Idiopathic pulmonary fibrosis (IPF) is a severe and often fatal disease. Diagnosis of IPF requires considerable expertise and experience. Since publication of the international IPF guideline in the year 2011 and Update 2018 several studies and technical advances occurred, which made a new assessment of the diagnostic process mandatory. In view of the antifibrotic drugs which have been approved for the treatment of IPF patients, the goal of this guideline is to foster early, confident and effective diagnosis of IPF. The guideline focusses on the typical clinical setting of an IPF patient and provides tools to exclude known causes of interstitial lung disease including standardised questionnaires, serologic testing and cellular analysis of bronchoalveolar lavage. High resolution computed tomography remains crucial in the diagnostic work-up. If it is necessary to obtain specimen for histology transbronchial lung cryobiopsy is the primary approach, while surgical lung biopsy is reserved for patients who are fit for it and in whom bronchoscopic diagnosis did not provide the information needed. Despite considerable progress, IPF remains a diagnosis of exclusion and multidisciplinary discussion remains the golden standard of diagnosis.Die idiopathische Lungenfibrose (IPF) ist eine schwerwiegende, chronische Lungenerkrankung, deren Diagnosestellung eine profunde fachliche Expertise verlangt. Seit Veröffentlichung der internationalen IPF-Leitlinie im Jahr 2011 mit Update 2018 haben zahlreiche Studien und technische Fortschritte eine Neubewertung des diagnostischen Vorgehens notwendig gemacht. Angesichts der Indikation, symptomatischen Patienten eine antifibrotische Therapie sofort nach Diagnosestellung zu empfehlen, hat die aktuelle IPF-Diagnostik-Leitlinie zum Ziel, die Diagnose der IPF möglichst ohne Verzögerung, wenig invasiv und zuverlässig zu ermöglichen. Sie beschreibt den typischen klinischen Kontext der IPF sowie die Untersuchungen, die zum Ausschluss bekannter Ursachen einer Lungenfibrose erforderlich sind, einschließlich standardisierter Anamneseerhebung, serologischer Testungen und zellulärer Analyse der bronchoalveolären Lavage. Zentrale Bedeutung kommt dem hochauflösenden Computertomogramm in Dünnschichttechnik zu. Sofern eine Histologie zur Diagnoseeingrenzung erforderlich ist, soll diese in erster Linie mittels der transbronchialen Lungen-Kryobiopsie gewonnen werden. Die chirurgische Lungenbiopsie soll Patienten vorbehalten bleiben, die in ausreichender Verfassung für diesen Eingriff sind und bei denen die endoskopische Diagnostik keine Klärung der Diagnose ergab. Goldstandard ist auch weiterhin die interdisziplinäre Diskussion der erhobenen Befunde, um nach Ausschluss anderer Formen chronisch progredienter Lungenfibrosen die Diagnose der IPF zu stellen.

Multiple-choice questions (MCQs) provide useful information about correct and incorrect answers, but they do not offer information about students' confidence.Ninety and another 81 medical students participated each in a curricular neurology multiple-choice exam and indicated their confidence for every single MCQ. Each MCQ had a defined level of potential clinical impact on patient safety (uncritical, risky, harmful). Our first objective was to detect informed (IF), guessed (GU), misinformed (MI), and uninformed (UI) answers. Further, we evaluated whether there were significant differences for confidence at correct and incorrect answers. Then, we explored if clinical impact had a significant influence on students' confidence.There were 1818 IF, 635 GU, 71 MI, and 176 UI answers in exam I and 1453 IF, 613 GU, 92 MI, and 191 UI answers in exam II. Students' confidence was significantly higher for correct than for incorrect answers at both exams (p 0.001). For exam I, students' confidence was significantly higher for incorrect harmful than for incorrect risky classified MCQs (p = 0.01). At exam II, students' confidence was significantly higher for incorrect harmful than for incorrect benign (p 0.01) and significantly higher for correct benign than for correct harmful categorized MCQs (p = 0.01).We were pleased to see that there were more informed than guessed, more uninformed than misinformed answers and higher students' confidence for correct than for incorrect answers. Our expectation that students state higher confidence in correct and harmful and lower confidence in incorrect and harmful MCQs could not be confirmed.

ZusammenfassungDie idiopathische Lungenfibrose (idiopathische pulmonale Fibrose, IPF) ist eine schwerwiegende Lungenerkrankung, die häufig innerhalb von zwei bis vier Jahren nach Diagnosestellung zum Tod führt. Seit Veröffentlichung der deutschen IPF-Leitlinie im Jahr 2013 liegen neue Therapiestudien vor, die eine Neubewertung der Behandlungsstrategien erfordern. Abweichend von der Vorgängerleitlinie wurde in der aktuellen Überarbeitung nicht mehr das GRADE-System sondern die Oxford Evidenzsystematik mit drei Empfehlungsgraden (A, B, C) verwendet, weil dieses System eine differenziertere Betrachtung erlaubt. Folgende Medikamente wurden mit dem Empfehlungsgrad A und dem Evidenzgrad 1-b als nicht geeignet für die Behandlung der IPF klassifiziert: Triple-Therapie aus Prednisolon, Azathioprin und Acetylcystein; Antikoagulation mit Vitamin-K-Antagonisten; Imatinib; Ambrisentan; Bosentan; Macitentan. Weniger eindeutig ist die negative Bewertung des Phosphodiesterase-5-Inhibitors Sildenafil und der Acetylcystein-Monotherapie (Empfehlungsgrad B, Evidenzgrad 2-b). Eindeutig positiv fiel die Empfehlung für Nintedanib und Pirfenidon zur Behandlung von IPF-Patienten aus (Empfehlungsgrad A, Evidenzgrad 1-a). Mit Empfehlungsgrad C und Evidenzgrad 4 wurde der generelle Einsatz von Antazida zur Behandlung der IPF als nicht zu empfehlen bewertet, da die Datenlage widersprüchlich ist; hier weicht die deutsche Leitlinie auch am deutlichsten von der internationalen Leitlinie ab. Am Ende der Leitlinie wird aus Expertensicht zu offenen Fragen in der Therapie der IPF Stellung genommen, für die bisher keine ausreichende Evidenzbasis existiert.

