Topper (3) Keywords: Key Words Infectious disease service; Cost control; Multiresistant pathogens; Intensive care; Antibiotics Abstract: Background: A routine infectious disease service was established in January 1998 in order to optimize the antibiotic usage and prescription pattern of a neurologic intensive care unit (NICU). Methods: Treatment guidelines for the most prevalent infections wer implemented and individual antibiotic regimes were discussed at the bedside with infectious disease experts. Results: This interdisciplinary cooperation reduced the total number of antibiotics prescribed by 38.1%, from 7,789 in 1997 to 4,822 in 1998, without comprimising patient outcomes (mortality rate: 22/313 patients in 1997 vs 32/328 patients in 1998). Total patient days (2,254 days vs 2,296 days) and average length of stay in the NICU (7.2 days vs 7.0 days) were comparable. Antimicrobial expenditure decreased by 44.8% (71,680 Euros in 1997 vs 39,567 Euros in 1998). Taking into account the costs for the infectious disease service (approximately 8,000 Euros in 1998), a total saving of 24,113 Euros was made. The dramatic reduction in antibiotic usage (mainly of carbapenems) resulted in a statistically significant decreased isolation of Stenotrophomonas maltophilia (p &lt 0.05), Enterobacter cloacae (p &lt 0.05), multiresistant Pseudomonas aeruginosa (p &lt 0.05) and Candida spp. (p &lt 0.05), without any change in the infection control guidelines. Conclusion: These data show that an infectious disease service can optimize and reduce antibiotic usage. This results in a decrease in the occurrence of multiresistant gram-negative pathogens and Candida spp. in intensive care units, and, at the same time, saves costs. Abstract

Aim: To evaluate general shortcomings and faculty-specific pitfalls as well as to improve antibiotic prescription quality (ABQ) in non-ICU wards, we performed a prospective cluster trial. Methods: An infectious-disease (ID) consulting service performed a prospective investigation consisting of three 12-week phases with point prevalence evaluation conducted once per week (=36 evaluations in total) at seven non-ICU wards, followed by assessment of sustainability (weeks 37–48). Baseline evaluation (phase 1) defined multifaceted interventions by identifying the main shortcomings. Then, to distinguish intervention from time effects, the interventions were performed in four wards, and the 3 remaining wards served as controls; after assessing effects (phase 2), the same interventions were performed in the remaining wards to test the generalizability of the interventions (phase 3). The prolonged responses after all interventions were then analyzed in phase 4. ABQ was evaluated by at least two ID specialists who assessed the indication for therapy, the adherence to the hospital guidelines for empirical therapy, and the overall antibiotic prescription quality. Results: In phase 1, 406 of 659 (62%) patients cases were adequately treated with antibiotics; the main reason for inappropriate prescription was the lack of an indication (107/253; 42%). The antibiotic prescription quality (ABQ) significantly increased, reaching 86% in all wards after the focused interventions (502/584; nDf=3, ddf=1,697, F=6.9, p=0.0001). In phase 2 the effect was only seen in wards that already participated in interventions (248/347; 71%). No improvement was seen in wards that received interventions only after phase 2 (189/295; 64%). Abstract

We evaluated the influence of environmental and density-dependent factors (intraspecific and interspecific) on clutch size, brood size, and nesting success of Little Egrets (Egretta garzetta) in the Camargue of southern France. We recorded these reproductive parameters in most years from 1970 to 1998. We used a generalized linear modeling approach (model selection based on AIC) to examine the environmental effects of spring rainfall, winter temperature, and wind on these parameters. We also examined density dependence of these parameters based on the total number of Little Egrets and the total number of tree-nesting herons nesting in these mixed-species colonies. Clutch size was positively associated with rainfall and negatively associated with the number of Little Egret nests in the Camargue. Brood size was negatively associated with the number of Little Egret nests, although rainfall was only significant as an interaction effect with these two effects. Nesting success was negatively associated with the number of tree-nesting herons, the proportion of each colony consisting of Cattle Egrets (Bubulcus ibis), wind speed, and several interactions among these variables. Virtually all of the reproductive parameters that we evaluated were negatively associated with the number of Little Egret nests or the number of tree-nesting herons. Anecdotal evidence suggests that Cattle Egrets displace Little Egrets at some centrally located nest sites. Such sites are better protected from strong winds, which are a common cause of nesting failure.

