This study aimed to investigate the impact of sleep and psychological well-being on the academic and clinical performance of nursing students in Saudi Arabia.Methods: This study used a cross-sectional approach with 218 student nurses selected through convenience sampling from King Khalid University, Abha, Saudi Arabia. The sleep quality, psychological wellbeing, academic performance and clinical performance questionnaires were adopted to measure the variables. The Google Forms survey was used to gather the data, which were collected in August through October 2023. t-test and chi-square were used to analyze the data.Results: The sleep quality was higher with good/very good performers (p < 0.001). There was a significant difference with academic performance on positive relations (p < 0.007) and purpose in life (p < 0.004). Additionally, there was a significant difference in autonomy (p < 0.01), environmental mastery (p < 0.026), positive relations (p < 0.001), and purpose in life (p < 0.001) with clinical performance. Among students with good academic performance, those who were older than 25 years of age (p < 0.043) and level 3 students (p < 0.001) were found to have a significant correlation. Furthermore, those who were 25 years of age (p < 0.012) and level 3 students (p < 0.001) were also found to have a significant correlation with clinical performance.Conclusion: The sleep quality, positive relations, purpose in life, autonomy, and environmental mastery are all important factors for academic and clinical success. Moreover, older students and level 3 students may be more likely to have the skills and knowledge necessary to succeed in these areas. Abstract

A 3-day international workshop on atmospheric monitoring and calibration for high-energy astroparticle detectors, with a view towards next-generation facilities. The atmosphere is an integral component of many high-energy astroparticle detectors. Imaging atmospheric Cherenkov telescopes and cosmic-ray extensive air shower detectors are the two instruments driving the rapidly evolving fields of very-high- and ultra-high-energy astrophysics. In these instruments, the atmosphere is used as a giant calorimeter where cosmic rays and gamma rays deposit their energy and initiate EASs; it is also the medium through which the resulting Cherenkov light propagates. Uncertainties in real-time atmospheric conditions and in the fixed atmospheric models typically dominate all other systematic errors. With the improved sensitivity of upgraded IACTs such as H.E.S.S.-II and MAGIC-II and future facilities like the Cherenkov Telescope Array (CTA) and JEM-EUSO, statistical uncertainties are expected to be significantly reduced, leaving the atmosphere as the limiting factor in the determination of astroparticle spectra. Varying weather conditions necessitate the development of suitable atmospheric monitoring to be integrated in the overall instrument calibration, including Monte Carlo simulations. Abstract

The Cherenkov Telescope Array (CTA) is the next generation of Imaging Atmospheric Cherenkov Telescopes. It would reach unprecedented sensitivity and energy resolution in very-high-energy gamma-ray astronomy. In order to reach these goals, the systematic uncertainties derived from the varying atmospheric conditions shall be reduced to the minimum. Different instruments may help to account for these uncertainties. Several groups in the CTA consortium are currently building Raman LIDARs to be installed at the CTA sites. Raman LIDARs are devices composed of a powerful laser that shoots into the atmosphere, a collector that gathers the backscattered light from molecules and aerosols, a photosensor, an optical module that spectrally select wavelengths of interest, and a read-out system. Raman LIDARs can reduce the systematic uncertainties in the reconstruction of the gamma-ray energies down to 5 % level. All Raman LIDARs subject of this work, have design features that make them different than typical Raman LIDARs used in atmospheric science, and are characterized by large collecting mirrors ($\sim2~$m$^2$). They have multiple elastic and Raman read-out channels (at least 4) and custom-made optics design. Abstract

Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

Abstract

Aims Treatment decisions in patients with scimitar syndrome (SS) are often challenging, especially in patients with isolated SS who are often asymptomatic and who might be diagnosed accidentally. We queried a large multi-institutional registry of SS patients to evaluate the natural history of this condition and to determine the efficacy of surgical treatment in terms of survival and clinical status. Methods and results We collected data on 485 SS patients from 51 institutions; 279 (57%) patients were treated surgically (STPs) and 206 (43%) were clinically monitored (CMPs). Median age at last follow-up was 11.6 years (interquartile range 4-22 years). Abstract

