Background: The objective of this study was to evaluate differences in various clinical endpoints in patients with reconstructive surgery by renal partial nephrectomy for tumors up to 4 cm compared to tumors larger than 4 cm.Material and methods: A total of 170 partial renal resection patients that presented malignant tumors were included in the retrospective study. Data was analyzed retrospectively based on internal clinic files, as well as a questionnaire to enhance the follow-up clinical outcomes data obtained. The most important outcomes determined included post-operative renal function, intra- and post-operative complications, local recurrence rate and total survival time.Results: The local recurrence rate was 6.1% for tumors up to 4 cm in size, compared to 14.9% for tumors that were larger than 4 cm. Compared to results for partial resection of T1a tumors, results for partial resection of tumors larger than 4 cm are worse in terms of post-operative renal function (p=0.007), as well as in terms of a total complications rate (p=0.048). It is important to note that there was not only a higher risk of post-operative bleeding that required transfusions (p=0.012), but also a higher risk of a hypertensive episode during the post-operative period reviewed (p=0.022). In addition, the total survival time for patients presenting tumors of up to 4 cm in size was significantly better (p=0.003). Conclusion: The results of our retrospective study of 170 patients that underwent partial renal resection after the diagnosis of malignant tumors, is that partial renal resection presents an oncologicaly safe surgical solution with low local recurrence rates. Additionally, partial resection in case of tumors that are larger than 4 cm showed worse post-operative renal function, a higher complications rate and a worse survival rate.

Objective: To propose possible success-driven solutions for problem and complication rates encountered with the ATOMS sling system, based on first-hand experience; and to provide possible actual alternative scenarios for the treatment of male . Patients and methods: During the defined period (between 4/2010 and 04/2014), 36 patients received ATOMS system implants at our clinic. We collected pre- and post-operative evaluation data using the International Consultation on Incontinence Questionnaire Short Form (ICIQ SF). As an expansion of the questionnaire, we added questions about post-operative perineal pain, the general satisfaction with the results of the intervention and willingness to recommend the operation to a best friend. Results: Our data shows a relatively high explantation rate, but a surprisingly high patient satisfaction rate. Explantation was required mainly due to late onset infections or other symptomatic factors. Compared to other studies early onset infections were rare. Conclusion: A non-invasive, uncomplicated adjustable system to alleviate male stress urinary incontinence remains a challenge. Although there are various systems available for the treatment of male stress urinary incontinence, it seems that despite the advantages of the ATOMS system, an artificial sphincter system may pose more advantages based on our experience, understanding and knowledge of its well-documented long-term solutions and problems.

For bulbar urethral strictures up to 2.5 cm in length, the one-stage urethral plastic surgery with stricture excision and direct end-to-end anastomosis remains the best procedure to guarantee a high success rate.This retrospective review shows the results of 21 patients who underwent bulbar end-to-end anastomosis from 2010–2013.In 20 cases (95.3%) good results were archived. The criteria of success were identified by pre- and postoperative radiological diagnostics and uroflowmetry.

Despite the progress of conservative treatment options for many urological diseases, surgical treatment is essential. The establishment and development of reconstructive urology has the objective of preservation, restoration or replacement of organ functions. Primarily, it is a substitute for radical reconstructive surgery of urological tumors in the region of the upper and lower urinary tract and the surgical correction of congenital and acquired anomalies and malformations of the urinary tract. Congenital malformations, trauma or cancer surgery often require so-called reconstructive surgical techniques to normalize or organ function restore; i.e. plastic and esthetic unprovments achieved by pyeloplasty for UPG or organ replacement for post cystectomy patients. It may be plastic, aesthetic corrections, as in case of malfunction of the urinary tract (stenosis of the uper uritric pelvic junction) or organ replacement, as state after radical cystectomy for bladder cancer. Through the application of minimally invasive surgical techniques in urology such as endoscopy and laparoscopy, this led in recent years to an expansion of the spectrum of reconstructive surgical urology in the treatment of urological emergencies as part of multiple trauma and improved the quality of life of patients. This paper deals with the developments of some selected reconstructive surgical techniques in recent years, that had a major change in the indication and surgical techniques.

