Summary: Background: Closed-loop insulin delivery systems have the potential to address suboptimal glucose control in children and adolescents with type 1 diabetes. We compared safety and efficacy of the Cambridge hybrid closed-loop algorithm with usual care over 6 months in this population. Methods: In a multicentre, multinational, parallel randomised controlled trial, participants aged 6–18 years using insulin pump therapy were recruited at seven UK and five US paediatric diabetes centres. Key inclusion criteria were diagnosis of type 1 diabetes for at least 12 months, insulin pump therapy for at least 3 months, and screening HbA1c levels between 53 and 86 mmol/mol (7·0–10·0%). Using block randomisation and central randomisation software, we randomly assigned participants to either closed-loop insulin delivery (closed-loop group) or to usual care with insulin pump therapy (control group) for 6 months. Randomisation was stratified at each centre by local baseline HbA1c. The Cambridge closed-loop algorithm running on a smartphone was used with either (1) a modified Medtronic 640G pump, Medtronic Guardian 3 sensor, and Medtronic prototype phone enclosure (FlorenceM configuration), or (2) a Sooil Dana RS pump and Dexcom G6 sensor (CamAPS FX configuration). The primary endpoint was change in HbA1c at 6 months combining data from both configurations. The primary analysis was done in all randomised patients (intention to treat). Trial registration ClinicalTrials.gov, NCT02925299. Findings: Of 147 people initially screened, 133 participants (mean age 13·0 years [SD 2·8]; 57% female, 43% male) were randomly assigned to either the closed-loop group (n=65) or the control group (n=68). Mean baseline HbA1c was 8·2% (SD 0·7) in the closed-loop group and 8·3% (0·7) in the control group. At 6 months, HbA1c was lower in the closed-loop group than in the control group (between-group difference −3·5 mmol/mol (95% CI −6·5 to −0·5 [–0·32 percentage points, −0·59 to −0·04]; p=0·023). Closed-loop usage was low with FlorenceM due to failing phone enclosures (median 40% [IQR 26–53]), but consistently high with CamAPS FX (93% [88–96]), impacting efficacy. A total of 155 adverse events occurred after randomisation (67 in the closed-loop group, 88 in the control group), including seven severe hypoglycaemia events (four in the closed-loop group, three in the control group), two diabetic ketoacidosis events (both in the closed-loop group), and two non-treatment-related serious adverse events. There were 23 reportable hyperglycaemia events (11 in the closed-loop group, 12 in the control group), which did not meet criteria for diabetic ketoacidosis. Interpretation: The Cambridge hybrid closed-loop algorithm had an acceptable safety profile, and improved glycaemic control in children and adolescents with type 1 diabetes. To ensure optimal efficacy of the closed-loop system, usage needs to be consistently high, as demonstrated with CamAPS FX. Funding: National Institute of Diabetes and Digestive and Kidney Diseases.

Abstract Introduction Empowering women in Saudi Arabia has become a critical agenda, reflecting the nation’s shifting societal and economic dynamics. This review highlighted recent policy initiatives, societal changes, and institutional reforms aimed at enhancing the role of women in the Saudi health sector. Methods This scoping review explored academic literature and government reports related to gender equality and women’s empowerment in the Saudi health sector to provide an overview of the advancements made and the challenges that remain. Results The review found substantial progress in empowering female healthcare workers (HCWs), evidenced by increased participation in leadership roles, higher enrollment in medical education, and the implementation of workplace policies promoting gender equity. These efforts are aligned with Saudi Arabia’s Vision 2030, which emphasizes inclusivity and diversity as central pillars for sustainable development. However, persistent challenges such as traditional gender norms, workplace biases, and limited access to mentorship and leadership opportunities were identified as barriers to further progress. Conclusions While significant strides have been made, achieving gender equality in the Saudi healthcare sector requires sustained efforts to address societal and institutional challenges. The transformative potential of education, leadership development, and mentorship programs should be harnessed to empower female HCWs. Continued policy reforms and cultural shifts are crucial for fostering a gender-diverse healthcare system.

Abstract This work aims to investigate the impact of chitosan and hydrogen peroxide on the growth and seasonal yield of lime trees cultivated in commercial orchards in Qalyubiyya Governorate, Egypt, during the 2021 & 2022 seasons. The treatments include foliar spraying of two concentrations of CHI (100 & 200 ppm) and H2O2 (2 & 4 cm/L), either individually or in combination, at two distinct times, one month after fruit set (1st week of September) and after two months. The hypothesis was that applying chitosan and hydrogen peroxide would improve tree growth, fruit quality, and total production. The obtained results indicated that the combinations of CHI with H2O2 improved tree growth, leaf mineral composition, seasonal yield, and fruit quality parameters. CHI at 200 ppm + H2O2 at 4 cm/L was the most efficient treatment and achieved the largest tree canopy. Treatment of 100 ppm CHI with 2 cm/L H2O2 showed superior yield in terms of tree yield (23.56 & 29.64 kg/tree), total production (8.15 & 9.16 t/ha) compared to other treatments. Likewise, combinations of CHI and H2O2 improved fruit characteristics. Findings of this study demonstrated that the foliar application of CHI with H2O2 could be a promising application to improve seasonal lemon yield and fruit quality in commercial orchards.

