The present study explored the effect of co-amorphous technology in improving the dissolution rate and stability of silybin based on the puerarin-silybin co-amorphous system prepared by the spray-drying method. Solid-state characterization was carried out by powder X-ray diffraction(PXRD), polarizing microscopy(PLM), Fourier transform infrared spectroscopy(FT-IR), differential scanning calorimetry(DSC), etc. Saturated powder dissolution, intrinsic dissolution rate, moisture absorption, and stability were further investigated. The results showed that puerarin and silybin formed a co-amorphous system at a single glass transition temperature which was higher than that of any crude drug. The intrinsic dissolution rate and supersaturated powder dissolution of silybin in the co-amorphous system were higher than those of the crude drug and amorphous system. The co-amorphous system kept stable for as long as three months under the condition of 40 ℃, 75% relative humidity, which was longer than that of the single amorphous silybin. Therefore, the co-amorphous technology could significantly improve the dissolution and stability of silybin.

To evaluate the effects of Hydroxypropyl methylcellulose acetate succinate(HPMCAS MF) on absorption of silybin(SLB) from supersaturable self-nanoemulsifying drug delivery system which was pre-prepared at the early stage experiment. The cell toxicity of self-emulsifying preparation was evaluated by the MTT method, and the in vitro membrane permeability and absorption promoting effect of the self-emulsifying preparation were evaluated by establishing a Caco-2 cell monolayer model. The in vivo and in vitro supersaturation correlation was evaluated via the blood concentration of SLB. The results of MTT showed that the concentration of the preparation below 2 mg·mL~(-1)(C_(SLB) 100 μg·mL~(-1)) was not toxic to Caco-2 cells, and the addition of polymer had no significant effect on Caco-2 cells viability. As compared with the solution group, the transport results showed that the P_(app)(AP→BL) of the self-emulsifying preparation had a very significant increase; the transport rate of silybin can be reduced by polymer in 0-30 min; however, there was no difference in supersaturated transport between supersaturated SLB self-nanoemulsion drug delivery system(SLB-SSNEDDS) and SLB self-nanoemulsion drug delivery system(SLB-SNEDDS) within 2 hours. As compared with SLB suspension, pharmacokinetic parameters showed that the blood concentration of both SLB-SNEDDS and SLB-SSNEDDS groups were significantly increased, and C_(max) was 5.25 times and 9.69 times respectively of that in SLB suspension group, with a relative bioavailability of 578.45% and 1 139.44% respectively. C_(max) and relative bioavailability of SLB-SSNEDDS were 1.85 times and 197% of those of SLB-SNEDDS, respectively. Therefore, on the one hand, SSNEDDS can increase the solubility of SLB in gastrointestinal tract by maintaining stability of SLB supersaturation state; on the other hand, the osmotic transport process of SLB was regulated through the composition of its preparations, and both of them could jointly promote the transport and absorption of SLB to improve the oral bioavailability of SLB.

To verify the appropriate preparation process of extracts for the solid substance benchmark of Linggui Zhugan Decoction. The extracts were prepared by different preparation processes, namely the traditional process(process 1), the extract combined with volatile oil separated from traditional process extract liquid(process 2), the modern secondary reflux extraction process(process 3) and the process that volatile oil was extracted first, then prepared according to the traditional process, and combined with extract(process 4); based on the characteristic spectrum, index components of cinnamaldehyde, glycyrrhizin, ammonium glycyrrhizinate, cinnamic acid, and the dry extract rate of process 1, the differences and similarities of four extracts were compared. The results showed that the similarity of the characteristic spectrum of process 2, process 4 and process 1 were all greater than 0.97, while there was no significant difference for the content of 4 quality control components and dry extract rate; the similarity of the characteristic spectrum of process 3 and process 1 was 0.91, the absolute peak area of 13 out of 21 peaks and the relative peak area of 7 peaks increased significantly, and the content of 3 out of 4 quality control components and dry extract rate also significantly increased. In conclusion, the material standards of extracts by the process 2 and 4 are consistent with that of the traditional process, so the two processes are suitable.

