7. A rare variation: duplication of inferior vena cava.

Objective: The inferior vena cava is one of the largest single veins that carries the venous blood of most of the structures below the diaphragm to the heart. This vein is formed by the union of three different embryological origins and four pairs of embryological veins. Changes that occur at any stage of this complex development process can lead to the emergence of variations. Duplication of the inferior vena cava is one of these rare congenital vascular variations. According to the literature, its incidence is between 0.2% and 3% and it is usually asymptomatic. Methods: Here, we will present the duplicated inferior vena cava, which was seen incidentally on computerized tomography images of two male patients, one 32 years old with lumbar vertebral fracture and the other 50 years old with lumbar disc herniation. Results: Today, this variation can be detected better with the development of non-invasive imaging techniques. However, it has been reported that the variation is overlooked and evaluated as a mass lesion or lymphadenopathy, which may lead to misdiagnosis, and therefore, there are cases where unnecessary surgical intervention was performed. Conclusion: Knowledge of vascular anatomical variations is very important in preventing vital complications after retroperitoneal surgeries, venous interventional radiological procedures, deep vein thrombosis management and incomplete lymph node dissection. 8. Two anatomical variations of the craniovertebral region: paracondylar process and epitransvers process.

Objective: Congenital anomalies of different characteristics with common embryological origin can be seen at the craniovertebral junction. Paracondylar process and epitransvers process are two of them. Paracondylar process is a bony exostosis that is located lateral to the occipital condyle and develops in the form of a cone towards the transvers process of the atlas. The mirror image of this, the bony prominence rising from the transvers process of the atlas to the skull base is called the epitransvers process. The incidence of these variations is between 0.077%-0.29% according to the literature. However, the incidence of paracondylar process is reported to be 1-2%. Methods: Left unilateral paracondylar process was detected in these patients after computed tomography, one of which was taken to a 40-year-old female patient who came with the reason of falling, and the other to a 56-year-old male patient who applied to the emergency department with the diagnosis of stroke. Bilateral epitransvers process was detected after computerized tomography in a 38-year-old female patient who presented with the complaint of vertigo. In this study, variations of these three cases will be presented. Results: These rare anatomical variations are usually asymptomatic, as in the cases in our study, and are usually detected incidentally. However, rarely, the wide paracondylar process may fuse or articulate with the processus transversus of the atlas, restricting the atlanto-occipital movement, and the variation may become symptomatic. This may cause occipitocervical pain, functional limitations in head and neck movement, and even torticollis. Conclusion: Determining these variations before planned surgeries in the craniocervical region is important for safe and distant lateral approaches. [ABSTRACT FROM AUTHOR]

