BACKGROUND Left ventricular noncompaction (LVNC) is a heterogeneous entity with uncertain prognosis. OBJECTIVES This study sought to develop and validate a prediction model of major adverse cardiovascular events (MACE) and to identify LVNC cases without events during long-term follow-up. METHODS This is a retrospective longitudinal multicenter cohort study of consecutive patients fulfilling LVNC criteria by echocardiography or cardiovascular magnetic resonance. MACE were defined as heart failure (HF), ventricular arrhythmias (VAs), systemic embolisms, or all-cause mortality. RESULTS A total of 585 patients were included (45 +/- 20 years of age, 57% male). LV ejection fraction (LVEF) was 48% +/- 17%, and 18% presented late gadolinium enhancement (LGE). After a median follow-up of 5.1 years, MACE occurred in 223 (38%) patients: HF in 110 (19%), VAs in 87 (15%), systemic embolisms in 18 (3%), and 34 (6%) died. LVEF was the main variable independently associated with MACE (P < 0.05). LGE was associated with HF and VAs in patients with LVEF > 35% (P < 0.05). A prediction model of MACE was developed using Cox regression, composed by age, sex, electrocardiography, cardiovascular risk factors, LVEF, and family aggregation. C-index was 0.72 (95% confidence interval: 0.67-0.75) in the derivation cohort and 0.72 (95% confidence interval: 0.71-0.73) in an external validation cohort. Patients with no electrocardiogram abnormalities, LVEF $50%, no LGE, and negative family screening presented no MACE at follow-up. CONCLUSIONS LVNC is associated with an increased risk of heart failure and ventricular arrhythmias. LVEF is the variable most strongly associated with MACE; however, LGE confers additional risk in patients without severe systolic dysfunction. A risk prediction model is developed and validated to guide management.

The Influence of diabetes mellitus in the late outcome of coronary stenting remains controversial.The aim of this study was to determine the late clinical outcome of diabetics in comparison with non diabetics and to establish whether there are subgroups of diabetic patients with a greater need for target lesion revascularization.Two hundred sixteen consecutive patients (74 diabetics; 95 stents in 90 lesions and 142 non diabetics) who had successfully undergone coronary stenting were included in the study and followed over 17.6 +/- 10 months. The clinical events evaluated were target lesion revascularization, death and acute myocardial infarction. Independent predictive variables of target lesion revascularization were studied in both groups of patients.The diabetic patients presented greater cardiovascular mortality (6.7% vs 1.4%; p=0.02) but the incidence of infarction was similar in the two groups (2.7% vs. 3.5%; p=0.6). The accumulated rate of target lesion revascularization at two years was 18.2% in diabetics vs 13.3% in non diabetics (p=0.09), respectively. The presence of three vessel disease (p=0.014), history of arterial hypertension ([=0.011) and residual stenosis0% (p=0.005) were specific predictive factors of target lesion revascularization for diabetic patients and together with vessel diameter3mm (p0.001) subgroups of diabetics were independently selected with a significantly greater incidence of target lesion revascularization than the non diabetic patients.Following coronary stenting, diabetic patients show a greater cardiovascular mortality than non diabetics, but only some subgroups of diabetics (small vessels extensive coronary disease, associated arterial hypertension, residual stenosis) show a significantly greater risk of target lesion revascularization.

- Abstract
Clinical PET acquisitions of the heart suffer from artefacts and drops in image quality due to the poor spatial resolution of the PET system. Moreover, cardiac PET images are further degraded by the blur caused by the breathing and beating motions, thus hampering diagnosis and evaluation of myocardial pathologies. Anatomy-enhanced PET reconstruction, using a high-resolution CT, has proven useful in brain imaging. In cardiac datasets however, due to the motion artefacts, the application of any restoring technique on datasets affected by motion blur needs to be preceded by the validation of the proposed method on realistic static datasets. In this work, the validation is performed using static cardiac ex vivo datasets obtained from a number of sacrificed sheep, scanned on a clinical PET/CT scanner. The aim of this work is to assess the effectiveness of reconstructions of the acquired datasets with different CT-based anatomical priors, in comparison to reconstructions currently applied in clinical practise. The gold standard to which all reconstructions are compared consists of images of the same hearts scanned on a small-animal PET scanner, whose high spatial resolution allows for almost artefact-free images. Encouraging results were obtained so far, with improvements in volume delineation and uniformity of activity values when anatomical information was used. Fig 1 shows the gold standard image (left) compared to a regular clinical reconstruction (middle) and to a reconstruction using the high-resolution CT as anatomical information (right).

