Background. Sea water is a major source in a case of nutrients and useful components for food, minerals, and other raw materials. Using seawater can be solved some issues concerning purification of it. Generally, seawater is of harmful ions and various type of microorganisms. Therefore it is needed to carry out methods purification of seawater in terms of using it as a precious substance. A reverse osmosis method is advanced one, which can treat seawater with high quality. However, a reverse osmosis is of disadvantage due to application semiconducting membranes and consumption of high energy. In tis a case, precipitation approach is cost effective and less consumption of energy in comparing with osmosis technology. In this paper precipitation method is offered to remove CI- and SO4-2 ions off the sea water Purpose. The physical and chemical methods are utilized to make seawater as a fertilizer. Methodology. There are determined that heavy metal cations and nonmetal ions according to upon MS-1669347-05-04 standard method. Heavy Metals such as Fe, Cd, Co, Cu. Ni, Pb, Zn were determined by means ICP-OES (inductively Coupled plasma-optical emission spectrometer). Whereas, the anions such as NO2-3 --N, total nitrogen, phosphates and total phosphorus were determined by means the device CECIL 4004 spectrophotometer. Originality. The precipitation method is considered in order to remove large amount of CI- and SO4-2 ions using Pb(NO3)2. Using this method, it is conducted by doing the below mentionedcon versions. Stoichiometric consumption in amount of 100% is required to precipitate CI- and SO4-2 from metal containing salts. Findings. As results, seawater can be used after removing ions more than 90%, which are harmful for plants. [ABSTRACT FROM AUTHOR]

Aim/Purpose: Fibroblast activation protein-(FAP)-ligands, a novel class of tracers for PET/CT imaging, demonstrated promising results in previous studies in various malignancies compared to standard [
18 F]FDG PET/CT.68 Ga-labeled fibroblast activation protein inhibitor-([68 Ga]Ga-DOTA-FAPI)-PET/CT impresses with sharp contrasts in terms of high tumor uptake and low background noise leading to clear delineation. [18 F]FDG PET/CT has limited accuracy in bladder cancer due to high background signal. Therefore, we sought to evaluate the diagnostic potential of [68 Ga]FAPI in patients with bladder cancer. Material and Methods: This retrospective analysis consisted of 8 patients (median age 66), 7 of whom underwent both [68 Ga]FAPI and [18 F]FDG PET/CT scans with a median time interval of 5 days (range 1–20 days). Quantification of tracer uptake was determined with SUVmax and SUVmean . Furthermore, the tumor-to-background ratio (TBR) was derived by dividing the SUVmax of tumor lesions by the SUVmax of adipose tissue, skeletal muscle, and blood pool. Results: Overall, 31 metastases were detected in five patients including lymph node metastases (n = 23), bone metastases (n = 4), lung metastases (n = 3), and a peritoneal metastasis (n = 1). In one patient, [68 Ga]FAPI demonstrated significant uptake in the primary tumor located in the bladder wall. [68 Ga]FAPI-PET/CT demonstrated significantly higher uptake compared to [18 F]FDG PET/CT with higher mean SUVmax (8.2 vs. 4.6; p = 0.01). Furthermore, [68 Ga]FAPI detected additional 30% (n = 9) lesions, missed by [18 F]FDG. TBR demonstrated favorable uptake for [68 Ga]FAPI in comparison to [18 F]FDG. Significant differences were determined with regard to metastasis/blood pool ([68 Ga]FAPI 5.3 vs [18 F]FDG 1.9; p = 0.001). Conclusion: [68 Ga]FAPI-PET/CT is a promising diagnostic radioligand for patients with bladder cancer. This first described analysis of FAP-ligand in bladder cancer revealed superiority over [18 F]FDG in a small patient cohort. This study presents a provenance analysis of the Neolithic obsidian assemblages from the early to mid-sixth millennium bc settlement at Göytepe, Azerbaijan. The study is unique in that (1) it involves a complete, non-selected obsidian assemblage (901 artefacts) from one particular area of the site; (2) the material is derived from a well-stratified sequence of 10 securely radiocarbon-dated architectural levels; and (3) the use of an extraordinarily wide range of sources (more than 20) was identified by provenance analysis using energy-dispersive X-ray fluorescence. The results reveal a previously unknown diachronic change in obsidian use in the region, suggesting the occurrence of significant socioeconomic changes during the Late Neolithic of the southern Caucasus.