Abstract This study aims to: (1) examine the impact of cybersecurity disclosure on banks’ performance and (2) explore whether the existence of a chief risk officer (CRO), an information technology (IT) committee, and a board of directors (BOD)’ size moderates the association between cybersecurity disclosure and bank performance. The study used manual textual analysis to measure cybersecurity disclosure in a sample of listed banks in the MENA region countries based on data from 2019 to 2021. The data were collected from annual reports and financial statements of banks available at Orbis Bank Focus database. The study employed a random effect regression model to test the hypotheses and discuss the results. The findings show that banks in the MENA region are increasingly interested in disclosing cybersecurity information, where cybersecurity disclosure over the sample years is increasing from 17% in 2019 to 19.6% in 2021. In addition, the results show that cybersecurity disclosure has a positive and significant influence on bank performance. Furthermore, the findings indicate that the presence of a CRO moderates the relationship between cybersecurity disclosure and bank performance. These findings show that depending largely on a bank's CRO to handle complex and dynamic risks can have serious consequences for decision making processes connected to managing cybersecurity risk and disclosure. This paper creates a new research paradigm by focusing on the disclosure of cybersecurity information in the MENA banking sector, where exploring the moderating role of the CRO, IT committee, and board size in enhancing the cybersecurity disclosure-bank performance relationship is lacking. The findings provide practical implications for various stakeholders, where it reveals the current practices of cybersecurity disclosure of banks in the MENA region with the objective of minimizing information asymmetry, maintaining public trust, and identifying potential risks of financial distress. In addition, the results direct the attention of banks and regulators toward the role of CRO in risk governance, particularly in managing cyber risks within the banking industry.

Abstract The World Health Organization declared a state of emergency in 2022 because of monkeypox. This disease has raised international concern as it has spread beyond Africa, where it is endemic. The global community has shown attention and solidarity in combating this disease as its daily increase becomes evident. Various skin symptoms appear in people infected with this disease, which can spread easily, especially in a polluted environment. It is difficult to diagnose monkeypox in its early stages because of its similarity with the symptoms of other diseases such as chicken pox and measles. Recently, computer-aided classification methods such as deep learning and machine learning within artificial intelligence have been employed to detect various diseases, including COVID-19, tumor cells, and Monkeypox, in a short period and with high accuracy. In this study, we propose the CanDark model, an end-to-end deep-learning model that incorporates cancelable biometrics for diagnosing Monkeypox. CanDark stands for cancelable DarkNet-53, which means that DarkNet-53 CNN is utilized for extracting deep features from Monkeypox skin images. Then a cancelable method is applied to these features to protect patient information. Various cancelable techniques have been evaluated, such as bio-hashing, multilayer perceptron (MLP) hashing, index-of-maximum Gaussian random projection-based hashing (IoM-GRP), and index-of-maximum uniformly random permutation-based hashing (IoM-URP). The proposed approach’s performance is evaluated using various assessment issues such as accuracy, specificity, precision, recall, and fscore. Using the IoM-URP, the CanDark model is superior to other state-of-the-art Monkeypox diagnostic techniques. The proposed framework achieved an accuracy of 98.81%, a specificity of 98.73%, a precision of 98.9%, a recall of 97.02%, and fscore of 97.95%.

Purpose – This study examines the correlation between ownership structure (OS) and financial reporting integrity (FRI), with emphasis on the impact of earnings quality (EQ) in the Egyptian context. Design/methodology/approach – The study uses data from 472 firm-year observations of Egyptian publicly listed companies between 2014 and 2021 and carried out descriptive statistics, correlation tests, multiple regression analysis and two-stage least squares (2SLS) to test the hypotheses. Findings – The results revealed that blockholders and institutional ownership significantly enhance reporting integrity through effective oversight and monitoring. The findings underscore the vital role of concentrated OS in overseeing reporting practices and mitigating managerial opportunism, thereby improving the transparency and reliability of financial disclosures in Egypt. Practical implications – The findings enrich the literature on corporate governance and financial reporting quality and have important implications for policymakers, regulators and corporate stakeholders. Originality/value – This work contributes valuable insights on how OS and EQ can bolster FRI, offering crucial information for combating financial crises and facilitating smooth business operations in Egypt.

Abstract In the realm of green communications, the focus is on achieving high spectrum efficiency and low energy consumption. This paper addresses the crucial goal of reducing energy usage in green cognitive radio networks (CRNs) during communication between secondary users (SUs) and primary users (PUs). This paper proposed an energy consumption optimization model (ECOM) for green CRN utilizing collaborative spectrum sensing thereby minimizing the environmental impact and prolonging the operational lifetime of devices. The collaborative spectrum sensing proved its role in optimizing the energy consumption in the green CRN. An energy-efficient scheduling algorithm is implemented in ECOM, in which the SUs can be scheduled to perform their sensing process in a time-division manner to reduce energy consumption. Applied collaborative spectrum sensing serves as a valuable resource for researchers, network operators, and policymakers seeking to balance the increasing demand for wireless communication services with the imperative of sustainability. The simulation results and mathematical proof emphasize that ECOM demonstrates reduced energy usage and increased average effective throughput when compared to other recent models.

