The goal of this paper is to underline the evolution of breeding indices for Grey Steppe breed, based on the fact that this breed is on the verge of extinction. The biological material was represented by a number of 48 specimens of Grey Steppe breed from three locations (SC Boviselect SRL, Neamț, SC Modern Farm SRL, Cluj-Napoca and SCDCB - Dancu, Iaşi). The breeding indices were calculated based on the breeding records of the specialized units for cattle breeding and were statistically interpreted with the help of the program Microsoft Excel from the Microsoft Office package and finally the variation estimators (arithmetic average, the average standard deviation and the variation coefficient) for the analyzed parameters were established. The average values for the gestation period fell within the recommended interval according to the literature (278 - 290 days) for all three farms. The average duration for service- period exceeds the recommended interval by far (45 - 75 days), resulting 373 days for the second lactation and 310 days for the seventh lactation as the cattle had more time to recover between the two calving. The period between the two calving has exceeded 365 days since the duration of service-period was longer, coming up to average means of over 500 days. The age at the first calving had the average value above the recommended one of 24 months in the literature, due to breeding at greater ages of the specimens from Grey Steppe breed. The purpose of this paper was to establish the variation of milk production for Sura de Stepa breed based on the fact that this breed was the focus of many researchers lately. The biological material was represented by a number of 48 cows of Sura de Stepa breed from three locations (SC Boviselect SRL, Neamţ, SC Modern Farm SRL, Cluj-Napoca and SCDCB - Dancu, Iaşi). The indices of milk production were calculated based on the production records of the above mentioned units and were statistically interpreted with the help of the program Microsoft Excel from the Microsoft Office package and finally the variation estimators (arithmetic average, the average standard deviation and the variation coefficient) for the analyzed parameters were established. The milk production indices that were taken into account, were: the duration of the normal and total lactation, the fat and protein content in milk, procentually expressed also in kg. The duration of normal lactation has varied between 214,1 days for the cows from SC Boviselect SRL and 288,4 days for the cows from SC Modern Farm SRL and the total one was between 321 days for the cows from SC Modern Farm and 361 days from the specimens from SCDCB Dancu. The minimum milk production was of 1878 litres and the maximum was of 3726,8 litres, both values being registered at SCDCB Dancu. The fat content was of about 4% and the protein content was over 3% for all the cows from the three locations. [ABSTRACT FROM AUTHOR]

14. Ultrasound of the median arcuate ligament syndrome: a new approach to diagnosis.

- Abstract
Introduction: The currently accepted pathophysiological concept behind median arcuate ligament syndrome (MALS) is vascular compromise by atypically positioned median arcuate ligament. Despite many articles on MALS, only sparse data on the outcome in general and the rather poor outcome after intervention have been reported and this makes at least questionable the current concepts of the underlying disease.Material and Methods: A total of 364 patients with suspected celiac trunk pathology underwent a standardized ultrasound assessment procedure: suspected diagnostic features for MALS such as typically elevated peak flow velocities (PV) in the celiac trunk or atypical celiac trunk deflection-angles (DA) were defined in patients and in 20 matched volunteers.Results: All the 6 retrospectively clearly diagnosed MALS-patients as well as 40% (8/20) of volunteers presented a DA of over 50°. MALS-patients presented a mean inspiratory PV of 172cm/s (+/- 40.9 cm/s), a mean expiratory PV of 425cm/s (+/-130.1 cm/s) with a PV-amplitude of 249.1% (+/-68.9%). Volunteers presented a mean inspiratory PV of 126.9cm/s (+/-42 cm/s), a mean expiratory PV of 209.9cm/s (+/-80.1 cm/s) with a PV-amplitude of 169.4% (+/-54.3%).Conclusions: The combination of a maximum expiratory PV of over 350 cm/s and a DA higher than 50° seems to be a most reliable indicator for MALS in this small series of patients. Based on these data we propose that functional ultrasound should be the first line in screening for MALS. However, a clear pathophysiological definition of MALS remains still obscure. 17. Caseload and In-Hospital Outcome of Carotid Surgery Performed during the COVID-19 Pandemic vs. Previous Years: A Single-Centre Analysis.

- Abstract
With the beginning of the COVID-19 pandemic in March 2020, restrictions and challenges for elective and emergency vascular surgery as well as worse outcomes were reported. This study aims to compare our single-centre experience with carotid artery surgery during the pandemic and previous years.Our retrospective analysis included all consecutive patients undergoing carotid surgery for symptomatic and asymptomatic stenosis between January 2017 and December 2021. Caseload, operation specific parameters, and demographic data as well as in-hospital outcome were compared during the COVID-19 pandemic versus previous years.A total of 623 consecutive patients were included. The caseload comparison showed an average of 112 carotid artery surgeries per adjusted year (March 16th to December 31st) from 2017 to 2019, prior to the pandemic. The caseload reduction in the first year of the pandemic (2020) was 36.6% (n = 71) and 17.9% (n = 92) in the second year (2021). No rebound effect was observed. There was no significant difference (p = 0.42) in the allocation of symptomatic and asymptomatic patients (asymptomatic patients: 37.1% prior vs. 40.8% during the pandemic; symptomatic patients: 62.9 vs. 59.2%). Major adverse event rates in years prior to the pandemic were postoperative bleeding requiring revision: n = 31 (7.1%); stroke in symptomatic patients: n = 9 (3.3%) and stroke in asymptomatic patients: n = 4 (2.5%); symptomatic myocardial infarction (MCI): n = 1 (0.2%); death: n = 2 (0.5%). During the pandemic, major adverse event rates were postoperative bleeding requiring revision: n = 12 (6.5%); stroke in symptomatic patients: n = 1 (0.9%), stroke in asymptomatic patients: n = 1 (1.3%); symptomatic MCI: n = 1 (0.5%); death: n = 1 (0.5%).Since the beginning of the COVID-19 pandemic in March 2020, there has been a significant reduction in carotid artery surgery performed both in symptomatic as well as in asymptomatic patients. - Abstract
Purpose: Diabetes mellitus (DM) and carpal tunnel syndrome (CTS) are common pathologies. The diagnosis of CTS can be facilitated by the use of an ultrasound-based wrist-to-forearm ratio (WFR) of the nerve diameter. However, the applicability of WFR in DM-patients is not yet clear., Materials and Methods: 233 wrists of 153 patients were examined. Cross-sectional areas (CSA) of the median nerve were obtained using a linear array probe. The WFR was calculated., Results: Diabetics with CTS had significantly lower WFR values than non-diabetics with CTS (p = 0.002). There was no difference between the WFR of diabetics with and without CTS (p = 0.06). The diagnostic accuracy between diabetics with and without CTS was low for measurements of WFR (ROC AUC = 0.630, 95 % CI 0.541 - 0.715, p = 0.011)., Conclusion: Our findings suggest that the WFR has a low diagnostic accuracy in diabetic patients with CTS and should be used with caution in those patients., Key Points: · The diagnostic accuracy of WFR is low in patients with DM. · WFR should not be used in patients with DM. · The sonographic evaluation of the median nerve in patients with DM should focus on morphological changes., Citation Format: · Steinkohl F, Loizides A, Gruber L et al.