- Abstract
Purpose: Transthoracic 3D echocardiography (3DE) allows an unparalleled opportunity for quantifying the dynamic changes of the tricuspid annulus (TA). Accordingly, our aims were: (I) to assess the determinants of TA size during cardiac cycle in healthy subjects; (II) to propose an approach and timing for TA sizing using 3DE. Methods: In 50 healthy volunteers (45±14 yrs, range 18-74, 27 males, with no risk factors, symptoms, signs or history of cardiovascular disease and on no medication), a full-volume dataset of the right ventricle (RV) containing the tricuspid valve (TV) was acquired (Vivid E9, GE Healthcare). TA diameters (septo-lateral, SL; antero-posterior, AP) and areas were measured on multiplanar images (Flexi-slice, EchoPac BT12, GE Healthcare) at 5 time points during the cardiac cycle: OS (onset of systole, at TV closure); MS (mid-systole); ES (end-systole); ED (onset of diastole); LD (late diastole, after the P wave). RV volumes and ejection fraction (EF) were analyzed with commercial software (4D RV analysis, TomTec, D). Results: Temporal resolution of the 3D datasets was 32±4 vps (range 24-53). TA areas were more closely correlated with RV volumes and body surface area (BSA) than with either SL or AP diameters. TA areas increased during systole from OS (3.9±0.6 cm2/m2) to ES (4.9±0.8 cm2/m2) and reached its largest area in LD (6.7±1.0 cm2/m2). All 5 TA areas were correlated with BSA (r range 0.57-0.62) and RV volumes (r ranges 0.53-0.60 for end-diastolic volume and 0.43-0.50 for end-systolic volume, p<0.0001 for all). Indexed TA areas were not related to either age or gender. With multivariable analysis, both RV end-diastolic volume and BSA determined TA areas during systole and early diastole, while TA area at LD and at OS were independently related with BSA only. Conclusions: In healthy subjects, the main determinants of TA size are RV volume and BSA. The largest TA area occurs at LD and is independently related with BSA only. Therefore, normative values should be based on TA areas measured at LD and indexed for BSA. However, the rapid change in TA areas occurring from LD to OS underscores the importance of adequate temporal resolution of 3DE data sets for reliable TA measurements.

- Abstract
Background: Current guidelines accept planimetry of anatomic aortic valve stenosis (AVS) area (AVA) by means of three-dimensional transesophageal echocardiography (3D-TEE) as "reasonable when additional information is needed in selected patients", but it is not considered appropriate for direct clinical-decision making. We aimed to test the long-term prognostic value of 3D-TEE planimetered AVS area. Methods: 282 patients with moderate and severe AVS underwent 3D-TEE with planimetry of AVA. Vital status was assessed after 5.58 years of follow-up and analyzed by Kaplan-Meier and Cox methods.

In multivariate Cox regression analysis, AVA, EuroScoreII value as measure of co-morbidity and absence of aortic valve replacement were independently associated with higher mortality rate (Table). K-M curves showed increased mortality rate in patients with severe aortic stenosis (as defined by AVA≤0.8cm2) assessed by means of 3D-TEE (Figure), Log Rank p=0.022. - Abstract
Purpose: With the advent of percutaneous transcatheter device closures in congenital heart defects and the emergence of percutaneous left atrial appendage closure, there is an increasingly important role for echocardiographic guidance and control of device position and function. Disc occluder devices frequently present as an unexplained ‘figure-of-8’ on echocardiography. The aim of this study was to clarify this ‘figure-of-8’ display and to relate its morphology to transducer position and device type. Methods: A mathematical model was developed to resemble disc occluder geometry and to allow a numerical simulation of the echocardiographic appearance. In addition, we developed an in vitro set-up for echocardiographic analysis of various disc occluders and various transducer positions. Results: In the mathematical model of an epitrochoid curve (closely resembling disc occluder geometry) a ‘figure-of-8’ display is obtained when emphasizing points with tangent vector perpendicular to the direction of ultrasound waves. Decreasing imaging depth results in a more asymmetric ‘figure-of-8’, with small upper part and wide lower part. Clinical and in vitro data are in close agreement with these results (Figure 1). Furthermore a ‘figure-of-8’ display is only obtained in a coronal imaging position, and is similar for different commercially available disc occluder types. Conclusions: The ‘figure-of-8’ display in the ultrasound image of a disc occluder is an imaging artifact due to the specific ‘epitrochoidal’ geometry of a deployed device and its interaction with ultrasound waves. The morphology of the ‘figure-of-8’ depends on transducer position, i.e. imaging depth, and is similar for different device types.

