- Abstract
Osteoarthritis (OA) is calcification of the joint associated with severe painful sensation caused by joint instability. This joint instability is affected by destruction of cartilage that protects the bones. Osteoarthritis can be caused by various factors such as age, genetic as well as vigorous exercise or activity. As the cartilage destruction progresses, the unprotected bone will rub on the other bone in the joint which can lead to osteoarthritis. Management of OA consists of a variety of treatments, such as pharmacological, non-pharmacological, non-pharmacotherapy, and surgery treatments. Due to the development of science and technology, various modalities have emerged that can support the treatment of OA inter alia prolotherapy. Prolotherapy or regenerative injection could influence the recovery of the destructed area directly and relief the painful sensation through the actions of inflammatory cells, macrophages, immune cells, and cytokines, therefore, the improvement of the destructed areas could occur faster. It was reported that knee osteoarthritis could be treated with prolotherapy successfully. In conclusion, since prolotherapy influences the body to repair the destructed areas, hence it could be used as the new osteoarthritis treatment, especially knee osteoarthritis.Keywords: prolotherapy, knee osteoarthritis, chronic pain Abstrak: Osteoartritis (OA) merupakan pengapuran sendi yang disertai nyeri hebat, disebabkan oleh karena adanya ketidakstabilan sendi yang dipengaruhi oleh rusaknya tulang rawan yang berperan untuk melindungi tulang. Osteoartritis dapat disebabkan oleh berbagai faktor seperti usia, jenis kelamin, genetik, aktivitas maupun olahraga berat. Saat tulang rawan rusak, maka tulang yang tidak dilindungi dapat saling bersinggungan di sendi sehingga sendi hancur dan berujung pada OA. Pengobatan untuk OA terdiri dari beragam pengobatan baik farmakologi, non-farmakologi, non-farmakoterapi, maupun tindakan operasi. Seiring berjalannya perkembangan ilmu pengetahuan dan teknologi, maka muncul berbagai modalitas yang dapat menunjang pengobatan OA, antara lain proloterapi. Proloterapi atau injeksi regeneratif memengaruhi dan memberi dampak penyembuhan secara langsung pada area yang mengalami cedera maupun nyeri melalui kerja sel-sel radang, makrofag, sel-sel imun, dan sitokin sehingga dapat bekerja lebih cepat untuk perbaikan daerah yang cedera. Telah dilaporkan hasil bermakna dalam peng-gunaan proloterapi pada osteoartritis lutut. Simpulan penelitian ini ialah proloterapi merangsang tubuh untuk memperbaiki daerah yang cedera sehingga merupakan solusi terbaik yang dibutuhkan dalam pengobatan OA dewasa ini terutama pada OA lutut.Kata kunci: proloterapi, osteoartritis lutut, nyeri kronik

17. Dependence of COVID-19 Policies on End-of-Year Holiday Contacts in Mexico City Metropolitan Area: A Modeling Study.

- Abstract
Background. Mexico City Metropolitan Area (MCMA) has the largest number of COVID-19 (coronavirus disease 2019) cases in Mexico and is at risk of exceeding its hospital capacity in early 2021. Methods. We used the Stanford-CIDE Coronavirus Simulation Model (SC-COSMO), a dynamic transmission model of COVID-19, to evaluate the effect of policies considering increased contacts during the end-of-year holidays, intensification of physical distancing, and school reopening on projected confirmed cases and deaths, hospital demand, and hospital capacity exceedance. Model parameters were derived from primary data, literature, and calibrated. Results. Following high levels of holiday contacts even with no in-person schooling, MCMA will have 0.9 million (95% prediction interval 0.3-1.6) additional COVID-19 cases between December 7, 2020, and March 7, 2021, and hospitalizations will peak at 26,000 (8,300-54,500) on January 25, 2021, with a 97% chance of exceeding COVID-19-specific capacity (9,667 beds). If MCMA were to control holiday contacts, the city could reopen in-person schools, provided they increase physical distancing with 0.5 million (0.2-0.9) additional cases and hospitalizations peaking at 12,000 (3,700-27,000) on January 19, 2021 (60% chance of exceedance). Conclusion. MCMA must increase COVID-19 hospital capacity under all scenarios considered. MCMA's ability to reopen schools in early 2021 depends on sustaining physical distancing and on controlling contacts during the end-of-year holiday.

