However, high dose for effective treatment limits their use. We have synthesized multifunctional gold nanoclusters (GNCs) that can be used for imaging and radiation therapy. The designed GNCs have been characterized for their physicochemical properties, biocompatibility, and their radiation dose enhancement potential on PC3 cell lines. Objectives: To assess pain perception in patients undergoing manual cataract surgery versus femtosecond laser-assisted cataract surgery (FLACS) and pain perception of patients receiving anaesthesia at 2 different time points during the FLACS procedure. We also aimed to assess the factors affecting pain perception in these different study groups., Design: Prospective cohort comparison of patient-selected surgical approach., Participants: Patients undergoing delayed sequential bilateral cataract surgery at an ambulatory surgical centre in Ontario., Methods: Patients were assigned to 3 groups: a manual cataract surgery cohort (n = 30) and 2 FLACS cohorts, those who received neurolept anaesthesia just prior to draping for phacoemulsification (FLACS standard cohort; n = 38) and those who received neurolept anaesthesia prior to the femtosecond laser part of the procedure (FLACS early cohort; n = 35). Outcome metrics included pain scores on the visual analogue scale on postoperative day 0 and at postoperative week 1. Secondary outcome measures included anxiety scores, surgeon or anaesthesiologist patient cooperation scores, ocular metrics, and perioperative systemic vitals., Results: There were no significant differences in pain perception on postoperative day 0 and at postoperative week 1 among the manual cataract surgery and FLACS standard cohorts (p = 0.94 and p = 0.72, respectively) or FLACS early and FLACS standard cohorts (p = 0.76 and p = 0.67, respectively). Patients had higher pain scores during second-eye procedures than first-eye procedures., Conclusions: Cataract surgery technique or timing of anaesthesia for FLACS procedures does not affect pain perception postoperatively. The conventional sharing of information and education between doctor and patient is no longer what it used to be prior to Dr. Google's entry into the mix with voluminous data, not only transforming but often threatening the patient-doctor relationship. While patients no longer seek basic information from their physicians since they've already consulted Dr. Google, the wise physician accepts that patients are now more aware, more involved in their own care, and more empowered with information. The good old doctor who knew everything is now more a myth that exists largely in folklore. While doctors may be well versed in a variety of fields, they have often narrowed down their areas of specialisation, while they continue to apply what they learn from their daily encounters with patients, establishing a stronger relationship over time. The challenge arises when a patient, having consulted Dr. Google, starts to question their doctor with the little knowledge gained from the internet. Biased opinions based on prior knowledge, have lately placed the doctor-patient relationship in jeopardy.

