Résumé: Résumé: Les données d’enregistrement du cancer sur 20 années à partir du registre des tumeurs de Sétif fournit aux professionnels de la santé, aux chercheurs et aux décideurs des informations valides et détaillées sur l’incidence des cancers les plus courants du registre du cancer de Sétif, selon l’âge, le sexe, la période de diagnostic et la répartition géographique, avec des comparaisons sur la base des taux standardisés avec les registres nationaux et des cinq continents. La tendance des incidences des cancers sur 20 ans montre des résultats intéressants. Les données de survie fournies par le registre reflètent l’efficience du système de prise en charge du cancer. Matériel et méthodes: Il s’agit d’un enregistrement prospectif sur le registre de population de Sétif de 1986 à 2005. Les données sont recueillies, saisies, contrôlées et analysées par le logiciel Can reg Version 4 élaboré par l’unité d’épidémiologie descriptive du centre international de recherche sur le cancer (CIRC) de Lyon. L’étude de survie est réalisée avec Concord Study portant sur les sujets âgés de 15 à 99 ans durant la période 1990–1994, sur les cancers du sein, colorectaux et de la prostate. Résultats: Pour toutes localisations et tous sexes confondus, les taux d’incidence suivent graduellement une courbe ascendante durant cette période. Les cancers bronchopulmonaires se situent au premier rang du registre du cancer de Sétif, ayant une incidence en hausse passant de 11,7 à 21,9 pour 100 000 personnes par an chez l’homme. Le cancer de la prostate vient en seconde position avec une tendance croissante passant de 2 en 1986 à 7,2 pour 100 000 hommes par an en 2005. Les hausses sont observées aussi pour les cancers colorectaux chez les deux sexes. Entre 1986 et 2005, le nombre de nouveaux cas passe respectivement de 23 en 1986 à 176 cas en 2005 pour les hommes et de 17 à 149 cas pour les femmes. Le taux d’incidence du cancer de la vessie est aussi en augmentation passant de 2,2 en 1986 à 8,7 pour 100 000 personnes par an chez l’homme, alors que les taux d’incidence des cancers du nasopharynx, de l’estomac, des lymphomes et des leucémies sont restés stables au cours de ces deux dernières décennies. Chez la femme, les taux d’incidence du cancer du sein sont en nette augmentation, occupant la première place, avec des taux passant de 10,4 à 19,6 pour 100 000 femmes par an. Le nombre de nouveaux cas a pratiquement doublé en 20 ans, il est passé de 187 cas en 1986 à 391 cas en 2005. On note également une augmentation du taux d’incidence du cancer de la thyroïde chez la femme, passant de 1,1 en 1986 à 4,3 pour 100 000 personnes par an en 2005. Par contre, les taux sont stables pour le cancer du col de l’utérus, des voies biliaires et des lymphomes. Les données de survie relative à cinq ans ne dépassant pas 40 % d’après la dernière étude Concorde 1990–1994 pour les principales localisations du registre, les taux de survie relative à cinq ans des cancers du rectum, colorectaux et prostate chez l’homme sont respectivement de 25,9; 18,2 et 19,6 %. La survie relative à cinq ans chez la femme des cancers du sein, du côlon, du rectum et colorectaux sont respectivement de 38,5, 30,6, 22,5 et 22,6 %. Le taux global d’incidence du cancer chez les enfants est demeuré relativement stable depuis 1986. La tranche d’âge 0–4 ans est prédominante et l’enfant de sexe masculin est plus touché. Conclusion: Le cancer constitue actuellement un des problèmes majeurs de santé publique en Algérie. Avec une nette augmentation des taux d’incidence des cancers liés au tabagisme chez l’homme et au cancer du sein, de la thyroïde et colorectal chez la femme. La survie est faible car elle est liée à une accessibilité aux soins médiocre et une prise en charge tardive.