The electron cyclotron (EC) system of ITER for the initial configuration is designed to provide 20MW of RF power into the plasma during 3600s and a duty cycle of up to 25% for heating and (co and counter) non-inductive current drive, also used to control the MHD plasma instabilities. The EC system is being procured by 5 domestic agencies plus the ITER Organization (IO). F4E has the largest fraction of the EC procurements, which includes 8 high voltage power supplies (HVPS), 6 gyrotrons, the ex-vessel waveguides (includes isolation valves and diamond windows) for all launchers, 4 upper launchers and the main control system. F4E is working with IO to improve the overall design of the EC system by integrating consolidated technological advances, simplifying the interfaces, and doing global engineering analysis and assessments of EC heating and current drive physics and technology capabilities. Examples are the optimization of the HVPS and gyrotron requirements and performance relative to power modulation for MHD control, common qualification programs for diamond window procurements, assessment of the EC grounding system, and the optimization of the launcher steering angles for improved EC access. Here we provide an update on the status of Europe’s contribution to the ITER EC system, and a summary of the global activities underway by F4E in collaboration with IO for the optimization of the subsystems.

The study of structural safety when it is covered by important paintings requires a careful analysis without affecting the paintings. Therefore, non-destructive surveys are the only feasible method to obtain data about its state. In Cueva Pintada case, the problem requires a detailed but noninvasive analysis because this cave was dug in volcanic tuff. The tuff presents poor consolidation. This geological material is very sensitive to vibration. Furthermore, the proximity to the city downtown increases vibrations due to traffic. Recently, a small rockfall produces damage and alerted of the cave state of conservation. The painting walls constraint many of the NDT surveys. Two methodologies have been applied: ground-penetrating radar (GPR) and passive seismic. GPR analysis allowed determining the inner structure of the tuff cave, and results were used to determine the zones that required a wider and dense vibration study. Radar images show important anomalies at depths of about 0.2 m to 0.8 m and from 1.5 m to the top of the roof, almost horizontal. Amplification factor measured by passive seismic survey ranges from 0.03 to 93, with near half of the ceil with amplifications higher than 5 and about 10% with amplifications over 50 times. In Chile, since 2002, there are extracurricular enrichment programs complementary to regular education for students with high intellectual capacity. This paper describes a model for the educational inclusion of students, with special educational needs associated with high intellectual capacity, developed at the University of Concepción and its effects on its students, academics and undergraduate students that collaborate with the program. The Talentos UdeC Program was created in 2003 and is intended for 240 children and youth from 11 to 18 years old, from 15 communes of the Biobio region. The case Talentos UdeC is analyzed from a mixed qualitative study in which those participating in the educational model are considered. The sample was composed of 30 students, 30 academics, and 30 undergraduate students. In the case of students, pre and post program measurements were made to analyze their socio-emotional adaptation, academic motivation and socially responsible behavior. The mentioned variables are measured through questionnaires designed and validated by the University of Concepcion that included: The Socially Responsible Behavior Questionnaire (CCSR); the Academic Motivation Questionnaire (CMA) and the Socio-Emotional Adaptation Questionnaire (CASE). The information obtained by these questionnaires was analyzed through a quantitative analysis. Academics and undergraduate students were interviewed to learn their perception of the effects of the program on themselves, on students and on society. The information obtained is analyzed using qualitative analysis based on the identification of common themes and descriptors for the construction of conceptual categories of answers. Quantitative results show differences in the first three variables analyzed in the students, after their participation for two years in Talentos UdeC. Qualitative results demonstrate perception of effects in the vision of world, project of life and in other areas of the students’ development; perception of effects in a personal, professional and organizational plane by academics and a perception of effects in their personal-social development and training in generic competencies by undergraduates students.

