20. How Can Self-Regulated Learning Be Supported in Mathematical E-Learning Environments?

Abstract

This study compares two E-learning environments: E-learning supported with IMPROVE self-metacognitive questioning (EL+IMP), and E-learning without explicit support of self-regulation (EL). The effects were compared between mathematical problem-solving and self-regulated learning (SRL). Participants were 65 ninth-grade students who studied linear function in Israeli junior high schools. Results showed that EL+IMP students significantly outperformed the EL students in problem-solving procedural and transfer tasks regarding mathematical explanations. We also found that the EL+IMP students outperformed their counterparts in using self-monitoring strategies during problem solving. This study discusses both the practical and theoretical implications of supporting SRL in mathematical E-learning environments.

21. Pre-operative SARS-CoV-2 PCR test at between 48 and 72hours pre-operatively is safe for patients undergoing primary and revision hip and knee arthroplasty – a multicentre international study

Abstract

Introduction: Patients undergoing lower limb arthroplasty who are SARS-CoV-2 positive at the time of surgery have a high-risk of mortality. The National Institute for Health and Clinical Care Excellence and the British Orthopaedic Association advise self-isolation for 14-days pre-operatively in patients at high-risk of adverse outcomes due to COVID-19. The aim of the study is to assess whether pre-operative PCR for SARS-CoV-2 could be performed at between 48 and-72 hours pre-operatively with specific advice about minimising the risk of SARS-CoV-2 restricted to between PCR and admission. Methods: A multi-centre, international, observational cohort study of 1000 lower limb arthroplasty cases was performed. The dual primary outcomes were thirty-day conversion to SARS-CoV-2 positive and thirty-day SARS-CoV-2 mortality. Secondary outcomes included thirty-day SARS-CoV-2 morbidity. Results: Of the 1000 cases, 935 (94%) had a PCR between 48 and 72-hours pre-operatively. All cases were admitted to, and had surgery through a COVID-free pathway. Primary knee arthroplasty was performed in 41% of cases, primary hip arthroplasty in 40%, revision knee arthroplasty in 11% and revision hip arthroplasty in 9%. Six-percent of operations were emergency operations. No cases of SARS-CoV-2 were identified within the first thirty-days. Conclusion: Pre-operative SARS-CoV-2 PCR test between 48 and 72-hours pre-operatively with advice about minimising the risk of SARS-CoV-2 restricted to between PCR and admission in conjunction with a COVID-free pathway is safe for patients undergoing primary and revision hip and knee arthroplasty. Pre-operative SARS-CoV-2 PCR test alone may be safe but further, adequately powered studies are required. This information is important for shared decision making with patients during the current pandemic.

Abstract

In proteinous systems, time-resolved dynamics of proton transfer require that the solvent offer a potential well to handle the discharged proton and protect it from the negative charge of the conjugated base. It must give an efficient way for the proton to move out into the bulk. Physical chemical properties of solvents control dynamics of proton removal from an acidic moiety and movement into the bulk. Clustering of photon-binding sites on a surface can give that surface greater capacity to attract protons and to direct them to a given site. Microcavities are a factor.

49. Surgical considerations in a paediatric case of a large skull-base epithelioid haemangioendothelioma.

Abstract

Intracranial epithelioid haemangioendothelioma (EHE) is a rare intermediate grade vascular tumour with heterogeneous clinical and histopathological behaviour. We present the surgical considerations of an exceptionally large skull-based EHE in an 11-year old female who presented to our institution with headaches and a protuberance over the left parietal area. Magnetic resonance imaging (MRI) demonstrated a left sided 10.5 x 6.6 x 11.1 cm extra-axial tumour arising from the parieto-temporaloccipital region which was continuous with the calvarium. An initial biopsy confirmed EHE. Staged treatment involved preoperative angiography and embolization. The patient underwent an extensive tumour excision and acrylic cranioplasty. Residual tumour persists in the petrous temporal bone. No neurological deficit was sustained. Postoperatively, we proceeded to tumour surveillance rather than adjuvant therapies, and follow-up imaging up to 36 months postoperatively has shown no tumour progression. We illustrate our surgical management of this large EHE and review the literature of this rare pathological entity with variable tumour behaviour and potential role for adjuvant therapy.Copyright © 2018, Springer-Verlag GmbH Germany, part of Springer Nature.
