Abstract

In this work we study numerical construction of optimal clinical diagnostic tests for detecting sporadic Creutzfeldt-Jakob disease (sCJD). A cerebrospinal fluid sample (CSF) from a suspected sCJD patient is subjected to a process which initiates the aggregation of a protein present only in cases of sCJD. This aggregation is indirectly observed in real-time at regular intervals, so that a longitudinal set of data is constructed that is then analysed for evidence of this aggregation. The best existing test is based solely on the final value of this set of data, which is compared against a threshold to conclude whether or not aggregation, and thus sCJD, is present. This test criterion was decided upon by analysing data from a total of 108 sCJD and non-sCJD samples, but this was done subjectively and there is no supporting mathematical analysis declaring this criterion to be exploiting the available data optimally. This paper addresses this deficiency, seeking to validate or improve the test primarily via support vector machine (SVM) classification. Besides this, we address a number of additional issues such as i) early stopping of the measurement process, ii) the possibility of detecting the particular type of sCJD and iii) the incorporation of additional patient data such as age, sex, disease duration and timing of CSF sampling into the construction of the test., Comment: 32 pages, 12 figures, 1 table

31. How a Climbing Wall Became Part of a NEW Physical Education Program

Abstract

The introduction of a NEW physical education (PE) program at Ancaster Senior Public School had, at its root, the desire to make physical activity an inclusive domain for both athletic students and those not so inclined. With the growing concerns over the rapid and consistent rise in childhood obesity rates it was evident that the current model of competitive sport skill-related programs was not meeting the needs of many students. In an effort to address these concerns, it was necessary to look for ways to engage students that would meet their needs and interests. To accomplish this end, it was determined that many of the goals the school was attempting to meet could be addressed through the use of a climbing wall. The climbing wall was seen as a legitimate educational activity that would encourage the physical, mental, social and emotional growth of each participant. This article describes how a climbing wall became an integral part of Ancaster Senior's NEW PE program. The integration of a permanent climbing wall in the school as part of its NEW PE program has provided students with an invitation to revisit their childhood and "climb" the new challenges presented in the "new curriculum" of the 21st century.

Abstract

Introduction Sporadic Creutzfeldt-Jakob disease (sCJD) is a rapidly progressive neurodegenerative disease with public health implications. Mean age of onset is 68 years. Age-specific incidence declines after 80 years. This may arise from under-ascertainment or other biological features of the disease. Accurate characterisation of late-onset sCJD is important for early diagnosis, avoiding unnecessary investigations and improving ascertainment for public health purposes. Objective To phenotype the clinical features and investigation profile of sCJD in adults >80 years. Methods We analysed all probable and definite sCJD cases identified by the UK National CJD Research & Surveillance Unit over a 10-year period (2011–2021). Individuals were grouped by age of onset. Clinical features and investigation profiles were compared. Results 10.3% (123/1196) had an age of onset over 80. Median survival was shorter (3.2 vs 4.3 months; P  < 0.001). Pyramidal signs (48.3% vs 34.2%; P  = 0.008) and akinetic mutism (55.1% vs 33.2%; P  < 0.001) were more frequent. Psychiatric symptoms (26.3% vs 39.6%; P  = 0.01) and cerebellar signs (65.4% vs 78.6%, P  = 0.007) were less frequent. Cognitive impairment and myoclonus were highly prevalent regardless of age. Between age groups, the diagnostic sensitivity of cerebrospinal fluid real-time quaking-induced conversion (CSF RT-QuIC) (92.9% vs 91.9%, P  = 0.74) was comparable, electroencephalography was superior (41.5% vs 25.4%; P  = 0.006) and MRI was inferior (67.8% vs 91.4%; P  < 0.001). Conclusions Late-onset sCJD has distinct clinical features, shorter survival and a different profile of investigation sensitivity. CSF RT-QuIC, MRI brain and specialist CJD review is recommended in older adults with a rapidly progressive neurological disorder. Autopsy is valuable when the cause remains elusive. Abstract

