1. Evidence-based early rehabilitation for children with cerebral palsy: co-development of a multifaceted knowledge translation strategy for rehabilitation professionals

Abstract

BackgroundCerebral palsy (CP) is the most common childhood physical disability. Early and evidence-based rehabilitation is essential for improving functional outcomes in children with CP. However, rehabilitation professionals face barriers to adopting evidence-based practices (EBP)s. The objective of this project is to develop a knowledge translation (KT) strategy to support CP-EBP among pediatric rehabilitation professionals.MethodsWe follow an integrated KT approach by collaborating with clinician- and patient-partners. Partners engaged in co-design through team meetings and content review via email. The KT strategy comprises two components: (1) An electronic (e)-KT toolkit was created from summarized evidence extracted from randomized clinical trials on early rehabilitation for children with CP, and (2) a multifaceted online KT training program developed with guidance from a scoping review exploring effective KT strategies.ResultsThe e-KT toolkit summarizes twenty-two early interventions for children with or at risk for CP aged 0–5 years. Each module features an introduction, resources, parent/family section, and clinician information, including outcomes, intervention effectiveness, and evidence level. The KT training program includes three 10–15 min video-based training modules, text summaries, quizzes, and case studies. Site champions, identified as qualified rehabilitation professionals, were onboarded to support the site implementation of the training program. A champion-training booklet and 1-hour session were designed to equip them with the necessary knowledge/resources.ConclusionThe tailored, multifaceted, and co-designed KT strategy aims to be implemented in pediatric rehabilitation sites to support professional's uptake of CP-EBPs. Lessons learned from its development, including the co-development process and multifaceted nature, hold potential for broader applications in rehabilitation.

Abstract

Natural hydrogen exploration has been restricted in scope due to the predominance in thinking that various rock interactions with water in cratonic settings offer the best natural hydrogen sources. The limited exploration findings in these areas in conjunction with advances in the understanding of hydrogen generation via anthropogenic methods suggest that other source alternatives such as organic hydrogen generation need to be revisited. The ideas on the maturation of organic matter may need to be reassessed with respect to hydrogen. It is suggested that an overlapping thermo-catalytic set of processes occurs to produce hydrocarbons and hydrogen. Initially clay reacts with kerogen producing hydrocarbons, hydrogen and amorphous carbon, the alteration of clays releasing hydronium. During the late catagenic phase thermo-catalysis of hydrocarbons by amorphous carbon create shorter chained hydrocarbons and hydrogen whilst amorphous carbon degrades to carbon black. During expulsion of hydrocarbons and hydrogen the permeability of the source rock becomes more heterogeneous, isolating some reactants creating pyrobitumens and other carbonaceous materials. At higher temperatures during metagenesis isolated pyrobitumens and other carbonaceous materials in shales are turned to graphite releasing gaseous hydrocarbons and eventually diatomic hydrogen in a second phase by thermo-catalytic reaction with carbon black. This matches the temperatures and results at which laboratory experiments and petrochemical processes used to generate hydrogen are observed. The current conclusion of the hydrocarbon generation story at the methane preservation limit should be recognized as the start of hydrogen generation and graphitization as the end of the process. Instead of basin exploration focused solely on hydrocarbons and stopping due to concerns of over maturity of source and reservoir, exploration may continue deeper in search of organic hydrogen. This should be noted as a primary hydrogen generation mechanism globally and provide a suitable model to aid hydrogen exploration and lead the energy transition into a hydrogen economy.

Abstract

Summary: The memory B cell response consists of phenotypically distinct subsets that differ in their ability to respond upon antigen re-encounter. However, the pathways regulating the development and function of memory B cell subsets are poorly understood. Here, we show that CD62L and CD44 are progressively expressed on mouse memory B cells and identify transcriptionally and functionally distinct memory B cell subsets. Bcl6 is important in regulating memory B cell subset differentiation with overexpression of Bcl6 resulting in impaired CD62L+ memory B cell development. Bcl6 regulates memory B cell subset development through control of a network of genes, including Bcl2 and Zeb2. Overexpression of Zeb2 impairs the development of CD62L+ memory B cells. Importantly, CD62L is also differentially expressed on human memory B cells following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination and identifies phenotypically distinct populations. Together, these data indicate that CD62L expression marks functionally distinct memory B cell subsets.

