2. High volume-rate echocardiography for simultaneous imaging of electromechanical activation and cardiac strain of the whole heart in a single heartbeat in humans.

Background: Imaging both electrical and mechanical cardiac function can better characterize cardiac disease and improve patient care. Currently, there is no noninvasive technique that can simultaneously image both electrical and mechanical function of the whole heart at the point of care. Here, our aim is to demonstrate that high volume-rate echocardiography can simultaneously map cardiac electromechanical activation and end-systolic cardiac strain of the whole heart in a single heartbeat. Method: A 32x32 ultrasound matrix array connected to four synchronized ultrasound scanners were used for transthoracic high volume-rate imaging (840 volumes/s) in sixteen young volunteers (28.1±4.2 y.o.). An electromechanical activation map of the whole heart and volumetric end-systolic atrial and ventricular strain images were obtained. Results: The whole heart activation sequence was found to be consistent across volunteers and in agreement with previously reported normal electrical activation sequences. The mean electromechanical activation time was 72.6±15.2 ms in the atria, 132.4±19.7 ms in the ventricles and 154.5±19.6 ms in the whole heart. Volumetric right and left atrial as well as right and left ventricular strains were also consistent across all volunteers, with a mean end-systolic global longitudinal strain of 26.8±6.5% in the atria and -16.6±3.4% in the ventricles. Conclusions: This initial feasibility study demonstrates that noninvasive high-volume rate imaging of the heart in a single heartbeat is feasible and can provide electromechanical activation and systolic strains simultaneously in all four cardiac chambers. This technique can be further developed and used at the point of care to assist for screening, diagnosis, therapy guidance and follow-up of heart disease patients. [ABSTRACT FROM AUTHOR]

Quantitative ultrasound measurements at a skeletal site can assess bone quality. Our objective is to evidence and exploit waves that propagate circumferentially in the cortical shell of the femoral neck in order to characterize hip strength. Numerical simulations of wave propagation in femoral neck cross-sections were performed to investigate the phenomena involved. It was shown that the first arriving signal (FAS) in through transmission was always associated with circumferential waves. A strong correlation between time-of-flight of the FAS (TOFFAS) and geometrical parameters related to bone strength was observed. Circumferential waves were found to be sensitive to porosity and elasticity of the cortical shell. An experimental ultrasonic set-up was designed to investigate the relationship between TOFFAS and the bone strength obtained by mechanical testing. A first series of measurements on nine samples showed that TOFFAS can predict proximal femur strength (R ² = 0.79) at least as well as the reference technique (R ² = 0.78) using X-rays. This experimental setup can be optimized and combined to an existing ultrasound device dedicated to proximal femur in order to increase fracture risk prediction. We propose as a future perspective the use of DORT method to determine the phase velocity of circumferential waves in femoral neck from which geometrical and material properties of bone could be deduced.; Les mesures ultrasonores en transmission transverse permettent d'évaluer un site osseux. Notre objectif est de mettre en évidence et d'exploiter les ondes qui se propagent circonférentiellement dans la coque corticale du col fémoral pour caractériser la résistance de la hanche à la fracture. 50. Quantitative ultrasound of cortical bone in the femoral neck predicts femur strength: Results of a pilot study.

- Abstract
A significant risk of femoral neck (FN) fracture exists for men and women with an areal bone mineral density (aBMD) higher than the osteoporotic range, as measured with dual-energy X-ray absorptiometry (DXA). Separately measuring the cortical and trabecular FN compartments and combining the results would likely be a critical aspect of enhancing the diagnostic capabilities of a new technique. Because the cortical shell determines a large part of FN strength a novel quantitative ultrasound (QUS) technique that probes the FN cortical compartment was implemented. The sensitivity of the method to variations of FN cortical properties and FN strength was tested. Nine femurs (women, mean age 83 years) were subjected to QUS to measure the through transmission time-of-flight (TOF) at the FN and mechanical tests to assess strength. Quantitative computed tomography (QCT) scans were performed to enable analysis of the dependence of TOF on bone parameters. DXA was also performed for reference. An ultrasound wave propagating circumferentially in the cortical shell was measured in all specimens. Its TOF was not influenced by the properties of the trabecular compartment. Averaged TOF for nine FN measurement positions/orientations was significantly correlated to strength ( R
2 = 0.79) and FN cortical QCT variables: total BMD ( R2 = 0.54); regional BMD in the inferoanterior ( R2 = 0.90) and superoanterior ( R2 = 0.57) quadrants; and moment of inertia ( R2 = 0.71). The results of this study demonstrate that QUS can perform a targeted measurement of the FN cortical compartment. Because the method involves mechanical guided waves, the QUS variable is related to the geometric and material properties of the cortical shell (cortical thickness, tissue elasticity, and porosity). This work opens the way to a multimodal QUS assessment of the proximal femur, combining our approach targeting the cortical shell with the existing modality sensitive to the trabecular compartment.