The present 2019 S2k consensus guideline of the German Respiratory Society was written - in contrast to the predecessor more general S3 guidelines from 2004 and 2010 - for pneumologists, since 2014 the German College of General Practitioners and Family Physicians (DEGAM) published his own cough guidelines.The guideline contains 48 recommendations agreed by consensus and 16 statements, which are explained in the background text in the following nine chapters: epidemiology, physiology, classification, acute, subacute or chronic cough, diagnostics and therapy; an extra chapter was dedicated to chronic idiopathic cough. Further emphasis of the guideline is the physiology of cough in anticipation of the introduction of new drugs, as well as detailed treatises on cough triggered by affections in the upper respiratory tract or by gastroesophageal reflux. The guideline should provide the pneumologist with the latest knowledge from neighboring disciplines required for diagnosis and therapy of cough. The clinical chapters also contain a short summary, practical recommendations and a bibliography of their own. Three new, simplified algorithms for acute, subacute and chronic cough round off the Diagnostics chapter.Die vorliegende 2019 S2k-Leitlinie der DGP wurde im Gegensatz zu den S3-Vorgänger-Leitlinien aus dem Jahre 2004 und 2010 für Pneumologen verfasst, für Allgemeinärzte liegt eine Leitlinie der Deutschen Gesellschaft für Allgemeinmedizin aus dem Jahre 2014 vor. Die Leitlinie enthält 48 im Konsensusverfahren abgestimmte Empfehlungen und 16 Statements, die im Hintergrundtext in den folgenden neun Kapiteln erläutert werden: Epidemiologie, Physiologie, Klassifizierung, akuter, subakuter bzw. chronischer Husten, Diagnostik und Therapie; dem chronischen idiopathischen Husten wurde ein Extra-Kapitel gewidmet. Weitere Schwerpunkte der Leitlinie sind die Physiologie des Hustens in Erwartung der Einführung neuer Medikamente sowie ausführliche Abhandlungen des Hustens, der durch Affektionen im Bereich der oberen Atemwege oder durch gastroösophagealen Reflux getriggert wird. Hier sollte die Leitlinie dem Pneumologen die zur Diagnostik und Therapie erforderlichen neuesten Kenntnisse aus den Nachbardisziplinen vermitteln. Die klinischen Kapiteln enthalten auch eine kurze Zusammenfassung, praktische Empfehlungen und jeweils ein eigenes Literaturverzeichnis. Drei neue, vereinfachte Algorithmen für den akuten, subakuten und chronischen Husten runden das Kapitel Diagnostik ab.

Medication-based strategies to treat rheumatoid arthritis are crucial in terms of outcome. They aim at preventing joint destruction, loss of function and disability by early and consistent inhibition of inflammatory processes.Achieving consensus about evidence-based recommendations for the treatment of rheumatoid arthritis with disease-modifying anti-rheumatic drugs in Germany.Following a systematic literature research, a structured process among expert rheumatologists was used to reach consensus.The results of the consensus process can be summed up in 6 overarching principles and 10 recommendations. There are several new issues compared to the version of 2012, such as differentiated adjustments to the therapeutic regime according to time point and extent of treatment response, the therapeutic goal of achieving remission as assessed by means of the simplified disease activity index (SDAI) as well as the potential use of targeted synthetic DMARDs (JAK inhibitors) and suggestions for a deescalating in case of achieving a sustained remission. Methotrexate still plays the central role at the beginning of the treatment and as a combination partner in the further treatment course. When treatment response to methotrexate is inadequate, either switching to or combining with another conventional synthetic DMARD is an option in the absence of unfavourable prognostic factors. Otherwise biologic or targeted synthetic DMARDs are recommended according to the algorithm. Rules for deescalating treatment with glucocorticoids and-where applicable-DMARDs give support for the management of patients who have reached a sustained remission.The new guidelines set up recommendations for RA treatment in accordance with the treat-to-target principle. Modern disease-modifying drugs, now including also JAK inhibitors, are available in an algorithm.