The primary purpose of this article is to show the simulation of the optical behavior of the divergence adapter elements in the Ronchi deflectometry configuration to measure convex surfaces, which so far has not been reported in the literature. The use of divergence adapter elements is mandatory when Ronchi deflectometry is used to obtain the shape of a convex surface. Otherwise, light reflected by the surface cannot be collected into the measuring stage. Divergence adapters must be included in both arms of the Ronchi deflectometry configuration, keeping in mind that their optical behaviors affect the surface shape measurement. Before carrying out an experimental setup, we analyzed these elements' effects through an optical simulation of the selected Ronchi. From the analysis of the optical simulation, we can check the adequacy of the divergence adapter for the expected measuring range and required accuracy; the decision about its suitability has been taken by looking if the relations between slope and position at the apex of the convex surface was unequivocal. In this paper, we present the Ronchi configuration chosen to measure convex surfaces, the selected lenses for the divergence adapters, the thresholds linked to the sampler's elements, the optical simulation, and the criteria used to answer if the behavior of the divergence adapters is appropriated for the radii interval that we want to measure. Finally, a unique polynomial expression is proposed to transfer slope and position values from the sampling elements of the apex plane of the surface to be measured. [ABSTRACT FROM AUTHOR]

Abstract

Objective Epratuzumab, a monoclonal antibody that targets CD22, modulates B cell signaling without substantial reductions in the number of B cells. The aim of this study was to report the results of 2 phase III multicenter randomized, double‐blind, placebo‐controlled trials, the EMBODY 1 and EMBODY 2 trials, assessing the efficacy and safety of epratuzumab in patients with moderately to severely active systemic lupus erythematosus (SLE). Methods Patients met ≥4 of the American College of Rheumatology revised classification criteria for SLE, were positive for antinuclear antibodies and/or anti–double‐stranded DNA antibodies, had an SLE Disease Activity Index 2000 (SLEDAI‐2K) score of ≥6 (increased disease activity), had British Isles Lupus Assessment Group 2004 index (BILAG‐2004) scores of grade A (severe disease activity) in ≥1 body system or grade B (moderate disease activity) in ≥2 body systems (in the mucocutaneous, musculoskeletal, or cardiorespiratory domains), and were receiving standard therapy, including mandatory treatment with corticosteroids (5–60 mg/day). BILAG‐2004 grade A scores in the renal and central nervous system domains were excluded. Patients were randomized 1:1:1 to receive either placebo, epratuzumab 600 mg every week, or epratuzumab 1,200 mg every other week, with infusions delivered for the first 4 weeks of each 12‐week dosing cycle, for 4 cycles. Patients across all 3 treatment groups also continued with their standard therapy. The primary end point was the response rate at week 48 according to the BILAG‐based Combined Lupus Assessment (BICLA) definition, requiring improvement in the BILAG‐2004 score, no worsening in the BILAG‐2004 score, SLEDAI‐2K score, or physician's global assessment of disease activity, and no disallowed changes in concomitant medications. Patients who discontinued the study medication were classified as nonresponders. Results In the EMBODY 1 and EMBODY 2 trials of epratuzumab, 793 patients and 791 patients, respectively, were randomized, 786 (99.1%) and 788 (99.6%), respectively, received study medication, and 528 (66.6%) and 533 (67.4%), respectively, completed the study. There was no statistically significant difference in the primary end point between the groups, with the week 48 BICLA response rates being similar between the epratuzumab groups and the placebo group (response rates ranging from 33.5% to 39.8%). No new safety signals were identified. Conclusion In patients with moderate or severely active SLE, treatment with epratuzumab + standard therapy did not result in improvements in response rates over that observed in the placebo + standard therapy group.