Abstract

Native to the Indo-African area, the Cattle Egret Bubulcus ibis is now a cosmopolitan species, still in great range expansion. The factors affecting breeding success of the species remain however poorly known, and here we investigate the breeding biology of the Cattle Egret in a new breeding population in the Soummam Valley (Petite Kabylie, Algeria). The installation chronology and nest site selection are described, and their effect on breeding success investigated. The nesting season lasted generally from the end of March to the end of July. Clutch size, number of hatched chicks and breeding success were low compared to other breeding areas in the Mediterranean. The first Cattle Egrets arriving at the colony in March selected the highest trees and the highest nest supports close to the trunk. With new arrivals during the course of the season, a gradual decrease was observed in the height of the nests which were progressively built away from the trunk. This selective behaviour allowed the first installed pairs to have a better breeding success. This phenomenon is discussed here within the framework of territoriality and the breeding advantages of being high ranking in the ‘territorial hierarchy' for the individual.

The breeding biology of the Purple Heron, Ardea purpurea, was studied in 1997-1998 in the Camargue delta complex in southern France. The mean laying date differed significantly between colonies in only one of the two years while clutch size remained constantly low (2.9-3.4) and showed no significant inter-colony or inter-annual differences. Chick mortality was similar among colonies the same year but was significantly higher in 1997 than in 1998. Brood size, hatching rank order and laying date had a significant effect on chick growth. Analyses of food regurgitated by chicks revealed a large variety of prey categories and surprisingly few fish. Ringing data provided evidence on several occasions that young herons can colonize foreign nests to improve their conditions. Future studies should focus on the feeding ecology and on factors affecting the availability of suitable nest sites., La biologie de la reproduction du Héron pourpré, Ardea purpurea, a été étudiée en Camargue durant les années 1997-1998. La date de ponte a été significativement différente entre les colonies pour une des deux années alors que la taille de ponte, constamment faible (2.9-3.4), n’a présenté aucune différence ni entre les colonies, ni entre les années. Most species of long-distance migratory birds put on energy stores to fuel their travels. However, recent studies have highlighted the potential costs associated with carrying too much fuel, either through increased predation risk or decreased flight efficiency. Consequently, it is now widely accepted that migratory birds should carry optimal rather than maximum fuel loads. Information from 372 garganey (Anas querquedula) ringed and recaptured at least once during the same spring in the Camargue, southern France, was used to document fuelling rates of individual ducks in relation to environmental variation and individual variation in condition. On average, garganey added very little fuel stores in the Camargue (mean gain per day=0.33 g, less than 0.5% of mean body-mass in total over an average stay of 5 days). Fuelling rates were negatively correlated with body mass at capture, but it cannot be excluded that this pattern was a statistical artefact. Given their body-mass at ringing, garganey could potentially still fly long distances when they stop in the Camargue. It is therefore likely that the aim of their stay in southern France is more for resting than refuelling, a finding that may have implications for the proper management of stop-over sites. Abstract

Several pathophysiological processes cause reversible functional abnormalities before the appearance of any structural, irreversible damage, in which case the observation of these abnormalities can constitute an invaluable warning signal to prevent the occurrence of irreversible neurological sequelae. Several processes occurring in the operating room (OR) and intensive care unit (lCU) belong to this last category and justify the usefulness of clinical neurophysiology as a neuromonitoring tool during surgery and in coma. Recent technological advances offer new concepts in monitoring of brain signals. In ICU and OR, modern computer technology has recently provided a large number of sophisticated neuromonitoring systems. The dimensions of the helmet vary because it is adjustable. Therefore, the helmet takes into consideration the individual head size and shape. Electrodes for electroencephalography (EEG), stimulus-induced brain oscillations, early somatosensory EPs (SEPs), and brainstem auditory EPs (BAEPs) are generally placed according to the International 10–20 System. Improvement in EEG assessment may be obtained with optimal electrode montages and objective spectral parameters. The montages currently in use are symmetrical and favor central and occipital electrodes. Monitoring of carotid endarterectomy (CEA) is necessary when selective shunting is performed. Monitoring techniques differ in many respects, such as the physiological mechanism studied, the invasiveness of the technique, and subjective versus objective assessment.