Abstract

BackgroundAlthough SARS-CoV-2 vaccines have proven effective in eliciting a protective immune response in healthy individuals, their ability to induce a durable immune response in immunocompromised individuals remains poorly understood. Primary antibody deficiency (PAD) syndromes are among the most common primary immunodeficiency disorders in adults and are characterized by hypogammaglobulinemia and impaired ability to mount robust antibody responses following infection or vaccination.MethodsHere, we present an analysis of both the B and T cell response in a prospective cohort of 30 individuals with PAD up to 150 days following initial COVID-19 vaccination and 150 days post mRNA booster vaccination.ResultsAfter the primary vaccination series, many of the individuals with PAD syndromes mounted SARS-CoV-2 specific memory B and CD4+ T cell responses that overall were comparable to healthy individuals. Nonetheless, individuals with PAD syndromes had reduced IgG1+ and CD11c+ memory B cell responses following the primary vaccination series, with the defect in IgG1 class-switching rescued following mRNA booster doses. Boosting also elicited an increase in the SARS-CoV-2-specific B and T cell response and the development of Omicron-specific memory B cells in COVID-19-naïve PAD patients. Individuals that lacked detectable B cell responses following primary vaccination did not benefit from booster vaccination.ConclusionTogether, these data indicate that SARS-CoV-2 vaccines elicit memory B and T cells in most PAD patients and highlights the importance of booster vaccination in immunodeficient individuals.

Germline; Tumour-only sequencing Línia germinal; Seqüenciació només de tumors Línea germinal; Secuenciación solo de tumors Background The European Society for Medical Oncology Precision Medicine Working Group (ESMO PMWG) was reconvened to update its 2018/19 recommendations on follow-up of putative germline variants detected on tumour-only sequencing, which were based on an analysis of 17 152 cancers. Methods We analysed an expanded dataset including 49 264 paired tumour-normal samples. We applied filters to tumour-detected variants based on variant allele frequency, predicted pathogenicity and population variant frequency. For 58 cancer-susceptibility genes, we then examined the proportion of filtered tumour-detected variants of true germline origin [germline conversion rate (GCR)]. We conducted subanalyses based on the age of cancer diagnosis, specific tumour types and ‘on-tumour’ status (established tumour-gene association). Results Analysis of 45 472 nonhypermutated solid malignancy tumour samples yielded 21 351 filtered tumour-detected variants of which 3515 were of true germline origin. 3.1% of true germline pathogenic variants were absent from the filtered tumour-detected variants. 6. End Times and the Modern World: The Ahmadiyya in Colonial Ghana

Abstract

The Ahmadiyya, a messianic Muslim missionary movement that expanded globally from South Asia, provided religious, social, and educational services and offered a compelling End Times message in colonial Ghana. An Ahmadi missionary arrived at the invitation of African Muslims, who learned about the movement from the Ahmadiyya’s English-language publications. Africans negotiated the terms of the mission’s founding and supported the residence of a South Asian missionary. Other West African Muslim movements navigated the colonial era with reformed religious practices and organizational changes, and the Ahmadiyya was distinctive with its English-language schools and an eschatology based on its founder’s claims to receive divine revelation as the Messiah and Mahdi. Ghanaian Ahmadi Muslims were a small minority within an overall Muslim minority in Ghana. Their initiatives created a dynamic regional center in an expanding Ahmadiyya network.

The Aspen Economic Strategy Group's Annual Policy Volume Economic Policy in a More Uncertain World marks the group's 5th anniversary and is released against a backdrop historic economic and strategic uncertainty. The book's seven chapters, each written by leading experts and edited by AESG Director Melissa S. Kearney and Deputy Director Amy Ganz, provide a deep-dive on long-term economic headwinds confronting the country, including demographic changes—declining fertility and population aging—and what a smaller worker to population ratio means in terms of slower economic growth, reduced revenue, and lower productivity growth. Additional chapters on the US immigration system and US innovation policy highlight potential solutions for countering these trends. Another chapter explores potential adverse impacts on local labor markets from the green energy transition and highlights policies to avoid repeating painful mistakes of the past, including the response to the decline of the coal industry and rise of globalization and automation. A final chapter highlights lessons learned from the unprecedented federal aid to state and local governments during the COVID-19 pandemic.