Background Cancer is a major cause of death in children worldwide, and the recorded incidence tends to increase with time. Internationally comparable data on childhood cancer incidence in the past two decades are scarce. This study aimed to provide internationally comparable local data on the incidence of childhood cancer to promote research of causes and implementation of childhood cancer control. Methods This population-based registry study, devised by the International Agency for Research on Cancer in collaboration with the International Association of Cancer Registries, collected data on all malignancies and nonmalignant neoplasms of the CNS diagnosed before age 20 years in populations covered by high-quality cancer registries with complete data for 2001-10. Incidence rates per million person-years for the 0-14 years and 0-19 years age groups were age-adjusted using the world standard population to provide age-standardised incidence rates (WSRs), using the age-specific incidence rates (ASR) for individual age groups (0-4 years, 5-9 years, 10-14 years, and 15-19 years). All rates were reported for 19 geographical areas or ethnicities by sex, age group, and cancer type. The regional WSRs for children aged 0-14 years were compared with comparable data obtained in the 1980s. Findings Of 532 invited cancer registries, 153 registries from 62 countries, departments, and territories met quality standards, and contributed data for the entire decade of 2001-10. 385 509 incident cases in children aged 0-19 years occurring in 2.64 billion person-years were included. The overall WSR was 140.6 per million person-years in children aged 0-14 years (based on 284 649 cases), and the most common cancers were leukaemia (WSR 46.4), followed by CNS tumours (WSR 28.2), and lymphomas (WSR 15.2). In children aged 15-19 years (based on 100 860 cases), the ASR was 185.3 per million person-years, the most common being lymphomas (ASR 41.8) and the group of epithelial tumours and melanoma (ASR 39.5). Incidence varied considerably between and within the described regions, and by cancer type, sex, age, and racial and ethnic group. Since the 1980s, the global WSR of registered cancers in children aged 0-14 years has increased from 124.0 (95% CI 123.3-124.7) to 140.6 (140.1-141.1) per million person-years. Interpretation This unique global source of childhood cancer incidence will be used for aetiological research and to inform public health policy, potentially contributing towards attaining several targets of the Sustainable Development Goals. Several factors are involved in the development of colon cancer. These factors are characterized by complexity and uncertainty. The system is very complex to analyze using classical mathematical methods. Statistical analysis techniques are used. Due to the complexity of the system, these studies remain in the domain of the probable and the uncertain. A technique based on the principles of artificial intelligence is proposed. The principles of fuzzy inference are applied to the analysis of these factors. The proposed system makes it possible to take care of these incertitude’s inherent to the input variables. The result will be as accurate as possible. Just randomly set the input variables to instantly read the result at the output. This can be a tool to predict and prevent the occurrence of this type of cancer.

Cancer is one of the major public health problems in Algeria. In the last 25 years, a significant increase in the incidence of the major types of cancers has been observed in both sexes. Moreover, the 5-year survival rate is low for the severe tumors due to a difficulty in access to cancer care and an incomplete health care framework. Cancer Registry of Setif, Algeria, has been recording cancer incidence, mortality, and survival since 1986 in collaboration with International Agency for Research on Cancer (IARC) of Lyon. Cancer Registry of Setif is being a source of information for cancer planning and corresponding surveillance in the National Cancer Plan 2015-2019, starting in January 2015. Data is recorded by means of CanReg 5 software. This software is developed and provided by the International Agency for Research on Cancer (IARC) of Lyon. It is designed specifically for cancer registration, and standardized to capture, control, and process the data. Estimation of cancer incidence in Algeria and survival rates are very important for surveillance, control, and planning of care. In men the incidence of lung, colorectal, bladder, prostate, and laryngeal cancers has significantly and steadily increased in the last decade. In women, the incidence of breast, colorectal, thyroid, and lung cancers has also increased significantly in the same period. Five-year survival rates for cancer of the stomach, colon, rectum, liver, lung, breast, cervix, ovary, and prostate in adults, and childhood leukemia are relatively low compared with other countries. The aim of our study was to estimate incidence and survival by means of Setif cancer registry data.