Background: Uveitic macular edema is a complication of acute or chronic uveitis. Current treatment regiments frequently have numerous side effects, often requiring supplemental treatment. This study investigates the efficacy of dexamethasone (DEX) intravitreal implants as monotherapy for treatment of macular edema in non-infectious intermediate, posterior or panuveitis., Methods and Results: Retrospective chart review of thirty patients with intermediate, posterior and panuveitis treated with DEX. Outcomes measured were central retinal thickness (CRT) and best corrected visual acuity (BCVA). Baseline measurements of CRT and BCVA were measured within 1 month prior to intravitreal DEX implant and follow up measurements were collected until 1 year post implant. 48 implants on 39 eyes of 30 patients were included in this study; 64.1% of patients had an improvement in BCVA and 65.4% had a reduction in CRT. BCVA improved from 0.285 logMAR (SD: 0.312) at baseline to 0.175 logMAR (SD: 0.194) at 1 month and was sustained thereafter. Preliminary CRT data showed a decrease from 392 [Formula: see text] (SD: [Formula: see text]) at baseline to 303 [Formula: see text] (SD: [Formula: see text] at 1 month and 313 [Formula: see text] (SD: [Formula: see text] at 12 months., Conclusions: The DEX implant as monotherapy for macular edema in non-infectious uveitis was associated with a reduction in CRT and improvement in BCVA. The microparticle quality and reproducibility of Li(Ni 0.8 Co 0.1 Mn 0.1 )O 2 (NCM811) cathode materials are important for Li-ion battery performance but can be challenging to control directly from synthesis. Here, a scalable reproducible synthesis process is designed based on slug flow to rapidly generate uniform micron-size spherical-shape NCM oxalate precursor microparticles at 25-34 °C. The whole process takes only 10 min, from solution mixing to precursor microparticle generation, without needing aging that typically takes hours. These oxalate precursors are convertible to spherical-shape NCM811 oxide microparticles, through a preliminary design of low heating rates (e.g., 0.1 and 0.8 °C/min) for calcination and lithiation. The outcome oxide cathode particles also demonstrate improved tap density (e.g., 2.4 g mL -1 for NCM811) and good specific capacity (202 mAh g -1 at 0.1 C) in coin cells and reasonably good cycling performance with LiF coating., Competing Interests: The authors declare no competing financial interest., (© 2023 The Authors. Background: Acute annular outer retinopathy (AAOR) is an uncommon disease. To date, there are few documented cases in the literature. Our case report is the first to describe a case of acute annular outer retinopathy in a patient with invasive ductal breast carcinoma., Case Presentation: The patient presented with photopsias and visual loss approximately 3 weeks prior to a diagnosis of invasive ductal breast carcinoma. We have documented the outer annular white ring seen in the acute phase of this disease and correlate it anatomically with Spectral-domain optical coherence tomography (SD-OCT) imaging. We identified RPE atrophy with nodular hyperreflectivity and loss of ellipsoid layer within the white annular ring with corresponding visual field loss. Fundus autofluorescence correlated with structural alterations seen on SD-OCT and showed both presumed active hyperautofluorescent zones with patchy hypoautofluorescent zones of atrophy and a classic annular hyperautofluorescent border. This case provides additional information about the natural history of this rare entity and its prognosis and varied presentation., Conclusions: The authors report a single case of acute annular outer retinopathy in a patient with invasive ductal breast carcinoma with the corresponding SD-OCT, fundus autofluorescence and visual field findings, during the acute phase of the disease. Li[Ni 0.8 Co 0.1 Mn 0.1 ]O 2 (LNCMO811) is the most studied cathode material for next-generation lithium-ion batteries with high energy density. However, available synthesis methods are time-consuming and complex, restricting their mass production. A scalable manufacturing process for producing NCM811 hydroxide precursors is vital for commercialization of the material. In this work, a three-phase slug flow reactor, which has been demonstrated for its ease of scale-up, better synthetic control, and excellent uniform mixing, was developed to control the initial stage of the coprecipitation of NCM811 hydroxide. Furthermore, an equilibrium model was established to predict the yield and composition of the final product. The homogeneous slurry from the slug flow system was obtained and then transferred into a ripening vessel for the necessary ripening process. Finally, the lithium-nickel-cobalt-manganese oxide was obtained through the calcination of the slug flow-derived precursor with lithium hydroxide, having a tap density of 1.3 g cm -3 with a well-layered structure. As-synthesized LNCMO811 shows a high specific capacity of 169.5 mAh g -1 at a current rate of 0.1C and a long cycling stability of 1000 cycling with good capacity retention. The solar thermochemical CO 2 splitting (CDS) is scrutinized via a redox ZnO/Zn cycle. The second law efficiency analysis is carried out by acquiring the required thermodynamic data from HSC Chemistry software. The main focus of this study is to explore the influence of reduction temperature (T red ), molar flow rate of inert sweep gas (n˙ inert ), and energy required for the gas separation on the solar-to-fuel energy conversion efficiency (η solar-to-fuel ) of the ZnO/Zn cycle. All the calculations are conducted at a constant gas-to-gas heat recovery effectiveness (ε gg ) equal to 0.5. n˙ inert required is recorded to be too high (5050 mol/s) at T red equal to 1500 K and moderately low (15 mol/s) for T red equal to 2000 K. The amount of thermal energy required to heat the inert/O 2 gas mixture (from CDS temperature to separator-1 temperature) and inert sweep gas (from separator-1 temperature to reduction temperature) has a significant impact on the total thermal energy requirement of the cycle (Q˙ TC ). The rise in T red from 1500 K to 2000 K shows a considerable decline in Q˙ TC from 77417.5 kW to 1161.8 kW, respectively. Whereas recent clinical studies report metastatic melanoma survival rates high as 30-50%, many tumors remain nonresponsive or become resistant to current therapeutic strategies. Analyses of The Cancer Genome Atlas (TCGA) skin cutaneous melanoma (SKCM) data set suggests that a significant fraction of melanomas potentially harbor gain-of-function mutations in the gene that encodes for the ErbB4 receptor tyrosine kinase. In this work, a drug discovery strategy was developed that is based on the observation that the Q43L mutant of the naturally occurring ErbB4 agonist Neuregulin-2beta (NRG2β) functions as a partial agonist at ErbB4. NRG2β/Q43L stimulates tyrosine phosphorylation, fails to stimulate ErbB4-dependent cell proliferation, and inhibits agonist-induced ErbB4-dependent cell proliferation. Compounds that exhibit these characteristics likely function as ErbB4 partial agonists, and as such hold promise as therapies for ErbB4-dependent melanomas. Consequently, three highly sensitive and reproducible (Z' > 0.5) screening assays were developed and deployed for the identification of small-molecule ErbB4 partial agonists. Six compounds were identified that stimulate ErbB4 phosphorylation, fail to stimulate ErbB4-dependent cell proliferation, and appear to selectively inhibit ErbB4-dependent cell proliferation. The rechargeable lithium-sulfur (Li-S) battery is a promising candidate for the next generation of energy storage technology, owing to the high theoretical capacity, high specific energy density, and low cost of electrode materials. The main drawbacks in the development of long-life Li-S batteries are capacity fading and the sluggish kinetics at the cathode caused by the polysulfides shuttle. These limitations are addressed through the design of novel nanocages containing cobalt phosphide (CoP) nanoparticles embedded in highly porous nitrogen-doped carbon (CoP-N-GC) by thermal annealing of ZIF-67 in a reductive atmosphere followed by a phosphidation step using sodium hypophosphite. The CoP nanoparticles, with large surface area and uniform homogeneous distribution within the N-doped nanocage graphitic carbon, act as electrocatalysts to suppress the shuttle of soluble polysulfides through strong chemical interactions and catalyze the sulfur redox. As a result, the S@CoP-N-GC electrode delivers an extremely high specific capacity of 1410 mA h g -1 at 0.1 C (1 C=1675 mA g -1 ) with an excellent coulombic efficiency of 99.7 %. Moreover, capacity retention from 864 to 678 mA h g -1 is obtained after 460 cycles with a very low decay rate of 0.046 % per cycle at 0.5 C. Neutral aqueous zinc-air batteries (ZABs) are an emerging type of energy devices with substantially elongated lifetime and improved recyclability compared to conventional alkaline ZABs. However, their development is impeded by the lack of robust bifunctional catalyst at the air-electrode for the oxygen evolution reaction (OER) and the oxygen reduction reaction (ORR). Here, we report the controlled synthesis of NiFe 2 O 4 /FeNi 2 S 4 heterostructured nanosheets (HNSs) that are highly efficient in catalyzing OER and ORR, therefore enabling neutral rechargeable ZABs. Associated with the formation of abundant oxide/sulfide interfaces over NiFe 2 O 4 /FeNi 2 S 4 HNSs' surfaces, the catalyst's oxygen binding energy can be effectively tuned to enhance the OER and ORR activities, as revealed by the density functional theory calculations. In 0.2 M phosphate buffer solution, the optimized NiFe 2 O 4 /FeNi 2 S 4 HNSs present an excellent oxygen electrocatalytic activity and stability, with much lower OER and ORR overpotentials than single-component FeNi 2 S 4 or NiFe 2 O 4 and with negligible performance decay in accelerated durability testing. When used as an air-electrode, the NiFe 2 O 4 /FeNi 2 S 4 HNSs can deliver a power density of 44.4 mW cm -2 and a superior cycling stability (only 0.6% decay after 900 cycles at 0.5 mA cm -2 ), making the resultant ZAB the most efficient and robust one with a neutral aqueous electrolyte reported to date. This work highlights the essential function of the heterostructure interface in oxygen electrocatalysis, opening a new avenue to advanced neutral metal-air batteries.