In order to improve the supersaturation and maintenance time of drug dispersion in curcumin self-nanoemulsion(CUR-SNEDDS), precipitation inhibitors(PPIs) were introduced to prepare curcumin supersaturated self-emulsion(CUR-SSNEDDS). The composition of CUR-SNEDDS prescriptions was selected through the solubility test, the compatibility of oil phase and surfactant, the investigation of the emulsifying ability of the surfactant and the drawing of the pseudo-ternary phase diagram. Analytic hierarchy process was used in combination with central composite design-response surface method to optimize the prescription. The type and dosage of precipitation inhibitors(PPIs) were selected to maintain the supersaturated concentration and duration of CUR in artificial gastrointestinal fluids. At the same time, polarizing microscope was used to evaluate the crystallization inhibition effect and the quality and in vitro release behavior of CUR-SSNEDDS. The prepared CUR-SSNEDDS prescription was capryol 90-kolliphor RH40-transcutol HP-Soluplus(7.93∶66.71∶25.36∶5), with the drug loading of(65.12±1.25) mg·g~(-1). CUR-SSNEDDS was transparent yellow, and the nanoemulsion droplets were spherical with uniform distribution. The emulsification time was(21.02±0.13) s, the average particle size was(57.03±0.35) nm, the polydispersity index(PDI) was(0.23 ± 0.01), and the Zeta potential was(-18.10±1.30) mV. CUR-SSNEDDS significantly inhibited the generation and growth of crystals after in vitro dilution. The supersaturation could be maintained above 10 within 2 h, and the dissolution rate and degree of CUR in artificial gastrointestinal fluid were significantly increased. Soluplus could effectively maintain the supersaturated state of CUR and enhance CUR dissolution in vitro.

To improve the performance of recycled low-density polyethylene (LDPER), this paper describes the polymer modification of bitumen with gamma-irradiated recycled low-density polyethylene (γ-LDPER). The effects of the γ-LDPER on bitumen were investigated by means of morphological and chemical analyses, including Fourier-transform infrared spectroscopy and multiple stress creep recovery (MSCR) test. The results showed that the surface of the LDPER treated by gamma beam irradiation led to the formation of free radicals and some functional groups that may contribute to the creation of strong chemical bonds between the polymer modifier and bitumen, thus improving the compatibility of the modified asphalt. With the increased use of γ-LDPER, the modified asphalt rutting resistance also increased. However, due to the high γ-LDPER content of the modified asphalt, undertaking the high-temperature performance evaluation of the applicability of MSCR deserves further study.

Human immunodeficiency virus (HIV)-infected long-term non-progressors (LTNPs) are of particular importance because of their unique disease progression characteristics. Defined by the maintenance of normal CD4+T cells after more than 8 years of infection, these LTNPs are heterogeneous. Some LTNPs exhibit ongoing viral production, while others do not and are able to control viral production. The underlying basis for this heterogeneity has not been clearly elucidated. In this study, the miRNA expression profiles of LTNPs were assessed. The levels of microRNA-19b (miR-19b) were found to be significantly increased in peripheral blood mononuclear cells of LTNPs with lower rather than higher viral load. We made clear that miR-19b may regulate CD8+T cell functions in HIV infection, which has not been addressed before. Overexpression of miR-19b promoted CD8+T cell proliferation, as well as interferon-γ and granzyme B expression, while inhibiting CD8+T cells apoptosis induced by anti-CD3/CD28 stimulation. The target of miR-19b was found to be the “phosphatase and tensin homolog”, which regulates CD8+T cells function during HIV infections. Furthermore, we found that miR-19b can directly inhibit viral production in in-vitro HIV infected T cells. These results highlight the importance of miR-19b to control viral levels, which facilitate an understanding of human immunodeficiency virus pathogenesis and provide potential targets for improved immune intervention.