- Abstract
Pulmonary vascular dysfunction is claimed to be a contributor to the development of pulmonary hypertension (PH). Impaired systemic vascular reactivity is one of the essential factors in the pathogenesis of cardiovascular disease. The aim of the investigation was to study whether there is any association between systemic vascular function and pulmonary artery pressure (PAP) in patients who have associated causes for PH development, such as coronary heart disease (CHD) and chronic obstructive pulmonary disease (COPD). Methods: The brachial artery vasodilator responses were measured by the ultrasound technique in twenty patients with mild to moderate COPD (group I) and twenty age–matched and COPD stage-matched patients who had past history of myocardial infarction (NYHA II) (group II).Conventional echocardiographic variables were measured in the said patients too. Results: Both flow-mediated dilatation (FMD) and nitrate-mediated dilatation (NMD) were significantly lower, and PAP was significantly higher in the group II patients compared to the same parameters of group I patients. NMD was inversely correlated with PAP (r=-0.7, p=0.02) in group I patients. There was no interrelation between FMD and PAP in patients from group I. Neither FMD nor NMD were correlated with PAP in group II patients. A significant positive correlation between PAP and left ventricular mass index (r=0.8, p=0.003) was revealed in the said patients as well. Conclusions: Attenuated vasodilator response of brachial artery to nitroglycerine is associated with PAP elevation in COPD patients. PH is closely related to cardiac remodeling in COPD patients in whom CHD developed. These data suggest different "stages" of vascular and cardiac remodeling in patients with COPD alone and in coexistence with CHD. The obtained data can be useful in the selection of treatment as regards these patient categories.

- Abstract
Purpose: Paravalvular leak (PVL) is not an uncommon complication of valve surgery. PVL severity, size, shape and morphology are important factors in determining subsequent clinical presentation and the severity of heart failure. Symptoms may be due to hemolytic anemia as a result of hemolysis. RT-3D TEE is superior over 2D TEE as it provides additional information about the complex 3D anatomy. Methods: The study was comprised of 108 patients with severe mitral PVLs. The PVLs were visualized using two dimensional transesophageal echocardiography (2D TEE) and subsequent real time three dimensional transesophageal echocardiography (RT-3D TEE). The relationship between echocardiographic findings and development of hemolysis were studied. The presence and severity of hemolysis was assessed on the basis of serum lactic dehydrogenase, blood hemoglobin, bilirubin and reticulocyte levels. Results: The study group included mechanical (n= 98) and bioprosthetic valves (n= 10). The most common PVL localizations were anterolateral commisure, posteromedial commissures and posterolateral region. The mean PVL width measured by 2D TEE was 3.2 ± 1.5 (range; 2-7) mm and the mean width of defect measured by 3D TEE was 3.1 ± 1.7 (range; 2-7) mm (p= 0.6). The mean length of defect measured by 3D TEE was 12.5 ± 7 (range; 4-22) mm. The mean area of severe PVLs was 35 ± 25 mm2 (range; 8-100). Fifty-six patients (52 %) PVLs were oval/round shaped whereas 34 (31 %) were cresentric, 11 (10 %) were slit-like and 5 (5 %) were slope-tunnel shaped. Two posteriorly-localized defects (2 %) were not clearly visualized by RT-3D TEE. Nine cresentic-shaped (8%) and 4 slit-like shaped (% 4) PVLs were ‘multi-fenestrated’. Besides, 74 % of the cresentic shaped leaks had irregular borders (n=25). The oval / round, tunnel-like and slit-like defects had regular borders. The patients were followed-up for 4 ± 0.8 years. Hemolysis was observed in 65 (60 %) patients. The patients with cresentic and irregular defects had significantly higher severity of hemolysis compared to the others (p=0.02). The size of the defect did not correlate with severity of hemolysis. Conclusions: RT-3D TEE permits detailed structural evaluation of paravalvular leak shape which may contribute to the severity of hemolysis.