The work presented in this paper is to combine the Sumudu transform method with a variational iteration method for solving linear and nonlinear partial differential equations with local fractional derivative. We apply the proposed method to obtain approximate analytical solutions of Klein–Gordon equations with local fractional derivative. The results obtained for the local fractional Klein–Gordon equation in the three cases presented, are in the form of non-differentiable functions. Through this work it can be said that this method is an alternative analytical method for linear and nonlinear local fractional partial differential equations. [ABSTRACT FROM AUTHOR]

18. Aligning domestic policies with international coordination in a post-Paris global climate regime: A case for China

© 2017 Elsevier Inc. The Paris COP-21 reached a climate agreement on 2-degree global emissions stabilisation target. However, the likelihood of successfully implementing a legally binding global climate treaty will depend to a large degree on the macroeconomic impacts of different policy options on developing and emerging economies. It is thus crucial to evaluate the transition costs of implementing different climate policy architectures under socioeconomic and technological uncertainties from a multiregional perspective. Here we use a hybrid computable general equilibrium model, in which sub-optimalities, infrastructural inertia and technological uncertainties are explicitly introduced, to quantify the trajectories of variation in transitions costs under a range of climate policy scenarios in both Annex I and developing nations. The policy architectures are based around the implementation of the 'streamlined Paris Pledges' (SPP) in developing countries with a particular focus on China and India. Our results indicate that the distributional effects should be taken into account by policymakers through extension of SPP to 2050 as global climate policies may have asymmetrical economic impacts between Annex I and developing countries. A first-best policy of global cap-and-trade scheme alone could be welfare-deteriorating for some parties, reflected by a significant reduction in macroeconomic growth rate over the course of the next decades. Modelling results also suggest that articulating both global and national policies in a multiregional climate deal can provide a palatable solution for countries like China as this would allow for significant reduction in the economic losses associated with a unique-carbon-price global climate policy. © 2014 The Authors. This study explores a situation of staged accession to a global climate policy regime from the current situation of regionally fragmented and moderate climate action. The analysis is based on scenarios in which a front runner coalition - the EU or the EU and China - embarks on immediate ambitious climate action while the rest of the world makes a transition to a global climate regime between 2030 and 2050. We assume that the ensuing regime involves strong mitigation efforts but does not require late joiners to compensate for their initially higher emissions. Thus, climate targets are relaxed, and although staged accession can achieve significant reductions of global warming, the resulting climate outcome is unlikely to be consistent with the goal of limiting global warming to 2 degrees. The addition of China to the front runner coalition can reduce pre-2050 excess emissions by 20-30%, increasing the likelihood of staying below 2 degrees. Not accounting for potential co-benefits, the cost of front runner action is found to be lower for the EU than for China. Regions that delay their accession to the climate regime face a trade-off between reduced short term costs and higher transitional requirements due to larger carbon lock-ins and more rapidly increasing carbon prices during the accession period.

This paper provides an overview of the AMPERE modeling comparison project with focus on the implications of near-term policies for the costs and attainability of long-term climate objectives. Nine modeling teams participated in the project to explore the consequences of global emissions following the proposed policy stringency of the national pledges from the Copenhagen Accord and Cancún Agreements to 2030. Specific features compared to earlier assessments are the explicit consideration of near-term 2030 emission targets as well as the systematic sensitivity analysis for the availability and potential of mitigation technologies. Our estimates show that a 2030 mitigation effort comparable to the pledges would result in a further "lock-in" of the energy system into fossil fuels and thus impede the required energy transformation to reach low greenhouse-gas stabilization levels (450ppm CO2e). Major implications include significant increases in mitigation costs, increased risk that low stabilization targets become unattainable, and reduced chances of staying below the proposed temperature change target of 2°C in case of overshoot. With respect to technologies, we find that following the pledge pathways to 2030 would narrow policy choices, and increases the risks that some currently optional technologies, such as carbon capture and storage (CCS) or the large-scale deployment of bioenergy, will become "a must" by 2030.