The eudaimonic perspective in philosophy and psychology suggests that a good life is closely related to developing oneself in order to contribute to the well-being and happiness of other people and of the world as a whole. Educational psychology can help to achieve this through the design and validation of educative models. Since 2004, the University of Concepcion and other Chilean universities apply an educative model to train socially responsible professionals, people that in the exercise of their profession contribute to generate equity for the development and assess the impacts of their decisions, opting for those that serve the common good. The main aim is to identify if a relationship exists between achieved learning, attitudes toward social responsibility, self-attribution of socially responsible behavior, value type, professional behavior observed and, participation in a specific model to train socially responsible (SR) professionals. The Achieved Learning and Attitudes Toward Social Responsibility Questionnaire, interview with employers and Values Questionnaire and Self-attribution of SR Behavior Questionnaire is applied to 394 students and graduates, divided into experimental and control groups (trained and not trained under the educative model), in order to identify the professional behavior of the graduates. The results show that students and graduates perceive cognitive, affective and behavioral learning, with significant differences in attitudes toward social responsibility and self-attribution of SR behavior, between experimental and control. There are also differences in employers' perceptions about the professional practice of those who were trained under the model and those who were not. It is concluded that the educative model has an impact on the learning of social responsibility and educates for a full life. Journal Article; Multicenter Study; Research Support, Non-U.S. Gov't; BACKGROUND Patients with chronic obstructive pulmonary disease (COPD) have a modified clinical presentation of venous thromboembolism (VTE) but also a worse prognosis than non-COPD patients with VTE. As it may induce therapeutic modifications, we evaluated the influence of the initial VTE presentation on the 3-month outcomes in COPD patients. METHODS COPD patients included in the on-going world-wide RIETE Registry were studied. The rate of pulmonary embolism (PE), major bleeding and death during the first 3 months in COPD patients were compared according to their initial clinical presentation (acute PE or deep vein thrombosis (DVT)). RESULTS Of the 4036 COPD patients included, 2452 (61%; 95% CI: 59.2-62.3) initially presented with PE. PE as the first VTE recurrence occurred in 116 patients, major bleeding in 101 patients and mortality in 443 patients (Fatal PE: first cause of death). Multivariate analysis confirmed that presenting with PE was associated with higher risk of VTE recurrence as PE (OR, 2.04; 95% CI: 1.11-3.72) and higher risk of fatal PE (OR, 7.77; 95% CI: 2.92-15.7). CONCLUSIONS COPD patients presenting with PE have an increased risk for PE recurrences and fatal PE compared with those presenting with DVT alone. More efficient therapy is needed in this subtype of patients. Yes

Factors associated with the detection of raised systolic pulmonary artery pressure (sPAP) levels in patients with a prior episode of pulmonary embolism (PE) are not well known.We used the RIETE Registry database to identify factors associated with the finding of sPAP levels ≥50 mm Hg on trans-thoracic echocardiography, in 557 patients with a prior episode of acute, symptomatic PE.Sixty-two patients (11.1%; 95% CI: 8.72-14.1) had sPAP levels ≥50 mm Hg. These patients were more likely women, older, and more likely had chronic lung disease, heart failure, renal insufficiency or leg varicosities than those with PAP levels50mm Hg. During the index PE event, they more likely had recent immobility, and more likely presented with hypoxemia, increased sPAP levels, atrial fibrillation, or right bundle branch block. On multivariate analysis, women aged ≥70 years (hazard ratio [HR]: 2.0; 95% CI: 1.0-3.7), chronic heart or chronic lung disease (HR: 2.4; 95% CI: 1.3-4.4), atrial fibrillation at PE presentation (HR: 2.8; 95% CI: 1.3-6.1) or varicose veins (HR: 1.8; 95% CI: 1.0-3.3) were all associated with an increased risk to have raised sPAP levels. Chronic heart disease, varicose veins, and atrial fibrillation were independent predictors in women, while chronic heart disease, atrial fibrillation, a right bundle branch block or an S1Q3T3 pattern on the electrocardiogram were independent predictors in men.Women aged ≥70 years more likely had raised sPAP levels than men after a PE episode. Additional variables influencing this risk seem to differ according to gender.

[Background] Physicians need a specific risk-stratification tool to facilitate safe and cost-effective approaches to the management of patients with cancer and acute pulmonary embolism (PE). The objective of this study was to develop a simple risk score for predicting 30-day mortality in patients with PE and cancer by using measures readily obtained at the time of PE diagnosis., [Methods] Investigators randomly allocated 1,556 consecutive patients with cancer and acute PE from the international multicenter Registro Informatizado de la Enfermedad TromboEmbólica to derivation (67%) and internal validation (33%) samples. The external validation cohort for this study consisted of 261 patients with cancer and acute PE. Investigators compared 30-day all-cause mortality and nonfatal adverse medical outcomes across the derivation and two validation samples., [Results] In the derivation sample, multivariable analyses produced the risk score, which contained six variables: age > 80 years, heart rate ≥ 110/min, systolic BP < 100 mm Hg, body weight < 60 kg, recent immobility, and presence of metastases. In the internal validation cohort (n = 508), the 22.2% of patients (113 of 508) classified as low risk by the prognostic model had a 30-day mortality of 4.4% (95% CI, 0.6%–8.2%) compared with 29.9% (95% CI, 25.4%–34.4%) in the high-risk group. In the external validation cohort, the 18% of patients (47 of 261) classified as low risk by the prognostic model had a 30-day mortality of 0%, compared with 19.6% (95% CI, 14.3%–25.0%) in the high-risk group., [Conclusions] The developed clinical prediction rule accurately identifies low-risk patients with cancer and acute PE., We are grateful to Sanofi Spain for supporting this registry with an unrestricted educational grant. Summary. Background: The influence of recent immobilization or surgery on mortality in patients with pulmonary embolism (PE) is not well known. Methods: We used the Registro Informatizado de Enfermedad TromboEmbolica (RIETE) data to compare the 3-month mortality rate in patients with PE, with patients categorized according to the presence of recent immobilization, recent surgery, or neither. Results: Of 18 028 patients with PE, 4169 (23%) had recent immobilization, 2212 (12%) had recent surgery, and 11 647 (65%) had neither. The all-cause mortality was 10.0% (95% confidence interval [CI] 9.5–10.4), and the PE-related mortality was 2.6% (95% CI 2.4–2.9). One in every two patients who died from PE had recent immobilization (43%) or recent surgery (6.7%). Only 25% of patients with immobilization had received prophylaxis, as compared with 65% of the surgical patients. Fatal PE was more common in patients with recent immobilization (4.9%; 95% CI 4.3–5.6) than in those with surgery (1.4%; 95% CI 1.0–2.0) or those with neither (2.1%; 95% CI 1.8–2.3). On multivariate analysis, patients with immobilization were at increased risk for fatal PE (odds ratio 2.2; 95% CI 1.8–2.7), with no differences being seen between patients immobilized in hospital or in the community. Conclusions: Forty-three per cent of patients dying from PE had recent immobilization for ≥ 4 days. Many of these deaths could have been prevented.

