The protective effects of talk with parents about sex in delaying sex and reducing young people's risky sexual behaviour may extend from adolescence to emerging adulthood. However, little is known about the content and process of this communication, or how parents and their emerging adult children perceive their conversations about sex and relationships. This study offers a novel exploration of family talk about sex during emerging adulthood and addresses topics that are not typically assessed as part of communication research, such as consent and positive talk about sexuality. This study uses thematic analysis to investigate perceptions of family talk about sex in a qualitative sample of 16 pairs of parents and their emerging adult children in the USA, and includes talk about protection, sexual behaviour, pregnancy and parenting; the positive aspects of sex; consent; and sexual orientation. Findings identified variation across topics in terms of 1) similarities and differences in parents' and emerging adults' comfort in talking with each other about sexual topics; and 2) how they perceive this communication across a range of sexual issues. Findings can inform the development of resources to support parents on how to talk with their emerging adult children about sexual issues in a developmentally appropriate way.

This study compared the extent to which Hispanic teenagers talk with mothers, fathers and extended family members about risks of sex, protection and relational sex, as well as the role of teenagers' gender on the association between sexuality communication and risky sexual behaviours. The analysis is based upon 474 Hispanic young people's responses to a school-based survey recruited from six New England high schools. We (1) calculated descriptive statistics and tests of difference by teenagers' gender, and (2) ran logistic regression models for engaging in three different risky sexual behaviors separately by gender. There were significant gender differences in teenagers' talk with mothers and fathers, but not with extended family members. There was consistent 'gender matching' between teenagers and the extended family member they talked to. The content of sexuality communication, the family member the teenager talked to and the gender of the teenager all contributed to whether communication was protective for teenagers' sexual behaviour. There are substantial differences in the types of sexuality communication Hispanic teenagers have with different family members, which are closely tied to teenager's and the family member's gender. Results suggest that one size does not fit all when it comes to family communication about sex and sexuality.

Systemic lupus erythematosus (SLE) is a heterogeneous autoimmune disease with a strong genetic component characterized by autoantibody production and a type I interferon signature. Here we report a missense variant (g.74779296G>A; p.Arg90His) in NCF1, encoding the p47phox subunit of the phagocyte NADPH oxidase (NOX2), as the putative underlying causal variant that drives a strong SLE-associated signal detected by the Immunochip in the GTF2IRD1-GTF2I region at 7q11.23 with a complex genomic structure. We show that the p.Arg90His substitution, which is reported to cause reduced reactive oxygen species (ROS) production, predisposes to SLE (odds ratio (OR) = 3.47 in Asians (Pmeta = 3.1 × 10-104), OR = 2.61 in European Americans, OR = 2.02 in African Americans) and other autoimmune diseases, including primary Sjögren's syndrome (OR = 2.45 in Chinese, OR = 2.35 in European Americans) and rheumatoid arthritis (OR = 1.65 in Koreans). Additionally, decreased and increased copy numbers of NCF1 predispose to and protect against SLE, respectively. Our data highlight the pathogenic role of reduced NOX2-derived ROS levels in autoimmune diseases.

Leptin is abnormally elevated in the plasma of patients with systemic lupus erythematosus (SLE), where it is thought to promote and/or sustain pro-inflammatory responses. Whether this association could reflect an increased genetic susceptibility to develop SLE is not known, and studies of genetic associations with leptin-related polymorphisms in SLE patients have been so far inconclusive. Here we genotyped DNA samples from 15,706 SLE patients and healthy matched controls from four different ancestral groups, to correlate polymorphisms of genes of the leptin pathway to risk for SLE. It was found that although several SNPs showed weak associations, those associations did not remain significant after correction for multiple testing. These data do not support associations between defined leptin-related polymorphisms and increased susceptibility to develop SLE.