Cystic Fibrosis (CF) is the most common autosomal-recessive genetic disease affecting approximately 8000 people in Germany. The disease is caused by mutations in the Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) gene leading to dysfunction of CFTR, a transmembrane chloride channel. This defect causes insufficient hydration of the epithelial lining fluid which leads to chronic inflammation of the airways. Recurrent infections of the airways as well as pulmonary exacerbations aggravate chronic inflammation, lead to pulmonary fibrosis and tissue destruction up to global respiratory insufficiency, which is responsible for the mortality in over 90 % of patients. The main aim of pulmonary treatment in CF is to reduce pulmonary inflammation and chronic infection.Mukoviszidose (Cystic Fibrosis, CF) ist die häufigste, autosomal-rezessiv vererbte Multisystemerkrankung. In Deutschland sind ca. 8000 Menschen betroffen. Die Erkrankung wird durch Mutationen im Cystic Fibrosis Transmembrane Conductance Regulator (CFTR-) Gen verursacht; diese führen zu einer Fehlfunktion des Chloridkanals CFTR. Dadurch kommt es in den Atemwegen zu einer unzureichenden Hydrierung des epithelialen Flüssigkeitsfilms und somit zu einer chronischen Inflammation. Rezidivierende Infektionen der Atemwege sowie pulmonale Exazerbationen der Lunge führen im Verlauf zu zunehmender Inflammation, pulmonaler Fibrose und fortschreitender Lungendestruktion bis hin zur respiratorischen Globalinsuffizienz, die für über 90 % der Mortalität verantwortlich ist. Hintergrund Medikamentöse Therapiestrategien zur Behandlung der rheumatoiden Arthritis sind entscheidend für den Langzeitverlauf. Sie dienen dem Ziel, durch frühe und konsequente Unterdrückung der Entzündung Gelenkzerstörung zu verhindern und damit die Funktion zu erhalten. Ziel der Arbeit Erarbeitung eines Konsenses für evidenzbasierte Empfehlungen zur Behandlung der rheumatoiden Arthritis mit krankheitsmodifizierenden Medikamenten in Deutschland. Methoden Nach einer systematischen Literatursuche wurde ein strukturierter Konsensprozess durchgeführt. Ergebnisse Sechs übergeordnete Prinzipien und 10 Empfehlungen fassen die Ergebnisse des Konsensprozesses zusammen. Verschiedene Punkte sind gegenüber der Fassung von 2012 neu, so die differenzierte Anpassung des Therapieregimes nach Zeitpunkt und Ausmaß des Ansprechens, das Therapieziel Remission gemessen mithilfe des simplified disease activity index (SDAI) wie auch der potentielle Einsatz zielgerichteter synthetischer DMARDs (tsDMARDs), der JAK-Inhibitoren sowie Empfehlungen zur Deeskalation nach dem Erreichen einer anhaltenden Remission. Wie bisher steht Methotrexat (MTX) im Mittelpunkt der Therapie zu Beginn und als Kombinationspartner im weiteren Verlauf. Die Kombination mehrerer konventioneller synthetischer DMARDs, oder, bei ungünstigen prognostischen Faktoren, der Einsatz von biologischen oder tsDMARDs kommen nach dem Algorithmus bei nicht ausreichendem Ansprechen auf Methotrexat zur Anwendung. Empfehlungen für die Deeskalation der Therapie mit Glukokortikoiden und konsekutiv gegebenenfalls auch DMARDs geben Hilfe für den Umgang mit Patienten, die eine anhaltende Remission erreicht haben. Zusammenfassung Die neue S2-Leitlinie gibt Empfehlungen für die Therapie der RA nach dem Prinzip des ""Treat-to-Target"" (T2T) mit etablierten und neuen krankheitsmodifizierenden Medikamenten (DMARDs), einschließlich der Biologika und JAK-Inhibitoren und macht Vorschläge zur Deeskalation nach Erreichen einer anhaltenden Remission.

All mechanically ventilated patients must be weaned from the ventilator at some stage. According to an International Consensus Conference the criteria for “prolonged weaning” are fulfilled if patients fail at least 3 weaning attempts (i. e. spontaneous breathing trial, SBT) or require more than 7 days of weaning after the first SBT. This occurs in about 15 – 20 % of patients. Because of the growing number of patients requiring prolonged weaning a German guideline on prolonged weaning has been developed. It is an initiative of the German Respiratory Society (Deutsche Gesellschaft fur Pneumologie und Beatmungsmedizin e. V., DGP) in cooperation with other societies (see acknowledgement) engaged in the field chaired by the Association of Scientific and Medical Societies in Germany (Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften, AWMF). This guideline deals with the definition, epidemiology, weaning categories, underlying pathophysiology, therapeutic strategies, the weaning unit, transition to out-of-hospital ventilation and therapeutic recommendations for end of life care. This short version summarises recommendations on prolonged weaning from the German guideline.

Drug hypersensitivity reactions are unpredictable adverse drug reactions. They manifest either within 1–6 h following drug intake (immediate reactions) with mild to life-threatening symptoms of anaphylaxis, or several hours to days later (delayed reactions), primarily as exanthematous eruptions. It is not always possible to detect involvement of the immune system (allergy). Waiving diagnostic tests can result in severe reactions on renewed exposure on the one hand, and to unjustified treatment restrictions on the other. With this guideline, experts from various specialist societies and institutions have formulated recommendations and an algorithm for the diagnosis of allergies. The key principles of diagnosing allergic/hypersensitivity drug reactions are presented. Where possible, the objective is to perform allergy diagnostics within 4 weeks–6 months following the reaction. A clinical classification of symptoms based on the morphology and time course of the reaction is required in order to plan a diagnostic work-up. In the case of typical symptoms of a drug hypersensitivity reaction and unequivocal findings from validated skin and/or laboratory tests, a reaction can be attributed to a trigger with sufficient confidence. However, skin and laboratory tests are often negative or insufficiently reliable. In such cases, controlled provocation testing is required to clarify drug reactions. This method is reliable and safe when attention is paid to indications and contraindications and performed under appropriate medical supervision. The results of the overall assessment are discussed with the patient and documented in an „allergy passport“ in order to ensure targeted avoidance in the future and allow the use of alternative drugs where possible.