The motor imagery (MI) electroencephalographic (EEG) signals are leveraged as control commands to numerus engineering applications, such as operating a wheelchair through thought alone. EEG signals are characterized by their non-stationary nature and high dimensionality, posing noteworthy challenges for building robust identification models with limited data samples. The renewed deep convolutional neural network (CNN) has been harnessed in various fields due to its ability to autonomously extract and select latent features addressing problems across various types of EEG signals. However, CNN architectures comprise a vast parameter and hyperparameter space. Normally, parameters are optimized via the back-propagation algorithm, while hyperparameters are adjusted through a lengthy trial-and-error process. This work mitigates the aforementioned problems by employing the genetic algorithm (GA) as an artificial intelligence optimization tool for optimizing crucial architectural hyperparameters including the number of convolution filters, size of filters, and dropout probability. Furthermore, the two parameters of cropping augmentation, used for boosting the EEG training samples, are also optimized by the GA. The proposed model in this paper (GACNN) that includes dual optimization, achieved both by the back-propagation algorithm and GA, has attained promising results in the analysis of MI EEG signals. Experiments are performed on the known brain-computer interface competition IV 2a dataset (BCIC IV 2a). GACNN has recorded an increment of about 17% in Cohen’s kappa coefficient and a decrement of about 30% in standard deviation (SD) in comparison to state-of-the-art studies.

The present investigation proposed an innovative trial at the probable beneficial effects of nano-Bacillus amyloliquefaciens (NBA) dietary incorporation on the Nile tilapia (Oreochromis niloticus). The investigation included the impact on the growth, digestive functions, immune-antioxidant indices, and resistance to Aeromonas hydrophila challenge. A total of 135 fish (35.14 ± 0.12 g) were equally allocated into three groups (45 fish/group; 15 fish/replicate) in triplicates for 70 days. The control, NBA2, and NBA4 groups were fed basal diets enriched with 0, 102, and 104 CFU/kg NBA, respectively. Following the feeding experiment, all experimental groups were injected with 0.1 mL (1.5 × 106) A. hydrophila, and the fish mortalities were observed for 14 days. The outcomes showed that dietary NBA (NBA4 followed by the NBA2 diet) augmented the growth variables (final body weight, total weight gain, and specific growth rate) and condition factor and declined the feed conversion ratio. The intestinal digestive enzyme (amylase and lipase) and growth hormone levels were increased and the serum glucose level was decreased by dietary NBA. Furthermore, NBA diets enhanced the immune (total protein, globulin, lysozyme, complement 3, myeloperoxidase, and phagocytic activity) and antioxidant (superoxide dismutase, catalase, and total antioxidant capacity) parameters. The intestinal histology revealed no pathological lesions with a significant improvement in the intestinal histomorpho-measures (villus height and width, villus surface area, lamina propria thickness, and tunica muscularis thickness) by NBA diets in a dose-dependent manner. In the fish intestine, the B. amyloliquefaciens count was increased in the NBA groups (NBA4 group followed by NBA2 group) with no discernible difference in the total bacterial count. Fish resistance to the A. hydrophila challenge was increased by increasing the survival % in the NBA4 group (91.70 %) followed by the NBA2 group (83.30 %) compared to the control group (70.80 %). Overall, dietary NBA (especially 104 CFU/kg diet) could be a promising feed supplement in the Nile tilapia diets for improving their growth, health, and resistance to bacterial challenges.

Children's participation in armed conflicts has been a widespread phenomenon since antiquity. Therefore, the international community had to intervene to protect them and prohibit their recruitment. Global efforts have been made to eliminate the phenomenon of recruitment and use of children in military operations by prohibiting the recruitment of children in many international documents.

Abstract Background Duke Activity Status Index (DASI) is a widely used tool to assess functional capacity among patients, but there is no Sinhala version validated for patients in Sri Lanka. This study aimed to cross-culturally adapt and test the validity and reliability of the Sinhala version of DASI (DASI-S). Methods The translation and cross-cultural adaptation of the DASI questionnaire were conducted following the standard guidelines. It was pre-tested on ten pre-operative patients and further modified. The construct validity and reliability of DASI-S were evaluated by administering the modified final DASI-S, which comprised 12 items, along with the physical functioning sub-scale of the 36-item short-form health survey (SF-36), consisting of 10 items to eighty-one patients who were awaiting non-cardiac surgeries at university surgical wards, National Hospital of Sri Lanka (NHSL), and Colombo North Teaching Hospital (CNTH), Sri Lanka. Reliability was assessed through Cronbach alpha, while the validity was evaluated using factor analysis and Spearman’s correlation. The ethical approval was obtained from the Ethics Review Committee, Faculty of Medicine, University of Colombo, Sri Lanka. Results The mean age of the participants was 46.2 (± 16.6) years and the majority were females (54.3%). The mean height, weight, and body mass index of the sample were 160.5 (± 9.6) cm, 60.3 (± 11.9) kg, and 23.4 (± 4.5) kgm−2 respectively. Abstract Background Pseudomonas putida is a pathogenic bacterium that induces great losses in fishes, including Nile tilapia (Oreochromis niloticus). Currently, the application of nanomaterials in aquaculture practices has gained more success as it endows promising results in therapies compared to traditional protocols. Objective Therefore, the current perspective is considered the first report to assess the anti-bacterial efficacy of titanium dioxide nanogel (TDNG) against Pseudomonas putida (P. putida) in Nile tilapia. Methods The fish (n = 200; average body weight: 47.50±1.32 g) were allocated into four random groups (control, TDNG, P. putida, and TDNG + P. putida), where 0.9 mg/L of TDNG was applied as bath treatment for ten days. Results Outcomes revealed that P. putida infection caused ethological alterations (surfacing, abnormal movement, and aggression) and depression of immune-antioxidant variables (complement 3, lysozyme activity, total antioxidant capacity, superoxide dismutase, and reduced glutathione content). Additionally, a substantial elevation in hepatorenal biomarkers (aspartate and alanine aminotransferases and creatinine) with clear histopathological changes and immuno-histochemical alterations (very weak BCL-2 and potent caspase-3 immuno-expressions) were seen. Surprisingly, treating P. putida-infected fish with TDNG improved these variables and obvious restoration of the tissue architectures. Conclusion Overall, this report encompasses the key role of TDNG as an anti-bacterial agent for controlling P. putida infection and improving the health status of Nile tilapia.