Objective: We review our results after surgical reconstruction with the Nesbit’s procedure for congenital or acquired penile deviation. Patients and methods: Etiology of penile deviation, surgical outcome for straightening the penis, postoperative patient satisfaction and clinical findings were evaluated for 5 patients undergoing Nesbit’s procedure followed by a Medline review of contemporary literature regarding alternative surgical techniques. Follow-up included clinical examination, self-photography on erection and a standardized interview with erectile dysfunction assessment using IIEF-5 questionnaire. Results: Overall patient satisfaction was 100% in two patients, 2 patients were partly satisfied and 1 patient reported no satisfaction because of severe penile shortening with insufficiency for sexual intercourse postoperatively. Preoperative mean angulation of the penis was 42°. Four patients had Peyronie’s disease and 1 patient’s curvature resulted from an untreated penile fracture during sexual intercourse. Conclusion: The Nesbit technique can give satisfactory results for mild and moderate penile curvature. However, each technique for the reconstruction of penile deviation has its own advantages and disadvantages. Therefore proper patient selection has a major impact on further outcome.

Unmanned Ground Vehicles are used for various reasons. One of them is used in search and rescue operations which are known as Throwable Unmanned Ground Vehicles. It is a type of Unmanned Ground Vehicle which can sustain falls from several meters. Such vehicles are easily deployable, lightweight and impact resistant. Wheels play an important role since they absorb most of the impact. To optimize the performance of such vehicle, mathematical modeling is necessary. In this research paper, the main goal is to model the system mathematically. Parameters like impact forces, elasticity, Young’s modulus and duration of impact depend highly on material properties and structure of wheels. Results show the effect of certain parameters on wheel’s design and materials. For that purpose three materials are chosen PCTPE, carbon fiber, and vulcanized rubber for wheels. Impact forces, and duration of time are plotted against height of fall and diameter of the wheel for these materials.

Throwable Unmanned Ground Vehicles are light weight, small size, easily deployable and impact resistive vehicles mainly used in domestic as well as in military purposes where human life is compromised such as inspection of sewage pipes, and search and rescue operations etc. Main challenge while considering a Throwable UGV is its impact absorption capability which is seen through its material properties. Wheels of Throwable UGV absorb most of the impact without harming the internal structure. To solve this problem a honeycomb structured wheel is designed and simulated in ANSYS Workbench. Vulcanized rubber and Plastic composite PCTPE material wheels were impact tested using explicit dynamic analysis tool in ANSYS workbench. Total deformation, equivalent stress and strain results are tested in ANSYS under impact testing of wheel which is dropped from 10 meter height with velocity of 14m/s in a concrete surface. A plastic composite material PCTPE was 3D-printed and was used in Throwable Unmanned Ground Vehicle.

In electromechanical systems, backlash in gear trains can lead to a degradation in control performance. We propose a drive–anti-drive mechanism to address this issue. It consists of two DC motors that operate in opposite directions. One motor acts as the drive, while the other serves as the anti-drive to compensate for the backlash. This work focuses on switching between the drive and anti-drive motors, controlled by a switched-mode PID controller. Simulation results on an inverted pendulum demonstrate that the proposed scheme effectively compensates for backlash, improving position accuracy and control. This switched controller approach enhances the performance of electromechanical systems, particularly where gear backlash poses challenges to closed-loop performance. [ABSTRACT FROM AUTHOR]

A significant number of people, either seasonally or permanently, migrate from the Thar Desert in Pakistan each year due to droughts caused by climate change. This study aims to investigate the determinants and consequences of these migration decisions, examine the effectiveness of migration as a climate adaptation strategy, and identify challenges in adapting to these changes. Data were gathered from 400 migrated households in the Mithi sub-district. A mixed-method approach was used, combining qualitative and quantitative methods. The findings revealed that threats to the standard of living, including lack of food and clean drinking water, unemployment, and limited educational and medical opportunities, were the primary reasons for permanent and temporary migration from ancestral locations. Migration significantly impacted the origin and destination regions, with positive or negative effects. Specifically, migrants identified various consequences for both the origin and destination communities, including population decline (63%), changes in age structure, increased demand for housing, economic fluctuations (73%), alterations in healthcare services, and increased psychological stress (77%). The study also revealed that individuals who migrated from the Thar Desert experienced improved conditions compared to their previous location, such as diversification of income sources, increased job stability, access to clean water and food, reduced health risks, and overall improvements in their living conditions. However, the destination communities faced significant challenges due to widespread resource depletion and environmental deterioration. Migrants encountered barriers to developing resilient livelihoods in destination areas, including lack of proper knowledge and information, institutional and government issues, environmental and technological challenges, and social and cultural issues. The study highlights the urgent need for comprehensive policies and sustainable solutions to address the root causes of migration and support the resilience of vulnerable populations.