Today, invasive and non-invasive home mechanical ventilation have become a well-established treatment option. Consequently, in 2010 the German Society of Pneumology and Mechanical Ventilation (DGP) has leadingly published the guidelines on "Non-Invasive and Invasive Mechanical Ventilation for Treatment of Chronic Respiratory Failure". However, continuing technical evolutions, new scientific insights, and health care developments require an extensive revision of the guidelines.For this reason, the updated guidelines are now published. Thereby, the existing chapters, namely technical issues, organizational structures in Germany, qualification criteria, disease specific recommendations including special features in pediatrics as well as ethical aspects and palliative care, have been updated according to the current literature and the health care developments in Germany. New chapters added to the guidelines include the topics of home mechanical ventilation in paraplegic patients and in those with failure of prolonged weaning.In the current guidelines different societies as well as professional and expert associations have been involved when compared to the 2010 guidelines. Importantly, disease-specific aspects are now covered by the German Interdisciplinary Society of Home Mechanical Ventilation (DIGAB). In addition, societies and associations directly involved in the care of patients receiving home mechanical ventilation have been included in the current process. Importantly, associations responsible for decisions on costs in the health care system and patient organizations have now been involved.The currently updated guidelines are valid for the next three years, following their first online publication on the home page of the Association of the Scientific Medical Societies in German (AWMF) in the beginning of July 2017. A subsequent revision of the guidelines remains the aim for the future.Die außerklinische Beatmung über einen invasiven oder nichtinvasiven Beatmungszugang ist mittlerweile etablierter Standard. Entsprechend wurde bereits im Jahr 2010 unter der Federführung der Deutschen Gesellschaft für Pneumologie und Beatmungsmedizin e. V. (DGP) die Leitlinie „Nichtinvasive und invasive Beatmung als Therapie der chronischen respiratorischen Insuffizienz“ publiziert. Die stetige Weiterentwicklung der technischen Methoden, neue wissenschaftliche Erkenntnisse sowie neue versorgungsrelevante Entwicklungen machen jedoch eine umfangreiche Revision der Leitlinie notwendig.Entsprechend wurde nun die revidierte Fassung der Leitlinie publiziert. Dabei finden sich in Bezug auf die aktuelle Literatur sowie auf die Versorgungslandschaft in Deutschland überarbeitete Empfehlungen zu den bereits in der Erstversion der Leitlinie aufgenommenen Themenbereichen der technischen Aspekte, der Organisationsstrukturen in Deutschland, der Qualifikationskriterien für die Versorgung außerklinisch beatmeter Patienten, für die einzelnen Krankheitsentitäten inklusive der Besonderheiten in der Pädiatrie sowie der ethischen Aspekte und der Palliativmedizin. Neu hinzugenommen sind zusätzlich die Themen der außerklinischen Beatmung nach erfolglosem prolongierten Weaning sowie die außerklinische Beatmung bei Querschnittlähmung.Neu ist auch die Zusammensetzung der beteiligten Fachgesellschaften, Fachverbände und Vereinigungen. Dabei werden die einzelnen Krankheitsbilder und Organsysteme übergreifend auch über die Deutsche Interdisziplinäre Gesellschaft für Außerklinische Beatmung e. V. (DIGAB) vertreten, während zusätzlich die Einbindung von Verbänden wichtig war, die direkt mit der Versorgung beatmeter Patienten im außerklinischen Setting konfrontiert sind. Zusätzlich wurde die Einbindung der Kostenträgerseite und der Patientenverbände für wichtig erachtet.Diese Leitlinie hat eine Gültigkeit von drei Jahren, beginnend mit dem Datum der Veröffentlichung auf der Homepage der Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften e. V. (AWMF) Anfang Juli 2017. Ziel ist eine weitere zeitgerechte Überarbeitung der Leitlinie nach Fristablauf.

Background. Although CSF cytology and MRI are standard methods to diagnose neoplastic meningitis (NM), this complication of neoplastic disease remains difficult to detect. We therefore reevaluated the sensitivity of gadolinium (GD)-enhanced MRI and cerebrospinal-fluid (CSF)-cytology and the relevance of tumor type and CSF cell count.Methods. We retrospectively identified 111 cases of NM diagnosed in our CSF laboratory since 1990 with complete documentation of both MRI and CSF cytology. 37 had haematological and 74 solid neoplasms. CSF cell counts were increased in 74 and normal in 37 patients.Results. In hematological neoplasms, MRI was positive in 49% and CSF cytology in 97%. In solid tumors, the sensitivity of MRI was 80% and of cytology 78%. With normal CSF cell counts, MRI was positive in 59% (50% hematological, 72% solid malignancies) and CSF cytology in 76% (92% in hematological, 68% in solid neoplasms). In cases of elevated cell counts, the sensitivity of MRI was 72% (50% for hematological, 83% for solid malignancies) and of CSF cytology 91% (100% for haematological and 85% for solid neoplasms). 91% of cytologically positive cases were diagnosed at first and another 7% at second lumbar puncture. Routine protein analyses had a low sensitivity in detecting NM.Conclusions. The high overall sensitivity of MRI was only confirmed for NM from solid tumors and for elevated CSF cell counts. With normal cell counts and haematological neoplasms, CSF-cytology was superior to MRI. None of the analysed routine CSF proteins had an acceptable sensitivity and specificity in detecting leptomeningeal disease.

Currently, management of antibody deficient patients differs significantly among caregivers. Evidence and consensus based (S3) guidelines for the treatment of primary antibody deficiencies were developed to improve the management of these patients.

Hintergrund: Bei der Polyneuropathie handelt es sich um eine Schadigung mehrerer oder aller peripheren Nerven. Sie kann mit teils qualenden neuropathischen Schmerzen einhergehen und ist fur erhebliche Morbiditat, eine erhohte Mortalitat und Beeintrachtigung der Lebensqualitat verantwortlich. Durch die Polyneuropathie kommt es zudem zu haufigen Sturzen. Die Schmerzbehandlung basiert im Wesentlichen auf Medikamenten. Bei Morbus Parkinson und Ruckenmarksverletzungen konnte die stochastische Resonanztherapie zentral- und periphernervale Interaktionsmechanismen positiv beeinflussen, was mit einer Verbesserung der posturalen Kontrolle einherging. Ziel: Untersuchung des Einflusses der stochastischen Resonanztherapie bei Patienten mit peripherer Neuropathie auf Vibrationsempfinden, Schmerz, statische und dynamische posturale Kontrolle. Methode: Fur die randomisierte klinische Studie wurden 32 Patienten mit Polyneuropathie randomisiert in Experimental- und Kontrollgruppe eingeteilt. Die Experimentalgruppe erhielt uber einen Zeitraum von 8 Wochen 2-mal wochentlich stochastische Resonanztherapie, die Kontrollgruppe eine Wassertherapie. Ergebnisse: Die Schmerzintensitat auf der Visuellen Analogskala verringerte sich in beiden Gruppen: in der Experimentalgruppe um 18,8 mm, in der Kontrollgruppe um 5,06 mm. Die Verbesserungen im Intergruppenvergleich waren nicht signifikant. Das Vibrationsempfinden verbesserte sich im Pra-post-Vergleich ebenfalls. Am Messpunkt des linken Groszehengrundgelenks zeigte sich eine statistisch nicht signifikante Verbesserung des Vibrationsempfindens von 2,13 auf 2,47 (Experimentalgruppe) und von 1,82 auf 2,94 (Kontrollgruppe). Die Verbesserungen waren an allen anderen Messpunkten gleichermasen nicht signifikant. Die statische und dynamische posturale Kontrolle konnte verbessert werden, die Ergebnisse waren jedoch nicht signifikant (p > 0,05). Schlussfolgerungen: Die stochastische Resonanztherapie zeigt bei Patienten mit Polyneuropathie klinisch positive Effekte. Allerdings ist sie bei Patienten mit peripherer Neuropathie einer Wassertherapie nicht uberlegen. Die weitere Untersuchung von Ganzkorpervibrationen beim Krankheitsbild der peripheren Neuropathie ware notwendig, um mehr Wissen uber diesen Sachverhalt zu generieren.