Ground-penetrating radar (GPR) is a geophysical and close-range remote sensing technique based on the use of radar pulses to obtain cross-section images of underground features. This method is characterized by the transmission of an electromagnetic short length pulse (1-2 ns), presenting a centre frequency ranging from 10 MHz to 2.5 GHz. The principles of GPR operation are based on the ability of low frequency radar waves to penetrate into a non-conductive medium, usually subsoil, but also walls, concrete or wood. Those waves are detected after suffering a reflection in electromagnetic discontinuities of the propagation medium. Therefore, this is a suitable method to study changes in those physical properties, and also to characterize different mediums and the reflective targets providing information about their physical properties. The aim of this work is to describe and demonstrate different applications of GPR in forestry, showing the obtained results together with their interpretation. Firstly, in this paper, it is illustrated how GPR is able to map shallow bedrock, subsoil stratigraphy and also to estimate shallow watertable depth. Secondly, different tree trunks as well as dry timber are analyzed, evaluating the different radar data obtained in each particular case, and observing differences in their electromagnetic properties related to the GPR response. Finally, several measurements were taken in order to analyze the use of GPR to detect tree root systems using polarimetric techniques, being possible to detect medium and big size roots, together with groups of small roots., El geo-radar o radar de subsuelo (GPR), es una técnica de teledetección cercana que puede emplearse como herramienta de prospección geofísica y que se fundamenta en la emisión de un pulso electromagnético de muy corta duración (1-20 ns) caracterizado por una frecuencia central que puede variar entre los 10 MHz y los 2,5 GHz, en función de la antena seleccionada. Tras su emisión mediante una antena transmisora, las ondas de radar experimentan reflexiones en las discontinuidades electromagnéticas existentes en el medio por el que se propagan, siendo detectadas por una segunda antena receptora. El análisis de las reflexiones puede utilizarse para caracterizar tanto el medio de propagación como los reflectores, por lo que resulta un método adecuado para el estudio de determinadas propiedades físicas. El objetivo principal de este trabajo es mostrar diferentes aplicaciones del GPR en el ámbito forestal. Para ello se exponen los fundamentos básicos del método y su aplicabilidad como herramienta versátil en el ámbito forestal, así como posibles resultados que se pueden obtener en casos concretos. Como primera aplicación se presentan dos ejemplos que ilustran la capacidad de este método para detectar sustratos rocosos a poca profundidad, para analizar la estratigrafía superficial de un suelo y para localizar el nivel freático. Como segunda aplicación se analizan troncos de árboles sanos y también madera seca, analizando los datos obtenidos y mostrando que es un método que podría llegar a utilizarse para determinar las diferencias existentes en sus propiedades electromagnéticas. Como tercera aplicación se evalúa la utilización del GPR para detectar raíces en el subsuelo, habiendo sido posible determinar la posición de raíces de tamaño medio o grande, así como los grupos de pequeñas raíces.

- Abstract
This paper presents a proposed methodology developed to obtain the geometry of masonry walls. The methodology is based on the automatic recognition of boundaries. The geometry is used to proceed to the numerical simulation of damages based on a simple model, which allows determinate the cracking process due the tensile stresses in joints. Masonry units are considered as lineal and elastic elements, and the non-elastic processes are considered to exist only in the joints. Two examples are included showing two different walls: a regular mansonry wall built with bricks, and an irregular masonry wall with limestone stones as masonry units.
