Introduction : L'état de santé des militaires est une préoccupation constante pour mener à bien leur mission régalienne. L'état de santé visuel occupe une place importante, puisqu'ils peuvent être victime d'atteintes visuelles en situation opérationnelle ou non. Certaines affections oculaires par leurs complications peuvent être responsables d'une diminution de la capacité opérationnelle des militaires. En général peu d'étude dans le monde et en particulier au Mali sont connues sur les pathologies oculaires dont ils sont victime en situation opérationnelle ou non. Le but de notre étude était de déterminer les affections oculaires chez les militaires de Kati afin d'assurer leur prise en charge adéquate et une amélioration de leur capacité opérationnelle. Matériel et méthode : Notre étude transversale et prospective s'est déroulée entre janvier 2016 et septembre 2017 au centre de santé de référence de Kati/Mali. La population d'étude était tous les militaires en service à Kati consultés au niveau du service d'ophtalmologie du centre de santé de référence de Kati/Mali entre janvier 2016 et septembre 2017. L'échantillonnage avait été non aléatoire et exhaustif et la taille de l'échantillon était de 203 participants. Etaient inclus les militaires ayant bénéficié d'une consultation ophtalmologique complète et qui acceptaient de faire partir de l'étude. L'examen ophtalmologique clinique complet et la collecte des données ont été effectué par un médecin ophtalmologiste. Les données collectées reposaient sur l'interrogatoire, l'examen de la fonction visuelle par la mesure de l'acuité visuelle de loin sur l'échelle de Monnoyer et l'acuité visuelle de près sur l'échelle de Parinaud, la prise de la pression intraoculaire au tonomètre à l'aplanation, l'examen des annexes et du segment antérieur de l'œil à la lampe à fente ainsi que l'examen du segment postérieur de l'œil par la lentille de Volk 90. Les matériels utilisés sont : une colonne de refractomètre, l'échelle de Monnoyer, l'échelle de Parinaud, une boite de verres d'essais, un réfractomètre automatique, un biomicroscope, une lentille de Volk 90. Les données étaient recueils dans une fiche d'enquête qui comportait des questionnaires testées au préalable. La saisie et l'analyse des données a été faite par le logiciel SPSS version 21. Les variables analysés étaient l'âge le sexe, le grade, le corps d'appartenance, l'acuité visuelle, la pression intraoculaire, les atteintes des annexes, les atteintes du segment antérieur de l'œil et les atteintes du segment postérieur. Le teste utilisé est le x2 de Pearson avec une marge d'erreur de 5%. Le consentement libre et éclairé de tous les participants avait été également obtenu. Résultats : au total 203 participants ayant donnée leur consentement ont été examinés au cours de l'étude. L'âge moyen de nos patients est 39,5% (extrême de 19 à 60 ans). Le sexe ratio était de 8 hommes / 1 femme. Les hommes du rang étaient les plus représentés soit 55% suivies des sous officiers 41%. L'armée de terre était plus représentée soit 91%. L'acuité visuelle moyenne était de 5/10 (extrême de pas de perception lumineuse à 10/10). et la pression intraoculaire moyenne était de 15 millimètres de mercure (extrême de 10 à 20 millimètres de mercure). Au total 194 atteintes oculaires ont été identifiées. Les amétropies représentaient la principale pathologie soit 21,67%. La principale affection annexielle était le ptérygion, tandis que les atteintes du segment antérieur et du segment postérieur étaient principalement la cataracte et le glaucome soit respectivement 16,74% et 9,85%. Conclusion : les affections oculaires sont nombreuses et multiformes chez les militaires de Kati. Leur identification dans cette étude permet d'assurer leur prévention, leur prise en charge et la réduction de leurs complications. Ils contribueront ainsi à l'amélioration de la capacité opérationnelle dans l'armée.