This paper explores a multi-model scenario ensemble to assess the impacts of idealized and non-idealized climate change stabilization policies on fossil fuel markets. Under idealized conditions climate policies significantly reduce coal use in the short- and long-term. Reductions in oil and gas use are much smaller, particularly until 2030, but revenues decrease much more because oil and gas prices are higher than coal prices. A first deviation from optimal transition pathways is delayed action that relaxes global emission targets until 2030 in accordance with the Copenhagen pledges. Fossil fuel markets revert back to the no-policy case: though coal use increases strongest, revenue gains are higher for oil and gas. To balance the carbon budget over the 21st century, the long-term reallocation of fossil fuels is significantly larger -- twice and more -- than the short-term distortion. This amplifying effect results from coal lock-in and inter-fuel substitution effects to balance the full-century carbon budget. The second deviation from the optimal transition pathway relaxes the global participation assumption. The result here is less clear-cut across models, as we find carbon leakage effects ranging from positive to negative because trade and substitution patterns of coal, oil, and gas differ across models. In summary, distortions of fossil fuel markets resulting from relaxed short-term global emission targets are more important and less uncertain than the issue of carbon leakage from early mover action.

International audience; This article revisits the role of the time profile of carbon emission reductions in the design of climate policies. Using the CGE energy-economy model Imaclim-R, we demonstrate that the emission profile does not significantly change the time profile and the magnitude of mitigation costs. Recycling carbon tax revenues towards lower labor taxes and an early action on long-lived infrastructures offer very important reductions of mitigation costs. These complementary measures are as an important determinant of mitigation costs as the time profile of emissions and the sequencing of these options is closely related to the intertemporal tradeoff on emission reduction.

International audience; In current debates on a post-2012 global climate agreement, a key challenge is how to involve developing countries, in particular China, beyond the pledges they put forward after COP15 in Copenhagen. This article gives a macroeconomic assessment of different options for an extended Copenhagen pledges framework in order to alleviate the medium and long term costs of climate change mitigation. To do so, it uses a hybrid general equilibrium framework, in which suboptimalities are explicitly introduced. The results highlight that only an extending Copenhagen pledges with tradable permits of carbon emissions integrating non Annex 1 countries, as well as appropriate domestic accompanying measures may contribute to lowering the global costs to achieving the long term climate mitigation targets. It is also argued that articulating global and national policies can be an acceptable framework for China since it is aligned with its specific development priorities and offers the flexibility for domestic policies to support the transition. This mixed policy framework can be regarded as a reasonable first step towards a more ambitious global climate agreement on the long term.

Africa was the birthplace of mankind, and Homo sapiens and his ancestors were confined to the African continent for most of their evolutionary period, before migration began to other areas of the world some 140,000 years ao. As a result, there is more genetic diversity between populations living within the African continent, than between Africans and the rest of mankind. Africa also demonstrates a very wide range of environments - with respect to climate, vegetation and zoology (including micro-organisms and human parasites). One might therefore expect a correspondingly wide diversity of cancer patterns, the study of which would be illuminating to our understanding of the causes of human cancer.Until quite recently, knowledge of cancer patterns in Africa was based mainly on the work of pioneering clinicians and pathologist5s. The clinical and pathological case series which they described enriched the literature in the 1950's and 1960's, but comparisons based upon relative frequency of differenct cancers in case series can be misleading. To evaluate differences in risk between populations requires incidence rates, derived from population-based cancer registries, which aim to record information on all new cases of cancer occurring in a defined population. Cancer in Africa includes a description of all cancer registration activity ongoing in Africa today, as well as in the past.

