Abstract

TlCuCl$_3$ is a coupled spin dimer system, which has a singlet ground state with an excitation gap of $\Delta/g\mu_{\mathrm B}$ = 5.5 T. TlCu$_{1-x}$Mg$_x$Cl$_3$ doped with nonmagnetic Mg$^{2+}$ ions undergoes impurity-induced magnetic ordering. Because triplet excitation with a finite gap still remains, this doped system can also undergo magnetic-field-induced magnetic ordering. By specific heat measurements and neutron scattering experiments under a magnetic field, we investigated the phase diagram in TlCu$_{1-x}$Mg$_x$Cl$_3$ with $x\sim 0.01$, and found that impurity- and field-induced ordered phases are the same. Abstract

Accurate and reliable predictions and an understanding of future changes in the stratosphere are major aspects of the subject of climate change. Simulating the interaction between chemistry and climate is of particular importance, because continued increases in greenhouse gases and a slow decrease in halogen loading are expected. These both influence the abundance of stratospheric ozone. In recent years a number of coupled chemistry–climate models (CCMs) with different levels of complexity have been developed. They produce a wide range of results concerning the timing and extent of ozone-layer recovery. Interest in reducing this range has created a need to address how the main dynamical, chemical, and physical processes that determine the long-term behavior of ozone are represented in the models and to validate these model processes through comparisons with observations and other models. A set of core validation processes structured around four major topics (transport, dynamics, radiation, and stratospheric chemistry and microphysics) has been developed. Each process is associated with one or more model diagnostics and with relevant datasets that can be used for validation. This approach provides a coherent framework for validating CCMs and can be used as a basis for future assessments. Similar efforts may benefit other modeling communities with a focus on earth science research as their models increase in complexity.

12. Compressive Non-Toxic Intrathoracic Goiter in Patient with Contraindication to Surgical Treatment: Case Report

Abstract

Introduction:Thyroidectomy is the treatment of choice for non-toxic compressive Multinodular Goiter (MNG). However, when surgery is contraindicated, other therapeutic options should be evaluated. In this case report, non-surgical therapeutic possibilities are reviewed, and the results obtained with Radioiodine Therapy (RAI) after stimulating with Recombinant Thyroid-Stimulating Hormone (rhTSH) are described. Case report:A 92-year-old patient with multiple comorbidities, non-toxic MNG, and symptomatic compression of the trachea and esophagus. Accordingly, RAI was chosen due to the high surgical risk. Initially, pre-treatment with methimazole was performed to increase TSH, which was interrupted due to drug hepatitis. Then, RAI was chosen in the post-stimulus with a low dose of rhTSH, with significant and early reduction of goiter and symptoms. Discussion:Although total thyroidectomy is the treatment of choice for non-toxic and symptomatic intrathoracic MNG; it does present risks, especially in patients with multiple comorbidities. Thus, interest in less invasive techniques is increasing. Thermal radio-ablations exhibit satisfactory results, however it is still an inaccessible technique. Radioiodine is an effective treatment option subsequent to the use of rhTSH or hypothyroidism methimazole-induced. RhTSH and methimazole can increase the level of radiation absorbed by the gland, which can lead to a reduction in the required I-131 dose. Conclusion:It is possible to mitigate the symptoms and improve the quality of life of patients with non-toxic and symptomatic MNG and multiple comorbidities, without surgical intervention. In this case report we describe alternatives to surgical intervention and show the effectiveness and safety of RAI along with rhTSH in the management of MNG.

Abstract

An analysis is performed on the dynamical coupling between the variability of the extratropical stratospheric and tropospheric circulations during the Northern Hemisphere winter. Obtained results provide evidence that in addition to the well-known Charney and Drazin mechanism by which vertical propagation of baroclinic Rossby waves is nonlinearly influenced by the zonal mean zonal wind, topographic forcing constitutes another important mechanism by which nonlinearity is introduced in the troposphere--stratosphere wave-driven coupled variability. On the one hand, vortex variability is forced by baroclinic Rossby wave bursts, with positive (negative) peaks of baroclinic Rossby wave energy occurring during rapid vortex decelerations (accelerations). On the other hand, barotropic Rossby waves of zonal wavenumbers s = 1 and 3 respond to the vortex state, and strong evidence is presented that such a response is mediated by changes of the topographic forcing due to zonal mean zonal wind anomalies progressing downward from the stratosphere. It is shown that wavenumbers s = I and 3 are the dominant Fourier components of the topography in the high-latitude belt where the zonal mean zonal wind anomalies are stronger; moreover, obtained results are in qualitative agreement with the analytical solution provided by the simple topographic wave model of Charney and Eliassen. Finally, evidence is provided that changes of barotropic long (s [less than or equal to] 3) Rossby waves associated with vortex variability reproduce a NAO-like dipole over the Atlantic Ocean but no dipole is formed over the Pacific Ocean. Moreover, results suggest that the nonlinear wave response to topographic forcing may explain the spatial changes of the NAO correlation patterns that have been found in previous studies.

