This study compared two types of therapies, CBT and Brief Relational Therapy, as applied to teenagers with depression. The comparisons were made in order to analyze whether adherence to the therapeutic method and handling of problems within the therapeutic alliance affects the extent of depressive symptoms, and in what way adherence and alliance interacts with each other in the therapeutic methods. Assessments of adherence and alliance were made by observing short sequences of each therapy session. A positive correlation was noted regarding the interaction of observer-rated adherence and alliance for Brief Relational Therapy but no such correlation was found for therapist and patient rated alliance. No correlations were found between adherence or alliance and change in depressive symptoms. Thus, no statistically reliable results were found to confirm our hypothesis. This could be due to limited statistical power. However, this study addresses the need to further investigate therapeutic mechanisms in psychotherapy and raises the question of in comparing therapeutic methods to find answers to therapeutic outcomes., Denna uppsats har haft som utgångspunkt att i två tonårsterapier, KBT och Relationell korttidsterapi, via observatörskattningar av terapisamtal uppdelade i korta sekvenser undersöka huruvida följsamhet till metod bidrar till utfall av depressionssymtom vid nästkommande samtal, huruvida hantering av alliansproblem bidrar till utfall av depressionssymtom vid nästkommande samtal och hur följsamhet och allians interagerar i respektive metod. Det gick inte att slå fast att följsamhet till metod eller grad av allians påverkade utfall. Avseende interaktion mellan följsamhet och observatörsskattad allians noterades en positiv korrelation vid behandling med Relationell korttidsterapi. Ingen korrelation noterades avseende följsamhet och allians skattad av terapeut och/eller patient. Det var följaktligen svårt att ge tydliga svar på uppsatsens frågor vilket möjligen beror på det begränsade underlaget. Arbetet tar dock upp frågor om vad som är psykoterapis verksamma mekanismer och belyser att dessa möjligen inte låter sig besvaras genom att enbart undersöka skillnad mellan olika metoder. Större forskningsstudier med målsättning att på nationell nivå vara vägledande för val av metod skulle sannolikt vinna på att inkludera studie av följsamhet, när och hur interventioner initieras och vilka generella faktorer som har betydelsefullt förklaringsvärde.