29. What factors best explain attitudes to snow leopards in the Nepal Himalayas?

Abstract

The snow leopard Panthera uncia is a vulnerable wild felid native to mountainous regions of 12 Asian countries. It faces numerous overlapping threats, including killings by herders retaliating against livestock losses, the illegal wildlife trade, loss of prey and habitat, infrastructure, energy and mining developments, and climate change. The species ranges over large territories that often lie outside of protected areas (PA), so coexistence with human populations across its range is key to its persistence. Human attitudes to snow leopards may be an important factor to consider in reducing overlapping threats to this species. However, this nexus has not been widely studied to date. Attitudes to snow leopard conservation, including actors and interventions, may also be a significant aspect of coexistence. These have also received limited empirical attention. This study therefore explored human attitudes to snow leopards and to snow leopard conservation, the motivations for these attitudes and the individual factors that best explained them. Using systematic sampling, a quantitative questionnaire was administered to 705 households at two sites in the Nepal Himalayas: Sagarmatha National Park, with a less decentralised governance model, and Annapurna Conservation Area, with a more decentralised model. Linear regression models were the main form of analysis. Based on these, attitudes to snow leopard conservation emerged as the strongest influence on local attitudes to snow leopards, and vice versa. This was true in both PAs, despite their differing management regimes. Other important explanatory factors included numbers of livestock owned, years of education, household livelihoods and age. Furthermore, a positive intrinsic motivation was the most common reason given by respondents to explain their attitudes to both snow leopards and snow leopard conservation. These findings demonstrate that, in addition to the usual suite of factors that influence attitudes to a species, the way in which its conservation is pursued and perceived also needs consideration. How the snow leopard is conserved may strongly influence its coexistence with local communities.

31. Multiple sediment incorporation events in a continental magmatic arc: Insight from the metasedimentary rocks of the northern North Cascades, Washington (USA)

Abstract

The rheology and composition of arc crust and the overall evolution of continental magmatic arcs can be affected by sediment incorporation events. The exhumed Cretaceous-Eocene North Cascades arc exposes abundant meta-sedimentary rocks that were incorporated into the arc during multiple events. This study uses field relationships, detrital zircon geochronology, bulk rock geo-chemistry, geothermometry, and quartz-in-garnet geobarometry to distinguish approximate contacts and emplacement depths for different metasedimentary units to better understand their protolith incorporation history and impact on the arc. The Skagit Gneiss Complex is one of the main deep crustal units of the North Cascades arc. It includes metasedimentary rocks with distinct detrital zircon signatures: Proterozoic-Cretaceous (Group 1) or Triassic-Cretaceous (Group 2) zircon populations. Both metasedimentary groups achieved near-peak metamorphic conditions of 640-800 degrees C and 5.5-7.9 kbar; several Group 2 samples reveal the higher pressures. A third group of meta sedimentary rocks, which was previously interpreted as metamorphosed equivalents of backarc sediments (Group 3), exhibited unimodal Triassic or bimodal Late Jurassic- Early Cretaceous detrital zircon signatures and achieved near-peak conditions of 570-700 degrees C and 8.7-10.5 kbar. The combined field and analytical data indi-cate that protoliths of Group 1 and Group 2 metasedimentary rocks were successively deposited in a forearc basin and underthrusted into the arc as a relatively coherent body. Group 3 backarc sediments were incorporated into the arc along a transpressional step-over zone. The incorporation of both forearc and backarc sediments was likely facilitated by arc magmatism that weakened arc crust in combination with regional transpression. Abstract

Background: breast cancer has a significant heritable basis, of which ∼60% remains unexplained. Testing for BRCA1/BRCA2 offers useful discrimination of breast cancer risk within families, and identification of additional breast cancer susceptibility genes could offer clinical utility. Patients and methods: we included 2135 invasive breast cancer cases recruited via the Breast and Ovarian Cancer Susceptibility study, a retrospective UK study of familial breast cancer. Eligibility criteria: female, BRCA-negative, white European ethnicity, and one of: (i) breast cancer family history, (ii) bilateral disease, (iii) young age of onset (Results: 159/2135 (7.4%) cases had a qualifying variant in an established breast cancer susceptibility gene, with minimal evidence of signal in other cancer susceptibility genes. Known breast cancer susceptibility genes PALB2, CHEK2, and ATM were the only genes to retain statistical significance after correcting for multiple testing. Due to the enrichment of hereditary cases in the series, we had good power (>80%) to detect a gene of BRCA1-like risk [odds ratio (OR) = 10.6] down to a population minor allele frequency of 4.6 × 10 −5 (1 in 10 799, less than one-tenth that of BRCA1)and of PALB2-like risk (OR = 5.0) down to a population minor allele frequency of 2.8 × 10 −4 (1 in 1779, less than half that of PALB2). Power was lower for identification of novel moderate penetrance genes (OR = 2-3) like CHEK2 and ATM. Conclusions: this is the largest case-control whole-exome analysis of enriched breast cancer published to date. Whilst additional breast cancer susceptibility genes likely exist, those of high penetrance are likely to be of very low mutational frequency. Contention exists regarding the clinical utility of such genes.