The ever-increasing global energy consumption necessitates the development of efficient energy conversion and storage devices. Nitrogen-doped porous carbons as electrode materials for supercapacitors feature superior electrochemical performances compared to pristine activated carbons. Herein, a facile synthetic strategy including solid-state mixing of benzimidazole as an inexpensive single-source precursor of nitrogen and carbon and zinc chloride as a high temperature solvent/activator followed by pyrolysis of the mixture ( T = 700-1000 °C under Ar) is introduced. The addition of ZnCl 2 prevents early sublimation of benzimidazole and promotes carbonization and pore generation. The sample obtained under the optimal carbonization temperature of 900 °C and ZnCl 2 /benzimidazole weight ratio of 2/1 (ZBIDC-2-900) features a moderate specific surface area of 855 m 2 g -1 , high N-doping level (10 wt%), and a wide micropore size distribution (∼1 nm). ZBIDC-2-900 as a supercapacitor electrode exhibits a large gravimetric capacitance of 332 F g -1 (at 1 A g -1 in 1 M H 2 SO 4 ) thanks to the cooperative advantages of the electrochemical activity of the nitrogen functional groups and the accessible porosity. We report here a novel observation that immobilization of heparinase I on CNBr-activated Sepharose results in heparin degradation properties that are different from heparinase I in the free solution form. Studies over a range of pHs (5-8) and temperatures (5-50°C) as well as under batch and flow conditions show that immobilized heparinase 1 displays altered pH and temperature optima, and a higher propensity for generation of longer chains (hexa- and octa-) with variable sulfation as compared to that in the free form, which is known to yield disaccharides. The immobilized enzyme retained good eliminase activity over at least five cycles of reuse. Heparin is a member of the glycosaminoglycan (GAG) family composed of glucosamine and uronic acid units containing O-sulfo, N-acetyl and N-sulfo groups, which are alternating in the chain and linked by 1→4 manner. It is a naturally occurring anticoagulant that prevents the formation of clots and their growth within blood. Certain low molecular weight heparins (LMWHs) are considered as better therapeutic agents than natural heparin because of the reduced side effects and smaller risk of bleeding. LMWHs can be produced from heparin by chemical or enzymatic depolymerizations. Heparinases catalyze the cleavage of glycosidic linkage between amino sugars and uronic acids in heparin. There are three kinds of heparinases which are frequently used for depolymerization of heparin. Despite wide range of applications of heparinases in health care, their use still has been hampered due to poor stability and high cost. To overcome this problem heparinases are recommended for immobilization to reduce the cost of product and enhance stability. Heparinases have been successfully immobilized using various methods and supports, mostly for deheparinization of blood through extracorporeal devices. Background: The classical theory of Müllerian origin of upper vagina fails to explain complex urogenital malformations like OHVIRA syndrome; the Acien's hypothesis, however, unravels the hidden embryopathogenesis. As per Acien, Wolffian (mesonephric) ducts instead of Müllerian ducts and sinovaginal bulbs, give rise to the vagina. The new hypothesis, however, retains the concept of origin of the ureters (with ureters inducing renal development) by the former and the uterus by Müllerian ducts. Thus, a failure of development of mesonephros/mesonephric duct gives rise to absent ureters and hence absent homolateral kidney; blind ending (obstructed) ipsilateral hemivagina and cessation of support to paramesonephric ducts which leads to unfused uterus (uterus didelphys). Hence, the new hypothesis explains all components of OHVIRA syndrome. On a parallel track, unilateral anomalous development of the mesonephros in males causes atresia of the homolateral ejaculatory duct that results in obstruction of the proximally placed seminal vesicle. Besides, there is absence of the ipsilateral kidney (Zinner syndrome)., Case Report: In this manuscript, we describe four cases of OHVIRA syndrome. Case 1 was a 34-year-old nulligravida, married since fourteen years, who presented with a 5-month history of pelvic inflammatory disease and dyspareunia. Regular menses in the patient and azoospermia in her husband delayed the diagnosis. Case 2 was a 14-year-old girl who presented with dysmenorrhea and lower abdominal pain since a few months. Case 3 was a 27-year-old female who presented with infertility and dysmenorrhea. Case 4 was a 15-year-old female who presented with a one-year history of dysmenorrhea and cyclic pelvic pain. In all cases, one of the uterine horns revealed collection due to a hemivaginal septum and an absent ipsilateral kidney; thus, establishing the diagnosis of OHVIRA syndrome. The case 4 additionally revealed homolateral vaginal agenesis., Conclusions: On the basis of our 4 cases, we support the Acien's hypothesis of Wolffian origin of vagina which explains the development of OHVIRA syndrome. Besides, we emphasize the need to suspect this syndrome in a female with a pelvic mass and absence of homolateral kidney. Finally, we believe that OHVIRA due to its Wolffian origin is a female equivalent of Zinner syndrome in males. Therefore, we propose OSVIRA (Obstructed Seminal Vesicle and Ipsilateral Renal Agenesis) as an acronym for Zinner syndrome analogous to OHVIRA ., Competing Interests: Final supports/conflicts of interest None.