Item does not contain fulltext, Importance: Primary open-angle glaucoma presents with increased prevalence and a higher degree of clinical severity in populations of African ancestry compared with European or Asian ancestry. Despite this, individuals of African ancestry remain understudied in genomic research for blinding disorders. Objectives: To perform a genome-wide association study (GWAS) of African ancestry populations and evaluate potential mechanisms of pathogenesis for loci associated with primary open-angle glaucoma. Design, Settings, and Participants: A 2-stage GWAS with a discovery data set of 2320 individuals with primary open-angle glaucoma and 2121 control individuals without primary open-angle glaucoma. The validation stage included an additional 6937 affected individuals and 14917 unaffected individuals using multicenter clinic- and population-based participant recruitment approaches. Study participants were recruited from Ghana, Nigeria, South Africa, the United States, Tanzania, Britain, Cameroon, Saudi Arabia, Brazil, the Democratic Republic of the Congo, Morocco, Peru, and Mali from 2003 to 2018. Individuals with primary open-angle glaucoma had open iridocorneal angles and displayed glaucomatous optic neuropathy with visual field defects. Elevated intraocular pressure was not included in the case definition. Control individuals had no elevated intraocular pressure and no signs of glaucoma. Exposures: Genetic variants associated with primary open-angle glaucoma. Main Outcomes and Measures: Presence of primary open-angle glaucoma. Genome-wide significance was defined as P < 5 x 10-8 in the discovery stage and in the meta-analysis of combined discovery and validation data. Esthetic and functional surgeries in the periocular region fall into the domain of oculoplastic, or plastic and reconstructive surgery and otorhinolaryngology. Oculoplasty is the largest surgical spectrum in ophthalmology including eyelids, orbits and the lacrimal system. Our purpose was to study the frequency of eyelid, lacrimal and orbital (oculoplastic) surgery at the CHU IOTA. Material and methods: Retrospective medical chart review of all the patients who underwent oculoplastic surgery was conducted from October 2015 to September 2016. Results: A total of 233 patients were included with 52% female and 48% male. Patients were 37 years old on average. In total, the surgery was performed on the eyelids in83 cases (35.6%), the orbits in 113 cases (48.5%) and the lachrymal system in 37 cases (15.9%).Eyelid surgery commonly resulted from eye trauma (43,3%), followed by malpositions (30,2%). Mutilating surgery accounted for 92% of the orbital surgery. External dacryocystorhinostomy was the main lacrimal surgery (42%) followed by canalicular lacerations 25%. Conclusion: Oculoplasticsurgeryoccupies an important place in the surgical activities of the ophthalmologic University hospital despite a larger volume for cataractsurgery. Our studydoesn't highlight the outcome of the surgery itself but the epidemiology to help decision makers in their eye health policy including the reduction of mutilating surgery and oculoplastic training. Introduction: Oculo-orbital tumors are common. Their clinical and histological features are multiple. The management of oculo-orbital tumors is a real challenge in sub-Saharan Africa, especially in our context. The aim of this study was to contribute to the improvement of the management of oculo-orbital tumors at the Bangui Teaching Hospital (CNHUB)., Methodology: This was a prospective, observational study of oculo-orbital tumors over 12 months from January 1 to December 31, 2022 in the ophthalmology department of the CNHUB. All consenting patients with histologically proven tumors of the eyeball and/or its adnexa were included in this study, regardless of age and sex.The parameters studied were:epidemiological: age, sex, occupation, origin;clinical: time of consultation, functional signs and associated signs, physical examination signs, risk factors;paraclinical: report of the anatomopathological examination of the operating room;therapeutic: treatment modalities (chemotherapy, surgery, palliative treatment, radiotherapy).These data were collected on a pre-established survey form for direct administration. The processing and the production of the results were carried out with Excel and Epi Info 7.0 software., Results: We included 97 patients with a mean age of 37.5 years, ranging from 2 to 70 