Die Standardisierung der Therapie der Juvenilen Idiopathischen Arthritis (JIA) hat zu einer Verbesserung der Lebensqualitat von Kindern und Jugendlichen mit JIA gefuhrt. Um die Versorgungsqualitat dieser Patienten weiter zu erhohen, ist eine Aktualisierung der bisherigen S2-Leitlinie von 2008 erforderlich. Durch eine neue systematische Literaturanalyse (Stichworter: juvenile idiopathic (rheumatoid) arthritis, therapy; Suchbegrenzungen (Limits) in PUBMED: humans, published in the last 3 years, all child: 0–18 years, clinical trial; Stichtag 15.01.2011) wurden 17 relevante Studien gefunden. Studien zur Diagnostik der JIA, Uveitis, Impfung, Transition wurden ausgeschlossen. Zu moderierten Konsensuskonferenzen wurden Vertreter folgender Fachgesellschaften und Vereinigungen eingeladen: Berufsverband der Kinder- und Jugendarzte (BVKJ), Deutsche Gesellschaft fur Kinder- und Jugendmedizin (DGKJ), Deutsche Gesellschaft fur Rheumatologie (DGRh), Deutsche Ophthalmologische Gesellschaft (DOG), Deutsche Rheuma-Liga Bundesverband, Verein zur Forderung und Unterstutzung rheumatologisch erkrankter Kinder und deren Eltern, Vereinigung fur Kinderorthopadie, Zentraler Verband der Physiotherapeuten und Krankengymnasten (ZVK). Die Konsensuskonferenzen fanden unter > 95% Teilnahme statt. Konsensus-Statements wurden formal per Nominale Gruppentechnik und im Delphiverfahren gewonnen. Zu den Themen medikamentose, symptomatische und chirurgische Therapie wurden aktualisierte Statements im Konsens erarbeitet und nach Kriterien evidenzbasierter Medizin bewertet. Neue, relevante Studien sind in der 2. Auflage der S2-Leitlinie fur die Therapie der JIA berucksichtigt.

The present guideline on allergen-specific immunotherapy (SIT) was established by the German allergy societies in conjunction with other scientific and medical societies (dermatology, ear-nose-throat, pediatrics, lung and airway diseases) and a patient support group according to criteria of the Association of the Scientific Medical Societies in Germany (AWMF). Subcutaneous immunotherapy (SCIT) is a unique causal treatment of IgE-mediated allergic diseases and induces longterm tolerance to the applied allergens due to numerous immunologic effects. Non-modified allergens are used as aqueous or physically coupled (depot) allergen extracts, chemically modified allergens (allergoids) are used as depot extracts for SCIT. Efficacy of SCIT has been demonstrated for pollen and house dust mite allergens in a large number of studies in patients with allergic rhinoconjunctivitis, and for animal dander (cat) and mold allergens (Alternaria, Cladosporium) in few studies. SCIT has been well-studied in intermittent and mild persisting IgE-mediated allergic asthma and is recommended as a therapeutic option besides allergen avoidance and pharmacotherapy. Preventive aspects, particularly reduced development of bronchial asthma and less novel allergic sensitizations, are increasingly considered during the decision for SCIT. In case of systemic reactions due to Hymenoptera (bee, wasp) venom allergy SCIT has excellent efficacy and should be continued for at least 3 - 5 years. An extended, sometimes lifelong SCIT, is necessary in a few patients. SCIT is indicated in patients with IgE-mediated sensitizations and corresponding clinical symptoms to allergens which do not permit allergen avoidance and which are available as suitable extracts. Diagnostic procedures, indication and selection of appropriate allergens for SCIT are made by a physician with certified training or qualified knowledge and skills in allergology. Contraindications have to be considered on an individual basis. Injections of SCIT are administered by a physician experienced in this therapy and who is able to perform emergency treatment in case of an allergic adverse event. Patients information and documentation are mandatory previous to the start of SCIT. Children tolerate SCIT very well and benefit especially from its immuno-modulatory effects. Systemic adverse reactions can occur due to SCIT, being rare in case of complete adherence to safety standards. Most adverse events are mild to moderate and easily treatable. Risk factors for and sequels of unwanted systemic effects can effectively be minimized by training the staff members involved, adhering to safety standards and immediate emergency treatment. Sublingual immunotherapy (SLIT) is an option for adults with allergic rhinoconjunctivitis due to pollen allergens, particularly if SCIT is not suitable. In house dust mite allergy and allergic asthma, SLIT does not substitute SCIT. Due to the present data, SLIT is not recommended for routine use in children and adolescents. A final conclusion on SLIT for children and adolescents is warranted if further study results are available. Various research fields like allergen characterization, routes of application, adjuvants, updosing regimen and preventive aspects demonstrate new developments in SIT being currently examined for clinical efficacy.