To improve the diagnosis and treatment of Penicilliosis marneffei without human immunodeficiency virus infection. Analyze and review the clinical features, diagnosis and treatment of six cases of P. marneffei without human immunodeficiency virus infection at The First Affiliated Hospital of Fujian Medical University. Two cases were diagnosed in the ENT Department, three cases in the respiratory department and one case in the dermatological department. Penicillium marneffei infection was confirmed by sputum culture, blood culture and tissue biopsy. After definite diagnosis, one refused further treatment, and others showed significant improvement. Penicilliosis marneffei is insidious onset and easy to be escaped and misdiagnosed. To achieve early diagnosis and appropriate treatment, doubtful cases should be alerted for the diagnoses as P. marneffei.

To investigate the molecular subtypes of prevalent HIV-1 strains and characterize the genetics of dominant strains among men who have sex with men. Molecular epidemiology surveys in this study concentrated on the prevalent HIV-1 strains in Liaoning province by year. 229 adult patients infected with HIV-1 and part of a high-risk group of men who have sex with men were recruited. Reverse transcription and nested PCR amplification were performed. Sequencing reactions were conducted and edited, followed by codon-based alignment. NJ phylogenetic tree analyses detected two distinct CRF01_AE phylogenetic clusters, designated clusters 1 and 2. Clusters 1 and 2 accounted for 12.8% and 84.2% of sequences in the pol gene and 17.6% and 73.1% of sequences in the env gene, respectively. Another six samples were distributed on other phylogenetic clusters. Cluster 1 increased significantly from 5.6% to 20.0%, but cluster 2 decreased from 87.5% to 80.0%. Genetic distance analysis indicated that CRF01_AE cluster 1 in Liaoning was homologous to epidemic CRF01_AE strains, but CRF01_AE cluster 2 was different from other scattered strains. Additionally, significant differences were found in tetra-peptide motifs at the tip of V3 loop between cluster 1 and 2; however, differences in coreceptor usage were not detected. This study shows that subtype CRF01_AE strain may be the most prevalent epidemic strain in the men who have sex with men. Genetic characteristics of the subtype CRF01_AE cluster strain in Liaoning showed homology to the prevalent strains of men who have sex with men in other parts of China.

To assess the effect that hormonal disturbances have on HIV prognosis in male patients. A prospective follow-up study was conducted among male HIV patients who started antiretroviral therapy (ART) between July 1, 2011 and June 30, 2014. The final follow-up session occurred before December 31, 2014. We examined the correlation between pre-ART hormone levels and disease prognosis. The Kaplan–Meier method and the multivariate Cox proportional hazard model were used to identify hormone-related predictors of immunological failure and mortality. During the follow-up of 163 male HIV patients, mortality rate occurred at a rate of 16.0% (26/163). Of these deaths, 84.6% (22/26) were acquired immunodeficiency syndrome–related. Furthermore, 53 patients were found to have suffered from immunological failure. Both pre-ART CD4+ T cell counts and the clinical stage assigned to the patients correlated strongly with dehydroepiandrosterone sulfate levels. Hyponatremia, high cortisol levels, tuberculosis, and being at World Health Organization (WHO)-defined clinical stage 4 were characteristics that associated significantly with mortality. Being at WHO clinical stage 4 was, itself, a factor that significantly associated with immunological failure. High cortisol levels were found to be an important hormonal disorder that associated with mortality. None of the hormones examined in this study had a strong correlation with immunological failure.

