufficient (EPI) model was adapted and validated in the present study. A total of thirteen EPI pigs, aged 8 weeks old, were randomised into three groups and fed either a milk-based formula or a milk-based formula supplemented with either bacterial or fungal lipase. Digestion and absorption of fat was directly correlated with the addition of lipases as demonstrated by a 30% increase in the coefficient of fat absorption. In comparison to the control group, a 40 and 25% reduction in total fat content and 26 and 45% reduction in n-3 and n-6 fatty acid (FA) content in the stool was observed for lipases 1 and 2, respectively. Improved fat absorption was reflected in the blood levels of lipid parameters. During the experiment, only a very slight gain in body weight was observed in EPI piglets, which can be explained by the absence of pancreatic protease and amylase in the gastrointestinal tract. This is similar to newborn babies that have reduced physiological function of exocrine pancreas. In conclusion, we postulate that the EPI pig model fed with infant formula mimics the growth and lipid digestion and absorption in human neonates and can be used to elucidate further importance of fat and FA in the development and growth of newborns, as well as for testing novel formula compositions.

- Abstract
The first milk, colostrum, is an important source of nutrients and an exclusive source of immunoglobulins (Ig), essential for the growth and protection from infection of newborn pigs. Colostrum intake has also been shown to affect the vitality and behaviour of neonatal pigs. The objective of this study was to evaluate the effects of feeding colostrum and plasma immunoglobulin on brain development in neonatal pigs. Positive correlations were found between growth, levels of total protein and IgG in blood plasma and hippocampus development in sow-reared piglets during the first 3 postnatal days. In piglets fed an elemental diet (ED) for 24h, a reduced body weight, a lower plasma protein level and a decreased level of astrocyte specific protein in the hippocampus was observed, as compared to those that were sow-reared. The latter was coincident with a reduced microgliogenesis and an essentially diminished number of neurons in the CA1 area of the hippocampus after 72h. Supplementation of the ED with purified plasma Ig, improved the gliogenesis and supported the trophic and immune status of the hippocampus. The data obtained indicate that the development of the hippocampus structure is improved by colostrum or an Ig-supplemented elemental diet in order to stimulate brain protein synthesis and its development during the early postnatal period., (Copyright © 2014 ISDN. - Abstract
Colostrum is an indispensable source of antibodies (IgG) protecting the newborn pig against infection. We studied the effect of feeding colostrum and purified IgG on early structure and development of the gastrointestinal tract (GIT). Newborn littermate pigs were fed either colostrum, an elemental diet (ED), or an ED supplemented with purified serum IgG (ED + IgG) for 24 h or then only ED up to 72 h. Afterwards, pigs were slaughtered. Colostrum-fed pigs or ED supplemented with IgG (ED + IgG) increased thickness (P < 0.001) of stomach mucosa and muscularis (P < 0.05) compared to the ED group not receiving IgG. Feeding an ED supplemented with IgG improved morphology of the GIT towards that of colostrum-fed piglets and indicates a beneficial effect of IgG on GIT development in neonatal pigs. Immunohistochemical studies indicate that ED feeding may influence the expression of nitric oxide synthase in jejunal myenteric (but not submucous) neurons of newborn pigs.