The aim of our work was to identify risk factors specific to populations in our region for primary prevention of risk behaviors, particularly eating habits.We conducted a case-control study on dietary factors of nasopharyngeal carcinoma in the Setif area in Algeria. The study included 72 cases and 72 controls matched for age, sex and area of residence. The variables analyzed were identified by an anthropological study.Increased risk of disease was associated with consumption of traditionally preserved foods such as "harissa", pickled fruit and vegetables and dried and salted meat. These factors were commonly found in other endemic areas. Certain products appeared to be specific to the Setif area including dried and salted fat, especially after prolonged exposure. Rancid butter was related to a 4 to 7-fold increase in risk and use of pickling increased the risk of nasopharyngeal carcinoma 4 to 12-fold depending on the type of food.These data point to the need for biochemical analysis of food specimens to search for the carcinogenic agents and to the importance of an immediate information and education program on food habits for the populations living in this area.

- Abstract
BACKGROUND: The lifestyle risk factors for nasopharyngeal carcinoma (NPC) in North Africa are not known. METHODS: From 2002 to 2005, we interviewed 636 patients and 615 controls from Algeria, Morocco and Tunisia, frequency-matched by centre, age, sex, and childhood household type (urban/rural). Conditional logistic regression was used to evaluate the association of lifestyles with NPC risk, controlling for socioeconomic status and dietary risk factors. RESULTS: Cigarette smoking and snuff (tobacco powder with additives) intake were significantly associated with differentiated NPC but not with undifferentiated carcinoma (UCNT), which is the major histological type of NPC in these populations. As demonstrated by a stratified permutation test and by conditional logistic regression, marijuana smoking significantly elevated NPC risk independently of cigarette smoking, suggesting dissimilar carcinogenic mechanisms between cannabis and tobacco. Domestic cooking fumes intake by using kanoun (compact charcoal oven) during childhood increased NPC risk, whereas exposure during adulthood had less effect. Neither alcohol nor shisha (water pipe) was associated with risk. CONCLUSION: Tobacco, cannabis and domestic cooking fumes intake are risk factors for NPC in western North Africa.

Diabetes mellitus stands as a major public health issue in Algeria and has an important socioeconomical impact. Our study involved a representative sample of 1457 subjects and aimed at assessing the prevalence of type 2 diabetes and glucose intolerance in the population of Setif Wilaya, aged between 30 and 64 years old.Diagnosis was based on oral glucose tolerance test, according to World Health Organisation criteria.Diabetes prevalence was 8.2% (CI: 95%: 6.8% to 9.6%). It increased with age, while 50% of cases were undiagnosed, without any difference according to sex nor urban (7.3%)/rural (9.7%) distribution. Glucose intolerance prevalence was 7.1 (CI 95%: 5,8% to 8,4%). Age-standardized prevalence, according to world population data provided by WHO, was 9.08% for diabetes and 7.5% for glucose intolerance. When the new American Diabetes Association Criteria were used, prevalence of type 2 diabetes was 8.8 (CI: 95%: 7.3% to 10.2%) and that of fasting hyperglycemia was 6.9% (CI: 95%: 5.6 to 8.2). According to these new criteria, among the 66 cases with undiagnosed diabetes, 79% presented with a fasting blood glucoseor =126 mg/dl.This relatively high diabetes prevalence calls for an appropriate management and health education, particularly focused on high risk subjects. These results bring the the first detailed prevalence data in an Algerian population.

Background: The age-standardized rate of breast cancer (BC) increased nearly nine-fold in the last 30 years in Setif, Algeria. A case-control study was carried out to investigate the role of sociodemographic characteristics and reproductive factors in the etiology of BC in this young Arab/Berber population of Setif., Methods: Cases were 612 women with incident BC admitted to major teaching and general hospitals of Setif during 2012-2017. Controls were 615 women admitted for acute, non-neoplastic conditions to the same hospital network of cases. Information was elicited using a standardized questionnaire. Odds ratios (ORs) and 95% confidence intervals (CIs) were computed after allowance for age and education., Results: Significant associations with BC risk emerged for family history of BC (OR= 4.15 for yes vs. no; 95%CI: 2.22-7.77), for the generation of oral contraceptive used (OR=1.57 for II-generation vs. III-generation; 95%CI:1.01-2.44), and education (OR=0.63 for >11 years vs. no schooling; 95%CI: 0.46-0.86). Conversely, parity and body mass index were not associated with BC risk, whereas a late age at menarche was linked with a non statistically significant BC risk (OR=1.20 for >15 vs.