Background and Objectives: The clinical manifestation in sickle cell disease (SCD) patients varies from one individual to another due to factors like the presence of alpha-thalassaemia mutation, foetal haemoglobin, and β-globin gene haplotype. The present study enumerates the clinical profile of sickle cell anaemia patients from Central India., Methods: Seven hundred seventy-six SCD patients from Jabalpur and surrounding districts (Madhya Pradesh) in central India were registered with the sickle cell clinic of NIRTH, Jabalpur. The present study reveals recorded signs and symptoms of genetically confirmed sickle cell anaemia (404) and sickle beta thalassaemia (92) patients., Results: Majority of the patients were from scheduled caste communities (47.9%) and Gond tribal community (13.8%). Splenomegaly was the most common clinical manifestation observed (71.4%). Overall, 63.5% patients had a history of blood transfusion. The most frequent signs and symptoms observed were Pallor, Icterus, Joint pain, Fever, and Fatigue. Majority of the patients revealed onset of disease prior to attaining the age of 3 years (sickle cell anaemia 44.3% and sickle beta thalassaemia 35.9%). Mean haemoglobin levels among SCA individuals were marginally higher than SBT patients. On the other hand, mean foetal haemoglobin levels among SBT individuals showed the reverse trend. Notably, the present study reports the first incidence of priapism recorded in Central India., Conclusions: The study revealed a high prevalence of SCD among scheduled caste, backward caste, and tribal communities. Dissemination of study findings, screening, pre-marriage counselling, and pre-natal diagnosis are fundamental to preventing or lowering of birth of sickle cell anaemia children in the affected populations.

This study was carried out to ascertain the allelic frequency of α(+)-thalassemia (α(+)-thal) in Scheduled caste and scheduled tribe populations of the Damoh district of Madhya Pradesh, India. Random blood samples of Scheduled tribe (267) and Scheduled caste (168), considering the family as a sampling unit, were analyzed for the presence of the -α(3.7) (rightward) (NG&#95;000006.1: g.34164&#95;37967del3804) and -α(4.2) (leftward) (AF221717) deletions. α(+)-Thal was significantly higher in the Scheduled tribals (77.9%) as compared to the scheduled caste population (9.0%). About 58.0% scheduled tribals carried at least one chromosome with the -α(3.7) deletion and 20.0% scheduled tribals carried the -α(4.2) deletion. Frequency for the -α(3.7) allele was 0.487 in the scheduled tribal populations in comparison to 0.021 in scheduled castes. Allelic frequency for -α(4.2) was 0.103 and 0.024, respectively, in the above communities. No Hardy-Weinberg equilibrium for α-thal gene (p < 0.05) was detected in the tribal population, indicating the presence of selection pressures in favor of α-thal mutation and adaptation.

Magnetically responsive oil-in-water emulsions are effectively stabilized by a halloysite nanotube supported superparamagnetic iron oxide nanoparticle system. The attachment of the magnetically functionalized halloysite nanotubes at the oil-water interface imparts magnetic responsiveness to the emulsion and provides a steric barrier to droplet coalescence leading to emulsions that are stabilized for extended periods. Interfacial structure characterization by cryogenic scanning electron microscopy reveals that the nanotubes attach at the oil-water interface in a side on-orientation. The tubular structure of the nanotubes is exploited for the encapsulation and release of surfactant species that are typical of oil spill dispersants such as dioctyl sulfosuccinate sodium salt and polyoxyethylene (20) sorbitan monooleate. The magnetically responsive halloysite nanotubes anchor to the oil-water interface stabilizing the interface and releasing the surfactants resulting in reduction in the oil-water interfacial tension. The synergistic adsorption of the nanotubes and the released surfactants at the oil-water interface results in oil emulsification into very small droplets (less than 20μm). The synergy of the unique nanotubular morphology and interfacial activity of halloysite with the magnetic properties of iron oxide nanoparticles has potential applications in oil spill dispersion, magnetic mobilization and detection using magnetic fields., (Copyright © 2015 Elsevier Inc. Naturally occurring halloysite clay nanotubes are effective in stabilizing oil-in-water emulsions and can serve as interfacially-active vehicles for delivering oil spill treating agents. Halloysite nanotubes adsorb at the oil-water interface and stabilize oil-in-water emulsions that are stable for months. Cryo-scanning electron microscopy (Cryo-SEM) imaging of the oil-in-water emulsions shows that these nanotubes assemble in a side-on orientation at the oil-water interface and form networks on the interface through end-to-end linkages. For application in the treatment of marine oil spills, halloysite nanotubes were successfully loaded with surfactants and utilized as an interfacially-active vehicle for the delivery of surfactant cargo. The adsorption of surfactant molecules at the interface serves to lower the interfacial tension while the adsorption of particles provides a steric barrier to drop coalescence. Pendant drop tensiometry was used to characterize the dynamic reduction in interfacial tension resulting from the release of dioctyl sulfosuccinate sodium salt (DOSS) from halloysite nanotubes. At appropriate surfactant compositions and loadings in halloysite nanotubes, the crude oil-saline water interfacial tension is effectively lowered to levels appropriate for the dispersion of oil. This work indicates a novel concept of integrating particle stabilization of emulsions together with the release of chemical surfactants from the particles for the development of an alternative, cheaper, and environmentally-benign technology for oil spill remediation.

Defect in the ventricular septum with obstruction to right ventricular outflow tract encompass a wide anatomic, physiological and clinical spectrum. Large ventricular septal defects occur with pulmonary stenosis that varies from mild to severe to complete (pulmonary atresia). Very large VSD (size 6.4 cm, in our case) with severe PS is a rare CHD and without surgical correction only 10% patient can survive beyond 20 year of age. With the help of noninvasive investigation (Echocardiography) we can diagnose CHD very easily.