years. There was a male predominance (sex-ratio = 1.8). Almost all patients lived in Bangui (96%). Patients under 40 years of age were most commonly affected (55%). The majority of tumors were benign (55%). Ocular pain (24%), lacrimation (22%) and orbital swelling (16%) were the main symptoms. Risk factors were mainly age (23%), diabetes (22%), HIV infection (16%) and alcohol and tobacco use (37%). The most common treatments were surgical removal (68%) and enucleation (16%). Chemotherapy was combined with surgery in 37% of cases., Conclusion: Oculo-orbital tumors are frequent in our context. Objectives: The main objective of this study was to evaluate the effect of CYP2B6 and CYP3A4 polymorphisms on the virological and immunologic responses of HIV patients. A total of 153 HIV-positive patients were enlisted for the study., Patients and Methods: Viral load and median CD4 T cell counts were evaluated at baseline and month 6 (M6). Samples were identified using TaqMan genotyping assays., Results: The AG in CYP2B6 rs2279343 was associated with VLS compared to homozygous AA. In the dominant model, the AG/GG genotypes were associated with VLS compared to the AA genotype. Moreover, in overdominant model, the AG genotype was associated with VLS compared to AA/GG. Regarding immunological response, only the AG in SNP rs2279343 CYP2B6 was associated with an increase in CD4 cell count between baseline and M6. In CYP2B6 rs3745274, the CD4 cell count at M6 was higher than that of baseline for GG carriers and for GT carriers. In CYP3A4 rs2740574, the TC carriers showed a higher median CD4 count at M6 compared to that of the baseline count, as well as for CC carriers. The best genotypes combination associated with CD4 cell count improvement were AA/AG in SNP rs2279343 and GG/GT in SNP rs3745274., Conclusion: Our findings support the fact that CYP2B6 rs2279343 could help in the prediction of VLS and both SNPs rs3745274 and rs2279343 in CYP2B6 and CYP3A4 rs2740574 were associated with immune recovery in Malian HIV-positive patients., (Copyright © 2022 Wolters Kluwer Health, Inc. Conjunctival tumors are common in tropical areas, where exposure to ultraviolet radiation is high and almost permanent. Malignant tumors are quite rare and the most represented is conjunctival squamous cell carcinoma. We report two cases of invasive squamous cell carcinoma of the conjunctiva received in the ophthalmology department of the Sominé Dolo hospital in Mopti, Mali. The patients were 25 and 51 years old, living in a rural desert area exposed to sunlight and dust. They presented with a mass developed in the area of the palpebral fissure, invading the cornea and preventing palpebral occlusion. The mass was raised, multi-lobulated, well circumscribed, pearly white in color and papillomatous in appearance with dilation of the feeder vessels. A wide surgical excision at 4 - 5 mm from the healthy tissue edges was performed with anatomopathological examination of the specimen, which confirmed a mature and invasive differentiated squamous cell carcinoma of the conjunctiva. The extension workup and HIV serology were negative. - Abstract
Background: Ocular damage, including damage to the conjunctiva, lacrimal gland, eyelids, and orbit, caused by Schistosoma haematobium is sporadic. We report a clinical case of orbital migration of schistosome eggs., Case Presentation: A 14-year-old boy of Malian nationality presented with a painless swelling of the upper right eyelid, which had been gradually increasing for approximately 3 months. Visual acuity was logMAR 0.10 and 0.00 in the right and left eye, respectively. External examination revealed a right palpebral mass, pushing the globe slightly downward and inward. Computed tomography revealed a mass of the right lacrimal gland. Total excision of the mass was performed by transpalpebral orbitotomy. Pathological examination revealed an inflammatory granulomatous infiltrate of the lacrimal gland consisting of lymphocytes, eosinophils, giant cells, epithelioid cell, histiocytes and calcified Schistosoma eggs with terminal spine. Urine examination revealed eggs of S. haematobium. Praziquantel 40 mg/kg was administered to the patient. The hematuria stopped after 1 week. After 3 years of follow-up, no recurrence was noted., Conclusions: The bilharzian granuloma of the lacrimal gland is an ectopic site of the parasite. In this case, the granuloma was cured by surgical excision followed by a course of Praziquantel.