48. Assessing Short-Term and Long-Term Economic and Environmental Effects of the COVID-19 Crisis in France.

- Abstract
In response to the COVID-19 health crisis, the French government has imposed drastic lockdown measures for a period of 55 days. This paper provides a quantitative assessment of the economic and environmental impacts of these measures in the short and long term. We use a Computable General Equilibrium model designed to assess environmental and energy policies impacts at the macroeconomic and sectoral levels. We find that the lockdown has led to a significant decrease in economic output of 5% of GDP, but a positive environmental impact with a 6.6% reduction in CO
2 emissions in 2020. Both decreases are temporary: economic and environmental indicators return to their baseline trajectory after a few years. CO2 emissions even end up significantly higher after the COVID-19 crisis when we account for persistently low oil prices. We then investigate whether implementing carbon pricing can still yield positive macroeconomic dividends in the post-COVID recovery. We find that implementing ambitious carbon pricing speeds up economic recovery while significantly reducing CO2 emissions. By maintaining high fossil fuel prices, carbon taxation reduces the imports of fossil energy and stimulates energy efficiency investments while the full redistribution of tax proceeds does not hamper the recovery., (© Springer Nature B.V. 2020.)- Published

49. Worldwide Age at Onset of Female Breast Cancer: A 25-Year Population-Based Cancer Registry Study.

- Abstract
A higher frequency of early onset female breast cancers (BC) has been observed in low/middle income countries than in high income countries. We quantified the role of population ageing to this pattern using data from all population-based cancer registries (CRs) worldwide. Patients' median age at BC onset and that of the general population were extracted for CRs listed in volumes VI (1983-1987 years) through XI (2008-2012 years) of Cancer Incidence in Five Continents. Their association was assessed at cross-sectional level by linear regression model and longitudinally considering 25-year ageing of the population in long-standing CRs listed at the beginning and at the end of the study. During 2008-2012, each one-year increase of population ageing was associated with a nearly ½ year increase of age at BC diagnosis. Population demographics explained forty-two percent of the age variance for BC. In 1983-1987, long-standing CRs with a median age at BC below age 61.8 years showed an increase of age at BC after 25-years. Worldwide, age at BC diagnosis essentially reflected the median age of the population. Changes in BC detection methodology likely lessened this association. Nevertheless, the elevated absolute number of BCs in young populations deserves strategies of BC prevention.

- Abstract
Background: Nasopharyngeal carcinoma (NPC) has a higher incidence in North Africa than in most parts of the world. In addition to environmental factors such as Epstein-Barr virus infection and chemical carcinogen exposure, genetic susceptibility has been reported to play a key role in the development of NPC. NAD(P)H: quinone oxidoreductase 1 is a cytosolic enzyme that protects cells from oxidative damage. A C to T transition at position 609 in the NQO1 gene (OMIM: 125860) has been shown to alter the enzymatic activity of the enzyme and has been associated with increased risk to several cancers. This study investigates for the first time the effect of this polymorphism on NPC susceptibility in a North African population., Methods: The NQO1 C609T polymorphism was genotyped using PCR-RFLP in 392 NPC cases and 365 controls from Morocco, Algeria, and Tunisia., Results: The allele frequencies and distributions of genotypes did not differ between cases and controls (p > 0.05). When stratifying according to smoking status, we observed two-fold higher NPC risk in ever-smokers carrying the CT or TT genotype. Multiple logistic regression analysis revealed that there was a significant interaction between T allele and smoking status (OR = 1.95, 95% CI = 1.20-3.19; interaction p = 0.007)., Conclusion: In this North African population, the functional NQO1 polymorphism was associated with a significantly higher risk of NPC among smokers and did not affect the risk among nonsmokers., (© 2018 The Authors.