Patients with sickle cell disease (SCD) produce significantly low levels of plasma nitric oxide (NO) during acute vaso-occlusive crisis. In transgenic sickle cell mice, NO synthesized by endothelial nitric oxide synthase (eNOS) enzyme of vascular endothelial cells has been found to protect the mice from vaso-occlusive events. Therefore, the present study aims to explore possible association of eNOS gene polymorphism as a potential genetic modifier in SCD patients. A case control study involving 150 SCD patients and age- and ethnicity-matched 150 healthy controls were genotyped by PCR-restriction fragment length polymorphism techniques for three important eNOS gene polymorphisms-eNOS 4a/b, eNOS 894G>T and eNOS -786T>C. It was observed that SCD patients had significantly higher frequencies of mutant alleles besides heterozygous and homozygous mutant genotypes of these three eNOS gene polymorphisms and low levels of plasma nitrite (NO2) as compared with control groups. The SCD severe group had significantly lower levels of plasma NO2 and higher frequencies of mutant alleles of these three SNPs of eNOS gene in contrast to the SCD mild group of patients. Haplotype analysis revealed that frequencies of one mutant haplotype '4a-T-C' (alleles in order of eNOS 4a/b, eNOS 894G>T and eNOS -786T>C) were significantly high in the severe SCD patients (P<0.0001), whereas the frequency of a wild haplotype '4b-G-T' was found to be significantly high (P<0.0001) in the SCD mild patients, which indicates that eNOS gene polymorphisms are associated with SCD patients in India and may act as a genetic modifier of the phenotypic variation of SCD patients.

Switchgrass was liquefied in supercritical water (SCW) using Ca(HCOO)2 as an in-situ source of hydrogen to enhance deoxygenation and the quality of the biocrude obtained. In SCW, Ca(HCOO)2 produces hydrogen via decomposition and hydrolysis reactions, and simultaneously switchgrass hydrolyzes to form oxygenated hydrocarbon compounds. Because of the close proximity of the newly-formed hydrogen and active hydrocarbons, hydrodeoxygenation occurred whereby some of the oxygenated compounds were upgraded by the removal of oxygen in the form of water. The analysis of the so-formed biocrude revealed the presence of benzene, polyaromatic hydrocarbons, and alkyl phenolics. The benzene formation is attributed to the hydrogenation of phenols but also due to the Diels-Alder alkene addition, followed by dehydrogenation. The addition of Ca(HCOO)2 doubled the yield of biocrude (10 wt%) and increased the heating value from 28 to 34 kJ/g. Cocrystals of itraconazole, an antifungal drug with poor bioavailability, and succinic acid, a water-soluble dicarboxylic acid, were formed by gas antisolvent (GAS) cocrystallization using pressurized CO(2) to improve itraconazole dissolution. In this study, itraconazole and succinic acid were simultaneously dissolved in a liquid solvent, tetrahydrofuran, at ambient conditions. The solution was then pressurized with CO(2), which decreased the solvating power of tetrahydrofuran and caused crystallization of itraconazole-succinic acid cocrystals. The cocrystals prepared by GAS cocrystallization were compared to those produced using a traditional liquid antisolvent, n-heptane, for crystallinity, chemical structure, thermal behavior, size and surface morphology, potential clinical relevance, and stability. Powder X-ray diffraction, Fourier transform infrared spectroscopy, differential scanning calorimetry, and scanning electron microscopy analyses showed that itraconazole-succinic acid cocrystals with physical and chemical properties similar to cocrystals produced using a traditional liquid antisolvent technique can be prepared by CO(2) antisolvent cocrystallization. The dissolution profile of itraconazole was significantly enhanced through GAS cocrystallization with succinic acid, achieving over 90% dissolution in less than 2 h. The cocrystals appeared stable against thermal stress for up to 4 weeks under accelerated stability conditions, showing only moderate decreases in their degree of crystallinity but no change in their crystalline structure. This study shows the utility of an itraconazole-succinic acid cocrystal for improving itraconazole bioavailability while also demonstrating the potential for CO(2) to replace traditional liquid antisolvents in cocrystal preparation, thus making cocrystal production more environmentally benign and scale-up more feasible.