- Abstract
Introduction: During the COVID-19 pandemic, we have witnessed a world-wide lock-down of the population. This government action combined with the application of social distancing should in principle reduce the frequency of occurrence of ocular injuries. The goal of our work is to try to understand the circumstances of the occurrence of ocular injuries at the IOTA Teaching Hospital during the lock-down period of the COVID-19 health crisis., Methodology: This was a cross-sectional, descriptive study. The data were collected prospectively. Our study covered the period from March to May 2020. All consenting patients seen at the IOTA Teaching Hospital for ocular trauma regardless of gender, age, circumstances in which the trauma occurred or the nature of the injuries were included by non-probability sampling. Excluded from the study were patients who did not consent or who consulted for a non-traumatic ophthalmologic condition., Results: There were a total of 138 cases, of which 84 were male and 54 female, for a gender ratio of M/F=1.5. Children aged 0 to 5 years represented more than 3/4 (79.14%) of our sample. Trauma occurred in 45.83% of cases during leisure activities and 3.60% of cases involved domestic violence., Discussion: According to the authors, measures aimed at limiting public movement, particularly the curfews introduced by the Malian government to contain the spread of the COVID-19 pandemic, may actually result in trauma., Conclusion: Raising public awareness of the social and psychological consequences of lock-down through audiovisual means might significantly reduce the frequency of these ocular traumas., (Copyright © 2020 Elsevier Masson SAS. - Abstract
Importance: Primary open-angle glaucoma presents with increased prevalence and a higher degree of clinical severity in populations of African ancestry compared with European or Asian ancestry. Despite this, individuals of African ancestry remain understudied in genomic research for blinding disorders., Objectives: To perform a genome-wide association study (GWAS) of African ancestry populations and evaluate potential mechanisms of pathogenesis for loci associated with primary open-angle glaucoma., Design, Settings, and Participants: A 2-stage GWAS with a discovery data set of 2320 individuals with primary open-angle glaucoma and 2121 control individuals without primary open-angle glaucoma. The validation stage included an additional 6937 affected individuals and 14 917 unaffected individuals using multicenter clinic- and population-based participant recruitment approaches. Study participants were recruited from Ghana, Nigeria, South Africa, the United States, Tanzania, Britain, Cameroon, Saudi Arabia, Brazil, the Democratic Republic of the Congo, Morocco, Peru, and Mali from 2003 to 2018. Individuals with primary open-angle glaucoma had open iridocorneal angles and displayed glaucomatous optic neuropathy with visual field defects. Elevated intraocular pressure was not included in the case definition. Control individuals had no elevated intraocular pressure and no signs of glaucoma., Exposures: Genetic variants associated with primary open-angle glaucoma., Main Outcomes and Measures: Presence of primary open-angle glaucoma. Genome-wide significance was defined as P < 5 × 10-8 in the discovery stage and in the meta-analysis of combined discovery and validation data., Results: A total of 2320 individuals with primary open-angle glaucoma (mean [interquartile range] age, 64.6 [56-74] years; 1055 [45.5%] women) and 2121 individuals without primary open-angle glaucoma (mean [interquartile range] age, 63.4 [55-71] years; 1025 [48.3%] women) were included in the discovery GWAS. The GWAS discovery meta-analysis demonstrated association of variants at amyloid-β A4 precursor protein-binding family B member 2 (APBB2; chromosome 4, rs59892895T>C) with primary open-angle glaucoma (odds ratio [OR], 1.32 [95% CI, 1.20-1.46]; P = 2 × 10-8). The association was validated in an analysis of an additional 6937 affected individuals and 14 917 unaffected individuals (OR, 1.15 [95% CI, 1.09-1.21]; P < .001). Each copy of the rs59892895*C risk allele was associated with increased risk of primary open-angle glaucoma when all data were included in a meta-analysis (OR, 1.19 [95% CI, 1.14-1.25]; P = 4 × 10-13). The rs59892895*C risk allele was present at appreciable frequency only in African ancestry populations. In contrast, the rs59892895*C risk allele had a frequency of less than 0.1% in individuals of European or Asian ancestry., Conclusions and Relevance: In this genome-wide association study, variants at the APBB2 locus demonstrated differential association with primary open-angle glaucoma by ancestry. If validated in additional populations this finding may have implications for risk assessment and therapeutic strategies.

29. [Assessment of quality of life in patients with primary open-angle glaucoma in urban areas].