ObjectiveAn increased frequency of atherosclerosis (ATH) in systemic lupus erythematosus (SLE) is well-documented but not fully explained by the presence of traditional cardiac risk factors. Several nontraditional biomarkers, including proinflammatory high-density lipoprotein (piHDL) and leptin, have been individually associated with subclinical ATH in SLE. The aim of this study was to examine whether these and other biomarkers can be combined into a risk profile, the Predictors of Risk for Elevated Flares, Damage Progression, and Increased Cardiovascular Disease in Patients with SLE (PREDICTS), that could be used to better predict future progression of ATH.MethodsIn total, 210 patients with SLE and 100 age-matched healthy control subjects (all women) participated in this prospective cohort study. The longitudinal presence of carotid plaque and intima-media thickness (IMT) were measured at baseline and followup (mean ± SD 29.6 ± 9.7 months).ResultsAt followup, carotid plaque was present in 29% of SLE patients. Factors significantly associated with plaque, determined using Salford Predictive Modeling and multivariate analysis, included age ≥48 years (odds ratio [OR] 4.1, P = 0.002), high piHDL function (OR 9.1, P < 0.001), leptin levels ≥34 ng/dl (OR 7.3, P = 0.001), plasma soluble TWEAK levels ≥373 pg/ml (OR 28.8, P = 0.004), and history of diabetes (OR 61.8, P < 0.001). Homocysteine levels ≥12 μmoles/liter were also a predictor. However, no single variable demonstrated an ideal combination of good negative predictive values (NPVs), positive predictive values (PPVs), sensitivity, and specificity. A high-risk PREDICTS profile was defined as ≥3 positive biomarkers or ≥1 positive biomarker plus a history of diabetes; for high-risk SLE patients, the PPV was 64%, NPV was 94%, sensitivity was 89%, and specificity was 79%. In multivariate analysis, SLE patients with the high-risk profile had 28-fold increased odds for the longitudinal presence of plaque (P < 0.001) and increased progression of IMT (P < 0.001).ConclusionA high-risk PREDICTS score confers 28-fold increased odds of the presence of any current, progressive, or acquired carotid plaque, both in patients with SLE and in control subjects, and is significantly associated with higher rates of IMT progression.

Abstract Introduction Osteopontin (OPN) has been implicated as a mediator of Th17 regulation via type I interferon (IFN) receptor signaling and in macrophage activity at sites of tissue repair. This study assessed whether increased circulating plasma OPN (cOPN) precedes development of organ damage in pediatric systemic lupus erythematosus (pSLE) and compared it to circulating plasma neutrophil gelatinase-associated lipocalin (cNGAL), a predictor of increased SLE disease activity. Methods cOPN and cNGAL were measured in prospectively followed pSLE (n = 42) and adult SLE (aSLE; n = 23) patients and age-matched controls. Time-adjusted cumulative disease activity and disease damage were respectively assessed using adjusted-mean SLE disease activity index (SLEDAI) (AMS) and SLICC/ACR damage index (SDI). Results Compared to controls, elevated cOPN and cNGAL were observed in pSLE and aSLE. cNGAL preceded worsening SLEDAI by 3-6 months (P = 0.04), but was not associated with increased 6-month AMS. High baseline cOPN, which was associated with high IFNalpha activity and expression of autoantibodies to nucleic acids, positively correlated with 6-month AMS (r = 0.51 and 0.52, P = 0.001 and 0.01 in pSLE and aSLE, respectively) and was associated with SDI increase at 12 months in pSLE (P = 0.001). Risk factors for change in SDI in pSLE were cOPN (OR 7.5, 95% CI [2.9-20], P = 0.03), but not cNGAL, cumulative prednisone, disease duration, immunosuppression use, gender or ancestry using univariate and multivariate logistic regression. The area under the curve (AUC) when generating the receiver-operating characteristic (ROC) of baseline cOPN sensitivity and specificity for the indication of SLE patients with an increase of SDI over a 12 month period is 0.543 (95% CI 0.347-0.738; positive predictive value 95% and negative predictive value 38%). Conclusion High circulating OPN levels preceded increased cumulative disease activity and organ damage in SLE patients, especially in pSLE, and its value as a predictor of poor outcome should be further validated in large longitudinal cohorts.

Family communication about sex can protect adolescents from risky behavior, like early sex and sex without protection. However, adolescents and parents often disagree about whether they talked with each other about sexual issues, limiting the protective effects of communication. Few studies explore these disagreements. This study included 27 pairs of early adolescents and parents. Adolescent and parent interviews were coded for agreement in reports of sexuality communication. Adolescents' interview themes were compared across levels of agreement between adolescent-parent dyads. Adolescents who highly agreed with their parents were more likely than those who did not to describe positive parental approaches to sexuality communication and awareness of parental perspectives, while no group differences were found for responses to parents' viewpoints. Study findings highlight the importance of sex education programs that support adolescent-parent sexuality communication, particularly for middle school students.