Microalgae are considered as an intriguing candidate for biofuel production due to their high biomass yield. Studies on bio-oil production through fast pyrolysis and upgrading to hydrocarbon fuels using algal biomass are limited as compared to other terrestrial biomass. Therefore, in this study, a fresh water green alga, Chlorella vulgaris, was taken for pyrolysis study. The average activation energy for pyrolysis zone was found to be 109.1 kJ/mol. Fixed-bed pyrolysis of algae gave a bio-oil yield of 52.7 wt.%, which accounts for 60.7 wt.% carbon yield. In addition, analytical pyrolysis of C. vulgaris was carried out in a Py/GC-MS to identify major compounds present in bio-oil with and without catalyst (H(+)ZSM-5). The study found that in catalytic-pyrolysis, as the catalyst loading increased from zero to nine times of the biomass, the carbon yield of aromatic hydrocarbons increased from 0.9 to 25.8 wt.%., (Copyright © 2012 Elsevier Ltd. Unlabelled: Goat milk fat includes several branched chain fatty acids (BCFAs), like 4-methyloctanoic acid, which when free, are responsible for goaty flavor. This flavor limits the market opportunities for goat milk. Prior research showed that cyclodextrins (CDs) can reduce goaty flavor, presumably by binding free fatty acids. This research extends that observation. In odor ranking trials in citrate buffer at pH 4.8, β-CD concentrations between 0% and 0.35% were increasingly effective in reducing odor intensity due to 4-methyloctanoic acid, but only when present in high molar excess. α-CD was also effective, but γ-CD was not. In lipase-treated goat milk only β-CD was effective but at much lower molar excess, a difference potentially explained by several factors. One was that BCFAs bind to CDs in marked preference to their straight chain isomers. Displacement experiments with phenolphthalein disproved that hypothesis. The ability of β-CD to reduce goaty flavor intensity extended to yogurt. An analytical panel showed that flavor of goat yogurt was reduced by addition of β-CD, but only if added before heating and fermentation. A hedonic trial showed that consumers preferred unsweetened and sweet/vanilla-flavored goat yogurt more when β-CD was included, P = 0.004 and 0.016, respectively. Males liked all yogurts more than females (P < 0.01), but there was a treatment × gender interaction (P = 0.016) for sweet/vanilla yogurt: sweet/vanilla masked the goaty flavor for males but not females. The ever-increasing growth of biorefineries is expected to produce huge amounts of lignocellulosic biochar as a byproduct. The hydrothermal carbonization (HTC) process to produce biochar from lignocellulosic biomass is getting more attention due to its inherent advantage of using wet biomass. In the present study, biochar was produced from switchgrass at 300 °C in subcritical water and characterized using X-ray diffraction, fourier transform infra-red spectroscopy, scanning electron micrcoscopy, and thermogravimetric analysis. The physiochemical properties indicated that biochar could serve as an excellent adsorbent to remove uranium from groundwater. A batch adsorption experiment at the natural pH (~3.9) of biochar indicated an H-type isotherm. The adsorption data was fitted using a Langmuir isotherm model and the sorption capacity was estimated to be ca. 2.12 mg of U g(-1) of biochar. The adsorption process was highly dependent on the pH of the system. An increase towards circumneutral pH resulted in the maximum adsorption of ca. 4 mg U g(-1) of biochar. The adsorption mechanism of U(VI) onto biochar was strongly related to its pH-dependent aqueous speciation. The results of the column study indicate that biochar could be used as an effective adsorbent for U(VI), as a reactive barrier medium. Itraconazole (ITZ) microflakes were produced by supercritical antisolvent (SAS) method and simultaneously mixed with pharmaceutical excipients in a single step to prevent drug agglomeration. Simultaneous ITZ particle formation and mixing with fast-flo lactose (FFL) was performed in a high-pressure stirred vessel at 116 bar and 40 °C by the SAS-drug excipient mixing (SAS-DEM) method. The effects of stabilizers, such as sodium dodecyl sulfate and poloxamer 407 (PLX), on particle formation and drug dissolution were studied. Drug-excipient formulations were characterized for surface morphology, crystallinity, drug-excipient interactions, drug content uniformity, and drug dissolution rate. Mixture of drug microflakes and FFL formed by the SAS-DEM process shows that the process was successful in overcoming drug-drug agglomeration. PLX produced crystalline drug flakes in loose agglomerates with superior dissolution and flow properties even at higher drug loadings. Characterization studies confirmed the crystallinity of the drug and absence of chemical interactions during the SAS process. This study is focused on hydrocarbon production through changing carrier gas and using zeolite catalysts during pyrolysis. A large reduction in high molecular weight, oxygenated compounds was noticed when the carrier gas was changed from helium to hydrogen during pyrolysis. A catalytic pyrolysis was conducted using two different methods based on how the biomass and catalysts were contacted together. For both methods, there was no significant change in the carbon yield with the change in pyrolysis environment. However, the mixing-method produced higher aromatic hydrocarbons than the bed-method. In addition, two methods were also tested using two ratios of biomass to catalyst. Nonetheless, there was no significant increase in hydrocarbon yield as the catalyst loading was increased from two to five times of biomass in the catalyst-bed method. A fast pyrolysis process produces a high yield of liquid (a.k.a. bio-oil) and has gained a lot of interest among various stakeholders. Nonetheless, some of the properties inherent by the bio-oil create significant challenges for its wider applications. Quality of the bio-oil and its yield are highly dependent on process parameters, such as temperature, feedstock, moisture content and residence time. In this study, the effect of temperature on bio-oil quality and its yield were examined using pine wood, an abundant biomass source in the southeastern part of the United States. Physical properties of bio-oil such as pH, water content, higher heating value, solid content and ash were analyzed and compared with a recently published ASTM standard. Bio-oil produced from pine wood using an auger reactor met specifications suggested by the ASTM standard. Thirty-two chemical compounds were analyzed. The study found that the concentration of phenol and its derivatives increased with the increase in pyrolysis temperature whereas the concentration of guaiacol and its derivatives decreased as the temperature increased. Concentration of acetic and other acids remained almost constant or increased with the increase in temperature although the pH value of the bio-oil decreased with the increase in temperature., (Copyright 2010 Elsevier Ltd. Microcrystalline cellulose (MCC) was pretreated with subcritical water in a continuous flow reactor for enhancing its enzymatic reactivity with cellulase enzyme. Cellulose/water suspension was mixed with subcritical (i.e., pressurized and heated) water and then fed into the reactor maintained at a constant temperature and pressure. After the reaction, product was immediately cooled in a double-pipe heat exchanger. The solid portion of the product (i.e., treated MCC) was separated and tested for molecular structure and enzymatic reactivity. Experiments were conducted at temperatures ranging from 200 to 315 degrees C, at 27.6 MPa, and for 3.4-6.2 s reaction times. The treated MCC was characterized for degree of polymerization (DP(v)) by viscosimetry, and crystallinity by X-ray diffraction (XRD). In addition, differential scanning calorimetry and scanning electron microscopy (SEM) analyses were carried out to study any transformation in the cellulose structure. As expected, DP(v) of cellulose steadily decreased with increase in the pretreatment temperature, with a rapid drop occurring above 300 degrees C. On the other hand, XRD analysis did not show any decrease in crystallinity upon pretreatment but, partial transformation of celluloses I-II structure was noticed in the MCC treated at 300 degrees C. Development of surface cracks and trenches were observed in the SEM images for all the treated samples. Enzymatic reactivity was increased after the treatment at > or = 300 degrees C.

Microparticles of a poorly water-soluble model drug, nevirapine (NEV) were prepared by supercritical antisolvent (SAS) method and simultaneously deposited on the surface of excipients such as lactose and microcrystalline cellulose in a single step to reduce drug-drug particle aggregation. In the proposed method, termed supercritical antisolvent-drug excipient mixing (SAS-DEM), drug particles were precipitated in supercritical CO(2) vessel containing excipient particles in suspended state. Drug/excipient mixtures were characterized for surface morphology, crystallinity, drug-excipient physico-chemical interactions, and molecular state of drug. In addition, the drug content uniformity and dissolution rate were determined. A highly ordered NEV-excipient mixture was produced. The SAS-DEM treatment was effective in overcoming drug-drug particle aggregation and did not affect the crystallinity or physico-chemical properties of NEV. The produced drug/excipient mixture has a significantly faster dissolution rate as compared to SAS drug microparticles alone or when physically mixed with the excipients., (Copyright 2009 Elsevier B.V. A study on the clinicoepidemiology of cerebral malaria (CM) and mild malaria (MM) among adults and children attending NSCB medical college hospital Jabalpur and civil hospital Maihar, Satna, in central India was undertaken. Of 1,633 patients, 401 were Plasmodium falciparum and 18 P. vivax. Of 401, 199 CM patients and 112 MM patients were enrolled. Severe complications among CM patients were jaundice (26%), acute renal failure (22%), respiratory distress (22%), severe malaria anemia (18%), hypotension (17%), hepatic encephalopathy (7.0%), and hematuria (5%). Among CM cases, seizures and severe malaria anemia were significantly higher in children (P < 0.0001) compared with adults, whereas jaundice (P < 0.0025), acute renal failure (P < 0.0001), and hematuria (P