- Abstract
Background: Physicians' adherence to guideline-recommended therapy is associated with short-term clinical outcomes in heart failure (HF) with reduced ejection fraction (HFrEF). However, its impact on longer-term outcomes is poorly documented. Here, we present results from the 18-month follow-up of the QUALIFY registry. Methods and results: Data at 18 months were available for 6118 ambulatory HFrEF patients from this international prospective observational survey. Adherence was measured as a continuous variable, ranging from 0 to 1, and was assessed for five classes of recommended HF medications and dosages. Most deaths were cardiovascular (CV) (228/394) and HF-related (191/394) and the same was true for unplanned hospitalizations (1175 CV and 861 HF-related hospitalizations, out of a total of 1541). According to univariable analysis, CV and HF deaths were significantly associated with physician adherence to guidelines. In multivariable analysis, HF death was associated with adherence level [subdistribution hazard ratio (SHR) 0.93, 95% confidence interval (CI) 0.87–0.99 per 0.1 unit adherence level increase; P = 0.034] as was composite of HF hospitalization or CV death (SHR 0.97, 95% CI 0.94–0.99 per 0.1 unit adherence level increase; P = 0.043), whereas unplanned all-cause, CV or HF hospitalizations were not (all-cause: SHR 0.99, 95% CI 0.9–1.02; CV: SHR 0.98, 95% CI 0.96–1.01; and HF: SHR 0.99, 95% CI 0.96–1.02 per 0.1 unit change in adherence score; P = 0.52, P = 0.2, and P = 0.4, respectively). Conclusion: These results suggest that physicians' adherence to guideline-recommended HF therapies is associated with improved outcomes in HFrEF. Objectives To evaluate the diagnostic performance of 14-core repeat biopsy protocol and the impact of prior biopsy scheme on repeat prostate biopsy group. Methods 211 patients had repeat biopsy using 14-core protocol consisting of 10-core peripheral zone (classical sextant+4 lateral peripheral cores) and 4-core transitional zone (TZ) biopsies. The diagnostic yield was determined both in patients who had previously undergone sextant or 10-core biopsy protocol. Results Overall cancer detection rate was 25.6%. 14-core biopsy technique detected cancer in 36.1 and 18.7% of the patients who had a previous sextant biopsy and 10-core biopsy protocol, respectively (P = 0.005). Patients with and without high-grade prostatic intraepithelial neoplasia (HGPIN) in the previous sextant biopsy had 56.5 and 28.3% cancer detection rates on the subsequent extended biopsy, respectively (P = 0.017) Patients who had previous 10-core biopsy with and without HGPIN revealed 22.9 and 17.2% cancer detection rates, respectively (P = 0.465) Additional four lateral peripheral cores detected 33% (3/30) and 17% (4/24) of cancers in patients with previous sextant and 10-core biopsy, respectively. 3.7% of the patients had tumor only in the TZ and none of them had prior extended biopsy. Conclusion The yield of extended 14-core repeat biopsy protocol was higher in patients with previous negative sextant biopsy compared to the patients with previous negative 10-core biopsy. HGPIN history found on previous sextant biopsy was a strong cancer predictor on repeat biopsy; same was not true for the patients with previous 10-core biopsy. The yield of lateral peripheral cores and TZ biopsies were lower in patients with prior negative extended biopsy.