Amaç: Bu çalışmada BT incelemesi yapılan hastalarda iyotlu kontast maddenin kontrast öncesi ve sonrası, böbrek ADC değerleri üzerindeki etkisini değerlendirmeyi amaçladık.Materyal-Metod: Temmuz 2015 tarihinden itibaren bölümümüze BT incelemesi için başvuran 40 yaş üstü hasta ve kontrol grubundan dahil olma ve dışlama kriterlerine uygun olanlar çalışmaya alındı. BT incelemesinden hemen önce ve 48-72 saat sonra iki kez böbreklere yönelik DAG-MRG(b=0,b=800) incelemesi yapıldı. üre-kreatinin değeri BT incelemesi öncesinde ve ikinci MRG çekiminin yapıldığı gün olacak şekilde iki kez bakıldı. DAG-MRG görüntüleri dört ve on yıllık deneyimli iki radyolog tarafından aynı seansta değerlendirildi. Her iki böbrek için kontrast öncesi ve sonrası üst, orta, alt kesimden ADC ölçümleri yapıldı. ROI 1.5 cm³'den küçük olacak şekilde ayarlandı.Bulgular: İki DAG-MRG'si olan 23 hasta, 25 kontrol grubu çalışmaya dahil edildi. Her iki böbrek için kontrast öncesi ve sonrası ortalama ADC değerleri karşılaştırıldığında kontrol grubunda kontrast sonrası ADC değerlerinde anlamlı düşme saptandı(sağ böbrek kontrast öncesi ADC: 2,11±0,17 x10ˉ³, sonrası 2,07±0,15 x10ˉ³ mm²/sn, p=0,016; sol böbrek için kontrast öncesi ADC: 2,11±0,17 x10ˉ³ mm²/sn, sonrası 2,04±0,14 x10ˉ³ mm²/sn, p=0,011). Hasta grubunda ise kontrast öncesi ve sonrası anlamlı farklılık izlenmedi(sağ böbrek için kontrast öncesi ADC: 1,97±0,22 x10ˉ³ mm²/sn, sonrası 1,97±0,24 x10ˉ³ mm²/sn, p=0,95; sol böbrek için kontrast öncesi 1,96±0,23 x10ˉ³ mm²/sn, sonrası 1,98±0,22 x10ˉ³ mm²/sn, p=0,64 ölçüldü).Ayrıca hasta ve kontrol grubunda ADC değerleri karşılaştırıldığında hasta grubunda kontrast öncesi ADC değerleri anlamlı düşük bulundu(Hasta ve kontrol grubunda sağ böbrek için kontrast öncesi değerleri sırası ile 1,97±0,22 x10ˉ³ mm²/sn, 2,11±0,17 x10ˉ³ mm²/sn p=0,016, sol böbrek için kontrast öncesi değerleri 1,96±0,23x10ˉ³ mm²/sn, 2,11±0,17 x10ˉ³ mm²/sn, p=0,018). Kontrast sonrası ise anlamlı farklılık izlenmedi.SONUÇ: ADC değerlerinde İyotlu non-iyonik kontrast madde kullanımı öncesine göre GFR'si 60'ın altında olanlarda değişiklik izlenmezken, GFR'si normal olanlarda anlamlı düşüş meydana gelmiştir.Anahtar sözcükler: ADC, GFR, DAG-MRG, Bilgisayarlı Tomografi, Kontrast madde aim: In this study, we aim to evaluate the efect of iodinated contrast agents to kidney ADC values, before and after the administration, on patients underwent Computed Tomography (CT) examination.Material-Methodology: Among the patients who applied to our department for CT examination, those were above 40 years old and found suitable for the exclusion and inclusion criterias to patient and control group, were taken into study. Diffusion weighted imaging (DWI)(b=0,b=800) was done before and 48-72 hours after the CT targeted at the kidneys. The value of urea-creatinin was measured twice before CT and on the day of DWI. DWI were evaluated by two experienced radiologists, one having four years and the latter having ten years of experience, at the same session. Apparent Diffusion coefficient (ADC) measures were taken from both kidneys before and after the contrast; and from upper, middle and lower part. Range of interest (ROI) was adjusted to be less than 1.5 cm².Findings: 23 patients and 25 control group. with two DWI were included in our study. ADC values before and after the contrast agent administration for both kidneys were compared and a significant decrease were detected in ADC values in the control group after administration (right kidney ADC before the administration: 2,11±0,17 x10ˉ³, after 2,07±0,15 x10ˉ³ mm²/sn, p=0,016; left kidney ADC before the administration: 2,11±0,17 x10ˉ³ mm²/sn, after 2,04±0,14 x10ˉ³ mm²/sn, p=0,011). WOS:000429700300012 PubMed: 29670827 Background: Brucellosis is an important multisystemic disease with many different clinical symptoms, and its early diagnosis and treatment are possible. Neurobrucellosis (NB) is a rare but serious finding of brucellosis. Brucella can be seen as meningitis and encephalopathy, and it can cause cranial nerve pathologies, vascular syndromes, myopathy, spinal diseases, and psychiatric disorders. In NB, vascular syndromes secondary to inflammation are rarely seen. Here, we present nine young patients with vascular and nonspecific neuropsychiatric findings who had NB as the etiology of stroke. Methods: Nine patients who were admitted to our Neurology Clinic between 2012 and 2017 for various reasons in whom brucellosis was found in the etiology were retrospectively studied. The patients' symptoms, physical examination, laboratory and radiographic findings, treatments, and treatment responses are discussed. Results: Of the nine patients who presented to our clinic in the 4-year period, five were female. The average age was 49 years. Five patients had small vessel vasculitis, three had great vessel vasculitis, and one had meningoencephalitis and pons abscess. Two patients had granuloma, and one had an aneurysm. Conclusions: We aimed to present our cases due to the fact that this disease should be kept in mind in the differential diagnosis of patients with stroke and similar neuropsychiatric findings.