This study explores the effect of using dental brushes with or without metacrylate-based modeling resins on long-term color stability and surface topographies of resin-based composites. This study examined the effects of two variables: (1) the type of brush used (Art brush, Micro-brush, or Mylar strip) and (2) the application of a modeling resin (applied or not applied). The specimens were artificially aged through 10,000 cycles of thermocycling and subsequently immersed in coffee for 30 days. Measurements of color and surface roughness were taken at baseline and after the aging, using a non-contact profilometer for surface roughness and a spectrophotometer for color. Data were analyzed using paired t-tests and one-way ANOVA. Resin-based composites smoothed with dental brushes or micro brushes without modeling resins exhibited lower color change (ΔE) than other groups. Paired t-tests revealed significant differences in average surface roughness (Ra) and valley depth (Rv) for each surfacing technique before and after aging ( p ⩽ 0.01). The root means square average of the profile heights (Rq) significantly increased in the control and micro-brush groups ( p ⩽ 0.01). In conclusion, the use of brushes in resin-based composites placement does not increase the susceptibility to staining. Instead, the inclusion of resin modeling contributes to discoloration over time.

Purpose – The objective of this study is to examine the correlation between the use of Canva-based SKI instructional videos and student motivation at MAS DDI As-Salman Allakuang. It aims to determine how Canva's integration influences aspects of student motivation such as interest, effort, and perseverance, providing insights into the effectiveness of digital learning tools in enhancing student engagement and motivation. Method – This study employs a quantitative correlational design to measure the impact of using Canva-based SKI instructional videos on student motivation at MAS DDI As-Salman Allakuang. A purposive sample of 54 students was selected. Data were collected using a validated motivation questionnaire covering aspects like interest, effort, and perseverance. Pearson correlation statistics were used to analyze the relationship between the use of Canva-based SKI instructional videos and student motivation. Findings – This study found a significant moderate positive correlation (r=0.617, p=0.001) between the use of Canva-based SKI instructional videos and student motivation in Aqidah Akhlak learning at MAS DDI As-Salman Allakuang. This indicates that more frequent use of Canva-based videos is associated with higher student motivation. These findings confirm the hypothesis and demonstrate that digital learning tools like Canva can enhance student motivation. Research Implications – Using Canva-based instructional videos in Aqidah Akhlak learning can practically enhance student motivation. However, to confirm this effect, more diverse, experimental, and long-term studies are needed due to limitations in causation and potential biases.

Abstract Soot stain from diverse sources is the most common stain that stains historical paper documents found in libraries, museums, and storage facilities. It is believed to be one of the types of deterioration that arises from the contact between stains and paper sheets. Therefore, in the present work, the effectiveness of the synthesized hybrid nanogel consisting of carbopol and nanoparticles (NPs) of ZnO, TiO2, or Fe3O4, in low and high-viscosity forms, for eliminating soot stains from historical paper manuscripts was conducted. The prepared hybrid nanogels were characterized utilizing the TEM, AFM, XRD, DLS, and Zeta Potential techniques. Following preparation, the soot-stained paper samples were heated to 105 °C for 6 days, a process known as accelerated thermal aging. Surface morphology, mechanical properties and the color change of the paper samples were investigated. The prepared hybrid nanogel had a spherical shape with well-defined edges and uniform size, with an average particle diameter ranging from 30 to 35 nm. There was no significant NPs agglomeration seen, suggesting uniform dispersion in the carbopol matrix. Additionally, the prepared gels' crystal structure and phase purity were revealed by the XRD analysis results. Paper fibers were visible in some parts of the treated sample with TiO2/carbopol hybrid nanogel at high-viscosity before aging. Cleaning soot-stained paper samples using high-viscosity Fe3O4/carbopol hybrid nanogel is more successful than using low-viscosity nanogel. the sample treated with high-viscosity ZnONPs/carbopol hybrid nanogel produced the highest total color differences (ΔE) (26.17). The paper sample treated with high-viscosity ZnO/carbopol hybrid nanogel exhibited the maximum tensile strength (61.8 N/mm2) and elongation at break (1.174%). The evaluation of the paper samples both before and after the cleaning process revealed that, the treatment with high-viscosity ZnO/carbopol hybrid nanogel gave the best results in removing soot stains from stained paper samples.