- Abstract
The article describes the method of calculating the strength of normal sections with the use of different diagrams of flexural rectangular reinforced-concrete elements. It has been shown that, the deformation diagram of the concrete created as fractional-rational function specified by Eurocode °b = Rb k -P-P2 , p=°^ 1 + (k - 2) -P' P sR k — Eb ' SR R and when applying two-dimensional and triple diagrams, the simplest variants of this diagram, the solving equations system can be written as Rb'b-h0 -Ç -Mb + Rsc 'As Rs - As — 0 M < M ult am Rb 'b-K + + RSc-A's-(h0 - a's) °С = Ç-(®b -Ç-Pb ) and , only the momentary nonlinear diagrams at this time should be calculated as m b . In the article, depending on the class of concrete for each used diagram for these new coefficients the values are given. Thus, with the application of nonlinear diagrams, the choice of load capacity and reinforcing bar has been brought to the traditional method of calculation. The comparative solution of the numerical examples shows that there is no need to apply complex diagrams for the defenition of load capacity, and it is desirable to apply the perfect deformation diagrams when selecting the reinforcing bar. This study presents preliminary results of mitochondrial DNA analyses of modern and ancient domestic goats in the southern Caucasus in order to examine their phylogenetic relationship with modern and ancient goats. For this purpose, seven ancient samples were obtained from two early agricultural villages in west Azerbaijan (Göytepe and Hacı Elamxanlı tepe, dated to ca. 6000-5500 cal bc, the Pottery Neolithic period), in addition to five modern goat samples in the same region. In the study, mitochondrial DNA segments of the control region (216 bp for the Neolithic samples and 481 bp for the modern samples) were amplified, and phylogenetic analyses were performed using previously published reference DNA sequences. As a result, all the haplotypes found in this study were grouped in the haplogroup A of goats. The finding of the haplogroup A among domestic goats in the southern Caucasus in the early sixth millennium bc can be interpreted as part of the geographic expansion of this lineage from the areas of initial domestication to surrounding areas that include also South and Southeast Europe. In the southern Caucasus, the haplogroup A probably continued to be a major lineage among domestic goats since their emergence in this area to the present. In contrast, this lineage has not been detected among local wild goats including Capra aegagrus, indicating the external origin of domestic goats. This possibility is consistent with archaeological records that indicate sudden appearance of agricultural lifeways in the southern Caucasus and cultural connections with northern Mesopotamia. Embaràs i malaltia cardíaca; Resultats de l’embaràs; Transposició de les grans artèries Embarazo y enfermedad cardíaca; Resultados del embarazo; Transposición de las grandes arterias Pregnancy and cardiac disease; Pregnancy outcomes, Transposition of the great arteries Background In the past 3 decades, the arterial switch procedure has replaced the atrial switch procedure as treatment of choice for transposition of the great arteries. Although survival is superior after the arterial switch procedure, data on pregnancy outcomes are scarce and transposition of the great arteries after arterial switch is not yet included in the modified World Health Organization classification of maternal cardiovascular risk. Methods and Results The ROPAC (Registry of Pregnancy and Cardiac disease) is an international prospective registry of pregnant women with cardiac disease, part of the European Society of Cardiology EURObservational Research Programme. Pregnancy outcomes in all women after an arterial switch procedure for transposition of the great arteries are described. The primary end point was a major adverse cardiovascular event, defined as combined end point of maternal death, supraventricular or ventricular arrhythmias requiring treatment, heart failure, aortic dissection, endocarditis, ischemic coronary events, and thromboembolic events. Altogether, 41 pregnant women (mean age, 26.7±3.9 years) were included, and there was no maternal mortality. A major adverse cardiovascular event occurred in 2 women (4.9%): heart failure in one (2.4%) and ventricular tachycardia in another (2.4%). One woman experienced fetal loss, whereas no neonatal mortality was observed. Conclusions Women after an arterial switch procedure for transposition of the great arteries tolerate pregnancy well, with a favorable maternal and fetal outcome. During counseling, most women should be reassured that the risk of pregnancy is low. Classification as modified World Health Organization risk class II seems appropriate. ObjectiveCardiac disease is a major cause of maternal mortality. Data regarding pregnancy outcomes in women with a systemic right ventricle (sRV) are scarce. We studied pregnancy outcomes in women with an sRV after the atrial switch procedure for transposition of the great arteries (TGA) or congenitally corrected TGA (CCTGA).MethodsThe ESC EORP Registry of Pregnancy and Cardiac Disease is an international prospective registry of pregnant women with cardiac disease. Pregnancy outcomes (maternal/fetal) in all women with an sRV are described. The primary end point was a major adverse cardiac event (MACE) defined as maternal death, supraventricular or ventricular arrhythmias requiring treatment, heart failure, aortic dissection, endocarditis, ischaemic coronary event and other thromboembolic events.ResultsAltogether, 162 women with an sRV (TGA n=121, CCTGA n=41, mean age 28.8±4.6 years) were included. No maternal mortality occurred. In 26 women, at least one MACE occurred, heart failure in 16 (9.8%), arrhythmias (atrial 5, ventricular 6) in 11 (6.7%) and others in 4 (2.5%). Prepregnancy signs of heart failure as well as an sRV ejection fraction ConclusionThe majority of women with an sRV tolerated pregnancy well with a favourable maternal and fetal outcome. Heart failure and arrhythmias were the most common MACE.