- Abstract
Introduction: Quality of life is defined by the physical, emotional and social well-being of an individual. Primary Open Angle Glaucoma (POAG) is a blinding optic neuropathy, it can change the quality of life by its chronic and irreversible nature. The objective of this study was to evaluate the quality of life in patients with POAG in urban areas., Patients and Methods: this was a quantitative cross-sectional study with a descriptive aim over 3 months. Patients followed for POAG at CHU-IOTA with an age ≥ 18 years were included and submitted to the GlauQOL-17 questionnaire for data collection., Results: We collected 100 glaucomatous patients, the mean age of patients was 52 years with a standard deviation of 14.02. The extreme ages are 18 and 81 years old. There was a male predominance; (54%) with a sex ratio (M / F) of 1.7. The good visual acuities were majority (higher or equal to 3/10). Among our patients, 35% were at the stage of glaucoma starting at OD and 30% at the stage of moderate glaucoma at OG. Our patients had a poor quality of life score (score <50%) in the Anxiety and Driving dimensions. In patients younger than 70 years old, we had a poor anxiety score in all progressive stages of the disease. Scores of self-image dimensions, constraints, and management were greater than 60% in patients over 70 years of age., Conclusion: Alteration of some of the domains studied was proportional to evolution of POAG. - Abstract
Objective: the goal of this clinical case is to report a case of ophthalmic zoster in children and to insist on the relevance of early antiviral treatment so as to minimize corneal affection and preserve visual function., Observation: we report an eleven-years-old-girl case, with a history of varicella infection at 5 years old, who came for consultation for a painful eruption affecting the forehead, the upper eyelid, the nose, and the upper lips. The clinical examination showed many vesicles erythematous affecting the left hemi face. The diagnosis of ophthalmic zoster has been retained. The biological checkup was done namely blood count, and glycemia in empty stomach and a HIV serology. This checkup was normal. The treatment was a local antiseptic based and systemic with acyclovir and antalgic (tramadol) during 8 days. The evolution was favorable. Zoster is rare in children. The ophthalmic form is exceptional. The diagnosis is clinical and should mention some bladdery lesions grouped in bunch with a disposition which follows a metamere. - Abstract
Chorioretinian coloboma often raises clinical issues leading sometimes to some confusion. Nevertheless, diagnosis must be as accurate as possible for several reasons: the association of some congenital anomalies of the papilla with other neurologic, endocrine and systemic disorders as well as issues in differential diagnosis between it and other ophthalmic disorders. We report a clinical case of a 6 year old child, the first boy in a family with 2 children, with bilateral papillar coloboma and without other associated malformations. Patient?s history: normal birth and updated immunisation status. His father and his 4-year-old sister underwent ophthalmic examination which was unremakable. However his mother had diverge strabismus in the right eye.

- Abstract
Esthetic and functional surgeries in the periocular region fall into the domain of oculoplastic, or plastic and reconstructive surgery and otorhinolaryngology. Oculoplasty is the largest surgical spectrum in ophthalmology including eyelids, orbits and the lacrimal system. Our purpose was to study the frequency of eyelid, lacrimal and orbital (oculoplastic) surgery at the CHU IOTA., Material and Methods: Retrospective medical chart review of all the patients who underwent oculoplastic surgery was conducted from October 2015 to September 2016., Results: A total of 233 patients were included with 52% female and 48% male. Patients were 37 years old on average. In total, the surgery was performed on the eyelids in83 cases (35.6%), the orbits in 113 cases (48.5%) and the lachrymal system in 37 cases (15.9%).Eyelid surgery commonly resulted from eye trauma (43,3%), followed by malpositions (30,2%). Mutilating surgery accounted for 92% of the orbital surgery. External dacryocystorhinostomy was the main lacrimal surgery (42%) followed by canalicular lacerations 25%., Conclusion: Oculoplasticsurgeryoccupies an important place in the surgical activities of the ophthalmologic University hospital despite a larger volume for cataractsurgery. - Abstract
Introduction: The treatment of cataract blindness is surgical, allowing restored vision. The purpose of this study was to evaluate the functional results of cataract surgery in adults in a tertiary care referral center., Patients and Methods: Prospective study of 2012 eyes operated for adult cataract from September 1, 2009 to August 31, 2010 (12 months). The results were analyzed by the Monitoring Cataract Surgical Outcomes software (MCSO). The postoperative functional data and the causes of poor outcomes were identified., Results: A total of 1044 women (51.9%) and 968 men (48.1%) underwent cataract surgery. Mean age was 65 years. Extracapsular cataract extraction (ECCE), and manual sutureless small incision cataract surgery (SICS) with posterior chamber IOL implantation in 98%, were the main surgical techniques. Functional results indicated that 45.5% of our patients had good visual acuity (≥3/10) with current spectacles, 33% had limited visual acuity (1/10-2/10), and 21.6% had poor visual acuity (<1/10). The proportion of patients with good results improved with best spectacle correction to 63%, vs. 22.9% with limited visual acuity and 14.1% with poor outcomes. The causes of poor outcomes were mainly related to surgical complications (42.1%) and refractive errors (34.8%)., Discussion: These results are inferior to WHO standards, which recommend a rate greater than or equal to 80% for good outcomes and a rate below 5% for bad outcomes., Conclusion: The identification of the causes of poor outcomes underscores the importance of improving surgical skills and the need for postoperative refraction., (Copyright © 2012 Elsevier Masson SAS.