In this paper, a measurement system is presented which enables fast and accurate determination of the relative angular emission intensity of light emitting devices, taking into account their specific features such as low light output, narrow spectral distribution, high spatial luminous intensity ratios, and small dimensions. Application of logarithmic sensors allows for fast measurement of relative emission by simple analog circuitry, while locating 18 of them on a fixed ring around the emitter allows a motionless measurement system. As a result, the proposed system can determine the relative angular emission in less than 100 ms with a resolution of 5° for symmetric emitters.

Chronic obstructive pulmonary disease (COPD) is a leading cause of death worldwide. Cigarette smoking is the main cause of COPD. Quitting smoking is thus the most effective treatment strategy and central in COPD prevention. A number of guidelines on prevention, diagnosis, therapy and rehabilitation of COPD have been published. To help implementing and standardizing smoking cessation in COPD a guideline was published 2008 in Germany focusing on this complex issue. The present guideline is an update of the 2008 guideline and has a high grade of evidence (S3 according to the AWMF; Arbeitsgemeinschaft wissenschaftlicher medizinischer Fachgesellschaften). The guideline gives comprehensive and practical information on how to integrate smoking cessation as an central part of COPD therapy.

Mechanical ventilation (MV) is an essential part of modern intensive care medicine. MV is performed in patients with severe respiratory failure caused by insufficiency of the respiratory muscles and/or lung parenchymal disease when/after other treatments, i. e. oxygen, body position, secretion management, medication or non invasive ventilation have failed.In the majority of ICU patients weaning is routine and does not present any problems. Nevertheless 40-50 % of the time during mechanical ventilation is spent on weaning. About 20 % of patients need continued MV despite resolution of the conditions which originally precipitated the need for MV.There maybe a combination of reasons; chronic lung disease, comorbidities, age and conditions acquired in ICU (critical care neuromyopathy, psychological problems). According to an International Consensus Conference the criteria for "prolonged weaning" are fulfilled if patients fail at least three weaning attempts or require more than 7 days of weaning after the first spontaneous breathing trial. Prolonged weaning is a challenge. An inter- and multi-disciplinary approach is essential for weaning success. Complex, difficult to wean patients who fulfill the criteria for "prolonged weaning" can still be successfully weaned in specialised weaning units in about 50% of cases.In patients with unsuccessful weaning, invasive mechanical ventilation has to be arranged either at home or in a long term care facility.This S2-guideline was developed because of the growing number of patients requiring prolonged weaning. It is an initiative of the German Respiratory Society (Deutsche Gesellschaft für Pneumologie und Beatmungsmedizin e. V., DGP) in cooperation with other societies engaged in the field.The guideline is based on a systematic literature review of other guidelines, the Cochrane Library and PubMed.The consensus project was chaired by the Association of Scientific Medical Societies in Germany (Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften, AWMF) based on a formal interdisciplinary process applying the Delphi-concept. The guideline covers the following topics: Definitions, epidemiology, weaning categories, pathophysiology, the spectrum of treatment strategies, the weaning unit, discharge from hospital on MV and recommendations for end of life decisions. Special issues relating to paediatric patients were considered at the end of each chapter.The target audience for this guideline are intensivists, pneumologists, anesthesiologists, internists, cardiologists, surgeons, neurologists, pediatricians, geriatricians, palliative care clinicians, nurses, physiotherapists, respiratory therapists, ventilator manufacturers.The aim of the guideline is to disseminate current knowledge about prolonged weaning to all interested parties. Because there is a lack of clinical research data in this field the guideline is mainly based on expert opinion.

General design: Presentation of a new type of a study protocol for evaluation of the effectiveness of an immune modifier (rhG-CSF, filgrastim): prevention of postoperative infectious complications and of sub-optimal recovery from operation in patients with colorectal cancer and increased preoperative risk (ASA 3 and 4). This part describes the design of the randomised, placebo controlled, double-blinded, single-centre study performed at an university hospital (n = 40 patients for each group).¶Objective: The trial design includes the following elements for a prototype protocol:¶ - The study population is restricted to patients with colorectal cancer, including a left sided resection and an increased perioperative risk (ASA 3 and 4).¶ - Patients are allocated by random to the control or treatment group.¶ - The double blinding strategy of the trial is assessed by psychometric indices.¶ - An endpoint construct with quality of life (EORTC QLQ-C30) and a recovery index (modified Mc Peek index) are used as primary endpoints. Qualitative analysis of clinical relevance of the endpoints is performed by both patients and doctors.¶ - Statistical analysis uses an area under the curve (AUC) model for improvement of quality of life on leaving hospital and two and six months after operation. A confirmatory statistical model with quality of life as the first primary endpoint in the hierarchic test procedure is used. Expectations of patients and surgeons and the negative affect are analysed by social psychological scales.¶Conclusion: This study design differs from other trials on preoperative prophylaxis and postoperative recovery, and has been developed to try a new concept and avoid previous failures.¶

We examine the temperature-induced changes of the optical spectra of thin C60 films deposited on silicon and gold. We have used thermoreflectance to study the spectral response of the lowest electronic states. We have found strong effects related to the phase transition and freezing of the rotations of C60 molecules upon cooling.

In einer Beobachtungsstudie wurden 178 ambulante Patienten mit Verletzungen an der unteren Extremitat, die nach Abwagung ihres individuellen Thromboserisikos anhand einer Checkliste keine medikamentose Thromboembolieprophylaxe erhielten; auf das Auftreten von tiefen Beinvenenthrombosen getestet. Nach Abnahme des Gipsverbands nach durchschnittlich 14,4 (Range 5–48) Tagen erfolgte eine klinische und farbkodierte duplexsonographische Untersuchung. Bei positiven Ausfall einer oder beider Untersuchungen wurde eine Phlebographie durchgefuhrt. Von den 178 untersuchten Studienpatienten entwickelten 2 Patienten eine isolierte Einbundelunterschenkelvenenthrombose (Thromboseinzidenz 1,1%, 95-%-Konfidenzintervall 0–4,4%). Eine Lungenembolie wurde bei keinem Patienten beobachtet. Aufgrund unserer Ergebnisse erscheint eine generelle medikamentose Thromboembolieprophylaxe bei jungen, gesunden Patienten mit einer Verletzung an der unteren Extremitat und einer Ruhigstellung im Gipsverband nicht indiziert.