Dissolution rate of a poorly water-soluble drug, fenofibrate, is increased by adsorbing the drug onto silica. The adsorption is achieved by first dissolving the drug in supercritical carbon dioxide and then depressurizing the solution onto silica. Loadings of up to 27.5 wt.% drug onto silica are obtained. Since solvents are not used in the loading process, the fenofibrate/silica formulation is free of any residual solvent, and carbon dioxide is freely removed upon depressurization. The formulation is characterized using infrared spectroscopy, ultraviolet spectroscopy, X-ray diffraction, differential scanning calorimetry and scanning electron microscopy. Based on in vitro dissolution study, a significant increase in the dissolution rate (approximately 80% drug release in 20 min) of drug-silica formulation is observed as compared to micronized fenofibrate (approximately 20% drug release in 20 min), which can be attributed to increase in the surface area and decrease in the crystallinity of drug after adsorption onto silica. Two different formulations are compared: (A) amorphous fenofibrate/silica and (B) slightly crystalline fenofibrate/silica. The second formulation is found to be more stable on storage.

This study has been carried out to assess the diverse floristic wealth in urban forest area of NEERI campus at Nagpur, Maharashtra (India). This urban forest is ecologically important to maintain the atmospheric temperature around 2 degrees C below and higher relative humidity as compared to other urban areas. The water table is also observed to be shallower in this area as compared to other areas. Therefore, the biological diversity of this urban forest was studied, as it is directly related to ecology of the area. Floristic survey of NEERI premises recorded 135 vascular plants including 16 monocots and 119 dicots, belonging to 115 genera and 53 families. The taxa included 4 types of grasses, 55 herbs, 30 shrubs and 46 trees. The large number of species within very small area (43 ha) indicates rich biodiversity in this forest area. It is also observed that this forest patch has tall trees, with good density and rich cover of shrubs and herbs on forest floor indicating well knit plant community. These characteristics have given immense ecological importance to this urban forest area. Detailed vegetation study revealed that positive co-operation in the plant communities can significantly maintain species diversity in the environment.

Poly(methyl methacrylate) (PMMA) is one of the most commonly plastics used as dental-base material, due to its good biological compatibility and mechanical properties. Chitosan has wide application in chemical, biochemical and biomedical fields of research. In this work, chitosan (CTS) was functionalized with glycidyl methacrylate (GMA), to ease a further reaction with MMA. The resulting co-polymer was finally blended with PMMA and poly(butyl acrylate) PBA which works as a damper, the polymers were cured by UV to obtain the final resin. Characterization of UV-cured resins was carried out by thermal measurements, X-ray diffraction, atomic force microscopy (AFM), micro and nanoindentation, water absorption and elution in water. As a result a higher thermal stability of the final resin compared with the precursor co-polymer ((CTS-GMA)-g-PMMA) was obtained. The resin presented roughness in the nanometer scale and nanoparticles embedded in the acrylic matrix producing a tough material. However, XRD measurements show that all materials are in an amorphous state. Values of hardness and elastic modulus results were very near to those of the dentine. The results of elution in water of the tested resin samples show them as clinically acceptable as a dental base material.

The identity of intermolecular contact residues in sickle hemoglobin (HbS) fiber is largely known. However, our knowledge about combinatorial effects of two or more contact sites or the mechanistic basis of such effects is rather limited. Lys16, His20, and Glu23 of the alpha-chain occur in intra-double strand axial contacts in the sickle hemoglobin (HbS) fiber. Here we have constructed two novel double mutants, HbS (K16Q/E23Q) and (H20Q/E23Q), with a view to delineate cumulative impact of interactions emanating from the above contact sites. Far-UV and visible region CD spectra of the double mutants were similar to the native HbS indicating the presence of native-like secondary and tertiary structure in the mutants. The quaternary structures in both the mutants were also preserved as judged by the derivative UV spectra of liganded (oxy) and unliganded (deoxy) forms of the double mutants. However, the double mutants displayed interesting polymerization behavior. The polymerization behaviour of the double mutants was found to be non-additive of the individual single mutants. While HbS (H20Q/E23Q) showed inhibitory effect similar to that of HbS (E23Q), the intrinsic inhibitory propensity of the associated single mutants was totally quelled in HbS (K16Q/E23Q) double mutant. Molecular dynamics (MD) simulations studies of the isolated alpha-chains as well as a module of the fiber containing the double and associated single mutants suggested that these contact sites at the axial interface of the fiber impact HbS polymerization through a coupled interaction network. The overall results demonstrate a subtle role of dynamics and electrostatics in the polymer formation and provide insights about interaction-linkage in HbS fiber assembly.

