Abstract

Zidovudine (AZT) is the only drug that has so far been approved for relieving the symptoms of HIV (human immunodeficiency virus) infection and prolonging the life of patients with AIDS, the acquired immunodeficiency syndrome. One of the side effects of zidovudine is the suppression of the bone marrow, which leads to anemia because the bone marrow manufactures red blood cells. The anemia is frequently severe enough to require blood transfusions on a regular basis. A study was performed to evaluate the possible benefits of giving erythropoietin, a hormone that stimulates production of red blood cells, to AIDS patients on zidovudine. Sixty-three patients participated; 29 received erythropoietin, while 34 took an inert placebo. During the second and third months of the study, fewer subjects taking erythropoietin were requiring blood transfusions, and those who were transfused needed fewer units of red blood cells. Further analysis of patient characteristics showed that those who had less need for transfusions were the individuals whose own erythropoietin levels at the start of the study were less than or equal to 500 IU (International Units) per liter. Patients whose baseline erythropoietin level was above 500 IU per liter did not benefit from the therapy. There was no greater incidence of side effects in the treatment group than in the placebo group. It appears that erythropoietin may be helpful to AIDS patients to counteract the anemia often caused by zidovudine, but more research is needed to clarify which patients will benefit. Abstract

Genes that code for CD4 and for an immunoglobulin molecule (IgG) were fused to form CD4 immunoadhesin, which has properties of both molecules. CD4 is a membrane protein on T lymphocytes (a type of white blood cell) that is the receptor for human immunodeficiency virus (HIV), which causes acquired immunodeficiency syndrome (AIDS). It can be isolated from the membrane and in that form is called soluble (not attached to a cell) CD4. It binds to the HIV protein gp120 and blocks HIV from binding and infecting T lymphocytes; it also has a long half-life in the blood plasma. Like the IgG molecule, CD4 immunoadhesin binds with a cellular receptor known as the Fc receptor. CD4 immunoadhesin can mediate the killing of HIV cells, an action known as antibody-dependent cell-mediated cellular cytotoxicity (ADCC). However, it does not mediate ADCC against cells containing the CD4 receptor with gp120 when these cells are not infected. This change is important because in AIDS patients, ADCC causes the death of cells that express CD4; CD4 would otherwise be involved in an immune response against the virus. In primates, CD4 immunoadhesin (like IgG) can cross the placenta and provide protection to the unborn fetus. CD4 immunoadhesins may be developed for therapeutic use, especially in preventing the transmission of HIV infection from the mother to the fetus. Abstract

Doxorubicin may not be an effective treatment for AIDS patients who have Kaposi's sarcoma. Kaposi's sarcoma is a cancer that affects the skin and occasionally the lymph nodes and viscera, or internal organs. Among 53 AIDS patients suffering from Kaposi's sarcoma who were treated with 15 milligrams of doxorubicin per square meter once a week, 26 had cutaneous (skin) disease with no symptoms and 27 had visceral disease with fluid accumulation and a history of opportunistic infections. Of 50 patients for whom treatment response was evaluated, 10% experienced a partial response to treatment, 64% experienced a minor response, 24% experienced no response and 2% experienced disease progression. Twenty percent of the patients with cutaneous disease experienced a partial response to treatment, compared to none of the patients with visceral disease.