For adolescents who have a father in their lives, father–teen conversations about sex and relationships can protect teens from risky sexual behaviors. However, little is known about the content and process of these conversations. This study explored topics of and approaches to fathers' talk with their teens about sex and relationships in interviews with a diverse sample of 43 fathers of high school-aged adolescents from across the United States. Interview data were analyzed using content analysis. The results showed how fathers talked with their adolescent children about topics of sexual behavior, risks of sex, dating and relationships, as well as less studied areas of diverse sexual and gender identities and consent, and how these conversations differed with male and female teens. Findings also showed that fathers took multiple approaches to talk about sex, including personal talk, talk about friends and family, and use of media and other distal contexts to start conversations. These findings show how fathers contribute to the sexual socialization of their adolescent children and suggest points of access for fathers who are unsure how to approach talk with their teens about sex and relationships. Background: School-based comprehensive sex education programs can reduce early adolescents' risky sexual behavior. The purpose of this study was to assess the effectiveness of a 3-year comprehensive sex education program in delaying vaginal sex for middle school students and whether the family component of the intervention contributes to its effectiveness. Methods: This longitudinal evaluation followed a cohort of 6th graders (N?=?2453) through the end of 8th grade. The design used random assignment of 24 schools into treatment and comparison conditions. The analysis included multiple-group logistic regression to assess differences in delay of sex between intervention and comparison groups. Results: In schools where the program was taught, 16% fewer boys and 15% fewer girls had had sex by the end of 8th grade compared to boys and girls at comparison schools. Completing family activities during the first year of the program predicted delayed sexual debut for boys. Conclusions: Theory-based, developmentally appropriate, comprehensive sex education programs that include parent involvement can be effective in delaying vaginal sex for middle school students. Parent involvement is particularly important for boys, as family activities may encourage parents to talk with their sons earlier and more frequently.

This mixed-methods study examined urban adolescents' perceptions of gender and racial/ethnic barriers to STEM (science, technology, engineering, and mathematics) success, and their meaning-making and coping regarding these experiences. The sample includes surveys from 1024 high school-aged students and interviews from 53 students. Logistic analysis showed that higher science aspirations significantly predicted perceived support for girls and women in science. Analysis of interviews showed themes of microaggressions, responses to microaggressions, and gender- and race-based support. Findings suggest participants vary in perceptions of barriers, yet are generally optimistic about overcoming such obstacles.

Background: Early sexual activity can undermine adolescents' future school success and health outcomes. The purpose of this study was to assess the role of a family homework component of a comprehensive sex education intervention in delaying sexual initiation for early adolescents and to explore what social and contextual factors prevent adolescents from completing these family homework activities. Methods: This mixed methods study included 6th-and 7th-grade survey responses from 706 students at 11 middle school schools receiving a sex education intervention, as well as interviews from a subset of 33, 7th-grade students from the larger sample. Results: Adolescents who completed more family homework assignments were less likely to have vaginal intercourse in 7th grade than those who completed fewer assignments, after controlling for self-reports of having had vaginal intercourse in 6th grade and demographic variables. Participants' explanations for not completing assignments included personal, curriculum, and family-based reasons. Conclusions: Family homework activities designed to increase family communication about sexual issues can delay sex among early adolescents and contribute to school-based sex education programs. Successful sex education programs must identify and address barriers to family homework completion.

In this article, we operationalize identification of mixed racial and ethnic ancestry among adolescents as a latent variable to (a) account for measurement uncertainty, and (b) compare alternative wording formats for racial and ethnic self-categorization in surveys. Two latent variable models were fit to multiple mixed-ancestry indicator data from 1,738 adolescents in New England. The first, a mixture factor model, accounts for the zero-inflated mixture distribution underlying mixed-ancestry identification. Alternatively, a latent class model allows classification distinction between relatively ambiguous versus unambiguous mixed-ancestry responses. Comparison of individual indicators reveals that the Census 2000 survey version estimates higher prevalence of mixed ancestry but is less sensitive to relative certainty of identification than are alternate survey versions (i.e., offering a "mixed" check box option, allowing a written response). Ease of coding and missing data are also considered in discussing the relative merit of individual mixed-ancestry indicators among adolescents. (Contains 6 tables, 3 figures and 1 footnote.)

This mixed-methods exploratory study examined the diverse content and situated context of White adolescents' racial-ethnic identities. The sample consisted of 781 9th-12th grade White adolescents from three New England schools, which varied in racial and economic make-up. Open-ended responses provided a range of thematic categories regarding the importance of race-ethnicity to the adolescents' identities, representing the diverse ideologies of White adolescents' explanations, ranging from colorblind claims to ethnic pride. This study also found significant relationships between racial-ethnic identity importance (centrality) and parents' education for White adolescents. These findings highlight the diversity of White adolescents' understanding of their racial-ethnic identities and the importance of context in shaping racial-ethnic centrality.