In this paper, we have studied the rate equation model of Kariotis and Lagally that can be used for a computer simulation of molecular beam epitaxial (MBE) growth. The advantage of such a model is the high speed of calculations so that results can be obtained in a very short time. The model is based on a solution of kinetic rate equations that govern the time dependence of the concentration of islands of varying sizes and differing heights on the top of the surface during MBE growth. One of the results of this model is an expression for the time dependence of the coverage of each layer . Within the present approach the MBE growth mode is evaluated by a calculation of the RHEED (reflection high-energy electron diffraction) intensity I(t). Similarly a calculation of the interface width IW(t), which determines the roughness of the growing surface, serves to confirm the growth mode of the MBE structure. In original model of Kariotis and Lagally the MBE growth has been affected by an appropriate choice of a set of the RE input parameters describing the probability of various kinetic processes appearing on the surface during the growth. In order to make the model applicable to simulations of MBE growth at `real' growth parameters (substrate temperature and deposition), we have included the substrate temperature dependence of all surface kinetic processes in the model. In this manner, and including processes described in the text, we have created a qualitatively new model that describes the MBE growth more accurately. This model has been used to simulate the MBE growth of GaAs at different substrate temperatures. The results have been compared with MC simulations and give an agreement in coverage profile

{211} polar surfaces of a Cd 0.96 Zn 0.04 Te chemically treated single crystal were investigated by Auger electron spectroscopy depth profiling. It was found that the use of a discriminatory black-white etchant causes the formation of a thick layer with cadmium having been severely depleted at both (211) A and (211) B faces. The thickness of this layer is large by a factor of 2 for the matt black face then for the bright and reflecting face. AES investigation of chemical treatment influence on (100) CdTe epitaxial layer surface has been performed. It has been established that etching in Br/methanol solution removes S, Cl and O containing contaminants from the surface, but simultaneously forms a Cd-depleted layer.

The interdisciplinary guidelines at the S3 level on the diagnosis of and therapy for hepatocellular carcinoma (HCC) constitute an evidence- and consensus-based instrument that is aimed at improving the diagnosis of and therapy for HCC since these are very challenging tasks. The purpose of the guidelines is to offer the patient (with suspected or confirmed HCC) adequate, scientifically based and up-to-date procedures in diagnosis, therapy and rehabilitation. This holds not only for locally limited or focally advanced disease but also for the existence of recurrences or distant metastases. Besides making a contribution to an appropriate health-care service, the guidelines should also provide the foundation for an individually adapted, high-quality therapy. The explanatory background texts should also enable non-specialist but responsible colleagues to give sound advice to their patients concerning specialist procedures, side effects and results. In the medium and long-term this should reduce the morbidity and mortality of patients with HCC and improve their quality of life.

These guidelines aim to facilitate high quality medical care of adults with snoring problems. The guidelines were devised for application in both in- and outpatient environments and are directed primarily at all those concerned with the diagnosis and therapy of snoring. According to the AWMF three-level concept, these represent S2k guidelines.A satisfactory definition of snoring does not currently exist. Snoring is the result of vibration of soft tissue structures in narrow regions of the upper airway during breathing while asleep. Ultimately, these vibrations are caused by the sleep-associated decrease in muscle tone in the area of the upper airway dilator muscles. A multitude of risk factors for snoring have been described and its occurrence is multifactorial. Data relating to the frequency of snoring vary widely, depending on the way in which the data are collected. Snoring is usually observed in middle-aged individuals and affected males predominate. Clinical diagnosis of snoring should comprise a free evaluation of the patient's medical history. Where possible this should also involve their bed partner and the case history can be complimented by questionnaires. To determine the airflow relevant structures, a clinical examination of the nose should be performed. This examination may also include nasal endoscopy. Examination of the oropharynx is particularly important and should be performed. The larynx and the hypopharynx should be examined. The size of the tongue and the condition of the mucous membranes should be recorded as part of the oral cavity examination, as should the results of a dental assessment. Facial skeleton morphology should be assessed for orientation purposes. Technical examinations may be advisable in individual cases. In the instance of suspected sleep-related breathing disorders, relevant comorbidities or where treatment for snoring has been requested, an objective sleep medicine examination should be performed. Snoring is not-at least as we currently understand it-a disease associated with a medical threat; therefore there is currently no medical necessity to treat the condition. All overweight patients with snoring problems should strive to lose weight. If snoring is associated with the supine position, positional therapy can be considered. Some cases of snoring can be appropriately treated using an intraoral device. Selected minimally invasive surgical procedures on the soft palate can be recommended to treat snoring, provided that examinations have revealed a suitable anatomy. The choice of technique is determined primarily by the individual anatomy. At an appropriate interval after the commencement or completion a therapeutic measure, a follow-up examination should be conducted to assess the success of the therapy and to aid in the planning of any further treatments.

Idiopathic pulmonary fibrosis is a fatal lung disease with a variable and unpredictable natural history and limited treatment options. Since publication of the ATS-ERS statement on IPF in the year 2000 diagnostic standards have improved and a considerable number of randomized controlled treatment trials have been published necessitating a revision. In the years 2006 - 2010 an international panel of IPF experts produced an evidence-based guideline on diagnosis and treatment of IPF, which was published in 2011. In order to implement this evidence-based guideline into the German Health System a group of German IPF experts translated and commented the international guideline, also including new publications in the field. A consensus conference was held in Bochum on December 3rd 2011 under the protectorate of the "Deutsche Gesellschaft für Pneumologie und Beatmungsmedizin (DGP)" and supervised by the "Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften" (AWMF). Most recommendations of the international guideline were found to be appropriate for the german situation. Based on recent clinical studies "weak negative" treatment recommendations for pirfenidone and anticoagulation were changed into "weak positive" for pirfenidone and "strong negative" for anticoagulation. Based on negative results from the PANTHER-trial the recommendation for the combination therapy of prednisone plus azathiorpine plus N-acetlycsteine was also changed into strong negative für patients with definite IPF. This document summarizes essential parts of the international IPF guideline and the comments and recommendations of the German IPF consensus conference.