Abstract

Few studies have evaluated the impact of lifetime abuse on quality of life (QoL) among older adults. By using a multinational study authors aimed to assess the subjective perception of QoL among people who have reported abuse during the course of their lifetime. The respondents (N = 4,467; 2,559 women) were between the ages of 60 and 84 years and living in seven European countries (Germany, Greece, Italy, Lithuania, Portugal, Spain, and Sweden). Lifetime abuse was assessed by using a structured questionnaire that allowed to assess lifetime experiences of abuse. QoL was assessed with the World Health Organization Quality of Life–Old module. After adjustment for potential confounders, authors found that to have had any abusive experience decreased the score of sensory abilities. Psychological abuse was associated with lower autonomy and past, present, and future activities. Physical abuse with injuries significantly decreased social participation. Intimacy was also negatively associated with psychological abuse, physical abuse with injury, and sexual abuse. The results of this study provide evidence that older people exposed to abuse during their lifetime have a significant reduction in QoL, with several QoL domains being negatively affected., ABUEL

Abstract

This paper describes the referral pathways taken by 1554 patients newly referred to the mental health services in 11 countries, and documents factors associated with delays in referral. The pathways in centres relatively well provided with psychiatric staff were dominated by general practitioners and to a lesser extent hospital doctors: the relatively less well resourced centres showed a variety of pathways with native healers often playing an important part. Delays were remarkably short in all centres regardless of psychiatric resources, but in some centres we found longer delays on pathways involving native healers. Somatic problems were a common presentation in all centres, and in some centres there was a tendency for patients presenting with somatic problems to have longer delays than those with symptoms of depression or anxiety. The implications of these findings are discussed in the context of an ongoing programme of WHO research activities aimed at improving the quality of mental illness care available in community settings

Abstract

Introducción: La remoción de lesiones etmoidales con extensión secundaria a la base cranealy/o región facial, entraña una elevada complejidad y se encuentra asociada a una elevada morbilidad. Objetivo: Determinar los resultados de la cirugía combinada craneofacial en pacientes con neoplasias etmoidales extendidas. Métodos: Se realizó un estudio descriptivo retrospectivo transversal. La muestra estuvo conformada por los pacientes intervenidos quirúrgicamente de lesiones de base craneal anterior mediante un abordaje combinado craneofacial durante el período: enero de 2009 a enero de 2012 en el Instituto Nacional de Oncología y Radiobiología con un seguimiento de 2 años. Se emplearon variables estadísticas descriptivas. Resultados: Fueron intervenidos 20 pacientes. El promedio de edad fue de 44,8 años. Predominó el sexo masculino (65%). La obstrucción nasal constituyó la presentación más frecuente. El adenocarcinoma y el carcinoma epidermoide constituyeron las lesiones malignas más frecuentes. Dentro de las lesiones benignas predominó el papiloma invertido. Las técnicas más empleadas fueron la craneotomía frontal bilateral y la etmoidectomía total. La complicación más frecuente fue la fístula de líquido cefalorraquídeo. La supervivencia global a los 2 años en los pacientes con lesiones malignas fue del 35%. Conclusiones: Se identificó un elevado número de complicaciones pero no afectaron la supervivencia ni la calidad de vida. Se logró una aceptable tasa de supervivencia en lesiones malignas. Introduction: The removal of ethmoidal tumors with secondary extension to the cranial base and/or facial region involves a high complexity and it is associated to a high morbility. Objective: To determine the results of craniofacial surgery in patient with ethmoid extended tumors. Methods: It was carried out a traverse retrospective descriptive study. The sample was conformed by the patients intervened surgically of anterior cranial base lesions by means of a combined craneofacial surgery during the period: January of the 2009 to January of the 2012 in the National Institute of Oncology and Radiobiology with a 2 year pursuit. Descriptive statistical variables were used. Results: 20 patients were intervened. The age average was of 44,8 years. It prevail the masculine sex (65%). Nasal obstruction constitutes the most frequent presentation. Adenocarcinoma and epidermoid carcinoma constituted the most frequent malignant lesions. Inside the benign lesions prevail the invertedpapiloma. The techniques more employees were the bilateral frontal craneotomy and total etmoidectomy. The most frequent complication was the cerebrospinal fluid leak. The 2 years overall survival in patients with malignant lesions was 35%. Conclusions: A high number of complications was identified but they didn’t affect the survival neither the quality of life. An acceptable rate of survival was achieved in malignant lesions.