To analyze the effects of long term nasal continuous positive airway pressure on the blood pressure of patients with OSAHS.From April 1997 to October 2008, 2898 patients from the First Affiliated Hospital of Fujian Medical University who complained snore during sleeping were studied. Nine hundred eighty cases were diagnosed as OSAHS with hypertension, and these patients were randomly divided into 2 groups: one group was treated with antihypertensive drugs and nasal continuous positive airway pressure (nCPAP), while the other group only received antihypertensive drugs. The polysomnography (PSG) was recorded during sleeping and the blood pressure was remeasured after 6 months or more. All patients were followed up for 5 years to observe the long-term effects of nCPAP or drugs.Systolic and diastolic blood pressure in the nCPAP group significantly decreased after 6 months [(125 ± 16) mm Hg (1 mm Hg = 0.133 kPa) vs (136 ± 19) mm Hg, (83 ± 10) mm Hg vs (95 ± 15) mm Hg, P0.05], and the decreasing extent of blood pressure in nCPAP group was more notable than antihypertensive drug group [decreasing extent of systolic blood pressure:(10 ± 11) mm Hg vs (4 ± 11) mm Hg; decreasing extent of diastolic blood pressure: (11 ± 7) mm Hg vs (6 ± 7) mm Hg; P0.05]. The total effective rate in nCPAP group was significantly higher than that in antihypertensive drug group (90% vs 38%, P0.01). One hundred and eighty three cases in nCPAP group and 157 cases in antihypertensive drug group completed the 5-year follow-up and the blood pressure was controlled within the normal range. Some patients could gradually reduce or stop the use of antihypertensive drugs and the blood pressure didn't appear to rebound. The number of antihypertensive drugs in the nCPAP group was significantly fewer as compared to the antihypertensive drugs group after 2, 3, 4 and 5 years' follow-up.nCPAP is a safe and effective treatment for high blood pressure in patients with OSAHS.

To investigate the prevalence and risk factors of obstructive sleep apnea-hypopnea syndrome (OSAHS) in adults aged over 20 years in Fuzhou city, there fore to provide epidemiological data for prevention and treatment of the disease, and establishing a data base for prospective study.A total of 5500 subjects were derived from a random and cluster sampling of the population in 5 districts of Fuzhou city. They were asked to answer the questions from a questionnaire at home. According to the degree of snoring, 315 subjects with a snoring scoreor = 3 degree and 100 subjects with a snoring score = 2 degree were selected at random to undergo polysomnography for a whole night. The prevalence of the disease was estimated and the risk factors for OSAHS were analyzed.4595 subjects (83.55%) responded, and validated questionnaires were obtained from 4286 subjects (effective power 93.28%); of whom 606 (14.14%) subjects had habitual snoring. The estimated prevalence of OSAHS defined by apnea-hypopnea index (AHI)or = 5 and Epworth sleepiness scale (ESS)or = 9 was 4.78%. Multivariate analysis revealed that age, smoking, family snoring, neck-circumference, waist circumference, and abnormality of the upper airway were significant risk factors for OSAHS.The estimated prevalences of snoring and OSAHS in adults aged over 20 years in Fuzhou city was high. Strategies based on the epidemiological data in Fuzhou city are needed to cut down the prevalence and harm of OSAHS by controlling modifiable risk factors.

To investigate the functional states of peripheral blood monocytes/macrophages and their relation with the disease progression in people living with HIV/AIDS (PWHA).Peripheral blood samples were collected from 57 PWHA who didn't receive antivirus treatment before and 32 healthy controls in Jilin, Liaoning, and Henan provinces. Flow cytometry was used to detect the expression of the early activation molecule CD69 in the CD14(+) cells. The plasma HIV RNA level was determined using real-time PCR.The CD69 expression rate in the CD14(+) T cells of the PWHA group was 27% +/- 4%, significantly higher than that of the controls (P0.01). The CD69 expression rate of the AIDS group was 39% +/- 3%, significantly higher than those of the HIV group and long-term nonprogressors (LTNP) group (26% +/- 3% and 20% +/- 4% respectively, both P0.05), that of the HIV groups being significantly higher than that of the LTNP group too (P0.05). The side scatter value of the CD14(+) T cells of the PWHA group was 76 +/- 16, significantly higher than that of the control group (50 +/- 12, P0.05). The monocyte/macrophage CD69 expression rate was significantly negatively correlated with the absolute value of CD4(+) T cells (r = -0.872, P0.01), and not correlated with the value of CD8(+) T cells (P0.05). The monocyte/macrophage CD69 expression rate was significantly positively correlated with the HIV-1 RNA viral load (r = 0.697, P0.01).The activation and the phagocytosis function of the monocytes/macrophages of the PWHA in China are higher than those of the healthy control. They are positively correlated with the disease progression.