BackgroundCardiovascular disease is the leading cause of death during pregnancy with thoracic aortic dissection being one of the main causes. Thoracic aortic disease is commonly related to hereditary disorders and congenital heart malformations such as bicuspid aortic valve (BAV). Pregnancy is considered a high risk period in women with underlying aortopathy.MethodsThe ESC EORP Registry Of Pregnancy And Cardiac disease (ROPAC) is a prospective global registry that enrolled 5739 women with pre-existing cardiac disease. With this analysis, we aim to study the maternal and fetal outcome of pregnancy in women with thoracic aortic disease.ResultsThoracic aortic disease was reported in 189 women (3.3%). Half of them were patients with Marfan syndrome (MFS), 26% had a BAV, 8% Turner syndrome, 2% vascular Ehlers-Danlos syndrome and 11% had no underlying genetic defect or associated congenital heart defect. Aortic dilatation was reported in 58% of patients and 6% had a history of aortic dissection. Four patients, of whom three were patients with MFS, had an acute aortic dissection (three type A and one type B aortic dissection) without maternal or fetal mortality. No complications occurred in women with a history of aortic dissection. There was no significant difference in median fetal birth weight if treated with a beta-blocker or not (2960 g (2358–3390 g) vs 3270 g (2750–3570 g), p value 0.25).ConclusionThis ancillary analysis provides the largest prospective data review on pregnancy risk for patients with thoracic aortic disease. Overall pregnancy outcomes in women with thoracic aortic disease followed according to current guidelines are good.