- Abstract
Aims: Late gadolinium enhancement (LGE) is frequently found in patients with dilated cardiomyopathy (DCM); there is little information about its frequency and distribution pattern according to the underlying genetic substrate. We sought to describe LGE patterns according to genotypes and to analyse the risk of major ventricular arrhythmias (MVA) according to patterns., Methods and Results: Cardiac magnetic resonance findings and LGE distribution according to genetics were performed in a cohort of 600 DCM patients followed at 20 Spanish centres. After exclusion of individuals with multiple causative gene variants or with variants in infrequent DCM-causing genes, 577 patients (34% females, mean age 53.5 years, left ventricular ejection fraction 36.9 ± 13.9%) conformed to the final cohort. A causative genetic variant was identified in 219 (38%) patients, and 147 (25.5%) had LGE. Significant differences were found comparing LGE patterns between genes (P < 0.001). LGE was absent or rare in patients with variants in TNNT2, RBM20, and MYH7 (0, 5, and 20%, respectively). Patients with variants in DMD, DSP, and FLNC showed a predominance of LGE subepicardial patterns (50, 41, and 18%, respectively), whereas patients with variants in TTN, BAG3, LMNA, and MYBPC3 showed unspecific LGE patterns. The genetic yield differed according to LGE patterns. Patients with subepicardial, lineal midwall, transmural, and right ventricular insertion points or with combinations of LGE patterns showed an increased risk of MVA compared with patients without LGE., Conclusion: LGE patterns in DCM have a specific distribution according to the affected gene. - Abstract
Aims: Genotype and left ventricular scar on cardiac magnetic resonance (CMR) are increasingly recognized as risk markers for adverse outcomes in non-ischaemic dilated cardiomyopathy (DCM). We investigated the combined influence of genotype and late gadolinium enhancement (LGE) in assessing prognosis in a large cohort of patients with DCM., Methods and Results: Outcomes of 600 patients with DCM (53.3 ± 14.1 years, 66% male) who underwent clinical CMR and genetic testing were retrospectively analysed. The primary endpoints were end-stage heart failure (ESHF) and malignant ventricular arrhythmias (MVA). During a median follow-up of 2.7 years (interquartile range 1.3-4.9), 24 (4.00%) and 48 (8.00%) patients had ESHF and MVA, respectively. In total, 242 (40.3%) patients had pathogenic/likely pathogenic variants (positive genotype) and 151 (25.2%) had LGE. In survival analysis, positive LGE was associated with MVA and ESHF (both, p < 0.001) while positive genotype was associated with ESHF (p = 0.034) but not with MVA (p = 0.102). Classification of patients according to genotype (G+/G-) and LGE presence (L+/L-) revealed progressively increasing events across L-/G-, L-/G+, L+/G- and L+/G+ groups and resulted in optimized MVA and ESHF prediction (p < 0.001 and p = 0.001, respectively). Hazard ratios for MVA and ESHF in patients with either L+ or G+ compared with those with L-/G- were 4.71 (95% confidence interval: 2.11-10.50, p < 0.001) and 7.92 (95% confidence interval: 1.86-33.78, p < 0.001), respectively., Conclusion: Classification of patients with DCM according to genotype and LGE improves MVA and ESHF prediction. Scar assessment with CMR and genotyping should be considered to select patients for primary prevention implantable cardioverter-defibrillator placement., (© 2022 The Authors. - Abstract
Background: Myocarditis occasionally is related to arrhythmogenic right ventricular dysplasia (ARVD) and sometimes overlaps during the early stages, which may lead to misdiagnosis. Acute myocarditis may reflect an active phase of ARVD., Objective: The purpose of this study was to evaluate the genetic basis of myocarditis in ARVD and to investigate the association with a poorer prognosis and a higher risk of ventricular arrhythmias., Methods: Two groups were analyzed: group A, which consisted of 131 affected patients-84 with ARVD (62% male, age 45 years [range 33-55 years]) and 47 with left-sided forms (arrhythmogenic left ventricular dysplasia [ALVD]) (47% male, age 45 years [range 25-61 years]); and group B, which consisted of 64 nonaffected mutation-carrying relatives (36% male, age 42 years [range 22-56 years]; 23 from classic ARVD families and 41 from ALVD families)., Results: Seven patients (3.5%) presented with a clinical diagnosis of acute myocarditis over median follow-up of 34 months. Myocarditis was the first clinical presentation in 6 of 7 cases. In 2 patients, acute myocarditis preceded worsening of left ventricular systolic function. In 1 case, myocarditis was associated with an increased gadolinium pattern in cardiac magnetic resonance. Two patients presented with ECG changes weeks after myocarditis resolution. Myocarditis preceded the development of ventricular tachycardia in 2 other patients. Myocarditis clustered in families bearing DSP Q447* and LDB3 c.1051A>G., Conclusion: Acute myocarditis reflects an active phase of ARVD that leads to changes in phenotype and abrupt progression of the disease. An active phase should be suspected in a patient with myocarditis associated with a family history of ARVD.