Background & Aims: Although inflammation is presumed to contribute to colonic neoplasia in ulcerative colitis (UC), few studies have directly examined this relationship. Our aim was to determine whether severity of microscopic inflammation over time is an independent risk factor for neoplastic progression in UC., Methods: A cohort of patients with UC undergoing regular endoscopic surveillance for dysplasia was studied. Degree of inflammation at each biopsy site had been graded as part of routine clinical care using a highly reproducible histologic activity index. Progression to neoplasia was analyzed in proportional hazards models with inflammation summarized in 3 different ways and each included as a time-changing covariate: (1) mean inflammatory score (IS-mean), (2) binary inflammatory score (IS-bin), and (3) maximum inflammatory score (IS-max). Potential confounders were analyzed in univariate testing and, when significant, in a multivariable model., Results: Of 418 patients who met inclusion criteria, 15 progressed to advanced neoplasia (high-grade dysplasia or colorectal cancer), and 65 progressed to any neoplasia (low-grade dysplasia, high-grade dysplasia, or colorectal cancer). Univariate analysis demonstrated significant relationships between histologic inflammation over time and progression to advanced neoplasia (hazard ration (HR), 3.0; 95% CI: 1.4-6.3 for IS-mean; HR, 3.4; 95% CI: 1.1-10.4 for IS-bin; and HR, 2.2; 95% CI: 1.2-4.2 for IS-max). This association was maintained in multivariable proportional hazards analysis., Conclusions: The severity of microscopic inflammation over time is an independent risk factor for developing advanced colorectal neoplasia among patients with long-standing UC.

Host genetic factors play an important role in susceptibility to HIV-1 infection and progression to AIDS. Mutations in genes encoding chemokine receptors and their ligands, viz., CCR5delta32, CCR2-64I and SDF1-3'A are implicated to have protective effects against HIV-1 infection and/or disease progression. The distribution of these gene polymorphisms and their role in the course of the disease varies between individuals of different racial, ethnic and risk groups. We have examined the allelic frequencies of CCR5delta32, CCR2-64I and SDF1-3'A in 500 healthy North Indians tested seronegative for HIV-1, by PCR-RFLP. The plasma levels of stromal derived factor (SDF-1) protein were estimated in 75 individuals using ELISA kit. Frequencies of CCR5delta32, CCR2-64I and SDF1-3'A alleles in 500 individuals were 1.5%, 9.1% and 20.4%, respectively. The SDF1-3'A homozygosity was confirmed by PCR product cloning and sequencing. The relative hazard values calculated on the basis of the three locus genotype of each individual revealed high relative hazard values (>0.9). The plasma levels of SDF-1 ranged from 1.77 to 3.42 ng/ml and were comparable between the three genotypes of SDF-1. This is the first study to assess the plasma level of SDF-1 protein in Asian Indians. Low frequency of the protective allele CCR5delta32 observed in this study suggests high vulnerability of North Indians to HIV-1 infection. The precise role of SDF1-3'A in HIV-1 infection needs to be elucidated.

Objective: The purpose of this study was to evaluate the influence of rollator use on health-related quality of life in patients with COPD., Design: Randomized controlled trial., Methods: Thirty-one postrehabilitation patients with COPD were randomized to receive a rollator (n = 18) or usual care (n = 13) for 8 weeks and to record the frequency of rollator use. Outcome measures at baseline, 4 weeks, and 8 weeks included the Chronic Respiratory Questionnaire (CRQ) and the 6-min walk (6MW)., Results: During acute testing, subjects consistently walked further when assisted (baseline 6MW: 292 +/- 67 m vs 263 +/- 67 m; 8 weeks: 283 +/- 65 m vs 259 +/- 68 m [+/-SD]; p = 0.013). However, provision of a rollator at home was not associated with group differences in the CRQ (p > 0.08) or in the unassisted 6MW (p = 0.4) or the assisted 6MW (p = 0.5). Eight of 18 subjects assigned to the rollator group used the rollator less than three times per week. Regular users demonstrated a consistent improvement in mastery compared with infrequent users (4 weeks: 4.7 +/- 0.6 vs 5.2 +/- 0.8, respectively; 8 weeks: 5.3 +/- 0.8 vs 4.7 +/- 0.4; p = 0.014)., Conclusions: Despite evidence of effectiveness during acute testing, this study did not demonstrate a rollator effect on quality of life or exercise capacity when the rollator was provided at home, for a longer period. Actual use of a rollator may be an important determinant of its effect. Therefore, when prescribing a rollator, health-care professionals should attempt to identify those most likely to use it.

Nanoparticles are of significant importance in drug delivery. Rapid expansion of supercritical solution (RESS) process can produce pure and high-quality drug particles. However, due to extremely low solubility of polar drugs in supercritical CO(2) (sc CO(2)), RESS has limited commercial applicability. To overcome this major limitation, a modified process rapid expansion of supercritical solution with solid cosolvent (RESS-SC) is proposed which uses a solid cosolvent. Here, the new process is tested for phenytoin drug using menthol solid cosolvent. Phenytoin solubility in pure sc CO(2) is only 3 micromol/mol but when menthol solid cosolvent is used the solubility is enhanced to 1,302 micromol/mol, at 196 bar and 45 degrees C. This 400-fold increase in the solubility can be attributed to the interaction between phenytoin and menthol. Particle agglomeration in expansion zone is another major issue with conventional RESS process. In proposed RESS-SC process solid cosolvent hinders the particle growth resulting in the formation of small nanoparticles. For example, the average particle size of phenytoin in conventional RESS process is 200 nm whereas, with RESS-SC process, the average particle size is 120 nm, at 96 bar and 45 degrees C. Similarly at 196 bar and 45 degrees C, 105 nm average particles were obtained by RESS and 75 nm average particles were obtained in RESS-SC process. The particles obtained were characterized by Fourier-transform infrared spectroscopy (FTIR), X-ray diffraction (XRD), dynamic light scattering (DLS) and differential scanning calorimetery (DSC) analyses. Phenytoin nanoparticle production rate in RESS-SC is about 400-fold more in comparison to that in RESS process.

The synthesis, characterization and degradation of a hybrid chitosan (CTS)/glycidyl methacrylate (GMA) material are reported. These versatile materials (natural-synthetic materials) are potential candidates for dental restoratives. All materials were characterized by infrared spectroscopy (FT-IR), X-ray diffraction and thermal (DSC) analysis. Particular attention was paid to the thermal stability and chemical resistance of the hybrid CTS materials. From dynamical rheological tests, it was concluded that CTS-GMA solutions behave as physical hydrogels. These pH-sensitive gels are an example of stimuli-responsive polymers, also known as 'smart polymers'.