Despite the localisation of the southern Caucasus at the outskirt of the Fertile Crescent, the Neolithisation process started there only at the beginning of the sixth millennium with the Shomutepe-Shulaveri culture of yet unclear origins. We present here genomic data for three new individuals from Mentesh Tepe in Azerbaijan, dating back to the beginnings of the Shomutepe-Shulaveri culture. We evidence that two juveniles, buried embracing each other, were brothers. We show that the Mentesh Tepe Neolithic population is the product of a recent gene flow between the Anatolian farmer-related population and the Caucasus/Iranian population, demonstrating that population admixture was at the core of the development of agriculture in the South Caucasus. By comparing Bronze Age individuals from the South Caucasus with Neolithic individuals from the same region, including Mentesh Tepe, we evidence that gene flows between Pontic Steppe populations and Mentesh Tepe-related groups contributed to the makeup of the Late Bronze Age and modern Caucasian populations. - Abstract
Archaeological and archaeogenetic evidence points to the Pontic-Caspian steppe zone between the Caucasus and the Black Sea as the crucible from which the earliest steppe pastoralist societies arose and spread, ultimately influencing populations from Europe to Inner Asia. However, little is known about their economic foundations and the factors that may have contributed to their extensive mobility. Here, we investigate dietary proteins within the dental calculus proteomes of 45 individuals spanning the Neolithic to Greco-Roman periods in the Pontic-Caspian Steppe and neighbouring South Caucasus, Oka-Volga-Don and East Urals regions. We find that sheep dairying accompanies the earliest forms of Eneolithic pastoralism in the North Caucasus. During the fourth millennium BC, Maykop and early Yamnaya populations also focused dairying exclusively on sheep while reserving cattle for traction and other purposes. We observe a breakdown in livestock specialization and an economic diversification of dairy herds coinciding with aridification during the subsequent late Yamnaya and North Caucasus Culture phases, followed by severe climate deterioration during the Catacomb and Lola periods. The need for additional pastures to support these herds may have driven the heightened mobility of the Middle and Late Bronze Age periods. Here, we report genome-wide data analyses from 110 ancient Near Eastern individuals spanning the Late Neolithic to Late Bronze Age, a period characterized by intense interregional interactions for the Near East. We find that 6
th millennium BCE populations of North/Central Anatolia and the Southern Caucasus shared mixed ancestry on a genetic cline that formed during the Neolithic between Western Anatolia and regions in today's Southern Caucasus/Zagros. During the Late Chalcolithic and/or the Early Bronze Age, more than half of the Northern Levantine gene pool was replaced, while in the rest of Anatolia and the Southern Caucasus, we document genetic continuity with only transient gene flow. Additionally, we reveal a genetically distinct individual within the Late Bronze Age Northern Levant. Overall, our study uncovers multiple scales of population dynamics through time, from extensive admixture during the Neolithic period to long-distance mobility within the globalized societies of the Late Bronze Age. 42. Trump's "America first" energy policy, contingency and the reconfiguration of the global energy order.

- Abstract
The global energy system is in transition to a new energy order characterized by the emergence of the United States as a net oil exporter, the shale revolution and the gradual shift towards low-carbon sources and renewables. The shale boom in the US was a game changer, as was the election of Donald Trump as US president. Trump pushed an ambitious "America first" agenda aimed at transforming the US into a global energy superpower. The purpose of this article is two-fold: first, it outlines the key pillars of the emerging global energy order. Second, it underscores the role of contingent events, a factor neglected by some previous studies because of their reliance on what A.O.Hirschman (1970) termed "paradigmatic thinking". The recent transition in the international energy order is an outcome of two paramount, yet largely unanticipated events: the shale revolution in the US and Trump's neomercantilist and unilateralist economic policies. While contingencies are an inherent feature of social reality, the scenario approach can be a useful heuristic for dealing with uncertainties. The Editors' Network of the European Society of Cardiology (ESC) provides a dynamic forum for editorial discussions and endorses the recommendations of the International Committee of Medical Journal Editors (ICMJE) to improve the scientific quality of biomedical journals. Authorship confers credit and important academic rewards. Recently, however, the ICMJE emphasized that authorship also requires responsibility and accountability. These issues are now covered by the new (fourth) criterion for authorship. Authors should agree to be accountable and ensure that questions regarding the accuracy and integrity of the entire work will be appropriately addressed. Objectives: Subsistence strategies are of great interest for understanding how prehistoric societies adapted to their environment. This is particularly the case for the southern Caucasus where relationships have been shown with the northern Caucasus and Mesopotamia since the Neolithic and where societies are alternately described as sedentary and mobile. This article aims, for the first time, to characterize human diets and their evolution using biochemical markers, from the Neolithic to the Bronze Age (sixth-first millenium BC), at Mentesh Tepe, a site in the middle Kura valley in Azerbaijan., Materials and Methods: The data set belongs to 40 humans, 32 domestic and wild animals, and 42 charred seeds discovered in situ and perfectly dated. Stable isotope analyses were performed, including (a) δ
13 Cco and δ15 N for animal and human bone collagens and for seeds, and (b) δ13 Cap for human bone apatite., Results: Almost all the data (25/31) suggest an increased contribution of cereals, lentils, and freshwater fish during the Neolithic, whereas afterwards, until the Late Bronze Age, all individuals consumed more animal proteins from their livestock. None of the biological criteria (age at death and sex) and burial types (mass/single graves) were found to be related to a specific diet over time. Comparisons with other isotopic data from contemporary sites in Georgia argue in favor of a wide variety of dietary sources in the vicinity of the Kura valley and for highly mobile populations. 45. Epizootology and Molecular Diagnosis of Lumpy Skin Disease among Livestock in Azerbaijan.