Die interdisziplinare Leitlinie der Qualitat S3 zur Diagnose und Therapie des hepatozellularen Karzinoms (HCC) ist ein evidenz- und konsensbasiertes Instrument, um die Diagnostik und Therapie des HCC zu verbessern, da die Diagnostik und Therapie des HCC ein auserst diffiziles Vorgehen erfordert. Es ist die Aufgabe der Leitlinie, dem Patienten (mit Verdacht auf HCC oder nachgewiesenem HCC) angemessene, wissenschaftlich begrundete und aktuelle Verfahren in der Diagnostik, Therapie und Rehabilitation anzubieten. Dies gilt sowohl fur die lokal begrenzte oder lokal fortgeschrittene Erkrankung als auch bei Vorliegen eines Rezidivs oder von Fernmetastasen. Die Leitlinie soll neben dem Beitrag fur eine angemessene Gesundheitsversorgung auch die Basis fur eine individuell zugeschnittene, qualitativ hochwertige Therapie bieten. Mit den erklarenden Hintergrundtexten ist es auch nicht spezialisierten, mitbehandelnden Kollegen moglich, den Patienten uber das Vorgehen der Spezialisten, Nebenwirkungen und Ergebnisse gut zu beraten. Mittel- und langfristig sollen so die Morbiditat und Mortalitat von Patienten mit HCC gesenkt und die Lebensqualitat erhoht werden.

We report the optical spectra of C-60 epitaxial films on mica with various thicknesses in the energy range from 1.4 eV to 6 eV, and for temperatures from 13 K to 300 K. The transmittance of toluene, hexane, and heptane solutions of known concentrations has also been studied; a significant shift of the absorption bands in these solvents has been found.

Treatment of Juvenile Idiopathic Arthritis (JIA) has improved quality of life in children and adolescents with JIA. Standardisation of care offers the chance to improve the quality of care of those patients. New studies have been published after completion of our last treatment guideline (2007). An updated consensus process is mandatory.A systematic literature analysis in PUBMED (key words: juvenile idiopathic (rheumatoid) arthritis, therapy; limits: humans, published in the last 3 years, all child 0-18 years, clinical trial) revealed 17 relevant studies. The following societies were invited: Berufsverband der Kinder- und Jugendärzte (BVKJ), Deutsche Gesellschaft für Kinder- und Jugendmedizin (DGKJ), Deutsche Gesellschaft für Rheumatologie (DGRh), Deutsche Ophthalmologische Gesellschaft (DOG), Deutsche Rheuma-Liga Bundesverband, Verein zur Förderung und Unterstützung rheumatologisch erkrankter Kinder und deren Eltern, Vereinigung für Kinderorthopädie, Zentraler Verband der Physiotherapeuten und Krankengymnasten (ZVK). Consensus conferences were each attended by more than 95% of the nominated representatives. Consensus statements were confirmed by nominal group technique and Delphi method.Updated consensus statements regarding drug therapy, symptomatic and surgical management of JIA were compiled and judged strictly by the criteria of Evidence-Based Medicine (EBM).

Methods We performed a systematic literature analysis in PubMed (deadline: 15 January 2010, terms “juvenile idiopathic (rheumatoid) arthritis”, “therapy”, limits: “humans”, “published in the last 3 years”, “all child 018 years”, “clinical trial”) and evaluated relevant studies for quality of methodology. A formal consensus process, i.e. Nominal group technique and Delphi method, was conducted at three moderated consensus conferences at Dusseldorf or Krefeld (Germany) on May 9, 2007, August 1, 2007 and January 15, 2010. Conferences were attended by 95% of the representatives who had been nominated by their scientific society or organizations.

Skin tests in patients with IgE-mediated immediate type allergy are performed with the intention to establish a contact between allergens and skin mast cells. The latter carry specific IgE antibodies on their surface. If mast cells get activated, mediators (mainly histamine) are released which induce a visible skin reaction (wheal and erythema).[nl]Skin tests are indicated, if an immediate type allergic disease is suspected. Systemic anaphylactic reactions at skin testing are very rare. However, it is necessary to take them into account and to provide emergency treatment. Relative contraindications comprise skin diseases in the test area, poor general condition and insufficiently treated severe asthma. If tests are used, which have a higher risk for a systemic anaphylactic reaction, pregnancy or beta-blocker therapy, are further contraindications.[nl]Skin test application does not depend on patient age. However, in pre-school age tests are reluctantly performed. It is essential to consider the half-life of drugs which may interfere with the test result, and which have to be discontinued early enough before testing. After anaphylactic reactions there may be a refractory period. Therefore, tests should not be done within the first week after such reactions. Skin prick tests are the procedures of first choice, intradermal tests are more sensitive than prick tests. Skin tests are performed at the flexor side of the forearm. As intradermal tests are more inconvenient, testing can be also done at a less susceptible site of the body (upper back).[nl]It is recommended to use standardized test extracts. However, if standardised extracts are not available or do not yield suitable test results, one may switch to other preparations. If the patient shows a positive reaction to a non-standardized substance, control tests should be performed in healthy subjects in order to exclude an unspecific reaction.[nl]The reaction is read after 15 to 20 min. Skin tests are regarded positive if the mean wheal diameter is ≥ 3 mm at the prick test, and ≥ 5 mm at the intradermal test.[nl]Skin test results may be negative although patients are allergic. If a skin test is positive, one will have to distinguish reactions, which are clinically relevant, from those, which are not. History and/or challenge tests help to clarify the relevance of a sensitization. Usually, a clinically irrelevant sensitization does not lead to practical consequences.