Abstract A comprehensive investigation into the green synthesis of metal oxide nanoparticles (NPs) has garnered significant attention due to its commendable reliability, sustainability, and environmentally friendly attributes. Green synthesis methods play a crucial role in mitigating the adverse effects associated with conventional approaches employed for nanostructure preparation. This research endeavors to examine the impact of ginger plant extract-assisted green synthesis of metal oxides NPs on the serum ferritin levels of anemic diabetic patients in vitro, focusing specifically on α-Fe2O3 and ZnO NPs. Sixty diabetic volunteers with anemia (35–50 years) and thirty healthy volunteers were enrolled as controls. The assessment was conducted using the VIDAS Ferritin (FER) assay. Photoluminescence (PL) spectroscopy measurements were performed to elucidate the intrinsic and extrinsic transitions of these NPs, affirming the successful formation of α-structured iron oxide. Density functional theory (DFT) calculations were carried out at the B3LYP/6-311++G(d,2p) level of theory to investigate the geometry optimization and molecular electrostatic potential maps of the NPs. Furthermore, TD-DFT calculations were employed to explore their frontier molecular orbitals and various quantum chemical parameters. The binding affinity and interaction types of ZnO and α-Fe2O3 NPs to the active site of the human H-Chain Ferritin (PDB ID: 2FHA) target were determined with the help of molecular docking. Results unveiled the crystalline structure of ZnO and the α-structure of α-Fe2O3. Analysis of the frontier molecular orbitals and dipole moment values demonstrated that ZnO (total dipole moment (D) = 5.80 µ) exhibited superior chemical reactivity, biological activity, and stronger molecular interactions with diverse force fields compared to α-Fe2O3 (D = 2.65 µ). Molecular docking of the metal oxides NPs with human H-chain ferritin provided evidence of robust hydrogen bond interactions and metal-acceptor bonds between the metal oxides and the target protein. This finding could have a great impact on using metal oxides NPs-ferritin as a therapeutic protein, however, further studies on their toxicity are required. Graphical abstract

With the extensive deployment of Internet-of-Things (IoT) in next generation wireless systems, the problems of energy efficiency and battery life become exacerbated, highlighting the pressing need for innovative solutions. Symbiotic radio (SR) is considered one of the emerging technologies that aims at providing an energy-efficient solution for the ubiquitous IoT applications. In this paper, we propose an SR system that is assisted with a double-faced active intelligent reflecting surface (DFA-IRS). The proposed system consists of an active transmitter (AT), an active receiver (AR), a backscatter receiver (BR), a DFA-IRS, and an IoT device that is connected to the DFA-IRS. We formulated a BR spectral efficiency maximization problem via optimizing the active beamforming vector at the AT, the power amplification factors of the IRS active elements, and the IRS phase shift at each active element under the constraints of a maximum power budget and the AR spectral efficiency requirements. The formulated problem is non-convex due to the coupling between different variables. Hence, we divided the main problems into three sub-problems and utilized the successive convex approximation (SCA) and the semidefinite relaxation (SDR) techniques to obtain a convex equivalent problem that can be solved using conventional optimization tools such as CVX. Simulation results show that the proposed algorithm converges in few number of iterations. Moreover, the proposed scheme achieves better BR spectral efficiency when compared to the case where a single-faced active IRS or a simultaneously transmitting and reflecting IRS (STAR-IRS) counterpart is used.

The proliferation of affordable LiDAR technology and photogrammetry sensors has revolutionized 3D data acquisition in built environments, enabling comprehensive data capture from citywide scales to interior structures. This data can be transformed into digital twins, providing valuable resources for city planners, architects, engineers, and decision-makers. However, current studies often overlook the limitations of real-world point cloud datasets derived from LiDAR systems, which are voluminous, noisy, incomplete, and lacking information, which hinders monitoring, interpretation, and automated analysis. To address these challenges, methods are required to prepare point cloud data, ensuring accurate and reliable 3D representations. This research proposes a detailed framework for point cloud data preparation in busy urban environments. It includes precise algorithms, software, and parameter guidelines, allowing for the creation of comprehensive point cloud datasets. The framework has been successfully implemented on datasets acquired in Toronto, converting point cloud data from various platforms and parameters into an integrated dataset. Results demonstrate the framework's effectiveness in producing accurate and complete point cloud datasets for applications such as classification, information extraction, 3D model generation, and smart cities' monitoring and management.

Background and objective: Tuberculosis regarded as a major cause of death word wide despite WHO trials to eradicate it. The aim of this study was to determine the effect of gender difference in treatment outcome and mortality. Methods: In this retrospective study on pulmonary tuberculosis at the Chest and Respiratory Disease Center in Erbil city. A total 430 patients with pulmonary tuberculosis were studied from January 1st, 2016 to December 31st, 2019 at the Chest and Respiratory Disease Center in Erbil city. The patients were divided in to two category groups; category one, includes severe extra pulmonary tuberculosis, Smear negative or positive seriously infected pulmonary disease, and category two: includes failure of treatment, relapses, and default. The recorded files of 430 patients with TB registered at Chest and Respiratory Disease Center in Erbil city as follows 103 (24%), 107 (24.9%), 109 (25.3%), and 111 (25.8%) patients since 2016, 2017, 2018, and 2019 were analyzed respectively. Abstract Methylene blue (MB) dye is considered a well-known dye in many industries and the low concentration of MB is considered very polluted for all environment if it discharged without any treatment. For that reason, many researchers used advanced technologies for removing MB such as the electrochemical methods that considered very simple and give rapid response. Considering these aspects, a novel quartz crystal microbalance nanosensors based on different concentrations of PVC@SiO2 were designed for real-time adsorption of MB dye in the aqueous streams at different pHs and different temperatures. The characterization results of PVC@SiO2 showed that the PVC@SiO2 have synthesized in spherical shape. The performance of the designed QCM-Based PVC@SiO2 nanosensors were examined by the QCM technique. The sensitivity of designed nanosensors was evaluated at constant concentration of MB (10 mg/L) at different pHs (2, 7 and 11) and temperatures (20 °C, 25 °C, and 30 °C). From the experimental, the best concentration of PVC@SiO2 was 3% for adsorbed 9.99 mg of cationic methylene blue at pH 11 and temperature 20 °C in only 5.6 min.