Abstract

Sertoli-Leydig cell tumours of the ovary (SLCT) are rare tumours predominantly caused by mutations in the DICER1 gene. We present a patient with a unilateral SLCT who had an underlying germline DICER1 gene mutation. We discuss the underlying pathology, risks, and screening opportunities available to those with a mutation in this gene as SLCT is only one of a multitude of other tumours encompassing DICER1 syndrome. The condition is inherited in an autosomal dominant fashion. As such, genetic counselling is a key component of the management of women with SLCT.

Abstract

During the 2008 to 2012 period, the US Border Patrol enacted new sanctions on migrants apprehended while attempting to enter the United States illegally. Using administrative records on apprehensions of Mexican nationals that include fingerprint-based IDs and other details, we detect if an apprehended migrant is subject to penalties and if he is later reapprehended. Exploiting plausibly random variation in the rollout of sanctions, we estimate econometrically that exposure to penalties reduced the 18-month reapprehension rate for males by 4.6 to 6.1 percentage points off of a baseline rate of 24.2 percent. These magnitudes imply that sanctions can account for 28 to 44 percent of the observed decline in recidivism in apprehensions. Further results suggest that the drop in recidivism was associated with a reduction in attempted illegal entry. (JEL K37, J15, J18)

Abstract

Rodent colony health surveillance has traditionally been accomplished by testing sentinel animals that have been exposed to soiled bedding from colony animals. Collecting samples from exhaust plenums on ventilated caging systems, followed by PCR analysis, has emerged as another promising method for health surveillance. However, environmental testing at the rack level is not effective for all ventilated rack designs. In this study, we tested whether media placed in soiled bedding is effective in detecting 3 adventitious agents: mouse norovirus (MNV), Helicobacter spp., and fur mites. Soiled bedding was collected from pathogen-positive colony mice and distributed to traditional sentinel mouse cages and mouse-free experimental cages every 1 to 2 wk for static and ventilated cages, respectively. Experimental cages contained 10 flocked swabs ('passive swabs') and 1 piece of filter media. After 90 d, fresh feces, pelage swabs, and blood were collected from the sentinel cages, and the passive swabs and filter media were collected from the experimental cages. Concurrently, 10 additional flocked swabs ('active swabs') were stirred through the cumulated soiled bedding of each experimental cage. Sentinel mice were positive for MNV and Helicobacter spp., but negative for fur mites by pelage swab PCR. All samples from experimental cages were positive for Helicobacter spp. and fur mites in both caging types. For MNV, passive swabs were most effective at detection (100%), followed by active swabs (80% to 100%) and filter media (60% to 80%). These findings suggest that testing media in pooled soiled bedding samples is more effective than traditional sentinel methods for colony health surveillance and is a viable option when sampling at the rack level is ineffective.

39. Who Will Fill China’s Shoes? The Global Evolution of Labor-Intensive Manufacturing

Abstract

In this paper, I review evidence on changing global specialization in labor-intensive exporting. Production of apparel, footwear, furniture, and related products are how many low-income countries first enter export manufacturing. Just as China’s rise as a powerhouse in these goods supplanted a role previously occupied by the East Asian Tigers, the world may again be on the cusp of significant change in where labor-intensive goods are produced. China’s prowess in these sectors peaked in the early 2010s; its share in their global exports, while still substantial, is now in decline. Mechanisms through which the global economy may adjust to China’s graduation into more technologically sophisticated activities include expanded labor-intensive export production in other emerging economies and labor-saving technological change in products currently heavily reliant on less-educated labor. Available evidence suggests that the first mechanism is operating slowly and the second hardly at all. As a third mechanism, China may in part replace itself by moving labor-heavy factories out of densely populated and expensive coastal cities and into the country’s interior. Such a transition, though still in its infancy, would mirror the decentralization of manufacturing production in the U.S. and Europe, which occurred after World War II.