Abstract

Background: Patients with breast cancer (BC) show strong interest in complementary and alternative medicine (CAM), particularly for adverse effects of adjuvant endocrine treatment - e.g., with letrozole. Letrozole often induces myalgia/limb pain and arthralgia, with potential noncompliance and treatment termination. This analysis investigated whether CAM before aromatase inhibitor (AI) therapy is associated with pain development and the intensity of AI-induced musculoskeletal syndrome (AIMSS) during the first year of treatment. Patients and methods: The multicenter phase IV PreFace study evaluated letrozole therapy in post-menopausal, hormone receptor-positive patients with early BC. Patients were asked about CAM use before, 6 months after, and 12 months after treatment started. They recorded pain every month for 1 year in a diary including questions about pain and numeric pain rating scales. Data were analyzed for patients who provided pain information for all time points. Results: Of 1396 patients included, 901 (64.5%) had used CAM before AI treatment. Throughout the observation period, patients with CAM before AI treatment had higher pain values, for both myalgia/limb pain and arthralgia, than non-users. Pain increased significantly in both groups over time, with the largest increase during the first 6 months. No significant difference of pain increase was noted regarding CAM use. Conclusions: CAM use does not prevent or improve the development of AIMSS. Pain intensity was generally greater in the CAM group. Therefore, because of the risk of non-compliance and treatment discontinuation due to the development of higher pain levels, special attention must be paid to patient education and aftercare in these patients. Abstract

The 3D structures of the free oscillations of an adiabatic and hydrostatic atmosphere around a basic state at rest were used as a physical filtering for atmospheric data. This filtering procedure allows for the consideration of the three primitive variables (u, v, [phi]) over the whole atmosphere simultaneously. Accordingly, the computed statistics do not simply rely on the information provided by a single variable of circulation, such as the 500-hPa geopotential field. Using this method, two classical patterns were isolated in the barotropic component of the circulation, one resembling the Pacific-North America (PNA) pattern, the other similar to the North Atlantic Oscillation (NAO) pattern in summer. Associating the barotropic and the second baroclinic components, a coupling in variability was retrieved between the strength of the winter stratospheric polar vortex and the tropospheric circulation over the North Atlantic. Until now these modes had only been recovered by means of statistical analysis. This study shows their existence in physically filtered fields. The obtained results make clear that the observed winter pattern of NAO is not a simple variability mode of the atmosphere, but results instead from mean flow wave interaction that modulates tropospheric planetary Rossby waves. The association between the NAO circulation variability patterns and the anomalies of the 850-hPa temperature field was also investigated. Abstract

Aim To assess the safety and tolerability of adjuvant subcutaneous trastuzumab (Herceptin® SC, H SC), delivered from an H SC Vial via hand-held syringe (Cohort A) or single-use injection device (Cohort B), with or without chemotherapy, for human epidermal growth factor receptor 2 (HER2)-positive stage I to IIIC early breast cancer (EBC) in the phase III SafeHer study (NCT01566721). Methods Patients received 600 mg fixed-dose H SC every 3 weeks for 18 cycles. The chemotherapy partner was at the investigators' discretion (H SC monotherapy was limited to ≤10% of the population). Data from the first H SC dose until 28 days (plus a 5-day window) after the last dose are presented. Results are descriptive. Results In the overall population, 2282/2573 patients (88.7%) experienced adverse events (AEs). Of the above, 128 (5.0%) patients experienced AEs leading to study drug discontinuation; 596 (23.2%) experienced grade ≥ 3 AEs and 326 (12.7%) experienced serious AEs. Grade ≥ 3 cardiac disorders were reported in 24 patients (0.9%), including congestive heart failure in eight (0.3%). As expected, the AE rates varied according to the timing of chemotherapy in both cohorts, with higher rates in concurrent versus sequential chemotherapy subgroups. In the concurrent chemotherapy subgroup, AEs were more common during the actual period of concurrent chemotherapy compared with the period when patients did not receive concurrent chemotherapy. Conclusion SafeHer confirms the safety and tolerability of the H SC 600 mg fixed dose for 1 year (every 3 weeks for 18 cycles) as adjuvant therapy with concurrent or sequential chemotherapy for HER2-positive EBC. These primary analysis results are consistent with the known safety profile for intravenous H and H SC.