Abstract

Objective: Large numbers of psychiatric patients either are involuntarily admitted to hospital treatment or feel coerced despite a legally voluntary admission. For ethical and clinical reasons, their perceived coercion should be reduced as far as possible. There is however limited evidence on patient characteristics associated with perceived coercion during hospital treatment. This study aimed to identify i) sociodemographic and clinical characteristics associated with perceived coercion at admission and ii) changes in symptoms and global functioning associated with changes in perceived coercion over time. Method: Three thousand and ninety three in-patients who were involuntarily admitted or felt coerced to hospital treatment despite a legally voluntary admission were recruited in the European evaluation of coercion in psychiatry and harmonization of best clinical practice - EUNOMIA project in 11 European countries. Perceived coercion, global functioning and symptoms were assessed after admission and at a 3-month follow-up. Results: Involuntary admission, female gender, poorer global functioning and more positive symptoms were associated with higher levels of perceived coercion at admission. Perceived coercion significantly decreased over time, and the improvements in global functioning and positive symptoms were associated with reduction in perceived coercion. Conclusion: Female patients perceive more coercion in psychiatric hospital treatment. Effective treatment for positive symptoms and improving patients' global functioning may lead to a reduction in perceived coercion.

Abstract

A 3-day international workshop on atmospheric monitoring and calibration for high-energy astroparticle detectors, with a view towards next-generation facilities. The atmosphere is an integral component of many high-energy astroparticle detectors. Imaging atmospheric Cherenkov telescopes and cosmic-ray extensive air shower detectors are the two instruments driving the rapidly evolving fields of very-high- and ultra-high-energy astrophysics. In these instruments, the atmosphere is used as a giant calorimeter where cosmic rays and gamma rays deposit their energy and initiate EASs; it is also the medium through which the resulting Cherenkov light propagates. Uncertainties in real-time atmospheric conditions and in the fixed atmospheric models typically dominate all other systematic errors. With the improved sensitivity of upgraded IACTs such as H.E.S.S.-II and MAGIC-II and future facilities like the Cherenkov Telescope Array (CTA) and JEM-EUSO, statistical uncertainties are expected to be significantly reduced, leaving the atmosphere as the limiting factor in the determination of astroparticle spectra. Varying weather conditions necessitate the development of suitable atmospheric monitoring to be integrated in the overall instrument calibration, including Monte Carlo simulations. With expertise distributed across multiple collaborations and scientific domains, an interdisciplinary workshop is being convened to advance progress on this critical and timely topic.

Abstract

The Cherenkov Telescope Array (CTA) is the next generation of Imaging Atmospheric Cherenkov Telescopes. It would reach unprecedented sensitivity and energy resolution in very-high-energy gamma-ray astronomy. In order to reach these goals, the systematic uncertainties derived from the varying atmospheric conditions shall be reduced to the minimum. Different instruments may help to account for these uncertainties. Several groups in the CTA consortium are currently building Raman LIDARs to be installed at the CTA sites. Raman LIDARs are devices composed of a powerful laser that shoots into the atmosphere, a collector that gathers the backscattered light from molecules and aerosols, a photosensor, an optical module that spectrally select wavelengths of interest, and a read-out system. Raman LIDARs can reduce the systematic uncertainties in the reconstruction of the gamma-ray energies down to 5 % level. All Raman LIDARs subject of this work, have design features that make them different than typical Raman LIDARs used in atmospheric science, and are characterized by large collecting mirrors ($\sim2~$m$^2$). They have multiple elastic and Raman read-out channels (at least 4) and custom-made optics design.