- Abstract
Lumpy skin disease (LSD) is a viral disease of livestock that can cause cutaneous and internal lesions, affecting milk production, hide quality and in some cases death of the infected animal. After an outbreak in neighboring Iran, a working group from the Azerbaijan State Veterinary Control Service was sent to the border rayons (administrative districts) to determine if any cattle in southern Azerbaijan were infected. The Rayonal Veterinary Offices were contacted to look for and report any cases of LSD in their rayons. Animals exhibiting clinical signs consistent with LSD infection were first observed in the rayon of Bilasuvar and more cases were subsequently identified in Jalilabad, Ujar, and Aghdash rayons. Samples were collected from blood, and/or lesions of suspected infected animals and internal organs of cattle that died and were tested at the Republican Veterinary Laboratory in Baku using real-time polymerase chain reaction (PCR). From June to November 2014, 2,762 cattle in Azerbaijan were reported to have clinical signs or gross necropsy lesions consistent with LSD. Of 269 samples tested for LSD virus by real-time PCR, 199 (74%) were positive. A total of 33 cattle died, which was 1.2% of those exhibiting clinical signs of disease. Samples from nodular cutaneous lesions were more frequently positive by PCR and had higher concentrations of virus than blood and pooled internal organ samples. Preventative measures including movement restrictions, vector control and vaccination were put into place to slow the spread of disease. Ongoing surveillance should continue as environmental persistence of the virus may lead to further outbreaks of disease.

46. Biosurveillance of avian influenza and Newcastle disease viruses in the Barda region of Azerbaijan using real time RT-PCR and hemagglutination inhibition.

- Abstract
The Azerbaijan State Veterinary Control Service (SVCS) has conducted active serological surveillance for avian influenza (AI) in poultry since 2006, when the first outbreak of AI H5N1 occurred in Azerbaijan. Samples are collected from September to May annually and tested using a hemagglutination inhibition (HI) assay to detect antibodies against H5 AI viruses. HI testing is also performed for Newcastle disease virus (NDV) upon request, but since this method cannot distinguish between natural infections and immune responses to vaccination, all positive results require follow-up epidemiological investigations. Furthermore, blood collection for the surveillance program is time-intensive and can be stressful to birds. In order to improve the national surveillance program, alternative sampling and testing methodologies were applied among a population of birds in the Barda region and compared with results of the national surveillance program. Tracheal and cloacal swabs were collected instead of blood. Rather than testing individual samples, RNA was pooled to conserve resources and time, and pools were tested by real-time reverse transcription polymerase chain reaction (rRT-PCR). Environmental sampling at a live bird market was also introduced as another surveillance mechanism. A total of 1,030 swabs were collected, comprising tracheal, and cloacal samples from 441 birds and 148 environmental surface samples from farms or the live bird market. During the same time, 3,890 blood samples were collected nationally for the surveillance program; 400 of these samples originated in the Barda region. Birds sampled for rRT-PCR were likely different than those tested as part of national surveillance. All swab samples tested negative by rRT-PCR for both AI and NDV. All blood samples tested negative for H5 by HI, while 6.2% of all samples and 5% of the Barda samples tested positive for exposure to NDV. Follow-up investigations found that positive samples were from birds vaccinated in the previous month. This study demonstrated that taking swabs was quicker and less invasive than blood collection. Results of rRT-PCR testing were similar to HI testing for H5 but also ruled out infection with all influenza type A viruses and not just H5. In addition, rRT-PCR testing was able to rule out active infections with NDV.