The Arafura and Timor Seas (ATS) comprise both near pristine and highly threatened marine ecosystems. There are four littoral nations (Indonesia, Timor-Leste, Australia, and Papua New Guinea), which have independently established Marine Protected Areas (MPAs). We design the first resilient MPA network for the entire region to protect critical habitats and species by considering transboundary features (i.e., connectivity of fisheries, rare, threatened, and protected species), uses and threats (i.e., fishing and climate change). We used Geographic Information System processing to identify gaps in the regional network based on existing and proposed MPAs and Areas of Interest (AOIs: previously identified as potential new MPAs) in each country, followed by systematic conservation planning to identify new AOIs for establishing MPAs to fill these gaps. The ATS MPA Network design (301,055 km2) has been endorsed by all four countries and includes all 93 existing (265,405 km2) and proposed (6,264 km2) MPAs, 13 previously identified AOIs (14,773 km2), and seven new AOIs identified in this study (14,613 km2). Of the new AOIs, three have been incorporated in Indonesia's national MPA network plan, one is in a new MPA currently being developed in Timor-Leste, and Australia is considering two as potential areas for new MPAs. 39. The concept of Ananke in Greek literature before 400 BCE

Abstract

This study seeks to explore the concept of ἀνάγκη (and the related terms ἀναγκαίος and ἀναγκαίως) in Greek literature written before 400 BCE. All passages containing these words from the time period were located, translated and analysed according to specific criteria concerning the usage and interpretation of the term. The resulting exploration was then split into five main sections: physical compulsion, moral compulsion, cosmology, circumstantial compulsion and the personification of compulsion. These sections were then examined according to both context and subtle differences in the meaning of ἀνάγκη terms within these contexts. The vast majority concerned some form of violence, physical force or fear of violent repercussions. Although the focus was on the interpretation of texts dating to before 400 BCE, owing to their fragmentary nature but considerable importance, the cosmological texts had to be examined in conjunction with later texts in order to shed more light on the meaning of ἀνάγκη in this context. Statistical analysis was performed on the 466 texts located and they were further analysed to track variations across time and genre-specific usages. Several types of usage were seen to develop only towards the end of the fifth century after 450 BCE including the notion of relative compulsions; the necessity for revenge and compelled alliances were seen to develop at this time. Recommendations were made with regards to the best and most appropriate translations; the majority of passages would require either the translation of coercion, constraint or compulsion for ἀνάγκη with the exception of the adjectival ἀναγκαίος which can mean blood relatives or similarly obligated individuals. The translation of necessity, although generally the given interpretation of ἀνάγκη was seldom appropriate since it did not grasp the entire meaning of the term in context.

Abstract

A theology of women’s priesthood can be developed through an examination of the symbol and narrative of Christian religion. Metaphors of the body, the bride and the whore form a leitmotif through Christian scripture and tradition, and these have traditionally been interpreted from a phallocentric viewpoint. A feminist approach to scripture and tradition reveals that the woman priest causes a shift in the interpretation of these metaphors that impacts on many areas of Christian worship and life. The Eucharist, the central Anglican rite and nexus between the narrative of faith and the praxis of discipleship, is laden with symbols that, if effective, are also transformative for the worshipper. The priest, instrumental in the liturgy, has a key symbolic function in offering such potential for the Church community. When celebrating the Eucharist, the priest who is a woman recovers ignored or undervalued meanings within the associated symbolism which give rise to new possibilities theologically, liturgically and morally for Christian teaching and discipleship. The multivalent symbolism of priesthood thus acquires a new breadth and richness that addresses our understanding of the nature of the triune God, and of ourselves as beings created in the image of God and members of the Body of Christ. The woman priest causes a ‘collision’ with the received wisdom of traditional teaching and practice, and invites consideration of the genderisation of symbol and narrative, the exclusion of women from sacred rituals and spaces, and the lack of a female religious imaginary. A feminist reading of Paul Ricoeur, together with an engagement with Luce Irigaray and Grace Jantzen, demonstrates that philosophical and psychoanalytical inquiry offers transformative possibilities for the religious imaginary, for the recognition of sexual difference and for the possibility of woman as subject of culture. The woman priest, representative both of the divine and of humankind, has a key role in this process.