Abstract The objectives of this study are to (i) examine the relationship between accounting conservatism and climate-related risk disclosure (CRRD) in the context of Egypt, and (ii) look into the moderating role of earnings quality (EQ) in such a relationship. To operationalize CRRD reported by Egyptian firms listed in the SP/EGX ESG index across the years from 2018 to 2022, the study uses a manual content analysis as a base for analysis and employs two (OLS) regression models to test the study’ hypotheses. EQ is measured using the cross-sectional modified Jones model, while accounting conservatism is gauged using the C-Score. Additionally, endogeneity checks in robustness tests are performed using system GMM. The findings show no impact of CRRD on the conservative accounting practices of Egyptian firms. However, there is a sizable adverse effect when EQ is used to moderate this association. This finding is consistent with the notion that firms report high levels of climate impacts and have fewer information asymmetry issues. This paper extends disclosure indexes by highlighting the frequent keywords of CRRD used in the annual narrative reports of Egyptian firms. Hence, this study is among the first to explore the moderation impact of EQ on the relationship between CRRD and accounting conservatism in the Egyptian setting.

Abstract The use of nanotechnology has the potential to revolutionize the detection and treatment of cancer. Developments in protein engineering and materials science have led to the emergence of new nanoscale targeting techniques, which offer renewed hope for cancer patients. While several nanocarriers for medicinal purposes have been approved for human trials, only a few have been authorized for clinical use in targeting cancer cells. In this review, we analyze some of the authorized formulations and discuss the challenges of translating findings from the lab to the clinic. This study highlights the various nanocarriers and compounds that can be used for selective tumor targeting and the inherent difficulties in cancer therapy. Nanotechnology provides a promising platform for improving cancer detection and treatment in the future, but further research is needed to overcome the current limitations in clinical translation. Abstract There are many paper manuscripts at museums, stores, and libraries that have different stains. The dust stains and soil remains played an important role in the degradation of these manuscripts. Therefore, the cleaning process for these stains is important to process. Unfortunately, the removal of stains by some traditional techniques can be hazardous to the paper's fibers. Therefore, this study aims to evaluate innovative nanoparticle gels in the cleaning process of dust stains from paper manuscripts. The synthesized nanomaterials [Fe3O4, TiO2, and ZnO]/Carbopol hybrid nanogel were examined in order to investigate the surface morphology, determine the physio-chemical properties, for phase structure, using scanning electron microscope (SEM), transmission electron microscope (TEM), AFM, DLS, XRD, and Zeta potential. Some stained paper samples were prepared and exposed to accelerated thermal aging at 100 °C for 72 h. The evaluation of the cleaning by nanogel was performed by different analytical procedures containing investigation with a USB digital microscope, SEM, color change, mechanical properties, and ATR-FTIR analysis. The results of this study showed that ZnO/Carbopol hybrid nanogel at high viscosity gave the best results in the cleaning process of mud stains through the ability to dismantle mud particles and increase surface contact with the solvent. The aged treated sample with high viscosity form of ZnO NPs/carbopol hybrid nanogel gave the highest tensile strength value of (56.0 N). The treated samples with the high viscosity form of ZnO NPs/carbopol hybrid nanogel gave the highest value of elongation (1.398%) before aging. It also does not affect the chemical composition of the paper after cleaning it, but rather on the whole, it bites the hydration of the paper, which positively affects the properties of the treated paper.

Abstract Background Effective communication among members of the dental team is essential for the delivery of high-quality dental care. However, an in-depth understanding of issues concerning the interrelationship between dental clinicians and laboratory technicians has not been previously undertaken. Therefore, the aim of the study was to explore factors influencing the interrelationship between dental clinicians and laboratory technicians. Methods Semi-structured interviews were conducted with dental clinicians and laboratory technicians using purposeful snowball sampling. Two trained researchers conducted the interviews based on a pre-piloted topic guide. The interviews were conducted via video conferencing platform, audio-recorded, and transcribed verbatim. Field notes were taken during the interviews. Framework Methodology was used to analyze the data. Results A total of 20 dental clinicians and laboratory technicians were interviewed. The average interview duration was 37 min. Participants mainly reported negative encounters and highlighted the importance of training and exposure, collaborative learning, and alignment of expectations of both parties in terms of cost of laboratory work, turnaround time, and patient preferences. The relationship between dental clinicians and laboratory technicians depends largely on effective teamwork dynamics and open communication channels. Increased workload, workforce shortage, availability of digital systems, management policies, and financial challenges were emphasized as organizational factors affecting the interrelationship between both groups. Participants highlighted the importance of shadowing, mentorship, education courses, joint discussions, patient-technician rapport, and adoption of digital technology for fostering collaborative practices between the professions. Conclusions A multitude of factors influencing the dental clinician-laboratory interrelationship at individual, interpersonal and organizational levels were identified. This study highlights the need to build a transformative relationship underpinned by mutual trust and respect. Such a collaborative relationship will facilitate optimal patient care and successful treatment outcomes. The outcome of this study can help stakeholders identify solutions for enhancing the interrelationship among the dental team, to ultimately improve patient care and efficiency of dental services.