This paper addresses contradictions between common perceptions of Asian Americans as a "model minority" and growing evidence of discrimination and its negative psychological implications for this group. The current study examined Chinese American early adolescents' distress from experiences of discrimination, its relationship with mental health and social functioning, and what factors may support their healthy development in the face of discrimination. The sample consisted of 158, 6th-8th grade, Chinese American youth. Study findings provided evidence for a relationship between distress from discrimination and social emotional health variables of depression and cooperation (a social competence construct). Results also revealed that peer support buffered the negative effects of discrimination on cooperation. These findings highlight the interpersonal context of discrimination among early adolescents, and the importance of developmentally and culturally appropriate supports.

The C4 gene copy-number variation (CNV) was investigated in 1241 European Americans, including patients with systemic lupus erythematosus (SLE), their first degree relatives by defining genotyping and phenotyping techniques. The results show that gene copy number of gene CNV provides data for an important immune defense protein in health and disease suggesting that low copy number is a risk factor for and high copy number is a protective factor against susceptibility to human SLE.

A genomic region on distal mouse chromosome 1 and its syntenic human counterpart 1q23-42 show strong evidence of harboring lupus susceptibility genes. We found evidence of linkage at 1q32.2 in a targeted genome scan of 1q21-43 in 126 lupus multiplex families containing 151 affected sibpairs (nonparametric linkage score 2.52, P = 0.006). A positional candidate gene at 1q32.2, complement receptor 2 (CR2), is also a candidate in the murine Sle1c lupus susceptibility locus. To explore its role in human disease, we analyzed 1,416 individuals from 258 Caucasian and 142 Chinese lupus simplex families and demonstrated that a common three-single-nucleotide polymorphism CR2 haplotype (rs3813946, rs1048971, rs17615) was associated with lupus susceptibility (P = 0.00001) with a 1.54-fold increased risk for the development of disease. Single-nucleotide polymorphism 1 (rs3813946), located in the 5' untranslated region of the CR2 gene, altered transcriptional activity, suggesting a potential mechanism by which CR2 could contribute to the development of lupus. Systemic lupus erythematosus (SLE) is an autoimmune disease with a complex genetic basis that includes susceptibility gene(s) within the chromosome 1q41-1q42 region. Toll-like receptor 5 (TLR5), the innate immune receptor for bacterial flagellin, maps to chromosome 1q41 and contains a common stop codon polymorphism that abrogates signaling (allele Cl174T) and is associated with an increased risk of infection. By using transmission disequilibrium testing in a cohort containing 199 affected patients and their 75 unaffected siblings and 326 parents, we found that allele 1174C, but not 1174T (with the stop codon), was preferentially transmitted to SLE-affected offspring (a 19:6 transmitted/not transmitted ratio, P = 0.009). In contrast, the alleles of the other three TLR5 SNPs did not exhibit preferential transmission. In addition, we found that allele 1174C was not preferentially transmitted to unaffected offspring (3:6 transmitted/not transmitted ratio, P value not significant). The allele frequency of 1174T in the probands was 3.2% compared with 5.8% in unaffected individuals, which was consistent with a protective association (odds ratio, 0.51; 95% confidence interval, 0.26-0.98; P = 0.041). Subjects with the TLR5 stop codon produced significantly lower levels of proinflammatory cytokines in comparison with individuals with the wild-type genotype. Together, these results indicate that the TLR5 stop codon polymorphism is associated with protection from the development of SLE. These data support a role for flagellated bacteria and the innate immune response in the development of SLE with implications for novel immunomodulatory treatment strategies. genetic markers | genetic predisposition to disease | immunity | inflammation

Drawing on developmental contextual theory, the authors examined the relationship of perceived barriers and support with school engagement and vocational attitudes among 9th-grade urban high school students in 2 studies. Study 1 (N = 174) showed that both perceived barriers and perceived support from family kin were associated with youths' commitment to school and aspirations for success in their future careers. Study 2 (N = 181) replicated and extended Study 1, demonstrating that perceived barriers, general perceptions of support, and kinship support were associated with behavioral and attitudinal indexes of school engagement, as well as with aspirations for career success, expectations for attaining Career goals, and the importance of work in one's future. The findings contribute to efforts to identify individual and contextual factors relevant to the educational and vocational lives of urban minority youth.