45. Abstraction, experience, reduction : time and periodicity in the work of Myron Stout and postwar American art history

Abstract

This dissertation is the first full-career study of the American painter Myron Stout (1908- 1987). Stout is best known for a series of small black and white abstract paintings he made between 1955 and 1980. He is most often associated with American hard-edge painting of the early 1960s, the movement which set the stage for Minimalism. However, Stout's work predates hard-edge painting in an important way, namely that he developed it as an extension of Abstract Expressionism. Stout intended the reduced palette, simplified shapes, and absence of gestural marks in his paintings to be effective containers for an expression of lived experience. Stout confounds typical art historical narratives in other ways. He was introduced to modernism in the 20s, but did not begin his life as a painter until the late 1940s. In an era of high production he made very few paintings-about fifteen over twenty-five years. His work "predicts" some of the issues that emerged in Minimalism, but by the mid-1960s Stout appeared very much a 50s artist. Whilst seemingly a die-hard, purist painter, his artistic milieu included artists innovating very different kinds of practices, from Happenings and Pop to realist painting. Finally, from today's perspective, Stout is a lesser-known artist, but he is and was nonetheless an insider in terms of his education and his artistic contacts. By addressing the practice of a single artist, this dissertation demonstrates that even in the face of newer, more inclusive methodologies, recent histories Abstract Expressionism and Minimalism still have problems with inclusivity, indeed depend to a large degree on narrow views. The approach here is to illuminate moments where Stout's history collides with conventional period-histories. Using Stout's chronological age in addition to his interface with the art world, it takes a longer view of his career, addressing early and late parts of his life. Stout's continuity through these periods raises questions about how models of périodisation continue to drive the way art history is written.

47. Brain-specific proteins in the diagnosis of dementia

Abstract

With the mean age of the population increasing, the number of patients with dementia is likely to increase. Although Alzheimer's disease is the most common form of dementia, it is recognised that other forms of dementia are more prevalent than initially thought. As drug therapies become available, it will become important to obtain an accurate clinical diagnosis. Creutzfeldt-Jakob disease (CJD) is a rare type of dementia but with the emergence of the new variant form of this disease which affects young patients, and is thought to be related to the ingestion of meat or meat products from cattle affected with bovine spongiform encephalopathy (BSE), it is possible that it may become more common. These concerns have led to the search for diagnostic tests which may help in the early diagnosis of these forms of dementia. This study investigated whether measurement of cerebrospinal fluid (CSF) brain- specific proteins could be used in the assessment of patients with dementia. Four proteins were investigated: S-100b, an astrocytic protein; and three neuronal proteins, neurone-specific enolase, tau protein and 14-3-3. A two site sandwich ELISA was developed and evaluated for the measurement of S-100b. Levels of all four brain-specific proteins were raised in the majority of patients with sporadic CJD. The presence of CSF 14-3-3 had the best combination of sensitivity and specificity for detecting sporadic CJD. CSF 14-3-3 has recently been included in the diagnostic criteria for probable sporadic CJD. The value of measurement of these proteins in the new variant, iatrogenic and familial forms of CJD was also investigated. None of the CSF brain-specific proteins investigated was of value in the diagnosis of Alzheimer's disease.

49. Improved Real-Time Quaking Induced Conversion for Early Diagnostics of Creutzfeldt–Jakob Disease in Denmark

Abstract

Cerebrospinal fluid-based real-time quaking-induced conversion (CSF RT-QuIC) is currently the most prominent method for early detection of sporadic Creutzfeldt–Jakob disease (sCJD), the most common prion disease. CSF RT-QuIC delivers high sensitivity (>90%) and specificity (100%), which has been demonstrated by large ring-trial studies testing probable and definitive sCJD cohorts. Following the inclusion of CSF RT-QuIC in the revised European CJD Surveillance Network diagnostic criteria for sCJD, it has become a standard diagnostic procedure in many prion disease reference or surveillance centers around the world. In this study, we present the implementation of the second-generation CSF RT-QuIC (commonly known as Improved QuIC or IQ) at the Danish Reference Center for Prion Diseases (DRCPD). The method’s sensitivity and specificity were evaluated and validated by analyzing 63 CSF samples. These 63 samples were also analyzed at the National CJD Research and Surveillance Unit (NCJDRSU), based at the University of Edinburgh, UK; analysis was carried out using the first generation or previous CSF RT-QuIC method (PQ). The sensitivity and specificity of PQ during tests at the NCJDRSU were 92% and 100%, respectively. Using these 63 CSF samples, the agreement between the two RT-QuIC generations at DRCPD and NCJDRSU prion laboratories was 100%.