Abstract

Has rising import competition contributed to the polarization of US politics? Analyzing multiple measures of political expression and results of congressional and presidential elections spanning the period 2000 through 2016, we find strong though not definitive evidence of an ideological realignment in trade-exposed local labor markets that commences prior to the divisive 2016 US presidential election. Exploiting the exogenous component of rising import competition by China, we find that trade exposed electoral districts simultaneously exhibit growing ideological polarization in some domains, meaning expanding support for both strong-left and strong-right views, and pure rightward shifts in others. Specifically, trade-impacted commuting zones or districts saw an increasing market share for the Fox News channel (a rightward shift), stronger ideological polarization in campaign contributions (a polarized shift), and a relative rise in the likelihood of electing a Republican to Congress (a rightward shift). Trade-exposed counties with an initial majority White population became more likely to elect a GOP conservative, while trade-exposed counties with an initial majority-minority population became more likely to elect a liberal Democrat, where in both sets of counties, these gains came at the expense of moderate Democrats (a polarized shift). In presidential elections, counties with greater trade exposure shifted toward the Republican candidate (a rightward shift). These results broadly support an emerging political economy literature that connects adverse economic shocks to sharp ideological realignments that cleave along racial and ethnic lines and induce discrete shifts in political preferences and economic policy. Abstract

The competitive shock to the U.S. manufacturing sector spurred by rising China import competition could either catalyze or stifle innovation. Using three distinct sources of variation to identify rising trade exposure, we provide a causal analysis of the effect of surging import competition on U.S. innovative activities. Applying a novel internet-based matching algorithm to map all U.S. utility patents granted by 2013 to firm-level data, and carefully accounting for the shifting concentration of patenting activity across sectors, we document a robust, negative impact of rising Chinese competition on firm-level and technology class-level patent production. Accompanying this fall in innovation, global employment, sales, profitability, and R&D expenditure all decline within trade-exposed firms. The trade-induced contraction along all margins of adjustment and for all measures of valuation suggest that the primary response of firms to greater import competition is to scale back their global operations.

Abstract

This paper reports on a search for an extension to the scalar sector of the standard model, where a new CP-even (odd) boson decays to a Z boson and a lighter CP-odd (even) boson, and the latter further decays to a b quark pair. The Z boson is reconstructed via its decays to electron or muon pairs. The analysed data were recorded in proton-proton collisions at a center-of-mass energy $$ \sqrt{s} $$ s = 13 TeV, collected by the CMS experiment at the LHC during 2016, corresponding to an integrated luminosity of 35.9 fb−1. Data and predictions from the standard model are in agreement within the uncertainties. Upper limits at 95% confidence level are set on the production cross section times branching fraction, with masses of the new bosons up to 1000 GeV. The results are interpreted in the context of the two-Higgs-doublet model.

Abstract

The trade war has received an enormous amount of attention. Its impacts today have been limited because the U.S. and China have been a bit restrained. Lots of firms have gotten exceptions to these tariffs and there have been lots of delays in their full implementation. It’s also been the case that U.S. firms have been delaying making adjustments to their global value chains. What has been striking is that the impact of tariffs on U.S. prices has been one-for-one. These tariffs are unlikely to have a significant impact on manufacturing employment in the United States: the job loss that we observed as a consequence of import competition from China came primarily from factory closure, furthermore U.S. tariffs didn’t target all U.S. imports. They targeted imports from China. Looking forward, to the extent the trade war is dampening GDP growth in China, itcould happen political consequences at home. As for us, this is a defining moment in which we’re going to see whether the U.S. is willing to maintain its commitments to more open borders.

Abstract

In this paper, we study how occupation (or industry) tradability shapes local labor‐market adjustment to immigration. Theoretically, we derive a simple condition under which the arrival of foreign‐born labor into a region crowds native‐born workers out of (or into) immigrant‐intensive jobs, thus lowering (or raising) relative wages in these occupations, and we explain why this process differs within tradable versus within nontradable activities. Using data for U.S. commuting zones over the period 1980–2012, we find—consistent with our theory—that a local influx of immigrants crowds out employment of native‐born workers in more relative to less immigrant‐intensive nontradable jobs, but has no such effect across tradable occupations. Further analysis of occupation labor payments is consistent with adjustment to immigration within tradables occurring more through changes in output (versus changes in prices) when compared to adjustment within nontradables, thereby confirming our model's theoretical mechanism. We then use the model to explore the quantitative consequences of counterfactual changes in U.S. immigration on real wages at the occupation and region level.