Introduction: The aim of this study was to investigate the characteristics of patients who were incidentally diagnosed with transitional cell carcinoma (TCC) during percutaneous nephrolithotomy (PCNL) surgery., Patients and Methods: We retrospectively analyzed the data of 1,406 patients who underwent PCNL for the removal of renal calculi. Tumoral lesions detected during diagnostic cystourethroscopy and nephroscopy were endoscopically resected and the resected specimens were sent to the pathology laboratory for definitive diagnosis., Results: Tumoral lesions were detected with cystoscopic examination in 6 patients and with nephroscopy in 4. Pathological examination of the lesions in the kidneys revealed stage T1G1 TCC in 2 patients, T1G3 TCC with carcinoma in situ in 1, and high-grade TCC with sarcomatoid features in 1 patient. The patient with sarcomatoid features died 2 months after surgery and all the other patients survived with no evidence of disease at the end of 26.67 +/- 18.58 months of follow-up., Conclusions: Since early diagnosis and management of urothelial lesions significantly improves the prognosis, a careful examination of urothelial mucosa during PCNL surgery is important. Instant diagnosis of a sarcomatoid component and imperative surgical intervention may be life-saving.

- Abstract
The aim of this clinical study was to determine whether there is a relationship between total serum testosterone, free testosterone, FSH (Follicle-Stimulating Hormone), LH (Luteinizing Hormone) and serum prostate specific antigen (PSA) levels. We postulated that such a correlation existed then the use of hormone specific reference ranges might enhance the usefullness of PSA concentrations <2.5 ng/mL as a marker for prostate cancer. Prior to digital rectal examination, serum was obtained from all patients between 8.30-10:00 AM for hormone and PSA concentrations. The study was performed on 210 male patients >40 years of age visiting our urology outpatient clinics. PSA was correlated to age (r = 0.23, p = 0.019), but there none between serum testosterone and age. No significant correlation was noted between testosterone or free testosterone and serum PSA levels, and none between serum FSH or LH and PSA. In age specific reference groups (41-49; 50-59; 60-69 years), we found no significant correlation between PSA and hormone concentrations. In this population of eugonadal men with serum PSA values less than 2.5 ng/ml, serum androgens and pituitary hormones do not appear to correlate with serum PSA.

50. The effect of prior biopsy scheme on prostate cancer detection for repeat biopsy population: results of the 14-core prostate biopsy technique.

- Abstract
Objectives: To evaluate the diagnostic performance of 14-core repeat biopsy protocol and the impact of prior biopsy scheme on repeat prostate biopsy group., Methods: 211 patients had repeat biopsy using 14-core protocol consisting of 10-core peripheral zone (classical sextant+4 lateral peripheral cores) and 4-core transitional zone (TZ) biopsies. The diagnostic yield was determined both in patients who had previously undergone sextant or 10-core biopsy protocol., Results: Overall cancer detection rate was 25.6%. 14-core biopsy technique detected cancer in 36.1 and 18.7% of the patients who had a previous sextant biopsy and 10-core biopsy protocol, respectively (P = 0.005). Patients with and without high-grade prostatic intraepithelial neoplasia (HGPIN) in the previous sextant biopsy had 56.5 and 28.3% cancer detection rates on the subsequent extended biopsy, respectively (P = 0.017) Patients who had previous 10-core biopsy with and without HGPIN revealed 22.9 and 17.2% cancer detection rates, respectively (P = 0.465) Additional four lateral peripheral cores detected 33% (3/30) and 17% (4/24) of cancers in patients with previous sextant and 10-core biopsy, respectively. 3.7% of the patients had tumor only in the TZ and none of them had prior extended biopsy., Conclusions: The yield of extended 14-core repeat biopsy protocol was higher in patients with previous negative sextant biopsy compared to the patients with previous negative 10-core biopsy. HGPIN history found on previous sextant biopsy was a strong cancer predictor on repeat biopsy; same was not true for the patients with previous 10-core biopsy. The yield of lateral peripheral cores and TZ biopsies were lower in patients with prior negative extended biopsy.