Abstract Background Previous studies have shown mortality benefits with corticosteroids in Coronavirus disease-19 (COVID-19). However, there is inconsistency regarding the use of methylprednisolone over dexamethasone in COVID-19, and this has not been extensively evaluated in patients with a history of asthma. This study aims to investigate and compare the effectiveness and safety of methylprednisolone and dexamethasone in critically ill patients with asthma and COVID-19. Methods The primary endpoint was the in-hospital mortality. Other endpoints include 30-day mortality, respiratory failure requiring mechanical ventilation (MV), acute kidney injury (AKI), acute liver injury, length of stay (LOS), ventilator-free days (VFDs), and hospital-acquired infections. Propensity score (PS) matching, and regression analyses were used. Results A total of one hundred-five patients were included. Thirty patients received methylprednisolone, whereas seventy-five patients received dexamethasone. After PS matching (1:1 ratio), patients who received methylprednisolone had higher but insignificant in-hospital mortality in both crude and logistic regression analysis, [(35.0% vs. 18.2%, P = 0.22) and (OR 2.31; CI: 0.56 – 9.59; P = 0.25), respectively]. There were no statistically significant differences in the 30-day mortality, respiratory failure requiring MV, AKI, acute liver injury, ICU LOS, hospital LOS, and hospital-acquired infections. Conclusions Methylprednisolone in COVID-19 patients with asthma may lead to increased in-hospital mortality and shorter VFDs compared to dexamethasone; however, it failed to reach statistical significance. Therefore, it is necessary to interpret these data cautiously, and further large-scale randomized clinical trials are needed to establish more conclusive evidence and support these conclusions.

Background and Objectives: Patients with type 2 diabetes (T2D) have a high prevalence of non-alcoholic fatty liver disease (NAFLD) (55%) and are at increased risk for developing non-alcoholic steatohepatitis, a severe form of NAFLD. Early detection of advanced fibrosis in patients with T2D and NAFLD is crucial and can prevent progression to chronic liver disease, cirrhosis, and hepatocellular carcinoma. However, screening for liver disease and risk-stratification pathways are not established in patients with T2D. We evaluated the efficacy of using the automated fibrosis-4 (FIB-4) index in routine clinical settings to identify patients requiring further specialist evaluation. Materials and Methods: In this prospective cohort study, individuals diagnosed with T2D were recruited from diabetes clinics at a tertiary university hospital. Demographic, clinical, and laboratory data were comprehensively collected. The FIB-4 index was automatically calculated and integrated into the hospital’s electronic medical records (EMRs), which were then stratified by age. Patients with advanced fibrosis (FIB-4 index ≥ 1.3) were referred to a specialist. Student’s t-test or the Mann–Whitney U test was used to analyze variables associated with advanced fibrosis. Logistic regression was used to identify predictors of advanced fibrosis. Results: Among the 318 patients with T2D, 9.7% had advanced fibrosis. The majority were females (54.7%) and Saudi nationals (89.6%). Several factors, including age, platelet count, total bilirubin, serum albumin, total cholesterol, low-density lipoprotein, transaminases, and gamma-glutamyl transferase (GGT), showed significant associations with advanced fibrosis (all p < 0.05). Older age, elevated total bilirubin and GGT levels, and prolonged international normalized ratio emerged as independent predictors of advanced fibrosis. Conclusions: Integrating the FIB-4 index into the EMR during the routine care of patients with T2D proved to be a valuable tool in effectively identifying individuals at risk of advanced fibrosis. Our findings emphasize the need for further research to refine screening strategies in this high-risk population.

Background: The choice of prosthesis for aortic valve replacement (AVR) remains challenging. The risk of anticoagulation complications vs. the risk of aortic valve reintervention should be weighed. This study compared the outcomes of bioprosthetic vs. mechanical AVR in patients older and younger than 50. Methods: This retrospective study was conducted from 2009 to 2019 and involved 292 adult patients who underwent isolated AVR. The patients were divided according to their age (above 50 years or 50 years and younger) and the type of valves used in each age group. The outcomes of bioprosthetic valves (Groups 1a (>50 years) and 1b (≤50 years)) were compared with those of mechanical valves (Groups 2a (>50 years) and 2b (≤50 years)) in each age group. Results: The groups had nearly equal rates of preexisting comorbidities except for Group 1b, in which the rate of hypertension was greater (32.6% vs. 14.7%; p = 0.025). This group also had higher rates of old stroke (8.7% vs. 0%, p = 0.011) and higher creatinine clearance (127.62 (108.82–150.23) vs. 110.02 (84.87–144.49) mL/min; p = 0.026) than Group 1b. Patients in Group 1a were significantly older than Group 2a (64 (58–71) vs. 58 (54–67) years; p = 0.002). There was no significant difference in the NYHA class between the groups. The preoperative ejection fraction and other echocardiographic parameters did not differ significantly between the groups. Re-exploration for bleeding was more common in patients older than 50 years who underwent mechanical valve replacement (p = 0.021). There was no difference in other postoperative complications between the groups. The groups had no differences in survival, stroke, or bleeding rates. Aortic valve reintervention was significantly greater in patients ≤ 50 years old with bioprosthetic valves. There were no differences between groups in the changes in left ventricular mass, ejection fraction, or peak aortic valve pressure during the 5-year follow-up. Conclusions: The outcomes of mechanical and bioprosthetic valve replacement were comparable in patients older than 50 years. Using bioprosthetic valves in patients younger than 50 years was associated with a greater rate of valve reintervention, with no beneficial effect on the risk of bleeding or stroke.

