Abstract

Abstract Objective Multiple sclerosis (MS) is a multifactorial disease with increasingly complicated management. Our objective is to use on‐demand computational power to address the challenges of dynamically managing MS. Methods A phase 3 clinical trial data (NCT00906399) were used to contextualize the medication efficacy of peg‐interferon beta‐1a vs placebo on patients with relapsing–remitting MS (RRMS). Using a set of reference patients (PORs), selected based on adequate features similar to those of an individual patient, we visualize disease activity by measuring the percentage of relapses, accumulation of new T2 lesions on MRI, and worsening EDSS during the clinical trial. Results We developed MS Vista, a functional prototype of clinical decision support system (CDSS), with a user‐centered design and distributed infrastructure. MS Vista shows the medication efficacy of peginterferon beta‐1a versus placebo for each individual patient with RRMS. In addition, MS Vista initiated the integration of a longitudinal magnetic resonance imaging (MRI) viewer and interactive dual physician‐patient data display to facilitate communication. Interpretation The pioneer use of PORs for each individual patient enables personalized analytics sustaining the dialog between neurologists, patients and caregivers with quantified evidence.

2. A Clinically-Compatible Workflow for Computer-Aided Assessment of Brain Disease Activity in Multiple Sclerosis Patients

Abstract

Over the last 10 years, the number of approved disease modifying drugs acting on the focal inflammatory process in Multiple Sclerosis (MS) has increased from 3 to 10. This wide choice offers the opportunity of a personalized medicine with the objective of no clinical and radiological activity for each patient. This new paradigm requires the optimization of the detection of new FLAIR lesions on longitudinal MRI. In this paper, we describe a complete workflow—that we developed, implemented, deployed, and evaluated—to facilitate the monitoring of new FLAIR lesions on longitudinal MRI of MS patients. This workflow has been designed to be usable by both hospital and private neurologists and radiologists in France. It consists of three main components: (i) a software component that allows for automated and secured anonymization and transfer of MRI data from the clinical Picture Archive and Communication System (PACS) to a processing server (and vice-versa); (ii) a fully automated segmentation core that enables detection of focal longitudinal changes in patients from T1-weighted, T2-weighted and FLAIR brain MRI scans, and (iii) a dedicated web viewer that provides an intuitive visualization of new lesions to radiologists and neurologists. We first present these different components. Then, we evaluate the workflow on 54 pairs of longitudinal MRI scans that were analyzed by 3 experts (1 neuroradiologist, 1 radiologist, and 1 neurologist) with and without the proposed workflow. We show that our workflow provided a valuable aid to clinicians in detecting new MS lesions both in terms of accuracy (mean number of detected lesions per patient and per expert 1.8 without the workflow vs. 2.3 with the workflow, p = 5.10−4) and of time dedicated by the experts (mean time difference 2′45″, p = 10−4). This increase in the number of detected lesions has implications in the classification of MS patients as stable or active, even for the most experienced neuroradiologist (mean sensitivity was 0.74 without the workflow and 0.90 with the workflow, p-value for no difference = 0.003). It therefore has potential consequences on the therapeutic management of MS patients.

Abstract

The recently published DICOM-Real Time Video (DICOM-RTV) standard allows real-time synchronization of transported video and medical metadata flows. In order to demonstrate the advantages of this synchronization by design, a demonstration of a DICOM-RTV Augmented Reality (AR) based solution is proposed. This solution consists in facilitating the surgeon's gesture by monitoring the positioning of the biopsy needle under ultrasound guidance and constructing virtual and augmented reality views of the needle's trajectory. In this paper, the platform architecture and the obtained results are presented. Furthermore, a comparison between our proposed solution and a setup that do not use DICOM-RTV is provided in order to show the difference in term of precision, synchronization and delay. The obtained results validate the contribution of real-time synchronization to improve the accuracy of surgical gesture aid applications such as AR.

Abstract

With the current advancement of the information technologies and the growing demand for data sharing, the risk of private data leakages increases. Moreover, the aggregation of multiple data, required for improving quality, may favor the prediction of unrevealed private and sensitive information, in particular in the medical data. Thus, with the current laws and regulations (GDPR, CNIL, HIPAA, etc.), it becomes required to protect the privacy of the patients when dealing with their sensitive data, and in particular when sending them outside the clinical site. Inline with this objective, the data anonymization and pseudonymization emerged among the solutions for safely sharing private data with remote peers. In this work, a particular interest is given to the pseudonymization of the DICOM Real-Time Video (DICOM-RTV) associated metadata that can be generated inside the operating room (OR), and shared via a streaming technology. The pseudonymization, by definition, includes both the notion of de-identification and re-identification, necessary for ensuring patient safety. Thus, the main objective of this paper is to define and conceptualize an architecture for de-identifying DICOM-RTV metadata on-the-fly before streaming them together with the DICOM-RTV video of the ongoing surgery outside the medical facility in real-time.

Abstract

International audience; Purpose The MEDIRAD project is about the effects of low radiation dose in the context of medical procedures. The goal of the work is to develop an informatics service that will provide the researchers of the MEDIRAD project with a platform to share acquired images, along with the associated dosimetric data pertaining to the radiation resulting from the procedure. Methods The authors designed a system architecture to manage image data and dosimetric data in an integrated way. DICOM and non-DICOM data are stored in separated repositories, and the link between the two is provided through a semantic database, i.e., a database whose information schema in aligned with an ontology. Results The system currently supports CT, PET, SPECT, and NM images as well as dose reports. Currently, two workflows for non-DICOM data generated from dosimetric calculations have been taken into account, one concerning Monte Carlo-based calculation of organ doses in Chest CT, and the other estimation of doses in nontarget organs in I-131 targeted radionuclide therapy of the thyroid. Conclusion The system is currently deployed, thus providing access to image and related dosimetric data to all MEDIRAD users. The software was designed in such a way that it can be reused to support similar needs in other projects.

Abstract

In order to overcome the challenges of managing real-time transfer of video, and/or audio, and associated medical metadata inside the medical theaters (e.g., operating room), a new DICOM communication service standard emerged. Its main objective is to deliver synchronized videos (potentially synchronized with their corresponding metadata) in real-time to surgeons during a surgery inside an operating room. Moreover, it allows, on one side, the transmission of real-time videos to subscribers with a quality of service comparable to the one inside the operating room, and on the other side, provides a standard that will allow the interoperability between the different medical equipment that produce/consume the media essences. This new DICOM extension is called DICOM Real-Time Video (DICOM-RTV). This paper is an introduction to this extension. It mainly presents the different challenges solved by this extension, illustrates it with relevant use cases, and provides the global architecture of the DICOM-RTV system. &nbsp

Abstract

This paper provides technical details regarding the worldwide premiere implementation of the DICOM Real-Time Video (DICOM-RTV), in which an experimental setup was deployed at the Rennes University Hospital in France during five Urological surgeries. The solution allows the real-time retransmission of medical images from different equipment in the operating room in a perfectly synchronized way. In this paper, we show how we have been able to transport and synchronize the video signal from a camera recording the hand gestures of the surgeon, and from an endoscope used by the same surgeon. This paper also demonstrates the interest of using the new DICOM-RTV standard in order to improve the surgical gestures precision and to provide precise, real-time and synchronized instructions to the different operators during a surgery, toward the next generation operation rooms.