Green approach on wastewater treatment strategies aims to promote sustainable and environmentally friendly methods for treating wastewater while also reducing the environmental impact of traditional wastewater treatment processes. Hence, the objective of this paper was to undertake a critical review of green approach on wastewater treatment strategies using standard techniques of harvesting data from secondary sources from 2015 to 2023. Information obtained reveals that energy-efficient treatment technologies, such as anaerobic digestion and membrane bioreactors, use less energy than traditional treatment technologies. Treating wastewater for reuse can reduce the demand for freshwater resources and the energy required for water treatment. Onsite wastewater treatment systems, such as septic systems and composting toilets, can reduce the amount of wastewater that needs to be transported and treated at centralized facilities. Incorporating green infrastructure, such as rain gardens and permeable pavement, into urban areas can help reduce the amount of stormwater runoff that enters the wastewater treatment system. Extracting nutrients, such as nitrogen and phosphorus, from wastewater can be used as a fertilizer for crops, reducing the need for synthetic fertilizers. Overall, these green wastewater treatment strategies aim to reduce the energy consumption, chemical usage, and environmental impact of traditional wastewater treatment processes, while promoting sustainable and efficient use of resources.

Abstract There has been a drastic decline in the number of cheetahs in the past century. Illegal pet trade poses a critical threat to the survival of the species and demands more attention. Genetic analysis of 55 cheetahs confiscated in Somaliland shows 100% as Acinonyx jubatus soemmeringii, suggesting that illegal trade subjects A. j. soemmeringii to significant pressure. Continued decline of the estimated 260–590 mature individuals qualifies this subspecies as “endangered” versus currently “vulnerable,” under criterion C2a(i) of the International Union for Conservation of Nature Red List. A population decline of ≥80% over the next 10 years or 3 generations would even fulfill criterion A3 of “critically endangered.” We urge reclassification of A. j. soemmeringii, as endangered, and appeal to the cheetah community to investigate further uplisting to critically endangered to reflect the likely extinction risk exacerbated by the illegal trade.

Background and objectives: Little is known about the implications of titanium dioxide nanoparticles (TiO2NPs) and cadmium chloride (Cd) co-exposure on the male reproductive system in mammals. As a result, this study researched the effects of oral TiO2NPs and/or Cd exposure on male reproduction and testicular functions. Additionally, a mitigation trial with co-enzyme Q10 (CoQ10) has also been conducted. Methods: In a 60-day experiment, seven experimental groups, each containing 10 male Sprague Dawley rats, were orally given distilled water (control), corn oil (vehicle control), CoQ10 (10 mg/kg b.wt), TiO2NPs (50 mg/kg b.wt), Cd (5 mg/kg b.wt), TiO2NPs + Cd, and TiO2NPs + Cd + CoQ10. Then, sperm quality, male sex hormones, oxidative stress indications, Ti and Cd testicular residues, testes and accessory gland architecture, and apoptotic and inflammatory markers in rat testes were assessed. Results: TiO2NPs and/or Cd exposure negatively impacted body weight, weight gain, testicular weights, semen quality, serum reproductive hormones, oxidative stress parameters, and Caspase-3 and tumor necrosis factor (TNF-α) immunoreactions. Histopathological changes were recorded in testicular, seminal vesicle, and prostatic tissues. Yet, co-administration of CoQ10 with TiO2NPs and Cd substantially mitigated these adverse consequences. The most notable aspect is that it effectively lowered testicular tissue Ti and Cd levels. It also improved oxidant status, hormonal profile, and sperm picture. CoQ10 minimized the testicular damage implied by histological examination. Furthermore, CoQ10 significantly diminished TiO2NPs and Cd-induced Caspase-3 and TNF-α immunoexpression in testicular tissue. Conclusion: As a result, CoQ10 could be utilized as a safe remedy to protect male reproductive physiology from TiO2NPs and Cd damage.

Abstract Background and Aims Direct‐acting antiviral agents (DAAs) have revolutionized the treatment of patients with chronic hepatitis C virus (HCV) infection, resulting in a high sustained virologic response (SVR) rate. However, the published data from the Eastern Province of Saudi Arabia are limited to small patient groups and specific DAAs used for patients with genotype‐4.(GT‐4). This study aimed to investigate the effectiveness and safety of DAAs for treating HCV infection in Saudi Arabia in a real‐life setting. Methods This retrospective study from January 2015 to December 2019 included all HCV‐infected patients who received DAAs at a tertiary university hospital in Saudi Arabia. Baseline characteristics and laboratory data were collected from health records, including HCV RNA level, genotype, and presence of liver cirrhosis or steatosis. The primary outcome was undetectable HCV RNA at 12 weeks posttreatment (SVR12). Results were stratified based on different DAAs and HCV genotypes. Treatment‐related adverse events were recorded. Statistical analyses were performed using SPSS version 25.0. Results Of the 117 patients included, 43.2% had advanced fibrosis or cirrhosis, and the majority (90.6%) were treatment‐naïve. The mean age was 50.1 ± 15.5 years, with 57.3% females. The most common genotype was GT‐4 (44.4%), followed by GT‐1 (40.2%). Most patients (64.3%) received sofosbuvir and daclatasvir ± ribavirin, while the remaining patients received various DAAs. Overall, 98.3% of the patients achieved SVR12. The therapy was well tolerated, with fatigue and headache being the most common side effects. Conclusions Treatment with DAAs is highly effective across different genotypes and various DDA regimens in the real world for treating HCV infection in the Eastern Province of Saudi Arabia, contributing to improved patient outcomes and the overall goal of HCV elimination.
